A large osteoid osteoma of the mandibular condyle causing conductive hearing loss: a case report and review of literature

  • Osteoid osteomas are benign skeletal neoplasms that are commonly encountered in the bones of the lower extremities, but are exceedingly rare in jaw bones with a prevalence of less than 1%. This unique clinical entity is usually seen in younger individuals, with nocturnal pain and swelling as its characteristic clinical manifestations. The size of the lesion is rarely found to be more than 2 cm. We hereby report a rare case of osteoid osteoma originating from the neck of the mandibular condyle that grew to large enough proportions to result in conductive hearing loss in addition to pain, swelling and restricted mouth opening. In addition, an effort has been made to review all the documented cases of osteoid osteomas of the jaws that have been published in the literature thus far.

Evaluation of skeletal maturity in the cervical vertebrae and hand-wrist in relation to vertical facial types

  • Objective: To evaluate differences in skeletal maturity in relation to vertical facial types and to compare differences in the skeletal maturity of the cervical vertebrae and hand-wrist in females. Methods: This study included 59 females aged 7 to 9 years with skeletal Class I malocclusion. The participants were categorized into three groups (low, normal, and high) according to the mandibular plane angle. Skeletal maturity was measured using skeletal maturity indicators (SMIs) and the Tanner-Whitehouse III (TW3) method on hand-wrist radiographs and by using cervical vertebrae maturation indicators (CVMIs) on lateral cephalometric radiographs. Results: The SMI was higher in the high-angle group than in the low-angle group (p = 0.014). The median TW3 bone age was 11.4 months higher in the high-angle group than in the low-angle group (p = 0.032). There was no significant difference in CVMI among the three groups. Skeletal maturity showed a weakly positive correlation with the mandibular plane angle (SMI, r = 0.391; TW3, r = 0.333; CVMI, r = 0.259). Conclusions: The skeletal maturity of the hand-wrist in females with a high mandibular plane angle was higher than that in females with a low mandibular plane angle. Obtaining additional hand-wrist radiographs may facilitate evaluation of skeletal maturity of females. In females with a high mandibular angle, the time to commence orthodontic treatment may be earlier than that in females with a low mandibular angle.

  • This investigation was designed to categorize Angle's class III malocclusion groups through analyzing horizontal and vertical components of craniofacial skeleton in Angle's class III malocclusion. The material selected for this study consisted in standard lateral cephalogram of one hundred and fifty five children, seventy four boys and eighty one girls, aged 6 through 15 years, having Angle's class III malocclusion. On the basis of findings of this study, the following results were obtained. 1. In horizontal skeletal classifications,16 groups were classified FMN-A-B, SE-FMN-A according to the Ba-SE-Me, Ba-SE/R. 2. The sequences that have relatively high frequency are as follow; a) Horizontal Group l b) Horizontal Group 3 c) Horizontal Group 5 d) Horizontal Group 9 e) Horizontal Group 4 3. In vertical skeletal classification, 8 groups were classified according to the PMV/PP, PMV/OP, PMV/MP. 4. The sequences that relatively high are as follows; a) Vertical Group 1 b) Vertical Group 3 c) Vertical Group 4 d) Vertical Group 8.

The influence of age on lip-line cant in adults: a cross-sectional study

  • Objective: The aims of this study were to assess the direction and degree of lip-line cant in Korean adult orthodontic patients and to identify the effects of sex and age on changes in the cant severity. Methods: In this cross-sectional retrospective study, lip-line cant was measured in the frontal photographs of 585 Korean patients (92 men and 493 women) aged 18-48 years. The outcome variables (direction and degree of lip-line cant) were assessed in terms of predictor variables (sex, age, sagittal skeletal relationship, and menton deviation angle). Results: The direction of lip-line cant did not differ according to sex, age, or skeletal classification. Patients had $1.6^{\circ}$ of lip-line cant on average before orthodontic treatment. Middle-aged adults displayed a significant trend toward a lower degree of lip-line cant compared to younger adults (p < 0.01). Multiple linear regression analysis showed that the degree of lip-line cant was weakly negatively correlated with age (p < 0.001). Conclusions: While the direction of lip-line cant did not differ according to the parameters explored here, the degree of cant was correlated with age in adults, independent of menton deviation. Specifically, middle-aged adults tended to display significantly lower degrees of lip-line cant than did younger adults.

  • This study was performed to verify the craniofacial skeletal characteristics in anterior openbite group, in contrast to normal occlusion group, and also to find out, vertical factors which shows, correlate to the amount of anterior openbite. 21 individuals, including 10 males and 11 females, without orthodontic treatment history and anterior - posterior skeletal malrelationships, were selected and analyzed basic morphology and vertical factors, using standard lateral cephalogram. The obtained results as follows: 1. Comparison of anterior open-bite group with normal occlusion group using Moyers analysis. a) In basis morphologic analysis, Ba-SE-Me was lesser and Ba-SE-Mn. P. and Mn.P/A-B were greater in both sexes of anterior openbite group than in normal occlusion group. b) In angle measurements of vertical analysis, PMV/Pal. P. was lesser in male anterior openbite group and PMV/Occ. P. and PMV/Mn. P. were greater in both sexes of anterior openbite group than in normal occlusion group. c) In height ratio of vertical analysis, ATFH/PTFH and ALFH/ATFH were greater in both sexes of anterior openbite group than in normal occlusion group. 2. The amount of anterior openbite was correlated with PMV/Occ. P. and PMV/Mn. P.

Moyers 분석법에 의한 한국인 정상교합자의 안면두개골격에 관한 연구 (A ROENTGENOCEPHALOMETRIC STUDY OF CRANIOFACIAL SKELETAL CHARACTERISTICS OF KOREAN NORMAL OCCLUSION BY MOYERS' ANALYSIS)

  • This study was performed to establish the cephalometric standards and to observe the nature of anatomic fit in the internal structural relationship of the craniofaciodental complex of the normal Korean by means of Moyers' method. Lateral cephalograms of 143 males and 144 females with normal occlusion and acceptable profile from 6 to 25 years of age, which were consisted of 5 groups that were 6 year-, 9 year-, 12 year-, 15 year- and adult-group were obtained. Data were gathered by traced digitizing the cephalograms and were statistically analyzed. The findings can be summerized as follows. 1. Norms of Korean males, females and both sexes in each group were established. 2. There was little significant sexual dimorphism in the form of craniofacial skeleton of all age groups. 3. The height and length of craniofacial skeleton was alike in each sexes in the 6 year-, 9 year- and 12 year-group, whereas it was larger in male than in female in the 15 year- and adult-group. 4. There were no significant sexual differences in the internal structural relationship of the craniofacial skeleton in all age groups.

Prenatal Stress Induces Skeletal Malformations in Mouse Embryos

  • Dexamethasone, a synthetic glucocorticoid (GC), is clinically administered to woman at risk for premature labor to induce fetal lung maturation. However, exposure to repeated or excess GCs leads to intrauterine growth restriction (IUGR) and subsequently increases risk of psychiatric and cardio-metabolic diseases in later life through fetal programming mechanisms. GCs are key mediators of stress responses, therefore, maternal nutrient restriction or psychological stress during pregnancy also causes negative impacts on birth and neurodevelopment outcome of fetuses, and other congenital defects, such as craniofacial and skeletal abnormalities. In this study, to examine the effect of prenatal stress on fetal skeletal development, dexamethasone (1 mg/kg [DEX1] or 10 mg/kg [DEX10] maternal body weight per day) was administered intraperitoneally at gestational day 7.5~9.5 and the skeletons were prepared from embryos at day 18.5. Seven out of eighteen (39%) embryos treated with DEX10 showed axial skeletal abnormalities in either the T13 or L1 vertebrae. In addition, examination of the sternum revealed that xiphoid process, the protrusive triangular part of the lower end of the sternum, was bent more outward or inward in DEX group embryos. In conclusion, our findings suggest a possible link to the understanding of the effect of uterine environment to the fetal skeletal features.

골격성 III급 부정교합자의 두개안면형태에 대한 하악이부의 형태 및 하악절치의 위치에 대한 연구 (A study of morphology of mandibular symphysis and location of lower incisor under the influence of the craniofacial skeleton in skeletal Class III malocclusion)

  • 골격성 III급 부정교합자의 두개안면골격형태가 하악이부형태와 하악전치의 위치에 미치는 영향을 알아보고자 연구를 시행하였다. 전후방적인 악골부조화가 나타나지 않고 교정치료의 경험이 없는 만 20세 이상의 성인남녀(정상군)와, 하악전돌의 악골부조화를 보이며 교정치료의 경험이 없 만 20세 이상의 성인남녀(하악전돌군)를 비교하여, 두개안면골격형태에 따른 하악이부형태와 하악전치의 위치에 관하여 다음과 같은 결론을 얻었다. 1. 두개안면골격형태의 비교에서 하악에 관련된 ${\angle}SN-Mn,\;{\angle}FMA,\;{\angle}Pal-Mn,\;{\angle}LFH$은 하악전돌군이 정상군에 비하여 크게 나타났지만,(p<0.05) 상악에 관련된 항목은 차이를 나타내지 아니하였다(p>0.n). 2. 하악전치를 둘러 싸고 있는 치조돌기는 하악전돌군이 정상군에 비해서 수평적으로 좁고,수직적으로 긴 형태를 나타내었다(p<0.05). 하악이부의 형태에서도 수평적으로는 하악전돌군이 정상군에 비해서 좁게 나타났으나, 수직적으로는 차이를 나타내지 아니하였다(p>0.05). 3. 하악전돌군의 두개안면골격형태와 이부계측항목과의 상관관계에서 두개저와 하악하연에 기인하는 수직적인 악골부조화가 증가할수록 LaABH, LiACBW, LaACBW, LiABBW, SW, ${\angle}LISA$가 감소하는 역상관관계를 나타냈지만 (p<0.05), 전후방적인 악골부조화와는 별다른 상관관계가 나타나지 않았다(p>0.05). 4. 하악전돌군에서 수직적인 두개안면골격형태를 나타내는 ${\angle}SN-Mn,\;{\angle}FMA,\;{\angle}Pal-Mn,\;{\angle}LFH$ 와 하악이부의 계측 항목인 LiACBW, LiABBW, SW, ${\angle}LISA$ 사이에 높은 상관관계가 존재하여, 수직적인 악골부조화로 하악이부의 형태와 하악전치의 경사도를 예측할 수 있었다(p<0.001).

Cone-beam computed tomography for the assessment of root-crown ratios of the maxillary and mandibular incisors in a Korean population

  • Objective: This retrospective, cross-sectional study aimed to establish reference data for normal crown and root lengths and the root-crown ratios (R/C ratios) for the mature maxillary and mandibular incisors in a Korean population by using cone-beam computed tomography (CBCT). Methods: We included 672 Korean patients (141 men and 531 women; mean age, $27.2{\pm}7.7years$) who underwent CBCT examinations during various dental treatments. Crown and root lengths and the R/C ratios of the maxillary and mandibular incisors were measured using CBCT data, which were analyzed to detect significant differences between demographic factors as well as sagittal and vertical skeletal or occlusal relationships. Results: Teeth of the same type in each half-arch were symmetrical. The mean R/C ratios varied from 1.1 to 1.2 for the maxillary incisors and from 1.3 to 1.4 for the mandibular incisors. Crown and root lengths were greater in men than in women, regardless of tooth type. Root lengths and R/C ratios for the mandibular incisors were significantly greater in patients with skeletal Class II malocclusion or an excessive overjet than in the other patients. The R/C ratios for the mandibular incisors were lower in patients with an open bite than in those with a normal or deep bite. Moreover, the R/C ratios for the mandibular incisors increased with age. Conclusions: The data obtained in our study can serve as reference values for crown and root lengths and the R/C ratios for the maxillary and mandibular incisors in the Korean population.

Miniscrew insertion sites of infrazygomatic crest and mandibular buccal shelf in different vertical craniofacial patterns: A cone-beam computed tomography study

  • Objective: To identify optimal areas for the insertion of extra-alveolar miniscrews into the infrazygomatic crest (IZC) and mandibular buccal shelf (MBS), using cone beam computed tomography (CBCT) imaging in patients with different craniofacial patterns. Methods: CBCT reconstructions of untreated individuals were used to evaluate the IZC and MBS areas. The participants were divided into three groups, based on the craniofacial pattern, namely, brachyfacial (n = 15; mean age, 23.3 years), mesofacial (n = 15; mean age, 19.24 years), and dolichofacial (n = 15; mean age, 17.79 years). In the IZC, the evaluated areas were at 11, 13, and 15 mm above the buccal cusp tips of the right and left first molars. In the MBS, the evaluated areas were at the projections of the first molars' distal roots and second molars' mesial and distal roots, at a 4- and 8-mm distance from the cementoenamel junction. Intergroup comparisons were performed with analysis of variance and the Tukey test. Results: There was no statistically significant difference in the IZC bone thickness among the groups. For MBS bone availability, some comparisons revealed no difference; meanwhile, other comparisons revealed increased MBS bone thickness in the brachyfacial (first molars distal roots) and dolichofacial (second molars mesial and distal roots) patterns. Conclusions: There was no significant difference in the IZC bone thickness among the groups. The facial skeletal pattern may affect the availability of ideal bone thickness for the insertion of extra-alveolar miniscrews in the MBS region; however, this variability is unlikely to be clinically meaningful.
