One Stage Correction of the Pectus Excavatum with Marfan Syndrome (Marfan증후군의 수술 교정 1례)

  • Pectus excavatum occasionally occurs in patients who have underlying cardiac disease, especially Marfan syndrome. This report describes a patient with pectus excavatum who had ascending aortic aneurysm with aortic regurgitation and anterior leaflet prolapse of mitral valve. This patient underwent replacement of aortic valve and ascending aorta with 25 mm SJM valved conduit graft[Bentall operation with Cabrol shunt , and mitral valve replacement with SJM 31 mm, the pectus excavatum was corrected at the time of completion of the intracardiac operation with the modified sternal turnover. This procedure offered excellent operative exposure for the inracardiac operation with prevention of low cardiac output after operation due to depressed sternum and maintained chest wall stability resulting good cosmetic chest wall appearance. This patient recovered and discharged in good postoperative result with minimal temporary peroneal nerve palsy in his left leg.

Treatment of Bell's Palsy by Stellate Ganglion Block (안면신경마비에 대한 성상교감신경절 차단요법)

  • Bell's palsy is not a serious disease and recovery is spontaneous in more half of the patients, but this recovery is long delayed in many instances and the cosmetic effect of an altered facial appearance is psychologically distressing. The etiology is unknown, but Kettle's ischemic hypothesis has been widely accepted. The aim of treatment is to reduce edema and improve circulation to the facial nerve. Stellate ganglion block (SGB) resulted in abolishing cerebral vascular spasm and in increasing cerebral blood flow. Thus, stellate ganglion block is effective in treatment of Bell's palsy. From 1978 to Oct. 1988, we have treated 222 patients (20 patients were recurred cases) with facial palsy by SGB and analysed the effect of SGB. We noticed that SGB was very effective in early repetitive treatment (90% recovery). This study is reported and the literature is reviewed.

A case of intramuscular xanthelasma palpebrarum found during blepharoplasty

  • Xanthelasma palpebrarum is the most common cutaneous xanthoma found on the medial side of the eyelid. The typical lesion is usually a flat and yellowish plaque on the skin. However, we report on a unique case of intramuscular xanthoma found during blepharoplasty for the correction of ptosis. A 53-year-old male patient visited our department with a complaint of a ptotic eyelid. He was concerned about the cosmetic appearance and the uncomfortable feeling while opening his eyes, and wanted these problems to be solved. A yellowish plaque of about $0.3{\times}0.3cm$ in size was found in the orbicularis oculi muscle during the surgery. The lesion was excised and xanthelasma was confirmed with biopsy. We have found this specific case of xanthelasma palpebrarum in the only muscle. Therefore, a careful approach to clinical and histologic examination and imaging is required for patients with these lesions.

Correction of pixie ear: infraauricular skin redraping method

  • Patients with pixie ear have an attached, tapering, and low-set earlobe. Traditional methods usually describe excision of the caudal portion of the lobule and reattachment in a more superior position. The present report suggests a simplified skin redraping method for correction of pixie ear. The procedure provides easy method to design and perform, which only requires elevation and trimming of the skin. Other ancillary procedures, such as flap design, anchoring, plication, and subdermal fixation, are not required. This method produces satisfactory results. Postoperative scar is invisible because the incision is on the retroauricular region, and the corrected earlobe has a more natural appearance than the repositioned earlobe. Moreover, skin redraping avoids tension, which contributes to minimization of the postoperative scar.

  • Appearance is a means of competitiveness for modern people, and one of the factors that harms the desires of modern people is white hair. For the modern man who needs to express beautiful appearance to others by covering white moth, we performed hair analysis after white moth hair dye treatment. The maximum modulus and tangential modulus according to the maximum load, maximum tensile strength, maximum elongation, breaking load, fracture strength, elongation at break, and evaluation interval between 1N-3N experimental group and control group of 1N-Respectively. Maximum load, maximum tensile strength, breaking load and breaking strength tended to be larger than those of the control group, while the maximum elongation and breaking elongation were the highest in the control group and the elongation decreased with the experimental group treatment. The maximum modulus and tangential modulus of the 1N-3N test group were higher than those of the control group at 0~0.15 and 0.15~2.5, respectively. Based on the study on the cosmetic changes of the hair before and after the treatment, it can be used as a basic data to select the correct oxidative hair dye product, the proper amount of application and the time to leave.

Experience of Reconstruction of an Extensive Chest Wall Defect with Extended External Oblique Musculocutaneous Flap (거대 흉벽 결손에 대한 확장 외복사근 근피판을 이용한 흉벽재건 치험례)

  • Purpose: The resection of locally advanced or recurred breast cancers frequently result in large chest wall defects and it leads to a great challenges to cover. Generally simple skin grafts are not a practical option for patients because of their poor cosmetic appearance and prognosis. The latissimus dorsi and rectus abdominis musculocutaneous flap have traditionally been recommended for closure of these large defects. Though the cosmetic result of reconstruction using these flaps is often excellent, but has significant drawbacks. Therefore, we thought that chest wall reconstruction using the external oblique musculocutaneous flap can be an alternative method for extensive chest wall defect related to large, locally advanced breast carcinoma. Methods & Results: We present a case of a 50-year-old Korean female, refered to our department with a left breast tumor for 10 months. CT demonstrate a large tumor on the left anterior chest wall and multiple nodules of varying size in the cervical areas and liver. FDG-PET showed areas of hot uptake throughout the left chest wall, mediastinum and liver. Biopsy was consistent with invasive ductal carcinoma (Grade III). The initial tumor was considered inoperable, so a series of chemotherapy was initiated. Though the size of the breast mass was slightly decreased, the patient continued to suffer from purulent discharge, unpleasant odor and contact bleeding of the mass, the salvage mastectomy was performed. Conclusion: We could reconstruct $23{\times}16\;cm$ sized large chest wall defect, resulting from the resection of a locally advanced breast carcinoma, using an external oblique musculocutaneous flap successfully. Immediate postoperatively checked flap was healthy. Overall result was good without any significant complications and discharged 3 weeks after operation.

Formation of Nano-emulsions with Resorcinol bis-ethylhexanoate upon Type of Emulsifiers (레조르시놀 비스-에틸헥사노에이트를 함유한 나노에멀젼의 유화제 종류에 따른 형성)

  • RS White (resorcinol bis-ethylhexanoate) is used in cosmetics as a skin whitening agent. In this study, we studied the possibility of nano-emulsion formation containing whitening agent, RS White, with different types of emulsifying agents. With Tween 80, 60, HCO 60 and 40 as a hydrophilic surfactants and Span 80 as ahydrophilic surfactant, nano-emulsions were formed at appropriate concentrations, but they were not formed in the system using the Myrj 52, Montanov L, and Tegocare 450 with Span 80. The diameter of nano-emulsion sphere was smaller than 100 nm. The emulsion showed a translucent appearance and maintained stability in stability evaluation with time. In vitro skin permeation experiments showed that amounts of skin permeated nano-emulsion for 24 h were $70.84{\mu}g/cm^2$ and those of O/W emulsion were $28.97{\mu}g/cm^2$. In conclusion, a stable nano-emulsion containing the resorcinol bis-ethylhexanoate is effective for potential efficacy system as an efficient delivery system of the functional materials into skin.

Reconstruction of the Face Using Thoracodorsal Artery Perforator Free Flap after Resection of Arteriovenous Malformation (안면부 동정맥 기형의 수술적 제거 후 흉배동맥 천공지유리피판을 이용한 결손의 재건)

  • Purpose: The treatment of arteriovenous malformation (AVM) of the face remains a difficult challenge in plastic surgery. Incomplete resection resulting in uncontrolled bleeding, postoperative enlargement of the remaining malformation, and a poor functional and cosmetic result could be the problems confronted by the surgeons. Methods: A 37 year-old male with large arteriovenous malformation in face treated with preoperative superselective transarterial embolization and free flap transfer. The size of the defect was $13{\times}9cm$. Sclerotheraphy without resection were performed several times but the results were unsatisfactory. Resection was performed the next day of embolization. We were able to repair with the thoracodorsal artery perforator free flap. And facial muscle reconstruction performed by simultaneous muscle and nerve transfer. Results: During the follow-up period 8 months the patient regained an acceptable cosmetic appearance. And he has shown no reexpansion of the malformation. Conclusion: The thoracodorsal artery perforator free flap could be a good choice for the reconstruction for massive defects of the face. A huge arteriovenous malformation could be safely removed and successfully reconstructed by the complete embolization, wide excision and coverage with a well vascularized tissue.

Partial Resection of Maxillary Ossifying Fibroma in a Thoroughbred Stallion

  • A 5-year-old Thoroughbred race horse was presented to Busan Korea Racing Authority equine hospital with a 3-year history of a slow-growing left rostral maxillary mass. The location and progressive growth of the mass eventually resulted in poor food prehension, quidding and mouth bit placement. The mass was solitary and hard, and covered by normal smooth oral mucosa. Radiographic examination of the maxillae showed a flocculated and mixed radiolucent lesion protruding outward and displacing the 202 and 203 teeth caudally. The 202 tooth was in normal size and the 203 tooth was hypoplastic on radiography. Under general anesthesia, a partial surgical resection of the mass was performed to minimize functional loss and facilitate prompt return to track. After surgery, there was improvement in food intake, mouth bit placement, and cosmetic appearance. Histopathological examination determined the resected maxillay mass to be an ossifying fibroma. However, there was continued growth of remnant mass in the maxilla. Equine ossifying fibroma is a rare condition and primarily affects the rostral mandible, and less commonly, the maxillae. In this case, the lesion was slow-growing, and caused cosmetic and functional impairments, including poor food intake and reduced trainability. Surgical resection was performed, but the effect of treatment was limited due to advanced size / stage of the tumor. Early dental care is suggested for horse owners to prevent belated identification and improve successful treatment of oral disorders like ossifying fibroma.

  • Skin irritations accompany the series of complex, physical and chemical changes. Alternative methods which reflect the biological phenomenon more sensitively are necessary for the purpose of reducing the animal tests and improving the accuracy and reliability. Chorioallantoic membrane (CAM) vascular assay is a in vitro method which utilizes the chorioallantoic membrane of hen's fertilized egg of about 10 days old. Test substances are placed directly onto the surface of the CAM and 1 hour later the CAM vasculature is subjectively evaluated to determine with a chemically related-injurious reponse including the appearance of haemorrage, congestion, coagulation, and so on. In this research, using the various surfactants, the correlations of CAMVA with in vivo models (intradermal safety test and human primary irritation test) were investigated. And CAMVA closely correlated to intradermal safety test (r=0.89) and human primary irritation test (r=0.90). From the result, it seems that CAMVA can also be used as a method for predicting the skin irritaions.
