Infection Rate of Chlamydia pneumoniae in Patients with Chronic Cough (만성기침 환자에서 Chlamydia pneumoniae 감염률)

  • Background: Persistent cough has recently been found to be associated with Chlamydia pneumoniae infection. We aimed to investigate the infection rate of C. pneumonia in adult patients with chronic cough. Methods: We recruited 68 patients with persistent cough lasting in excess of 3 weeks, who visited Kangdong Sacred Heart Hospital from January 2005 to August 2005. On the first visit, chest and paranasal sinuses radiography, skin prick test of common allergens, and induced sputum samples for C. pneumoniae were performed in all of patients. Further evaluation for diagnosis included a methacholine provocation test and eosinophil counts in induced sputum. Results: The most common cause of chronic cough was upper airway cough syndrome (UACS) (26.5%), followed by eosinophilic bronchitis (20.6%) and cough variant asthma (16.2%). Idiopathic chronic cough was the cause in 33.8% of patients. The mean duration of cough was 11.7 months. C. pneumoniae was isolated by polymerase chain reaction (PCR) from one patient who had upper respiratory air way syndrome. Conclusion: Chlamydia pneumoniae appears to have a minor role as a cause of chronic cough in patients.

The Effect of Low Dose Lidocaine on Fentanyl-Induced Cough, Mean Arterial Pressure, Heart Rate, Oxygen Saturation and Dizziness in Inhalation Anesthesia (흡입성 전신마취에서 저용량의 리도카인이 펜타닐-유도성 기침, 평균동맥압, 심박동수, 산소포화도 및 어지럼증에 미치는 영향)

  • Purpose: This study was performed to evaluate the effect of low-dose lidocaine on fentanyl-induced cough and hemodynamic changes under general anesthesia. This research was a randomized trial design and performed using a double-blind method. Methods: Data collection was performed from October 22, 2008, to May 4, 2009. One hundred and thirty two patients were randomly assigned to control group (Con G) and experimental group (Exp G) using a table of random numbers. Exp G (n=66) were administered 0.5 mg/kg lidocaine and Con G (n=66)) were administered saline. The occurrence of cough and vital sign were recorded within one minute after fentanyl bolus by an anesthesiologist. Collected data were analyzed using Repeated measures ANOVA using SPSS for Windows (Version 17.0). Results: The incidence of cough in Exp G was 13.6%, while Con G was 53%. The incidence cough in Exp G was significantly lower compared to Con G (p<.001). Lidocaine seemed not to suppress mean arterial pressure (p=.145), heart rate (p=.508), and oxygen saturation (p=.161). Conclusion: Intravenous administration of 0.5 mg/kg lidocaine seems to suppress fentanyl-induced cough without affecting mean blood pressure, heart rate and oxygen saturation. Therefore, we recommend intravenous 0.5 mg/kg lidocaine administration to suppress fentanyl-induced cough under general anesthesia.

Knowledge, Compliance, and affecting factors of the Cough Etiquette to Prevent the Respiratory Infectious Disease Transmission on Convergence Study (호흡기 감염 전파 예방을 위한 기침예절 지식과 실행수준 및 영향요인에 관한 융합 연구)

  • This study aimed to investigate the knowledge, compliance and affecting factors of the cough etiquette to prevent the respiratory infectious disease transmission. Data were collected 250 self reporting questionnaires above 19 years old adult from Aug. 13th to Sep. 19th 2018. 213 questionnaires were analyzed statistically. In the results, the score was converted in terms of 100 points, knowledge of the cough etiquette was average $70.99{\pm}19.92$ points, compliance was $70.63{\pm}10.25$ points. Knowledge and compliance were statistically positive significant correlation, In the multiple regression analysis, the affecting factors were gender, usually carry a handkerchief, usually carry a portable tissue paper, average number of handwashes per day, know about cough etiquette, knowledge of cough etiquette. Therefore, it is necessary to develop and apply an educational program that reflects the affecting factors of cough etiquette in this study result. The higher level of cough etiquette performance will help prevent respiratory infection transmission.

Effect of Myofacial Releasing of Abdominal Muscles on Chronic Cough by Oriental Medical Treatment : 2 Cases Report (한방치료를 통한 복근군의 근막이완으로 호전된 만성기침환자 치험 2례)

  • Objectives : We supposed that abdominal muscles are related to chronic cough, because abdominal pressure and respiratory muscles are influenced by abdominal muscle function. We have evaluated the effect of myofacial releasing of abdominal muscles with oriental medical treatment by experimenting two patients suffering from chronic cough. Methods : One patient was treated with acupuncture therapy and muscle stretching exercise for myofacial releasing of abdominal muscle. The other patient was treated with hot pack therapy and abdominal respiration training for myofacial releasing of abdominal muscle. Results and Conclusions : After oriental medical treatment of abdominal muscles in two cases, We figured out that the patients were on the mend. These results suggest that myofacial releasing of abdominal muscles using oriental medical treatment was effective in improving chronic cough.

  • Spinal Muscular Atrophy(SMA) is characterized by degeneration of the anterior horn cells leading to symmetrical muscle weakness and wasting of voluntary muscles. Depending on the age of onset, the maximum muscular activity achieved, and survivorship, 3 types of SMA are recognized: SMA type I=Werdnig-Hoffman disease; SMA type II=an intermediate form; SMA type III = Wohlfart-Kugelberg-Welander disease. We report on a 10-month-old male patient with SMA type II complained cough and sputum. We treated with Bopejungchungtang for his cough and sputum. After administration of Bopejunchungtang cough and sputum decreased and almost disappeared.

  • The cough is one of the most common respiratory symptom in children. Boeumjunggi-tang(BEJG) is herbal medicine mainly treat cough due to yin deficiency(陰虛咳嗽) in Oriental Medicine. During 3 momths from February, 1, 2000 to April, 30, 2000, BEGJ is administered to the pediatric out-patients with the cough due to yin deficiency have visited Pundang Dongguk Oriental medicine Hospital and among these patients, this study is performed for 50 patients of which the result of treatment could be gained. The conclusion is following. 1. In the distribution of patients, male(60%) is more than female(40%) and group of age in between 1 and 6 years(76%) are the first. 2. In the type of cough, group of chronic(more than 3 weeks), and group that the symptom make worse in night are the most. 3. In the distribution of remedial effect, group of symptom-relieved is the first(58%), symptom-disappearing is second(22%), symptom-unchanging is third(14%), symptom-aggravated is fifth(6%). 4. The chracteristic of cough have little influence on the remedial effect of BEGJ.

  • Objectives It is important to care aftereffects of the tuberculosis such as cough and sputum especially for the old. The purpose of this study is to report a case which showed symptoms improvement after treatment with Hyungbangjihwang-tang.Methods To evaluate the results of this treatment, Decrease of cough and sputum was assessed by Visual Analogue Scale(VAS). The patient`s oral intake and body weight were measured.Results The patient who suffered with cough and sputum after tuberculosis developed high fever and pleural effusion of unknown origin. After treatment with western and oriental medicine, high fever and pleural effusion were subsided but cough and sputum got worse and body weight was decreased after high fever and pleural effusion. So we prescribed Hyungbangjihwang-tang and then the symptoms of the patient were improved.Conclusions This study suggests that using Sasang constitutuional medical treatment is effective for Soyangin patient with afftereffects of the tuberculosis such as cough and sputum.

  • Cough is not only serves to maintain normal function of respiratory tract, but also many diseases may lead to this symptom. In Korean medicine, Zhudanxi(朱丹溪), was the first to classify cough according to the different stages of time. In this research, I investigated the stage of Nocturnal Cough(夜嗽) through 26 oriental medical books. As a result, deficiency of Yin(陰虛) was the most common causes of Nocturnal Cough and indigestion(食積) is the next. Liuweidihuang-tang(六味地黃湯) prescribed as the treatment for Yin Xu(陰虛) and Xeibaisan-jiawei(瀉白散加味) prescribed as the treatment for indigestion(食積).

Clinical Values of Cold-Heat Pattern Diagnosis by the Nasal Endoscopy for Patients with Cough (기침에 대한 비내시경을 이용한 한열변증(寒熱辨證)의 임상적 가치평가)

  • Objectives: This study was aimed to figure out an agreement between the diagnosis of nasal endoscopy and a preexisting questionnaire focusing on Cold-Heat pattern. Methods: 52 patients with cough who met the criteria filled out a pattern questionnaire and the examiner looked at their nasal cavities through nasal endoscopy. According to the checked questionnaire results, the subjects were identified by 6 patterns. After examining subject's mucous membrane of oropharynx and nasal cavity through nasal endoscopy, we classified each to the Cold or Heat group. Correlation between questionnaire and nasal endoscopy results was analyzed. Results: In diagnosing Cold-Heat, there was no significant difference by McNemar test (p=0.227) between nasal endoscopy and the questionnaire, and the two methods agreed moderately (${\kappa}=0.428$). The color of mucous membrane of oropharynx and the Cold-Heat pattern on questionnaire agreed slightly (${\kappa}=0.133$). The color of mucous membrane of nasal cavity and the Cold-Heat pattern on questionnaire agreed fairly (${\kappa}=0.384$). In the patients with cough related to upper respiratory tract, they got higher diagnosis accuracy than the patients with cough related to lower respiratory tract did. Similarly, external cough patients got higher diagnosis accuracy than internal cough patients did. Conclusions: To identify Cold or Heat, examining oropharynx and nasal cavity using nasal endoscopy is a meaningful method in patients with cough, showing that two diagnosis methods which use nasal endoscopy and questionnaire agreed moderately. Especially, it is more useful diagnosing patients with cough related to the upper respiratory tract than diagnosing the patients with cough related to the lower respiratory tract.

Clinical Assessment of 28 Patients with Cough Treated with Herbal Steam Therapy (훈증요법을 병행한 기침환자 28명에 대한 임상적 평가)

  • The purpose of this study was to investigate the clinical effects of herbal steam therapy on cough. Twenty-eight patients with cough who had treated with herbal steam therapy(chungsang, chungpae) between 3rd of January and 30th of April, 2013 were analyzed. The effects were evaluated the severity and frequency of cough as the modified questionnaire, severity of discomfort using visual analogue scale(VAS), and the patients' subjective satisfaction. Eighteen patients who had been treated herbal steam therapy over 3 times were analyzed improvements before and after therapy. Chungsang and Chungpae steam therapies significantly decreased the severity and frequency of cough. VAS scores also decreased from $7.78{\pm}0.878$ to $4.39{\pm}2.033$. Patients were satisfied with herbal steam therapy. Chungsang and Chungpae steam therapies seem to be effective in the treatment of cough. Further studies with a large number of cases will be needed in the future.
