Comparison of the Operative Results of Performing Endoscopic Robot Assisted Minimally Invasive Surgery Versus Conventional Cardiac Surgery (수술용 내시경 로봇(AESOP)을 이용한 최소 침습적 개심술과 동 기간에 시행된 전통적인 개심술의 결과에 대한 비교)

  • Background: The improvements in endoscopic equipment and surgical robots has encouraged the performance of minimally invasive cardiac operations. Yet only a few Korean studies have compared this procedure with the sternotomy approach. Material and Method: Between December 2005 and July 2007, 48 patients (group A) underwent minimally invasive cardiac surgery with AESOP through a small right thoracotomy. During the same period, 50 patients (group B) underwent conventional surgery. We compared the operative time, the operative results, the post-operative pain and the recovery of both groups. Result: There was no hospital mortality and there were no significant differences in the incidence of operative complications between the two groups. The operative $(292.7{\pm}61.7\;and\;264.0{\pm}47.9min$, respectively; p=0.01) and CPB times ($128.4{\pm}37.6\;and\;101.7{\pm}32.5min$, respectively; <0.01) were longer for group A, whereas there was no difference between the aortic cross clamp times ($82.1{\pm}35.0\;and\;87.8{\pm}113.5min$, respectively; p=0.74) and ventilator times ($18.0{\pm}18.4\;and\;19.7{\pm}9.7$ hr, respectively; p=0.57) between the groups. The stay on the ICU $(53.2{\pm}40.2\;and\;72.8{\pm}42.1hr$, respectively; p=0.02) and the hospitalization time ($9.7{\pm}7.2\;and\;14.8{\pm}11.9days$, respectively; p=0.01) were shorter for group A. The Patients in group B had more transfusions, but the difference was not significant. For the overall operative intervals, which ranged from one to four weeks, the pair score was significantly lower for the patients of group A than for the patients of group B. In terms of the postoperative activities, which were measured by the Duke Activity Scale questionnaire, the functional status score was clearly higher for group A compared to group B. The analysis showed no difference in the severity of either post-repair of mitral ($0.7{\pm}1.0\;and\;0.9{\pm}0.9$, respectively; p=0.60) and tricuspid regurgitation ($1.0{\pm}0.9\;and\;1.1{\pm}1.0$, respectively; p=0.89). In both groups, there were no valve related complications, except for one patient with paravalvular leakage in each group. Conclusion: These results show that compared with the median sternotomy patients, the patients who underwent minimally invasive surgery enjoyed significant postoperative advantages such as less pain, a more rapid return to full activity, improved cosmetics and a reduced hospital stay. The minimally invasive surgery can be done with similar clinical safety compared to the conventional surgery that's done through a median sternotomy.

Antioxidant and anti-inflammatory activities of extracts from Ledum palustre L. (백산차 추출물의 항산화 및 항염증 활성)

  • In this study, Ledum palustre L. was extracted by 4 different methods (LPW, hot water extraction; LPA, autoclave extraction; LPU, ultrasonification extraction; LPE, 70% ethanol extraction) and LPE was fractionated by using polarity difference of each solvent and used as 4 samples (LPE/H, the n-hexane layer; LPE/E, the EtOAc layer; LPE/B, the n-BuOH layer; LPE/W, the $H_2O$ layer). Antioxidant activities of Ledum palustre L. extracts were measured by DPPH and ABTS. As a result, the DPPH and ABTS radical scavenging showed high activities with LPE (82.3%, 99.8%) and LPE/E (91.8%, 99.6%) at the concentration of $1,000{\mu}g/mL$. The anti-inflammatory activities of LPE and LPE/E were measured by the inhibitory activity against NO, $PGE_2$, TNF-${\alpha}$, IL-$1{\beta}$ and IL-6 production on LPS-stimulated Raw 264.7 macrophages. As a result of MTT assay, cell viabilities of LPE and LPE/E were more than 90% at $25{\mu}g/mL$. NO and $PGE_2$ productions were inhibited by LPE (NO: 50%, $PGE_2$: 70%) and LPE/E (NO: 57%, $PGE_2$: 73%) at the concentration of $25{\mu}g/mL$. The inhibition activities against TNF-${\alpha}$, IL-$1{\beta}$, IL-6 production were 24%, 47% and 40% at the concentration of $25{\mu}g/mL$ of LPE. In particular, LPE/E showed 51%, 57% and 62% inhibition activities at the same concentration, respectively. From the above results, it can be concluded that $1,000{\mu}g/mL$ of LPE and LPE/E have the high antioxidant activities similar with Vitamin C, and $25{\mu}g/mL$, the low concetration of LPE and LPE/E have excellent anti-inflammatory activities. Therefore, if more research about anti-aging, whitening and antimicrobial activity of Ledum palustre L. extracts is carried out in the future, it will be possible to use them as effective materials for the prevention and treatment of inflammatory diseases and in the areas of functional foods and cosmetics.

Formation of Liquid Crystalline with Hydrogenated Lecithin and Its Effectiveness (수소첨가레시친을 이용한 액정 젤의 형성과 보습효과)

  • This study described about method that forms liquid crystal gel (LCG) by main ingredient with hydrogenated lechin (HL) in O/W emulsion system. Result of stability test is as following with most suitable LCG's composition. Composition of LCG is as following, to form liquid crystal, an emulsifier used 4.0wt% of cetostearyl alcohol (CA) by 4.0wt% of HL as a booster. Moisturizers contained 2wt% of glycerin and 3.0wt% of 1,3-butylene glycol (1,3-BG). Suitable emollients used 3.0wt% of cyclomethicone, 3.0wt% of isononyl isononanoate (ININ), 3.0wt% of cerpric/carprylic triglycerides (CCTG), 3.0wt% of macademia nut oil (MNO) in liquid crystal gel formation. On optimum conditions of LCG formation, the pHs were formed all well under acidity or alkalinity conditions (pH=4.0-11.0). Considering safety of skin, pH was the most suitable 6.0${\pm}$1.0 ranges. The stable hardness of LCG formation appeared best in 32 dyne/$\textrm{cm}^2$. Particle of LCG is forming size of 1-20$\mu\textrm{m}$ range, and confirmed that the most excellent LCG is formed in 1-6$\mu\textrm{m}$ range. According to result that observe shape of LCG with optical or polarization microscope, LCG could was formed, and confirmed that is forming multi -layer lamellar type structure around the LCG. Moisturizing effect measured clinical test about 20 volunteers. As a result, moisturizing effect of LCG compares to placebo cream was increased 36.6%. This could predicted that polyol group is appeared the actual state because is adsorbed much to round liquid crystal droplets to multi-lamellar layer's hydrophilic group. It could predicted that polyol group is vast quantity present phase that appear mixed because is adsorbed to round liquid crystal to multi-lamellar layer's hydrophilic group. This LCG formation theory may contribute greatly in cosmetics and pharmacy industry development.

Anti-aging Effects of the Extracts from Leaf. Stem, Fruit and Seed of Yew (Taxus cuspidata Sieb) by Solvent Extraction Method (용매추출법에 의한 주목의 잎, 줄기, 과실 추출물의 항 노화 효과)

  • Yew (Taxus cuspidata Sieb.) chose that grow as medicine, food, decorative plant in Korea's Kyong-Gi province surroundings. Extracts of yew extracted leaf of 250g and stems of 300g with 1,3-butylene glycol (l,3-BG), propylene glycol (PG) and water. As results, external appearance of leaf extract of yew was slightly brown clear extract. The pH was 5.3${\pm}$0.5, and specific gravity was 1.012${\pm}$0.05, and refractive index was l.375${\pm}$0.05. Also, appearance of stem's extract was slightly brown clear extract, and the pH was 5.4${\pm}$0.5, and specific gravity was 1.016${\pm}$0.05, and refractive index was 1.358${\pm}$0.05. Oil of yew separated from seeds, and extracted polysaccharide high purity from fruits. As a result, specific gravity of oil was 0.987, and obtained 40.0% of yield. Total polyphenols amount of yew extract is detected 0.563% in leaves, 0.325% in stems, whereas total tannins amount contained 0.054% and 0.037% each in leaves and stems. As effect in cosmetics, the anti-oxidative effect by DPPH method is 75.0% in leaves, and stems was 64.0%. Collagen synthesis rate was shown high activity by 54.16% in stem's extract, 33.18% in leaves' extract. Also, PPE-inhibitory activities were 13.7% and 23.5% each in leaves and stems. Anti-inflammatory effect of yew seed oil displayed superior effect of 41% than control. Polysaccharide's molecular weight that is gotten from fruits was 5${\times}$10$^4$-3${\times}$10$\^$5/ dalton, and got 20.0${\pm}$5% of yield.

Components and Antimicrobial Activity of Veiled Lady Mushroom, Dictyophora echinovolvata (흰돌기망태버섯(가칭; Dictyophora echinovolvata)의 일반성분 및 항미생물활성)

  • A strain of Dictyophora echinovolvata ASI 32002 showing good fruiting body formation was selected. Analyses of chemical and nutritional components as well as antimicrobial activity of different parts of the mushroom such as mycelium, egg, and fruiting body were carried out. There were differences in the chemical compositions and the quantities depending on developmental stages of veiled lady mushroom, D. echinovolvata ASI 32002. Nitrogen, phosphate, magnesium, and calcium in inorganic chemicals were abundant in mycelium, and potassium and mineral elements were abundant in the egg and fruiting body. Mannitol and trehalose were abundant in free sugar contents. Glutamic acid and arginine in mycelium and aspartic acid and glutamic acid in egg and fruiting body were abundant in free amino acid contents. Linoleic acid, an polyunsaturated fatty acid, was abundant in all parts of the Dictyophora species, but compositions and quantities of other fatty acids varied depending on the different parts of the mushroom. It was detected that malic acid, lactic acid and acetic acid in mycelium, formic acid, acetic acid and fumaric acid in egg, and malic acid, citric acid, lactic acid, fumaric acid in fruiting body were abundant. The methanol extracts of D. echinovolvata ASI 32002 mycelium showed antifungal activity with minimal inhibition concentration (MIC) of $62{\sim}125\;{\mu}g/ml$ that was similar levels of cyclohexamide against Aspergillus awamori, Hypocrea nigricance and Trichoderma virens. The MIC of extracts from mycelium and fruiting body against Candida albicans was $250\;{\mu}g/ml$, similar to that of tetracycline. In addition to the above results, further as food additives and ingredient of cosmetics.

Analysis of Ingredients Using 5 Species of Native Plants from Mt. Jiri. for the Development of Cosmetic Raw Materials, DPPH and ABTS Activity-II (지리산 자생식물 5종의 성분 분석 및 DPPH, ABTS 활성 실험을 통한 화장품 원료개발-II)

  • Five species of plants (Pimpinella brachycarpa (Kom.) Nakai, Hylotelephium erythrostictum (Miq.) H. Ohba, Magnolia sieboldii K. Koch, Sanguisorba hakusanensis Makino, Agastache rugosa (Fisch. & Mey.) Kuntze) native to the clean area of Mt. Jiri were selected. The collection period was from May to September 2021, and the five species plants were collected in their native habitats with flowers in full bloom. The collected plants were extracted with 70% EtOH and 17 kinds of polyphenol components were analyzed. Next, flowers, leaves and roots were separated from plants, extracted with 70% EtOH for each part and experiments were conducted on DPPH, ABTS, total polyphenols, and total flavonoids. The results are as follows. 1. There were a total of 13 polyphenol components contained in the 5 species of plants native to Mt. Jiri. Among them, the total content of the most detected substance was Sanguisorba hakusanensis Makino, which was 126.2 ppm, and the main substance was ellagic acid (=ELA), which was 122.4 ppm. 2. As a result of examining the DPPH radical scavenging activity, the flowers, stems, and roots of Sanguisorba hakusanensis Makino showed the best scavenging activity, while the flower extracts of other plants showed good scavenging activity. 3. The highest ABTS radical scavenging activity was Sanguisorba hakusanensis Makino flower extract at 13.5 ㎍/㎖, followed by roots at 16.8 ㎍/㎖ and leaves at 22.6 ㎍/㎖;. Although such a large ABTS radical scavenging activity could not be confirmed in other plants, the flower extract was overall better than the leaf and root extracts of other parts. 4. The highest total polyphenol content was found in the leaves of Magnolia sieboldii K. Koch, at 161.4mg GAE/g, followed by the highest content in the roots of Magnolia sieboldii K. Koch and the roots of Agastache rugosa (Fisch. & Mey.) Kuntze, at 130.0mg GAE/g. Also, the lowest was found to be Sanguisorba hakusanensis Makino. 5. The total flavonoid content was 186.2mg CAE/g in the roots of Agastache rugosa (Fisch. & Mey.) Kuntze, 166.9mg CAE/g in the flowers, and the lowest was 116.1mg CAE/g in the leaves. As shown in the results above, Sanguisorba hakusanensis Makino has excellent antioxidant efficacy and has a high polyphenol content, so it is expected to be fully utilized in the cosmetics industry in the future.

Differential Effects of Recovery Efforts on Products Attitudes (제품태도에 대한 회복노력의 차별적 효과)

  • Previous research has presupposed that the evaluation of consumer who received any recovery after experiencing product failure should be better than the evaluation of consumer who did not receive any recovery. The major purposes of this article are to examine impacts of product defect failures rather than service failures, and to explore effects of recovery on postrecovery product attitudes. First, this article deals with the occurrence of severe and unsevere failure and corresponding service recovery toward tangible products rather than intangible services. Contrary to intangible services, purchase and usage are separable for tangible products. This difference makes it clear that executing an recovery strategy toward tangible products is not plausible right after consumers find out product failures. The consumers may think about backgrounds and causes for the unpleasant events during the time gap between product failure and recovery. The deliberation may dilutes positive effects of recovery efforts. The recovery strategies which are provided to consumers experiencing product failures can be classified into three types. A recovery strategy can be implemented to provide consumers with a new product replacing the old defective product, a complimentary product for free, a discount at the time of the failure incident, or a coupon that can be used on the next visit. This strategy is defined as "a rewarding effort." Meanwhile a product failure may arise in exchange for its benefit. Then the product provider can suggest a detail explanation that the defect is hard to escape since it relates highly to the specific advantage to the product. The strategy may be called as "a strengthening effort." Another possible strategy is to recover negative attitude toward own brand by giving prominence to the disadvantages of a competing brand rather than the advantages of its own brand. The strategy is reflected as "a weakening effort." This paper emphasizes that, in order to confirm its effectiveness, a recovery strategy should be compared to being nothing done in response to the product failure. So the three types of recovery efforts is discussed in comparison to the situation involving no recovery effort. The strengthening strategy is to claim high relatedness of the product failure with another advantage, and expects the two-sidedness to ease consumers' complaints. The weakening strategy is to emphasize non-aversiveness of product failure, even if consumers choose another competitive brand. The two strategies can be effective in restoring to the original state, by providing plausible motives to accept the condition of product failure or by informing consumers of non-responsibility in the failure case. However the two may be less effective strategies than the rewarding strategy, since it tries to take care of the rehabilitation needs of consumers. Especially, the relative effect between the strengthening effort and the weakening effort may differ in terms of the severity of the product failure. A consumer who realizes a highly severe failure is likely to attach importance to the property which caused the failure. This implies that the strengthening effort would be less effective under the condition of high product severity. Meanwhile, the failing property is not diagnostic information in the condition of low failure severity. Consumers would not pay attention to non-diagnostic information, and with which they are not likely to change their attitudes. This implies that the strengthening effort would be more effective under the condition of low product severity. A 2 (product failure severity: high or low) X 4 (recovery strategies: rewarding, strengthening, weakening, or doing nothing) between-subjects design was employed. The particular levels of product failure severity and the types of recovery strategies were determined after a series of expert interviews. The dependent variable was product attitude after the recovery effort was provided. Subjects were 284 consumers who had an experience of cosmetics. Subjects were first given a product failure scenario and were asked to rate the comprehensibility of the failure scenario, the probability of raising complaints against the failure, and the subjective severity of the failure. After a recovery scenario was presented, its comprehensibility and overall evaluation were measured. The subjects assigned to the condition of no recovery effort were exposed to a short news article on the cosmetic industry. Next, subjects answered filler questions: 42 items of the need for cognitive closure and 16 items of need-to-evaluate. In the succeeding page a subject's product attitude was measured on an five-item, six-point scale, and a subject's repurchase intention on an three-item, six-point scale. After demographic variables of age and sex were asked, ten items of the subject's objective knowledge was checked. The results showed that the subjects formed more favorable evaluations after receiving rewarding efforts than after receiving either strengthening or weakening efforts. This is consistent with Hoffman, Kelley, and Rotalsky (1995) in that a tangible service recovery could be more effective that intangible efforts. Strengthening and weakening efforts also were effective compared to no recovery effort. So we found that generally any recovery increased products attitudes. The results hint us that a recovery strategy such as strengthening or weakening efforts, although it does not contain a specific reward, may have an effect on consumers experiencing severe unsatisfaction and strong complaint. Meanwhile, strengthening and weakening efforts were not expected to increase product attitudes under the condition of low severity of product failure. We can conclude that only a physical recovery effort may be recognized favorably as a firm's willingness to recover its fault by consumers experiencing low involvements. Results of the present experiment are explained in terms of the attribution theory. This article has a limitation that it utilized fictitious scenarios. Future research deserves to test a realistic effect of recovery for actual consumers. Recovery involves a direct, firsthand experience of ex-users. Recovery does not apply to non-users. The experience of receiving recovery efforts can be relatively more salient and accessible for the ex-users than for non-users. A recovery effort might be more likely to improve product attitude for the ex-users than for non-users. Also the present experiment did not include consumers who did not have an experience of the products and who did not perceive the occurrence of product failure. For the non-users and the ignorant consumers, the recovery efforts might lead to decreased product attitude and purchase intention. This is because the recovery trials may give an opportunity for them to notice the product failure.
