Trend of the Changes in the Level of Blood Lead, Urinary Arsenic and Urinary Cadmium of children in Ulsan ; 3-year Follow-up Study (울산지역 어린이들의 혈중 연, 요중 비소 및 카드뮴 농도 변화추이 - 3년 추적조사)

  • Objectives : To obtain basic data on blood lead level and urinary level of arsenic and cadmium of children living near a petrochemical estate and a suburban area in Ulsan, Korea and to observe the trend of the changes. In the level of these metals in these children. Methods : The study subjects comprised 626 children living near a petrochemical estate and 299 children living in a suburban area of Ulsan. We analyzed the level of lead, arsenic and cadmium using atomic absorption spectrometer. Results : The mean levels of blood lead in children living near the petrochemical estate were $5.25{\mu}g/dl,\;5.24{\mu}g/dl,\;and\;7.24{\mu}g/dl$, and in the years 1997, 1999, and 2000, respectively, whereas those of children living in the suburban area were $3.81{\mu}g/dl,\;4.75{\mu}g/dl,\;and\;7.19{\mu}g/dl$ respectively. The mean levels of urinary arsenic in children living near the petrochemical estate were $4.57{\mu}g/g$ creatinine, $4.78{\mu}g/g$ creatinine, and $6.02{\mu}g/g$ creatinine in the year 1997, 1999, and 2000 respectively, whereas those of children living in suburban area were $2.35{\mu}g/g$ creatinine, $4.75{\mu}g/g$ creatinine, and $7.07{\mu}g/g$ creatinine, respectively. The mean levels of urinary cadmium in children living near the petrochemical estate were $1.15{\mu}g/g$ creatinine, $1.05{\mu}g/g$ creatinine, and $1.71{\mu}g/g$ creatinine in the year 1997, 1999, and 2000, respectively, whereas those of the children living in the suburban area were $0.74{\mu}g/g$ creatinine, $1.29{\mu}g/g$ creatinine, and $1.48{\mu}g/g$ creatinine, respectively. There were increasing trends in the level of blood lead, urinary arsenic and cadmium of children in Ulsan, and the differences in the level of these metals were disappearing between the children living in other areas year by year. Conclusions : These results suggest that the amount of exposure to lead, arsenic, and cadmium is increasing from year to year, and there is a need for periodic biological and atmospheric monitoring of these metals in Ulsan.

Analysis Method of N-Nitrosamines in Human Urine by LC-MS/MS System (LC-MS/MS 시스템을 이용한 소변 중 N-니트로사민류 분석법 확립)

  • N-nitrosamines are the nitroso compounds which are produced by nitrosation reactions of the secondary amine and nitrite under acidic conditions. Approximately 300 species of N-nitrosamine have been tested for carcinogenicity in laboratory experiments, with 90% of them demonstrated carcinogenic effects different animal species, including higher primates. In 1978, IARC classified NDMA and NDEA as Group 2A, and NDPA, NDBA, NPIP, NPYR and NMOR as Group 2B. In this study, we established pretreatment and analytical method for N-nitrosamines (NDMA, NDEA, NMEA, NDPA, NDBA, NPIP, NPYR and NMOR) in human urine for biological monitoring of N-nitrosamines. The analytes were extracted using solid phase extraction (SPE), then quantitative analysis was performed by LC-(APCI)-MS/MS. The accuracies of the established method were between 85.8~108.7% and precisions were lower than 20%. The limit of detection (LOD) were between 0.0002 (NDBA) and 0.0793 (NDMA) ng/ml. The linearity obtained was satisfying for the 8 N-nitrosamines, with a coefficient of determination ($r^2$) higher than 0.999. The mean concentrations of N-nitrosamines in the urine were 2.645 mg/g creatinine for NDMA, 0.067 mg/g creatinine for NDEA, 0.009 mg/g creatinine for NMEA, 0.011 mg/g creatinine for NDBA, 0.271 mg/g creatinine for NPIP and 0.413 mg/g creatinine for NPYR. NDPA and NMOR were not detected. It can be used as a instrumental methodology for evaluation and risk assessment of human exposure to N-nitrosamines for the further research.

Changes of Renal Function and Treatment after CABG in Patients with Elevated Serum Creatinine (크레아티닌치가 높은 환자에서 관상동맥우회술 후 신장기능의 변화와 처치)

  • Background: Preoperative elevated serum creatinine values are associated with increased risk for both morbidity and mortality in patients undergoing on-pump coronary artery bypass surgery (CABG). We investigated the postoperative changes of renal function and proper management in the patients. Material and Method: Among 74 consecutive patients who underwent isolated on-pump CABG, 17 patients with increased serum creatinine level $(creatinine\;\geqq\;1.5\;mg/dL)$ within preoperative one week wereincluded in the study. Seven patients showed pre­operative serum creatinine level of 2.0 mg/dL or higher, and 3 of them had been undergoing hemodialysis. Preoperative hemodialysis was performed in the 3 patients due to end-stage renal failure (ESRD) the day before the operation. We started peritoneal dialysis immediately after the cardiopulmonary bypass in patients with ESRD or postoperative acute renal failure if it was necessary to remove intravascular volume and lower serum creatinine level. Result In most of the patients with CABG, postoperative serum creatinine level increased and recovered to the preoperative level at the discharge. In 2 of the 4 patients with serum creatinine level of 2.0 mg/dL or higher and 3 patients with ESRD, intravascular volume, serum creatinine level and serum electrolyte were controlled with peritoneal dialysis. Conclusion: Postoperative serum creatinine level increased transiently in most of CABG patients, and intravascular volume and serum creatinine level were controlled by peritoneal dialysis only in the patients with acute renal failure postoperatively and those depending on hemodialysis.

  • The effect of exercise on serum cholesterol and triglyceride concentration and on urinary creatinine and 3-methylhistidine excretion was studied in eight healthy women, aged 21 to 25yr. Subjects were tested before and after a 3-day experimental period, during which 1000 times rope-jumping a day were performed. Serum cholesterol and triglyceride levels decreased significantly after exercise. Urinary excretion of creatinine and 3-methylhistidine slightly increased after exorcise.

Relationship between Nutritionally-related Blood Metabolites and Gastrointestinal Parasites in Nguni Goats of South Africa

  • The objective of the study was to determine the relationship between faecal egg counts and nutritionally-related blood metabolites in Nguni goats of South Africa. Body weights, body condition scores (BCS), FAMACHA scores, faecal and blood samples were collected from 96 Nguni castrates. Faecal samples were analysed using the modified McMaster technique for nematodes and the sedimentation method for trematodes. Blood was analysed for packed cell volume (PCV), glucose, cholesterol, total protein, albumin, urea and creatinine. Season had an effect on glucose, globulin, total protein, creatinine, PCV and faecal egg counts (FEC). Globulin, PCV, creatinine and FEC were significantly higher in the wet season compared to the dry season. A quadratic relationship existed between faecal egg count loads and BCS whilst negative linear relationships were observed between faecal egg counts and creatinine, albumin and cholesterol levels of Nguni goats.

A Creatinine Biosensor with Reduced Interference from Creatine (크레아틴의 방해영향을 줄인 크레아티닌 바이오센서)

  • The planar-type amperometric creatinine biosensor employing an additional enzyme layer containing creatine kinase and adenosine triphosphate was developed to eliminate severe interference from creatine. In the additional enzyme layer, an interfering substance, creatine is converted to noninterfering product, phosphocreatine. Furthermore, the carbon electrode electroplated with Pt black(Pt-B) was employed to fabricate creatinine biosensors with improved sensor performance(e.g., sensitivity, reliability, and reproducibility). The creatinine levels in an unknown sample were determined within less than 5% errors using creatinine microsensors equipped in a flow-cell cartridge.

노출평가를 위한 TLV 근거 - 2-BUTOXYETHANOL(8)

  • 2-부톡시에탄올(2-butoxyethanol)에 대한 생물학적 노출지수 설정에 필요한 자료는 상당수 존재한다. 다양한 실험실 및 직업적 현장연구는 2-butoxyethanol에 노출된 후 소변에서 butoxyacetic acid의 배설을 조사하여 생물학적 노출지수를 구축할 데이터베이스를 제공하였다. 실험실 연구는 작업시간 동안보다는 짧은 시간을 노출시킨다. 휴식 중에 실시한 실험 자료들을 20ppm에 8시간 노출로 외삽하면 작업시작 종료 시점의 소변 중 butoxyacetic acid의 농도가 각각 192, 272, 190 mg/g creatinine이었다. 작업부하 30 watts와 50 watts의 연구 결과를 외삽하면 각각 304 butoxyacetic acid/g creatinine과 313 butoxyacetic acid/g creatinine이었다. 작업현장 연구는 소변을 가수분해시킨 후 총 butoxyacetic acid의 검출 범위가 광범위할 것으로 예상된다. 이러한 외삽의 결과는 일반적으로 1ppm 미만의 노출에서 나온 것이라는 점에 유의해야 한다. 피부 흡수의 가능성과 2-butoxyethanol의 상대적으로 낮은 휘발성을 감안할 때 현장 연구에서는 피부 경로가 흡입 연구보다 더 중요할 수 있다. 따라서 현장 조사는 휴식을 취한 자원봉사자를 대상으로 실시한 흡입 연구와 비교하면 소변 중 butoxyacetic acid 농도를 과대평가하는 경향이 있다. 활용 가능한 시뮬레이션 연구에서 휴식 시 2-butoxyethanol의 TLV-TWA 20ppm에 8시간 전신(흡입과 피부) 노출되는 경우 가수분해된 총 butoxyacetic acid가 210 mg/g creatinine이 예상되고 작업부하 50 watts에서는 373 mg/g creatinine이 예측된다. 50 watts의 작업량으로 20ppm에서 8시간 노출되는 경우 생물학적 노출지수(BEI)는 약 400 mg/g creatinine일 것이다. 그러나 생물학적 노출지수는 외삽에 대한 불확실성, 심각한 피부 노출 및 작업장 노출평가 자료의 부족을 반영하여 200 mg butoxyacetic acid/g creatinine으로 설정하였다. 인용한 자료들의 한계에도 불구하고 자원 봉사자 연구와 시뮬레이션 연구의 자료들이 일치성이 있었다. 연구에 따르면 butoxyacetic acid는 측정 가능한 양으로 존재하며 작업 교대 중에 검출될 수 있는 충분한 시간의 반감기를 가지고 있다. Sakai 등의 연구 자료와 일부 모델링 자료에서 노출농도가 높은 경우 주중에 약간의 축적 가능성을 보고하였지만 작업장에서 일반적으로 노출되는 농도 범위에서는 누적이 예상되지 않으며 작업 종료 후에 소변시료를 채취하는 것이 바람직하다. 대다수의 연구에서 소변 중 butoxyacetic acid 농도 수준의 개인 간 다양성을 보고하였다. 또한 대부분의 작업현장 연구에서는 2-butoxyethanol의 주변 환경 농도수준과 소변 중 butoxyacetic acid 수준에 유의한 상관관계를 발견하지 못하였다. 상호 연관성이 결여된 원인은 대부분 피부 노출의 영향으로 추정하였다. 개인 보호 장갑을 착용한 근로자들에게서는 유의한 상관관계가 발견되었다.

Serum Lactate, Creatinine and Urine Output: Early Predictors of Mortality after Initial Fluid Resuscitation in Severe Burn Patients (중증 화상에서 초기 수액치료 이후 소변량, 혈중젖산, 크레아티닌 수치 변화와 이에 따른 사망률 예측)

  • Purpose: PL, creatinine and urine output are biomarkers of the suitability and prognosis of fluid therapy in severe burn patients. The purpose of this study is to evaluate the usefulness of predicting mortality by biomarkers and its change during initial fluid therapy for severe burn patients. Methods: A retrograde review was performed on 733 patients from January 2014 to December 2018 who were admitted as severe burn patients to our burn intensive care unit (BICU). Plasma lactate, serum creatinine and urine output were measured at the time of admission to the BICU and after 48 hours. ABSI score, Hangang score, APACHEII, revised Baux index and TBSA were collected after admission. Results: 733 patients were enrolled. PL was the most useful indicators for predicting mortality in burn patients at the time of admission (AUC: 0.813) and after 48 hours (AUC: 0.698). On the other hand, mortality prediction from initial fluid therapy for 48 hours showed different results. Only creatinine showed statistical differences (P<0.05) in mortality prediction. But there were no statistical differences in mortality prediction with PL and UO (P>0.05). Conclusion: In this study, PL was most useful predictor among biomarkers for predicting mortality. Improvement in creatinine levels during the first 48 hours is associated with improved mortality. Therefore, efforts are needed to improve creatinine levels.

A Correlation Analysis of Serum Creatinine Based eGFR and Serum Cystatin C Based eGFR with Thyroid Dysfunction Patients (갑상선기능이상 환자에서 크레아티닌과 시스타틴을 이용한 사구체여과율 검사결과의 상관성 연구)

  • Serum creatinine-based eGFR and serum cystatin C-based eGFR are the most popular methods for measuring renal function. Thyroid hormone is known to affect serum creatinine-based eGFR and serum cystatin C-based eGFR; however, the clinical significance of thyroid dysfunctional patients of renal function evaluation has not been fully elucidated to date. This study examined the effect of thyroid hormone on serum creatinine-based eGFR and serum cystatin C-based eGFR. Moreover, we also evaluated the correlation analysis between serum creatinine-based eGFR and serum cystatin C-based eGFR in patients with thyroid dysfunction. A total of 442 patients with hypothyroidism and 284 patients with hyperthyroidism were investigated. A correlation analysis between thyroid hormone and serum creatinine- (and cystatin C-) based eGFR was performed. A correlation analysis between thyroid hormone and serum cystatin-C based eGFR indicated that serum cystatin-C based eGFR is more of an independent biomarker than serum creatinine-based eGFR in thyroid dysfunction patients. Therefore, serum cystatin C-based eGFR more accurately reflects renal function than serum creatinine-based eGFR in thyroid dysfunction patients.

Factors Associated with the Concentrations of Urinary Creatinine in Korean Children and Adolescents (한국 어린이와 청소년의 요중 크레아티닌 농도와 영향요인에 대한 연구)

  • Creatinine-adjustment is an important process in the urinary monitoring of the environmental exposure of children and adolescents. The purpose of this study was to investigate the concentrations of urinary creatinine and factors associated with them among Korean children and adolescents. We recruited 1,025 persons from 128 extracted schools. They were from three to 18 years old and supplied urine samples for measuring creatinine. The concentrations of urinary creatinine were 98.18 mg/dl (SD, 67.67) in arithmetic mean and 72.05 mg/dl (GSD 2.49) in geometric mean, were significantly higher among male children/adolescents than females in all age groups, and higher values appeared following increasing ages, heights and BMIs. The rates of the number who were below the lowest limit recommended by WHO (<30 mg/dl) were 25.57% among three to four year olds, 21.77% among five to six year olds, 20.0% among seven to eight year olds and 14.69% among nine to ten year olds, respectively. The rates of those above the highest limit (>300 mg/dl) were 0.0% among three to twelve year olds. The coefficient of determination R-square of the fitted regression model for urinary creatinine was 27.4% with general characteristic variables of sex, age, BMI and height. The significant variables among these were height (standardized beta = 0.372) and age (standardized beta = 0.129). Another coefficient of determination R-square was 15.3% with dietary habit variables of smoking, drinking, dining area, number of meals and snacks, and intake of milk food, cup-noodles, canned foods, popcorn, nachos, and hamburgers. In conclusion, the concentration of urinary creatinine was significantly lower in children than in adults, and was very significantly associated with the height of children. Therefore, children need the recommended concentrations for urinary creatinine, as distinguished from adults.
