Hypercoagulability is frequently observed in patients with severe coronavirus disease-2019 (COVID-19). Platelets are a favorable target for effectively treating hypercoagulability in COVID-19 patients as platelet hyperactivity has also been observed. It is difficult to develop a treatment for COVID-19 that will be effective against all variants and the use of antivirals may not be fully effective against COVID-19 as activated platelets have been detected in patients with COVID-19. Therefore, patients with less severe side effects often turn toward natural remedies. Numerous phytochemicals are being investigated for their potential to treat a variety of illnesses, including cancer and bacterial and viral infections. Natural products have been used to alleviate COVID-19 symptoms. Panax ginseng has potential for managing cardiovascular diseases and could be a treatment for COVID-19 by targeting the coagulation cascade and platelet activation. Using molecular docking, we analyzed the interactions of bioactive chemicals in P. ginseng with important proteins and receptors involved in platelet activation. Furthermore, the SwissADME online tool was used to calculate the pharmacokinetics and drug-likeness properties of the lead compounds of P. ginseng. Dianthramine, deoxyharrtingtonine, and suchilactone were determined to have favorable pharmacokinetic profiles.
Background: The coronavirus disease (COVID-19) pandemic increased awareness regarding the importance of hand hygiene in infection prevention. Although social distancing and vaccination are the strongest ways to prevent infection, personal hand hygiene is the most basic and easiest way to maintain public health. However, in addition to hand washing using running water, sanitizing tissues, and disinfection products are convenient for hand hygiene, especially outdoors. Therefore, it is necessary to improve the appropriateness of individual hand hygiene methods. In this study, we investigated the degree of hand hygiene offered by various hygiene products and hand drying methods for maintaining hand hygiene. Methods: An LED UV light kit was used for fluorescent observation of hand contamination. Bacteria from the hands were cultured to compare the degree of hand hygiene offered by various hygiene products. Bacteria were cultured in a hand-shaped medium dish to identify areas vulnerable to hand hygiene. Moreover, the degree of hand hygiene was observed according to the drying method using bacterial cultures. Results: We confirmed that hand washing under running water with antibacterial soap, sanitizing with alcohol gel disinfectant, and wiping with antibacterial wet wipes was effective for hand hygiene compared to washing under running water alone. However, for all hygiene products, a large number of bacteria were detected on the fingertips. We verified that natural drying, rather than rubbing, is effective in maintaining hand hygiene. Conclusion: These results suggest that hand hygiene products and drying methods are critical in hand hygiene management. Therefore, these results provide a basis for determining whether an individual's hand hygiene management method is appropriate.

Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection leads to various clinical symptoms including anemia. Lipocalin-2 has various biological functions, including defense against bacterial infections through iron sequestration, and it serves as a biomarker for kidney injury. In a human protein array, we observed increased lipocalin-2 expression due to parental SARS-CoV-2 infection in the Calu-3 human lung cancer cell line. The secretion of lipocalin-2 was also elevated in response to parental SARS-CoV-2 infection, and the SARS-CoV-2 Alpha, Beta, and Delta variants similarly induced this phenomenon. In a Calu-3 implanted mouse xenograft model, parental SARSCoV-2 and Delta variant induced lipocalin-2 expression and secretion. Additionally, the iron concentration increased in the Calu-3 tumor tissues and decreased in the serum due to infection. In conclusion, SARS-CoV-2 infection induces the production and secretion of lipocalin-2, potentially resulting in a decrease in iron concentration in serum. Because the concentration of iron ions in the blood is associated with anemia, this phenomenon could contribute to developing anemia in COVID-19 patients.
Background: Bacterial pneumonia occurring after respiratory viral infection is common. However, the predominant bacterial species causing pneumonia secondary to respiratory viral infections other than influenza remain unknown. The purpose of this study was to know whether the pathogens causing post-viral bacterial pneumonia vary according to the type of respiratory virus. Methods: Study subjects were 5,298 patients, who underwent multiplex real-time polymerase chain reaction for simultaneous detection of respiratory viruses, among who visited the emergency department or outpatient clinic with respiratory symptoms at Ulsan University Hospital between April 2013 and March 2016. The patients' medical records were retrospectively reviewed. Results: A total of 251 clinically significant bacteria were identified in 233 patients with post-viral bacterial pneumonia. Mycoplasma pneumoniae was the most frequent bacterium in patients aged <16 years, regardless of the preceding virus type (p=0.630). In patients aged ${\geq}16years$, the isolated bacteria varied according to the preceding virus type. The major results were as follows (p<0.001): pneumonia in patients with influenza virus (type A/B), rhinovirus, and human metapneumovirus infections was caused by similar bacteria, and the findings indicated that Staphylococcus aureus pneumonia was very common in these patients. In contrast, coronavirus, parainfluenza virus, and respiratory syncytial virus infections were associated with pneumonia caused by gram-negative bacteria. Conclusion: The pathogens causing post-viral bacterial pneumonia vary according to the type of preceding respiratory virus. This information could help in selecting empirical antibiotics in patients with post-viral pneumonia.
목 적 : 급성 바이러스성 하기도 감염의 원인으로 알려진 바이러스의 임상 양상 및 역학 을 연구하고자 하였으며, 특히 새롭게 밝혀진 hRV, HCoV-229E/NL63, HCoV-OC43, hMPV를 중심으로 하였다. 방 법 : 2005년 12월부터 2006년 11월까지 순천향대학교 천안병원 소아과에 하기도 감염으로 입원한 환아들 중 863명을 대상으로 환아들의 비인두 흡인물에서 multiplex RT-PCR 법을 이용하여 총 12개의 바이러스를 검출하였다. 결 과 : 총 12개의 호흡기 바이러스에 대하여 검사를 하였을 때 863명 중 474명(54.9%)에서 바이러스 양성이 나타났다. hMPV이 6.8%, HCoV-229E/NL63 1.4%, HCoV-OC43 2.1%, hRV 9%로 검출되었고, 혼합감염은 22.8%였다. 급성 바이러스성 하기도 감염은 1세 미만에서 가장 흔하게 나타났다. 임상진단은 폐렴이 59.5%로 가장 많았고, 세기관지염이 24.7%, 기관기관지염이 11.4%, 크룹이 4% 순이었다. 세기관지염의 가장 흔한 원인 바이러스는 RSV B이고, 반면에 hMPV, hRV, HCoV-229E/NL63은 폐렴의 주요 원인 바이러스로 나타났다. 바이러스별 유행 양상은 hMPV는 3월부터 5월까지 유행하였고, HCoV-OC43은 11월부터 2월까지 유행하였다. 반면에 HCoV-OC43, HCoV-229E/NL63과 hRV는 연중 고른 분포를 보였다. 결 론 : 본 연구는 비록 연구 기간이 1년으로 짧았지만, 다른 나라의 경우와 비교해 hMPV가 겨울엔 검출되지 않고 3월에서 4월까지 주로 검출된다는 점과 HCoV가 우리나라에서는 별로 검출되지 않아 하기도 감염에서는 그 비중이 매우 미미하다는 점을 알 수 있었다.
목적: 코로나바이러스감염증-19 (coronavirus disease 2019, 코로나19) 대유행이 시작된 이후 새로운 변이 바이러스들이 지속적 출현으로 반복되는 코로나19 유행이 장기간 지속되고 있다. 본 연구는 국내 코로나19 유행 제3기와 제4기 중반까지 소아 청소년 환자의 임상적, 역학적 특성을 파악하고자 하였다. 방법: 코로나19 확진으로 입원한 18세 이하 환자의 의무기록을 후향적으로 분석하였다. 연구기간은 제3기 유행(2020년 11월 13일-2021년 7월 6일)과 제4기 유행(2021년 7월 7일-10월 31일)로 구분하였다. 결과: 총 93명이 분석에 포함되었다(제3기 33명, 제4기 60명). 제3기에 비해 제4기 환자군의 중간 연령이 유의하게 높았다(6.7세 vs. 2.8세, P=0.014). 가정 내 전파가 전체 환자의 60.2%에서 보고되었고 두 시기에서 유사하였다(69.7% vs. 55.0%, P=0.190). 전체 환자 중 87.1% (81명)가 유증상 확인자였다. 이중 12.3%는 호흡기 증상이 없었다. 후각/미각 소실은 제4기 유행 환자군에 더 많이 관찰되었다(10.7% vs. 34.0%, P=0.032). 대부분의 호흡기 감염은 상기도 감염(94.4%, 67/71)이었고 4명 환자에서 폐렴이 동반되었다. 두 유행기 동안 진단 시 바이러스 수치의 유의한 차이는 없었다. 결론: 단일기관 후향적 연구의 제한점이 있으나 국내 코로나19 유행 제3기와 제4기 중반 동안 소아청소년 환자는 대체로 경증이었고 두 시기에서 임상적 특성 차이가 관찰되었다.
코로나바이러스감염증-19는 전 세계적으로 유행하고 있으며, 2021년 7월 15일 기준으로 1.88억명 이상의 확진자와 406만명 이상의 사망자가 발생하였다. 소아청소년에서는 성인에 비해 비교적 중증 감염 발생이 낮으나, 일부에서는 SARS-CoV-2 감염 약 2-6주후소아다기관염증증후군(multisystem inflammatory syndrome in children, MIS-C)라는 합병증이 발생할 수 있다. MIS-C는 어린영아부터 청소년까지 다양한 연령에서 발생할 수 있으며, 발열을 포함하여 다양한 장기와 관련된 증상을 보일 수 있다. 소화기 및 신경계 증상이 흔하며, 많은 경우 가와사키병과 유사한 피부점막증상 등이 나타난다. 특히 심장관련 증상으로 좌심실 기능저하, 심근염 등이 나타날 수 있으며, 관상동맥확장 및 관상동맥류가 나타날 수 있다. 경우에 따라 저혈압, 쇼크를 동반하며, 중환자실 치료 및 기계환기요법을 요하나, 적절한 치료 후 대체로 회복을 보이는 것으로 보고된다. 이와 같이 MIS-C는 소아청소년에서 SARS-CoV-2 이후 드물게 발생하는 중요한 합병증으로, 임상증상을 잘 인지하고 조기에 적절한 치료를 하는 것이 중요하다. 이에 본 종설에서는 MIS-C의 역학 및 임상 증상, 추정되는 병태생리, 진단적 접근 및 치료에 대해 다루고자 한다.
Objective: This study aimed to examine the influence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection on pregnancy in cytokeratin-18 (K18)-hACE2 transgenic mice. Methods: To determine the expression of hACE2 mRNA in the female reproductive tract of K18-hACE2 mice, real-time polymerase chain reaction (RT-PCR) was performed using the ovary, oviduct, uterus, umbilical cord, and placenta. SARS-CoV-2 was inoculated intranasally (30 μL/mouse, 1×104 TCID50/mL) to plug-checked K18-hACE2 homozygous female mice at the pre-and post-implantation stages at 2.5 days post-coitum (dpc) and 15.5 dpc, respectively. The number of implantation sites was checked at 7.5 dpc, and the number of normally born pups was investigated at 20.5 dpc. Pregnancy outcomes, including implantation and childbirth, were confirmed by comparison with the non-infected group. Tissues of infected mice were collected at 7.5 dpc and 19.5 dpc to confirm the SARS-CoV-2 infection. The infection was identified by performing RT-PCR on the infected tissues and comparing them to the non-infected tissues. Results: hACE2 mRNA expression was confirmed in the female reproductive tract of the K18-hACE2 mice. Compared to the non-infected group, no significant difference in the number of implantation sites or normally born pups was found in the infected group. SARS-CoV-2 infection was detected in the lungs but not in the female reproductive system of infected K18-hACE2 mice. Conclusion: In K18-hACE2 mice, intranasal infection with SARS-CoV-2 did not induce implantation failure, preterm labor, or miscarriage. Although the viral infection was not detected in the uterus, placenta, or fetus, the infection of the lungs could induce problems in the reproductive system. However, lung infections were not related to pregnancy outcomes.

코로나바이러스감염증-19 (COVID-19)의 임상 양상은 무증상부터 급성 호흡곤란 증후군에 이르기까지 다양하다. 점액 다당류증(mucopolysaccharidosis) 2형은글라이코스아미노글라이칸(glycosaminoglycan)의 일종인 헤파란 황산염(heparan sulfate)과 더마탄 황산염(dermatan sulfate)의 분해를 촉매하는 효소 결핍에 의해 상기 물질이 리소좀(lysosome)에 축적되는 질환으로 전신 침범, 특히 호흡기침범을 특징으로 한다. 따라서 박테리아나 바이러스에 의한 호흡기 감염은 예후에 치명적일 수 있다. 현재 점액 다당류증 환자에서 제 2형 중증급성호흡기증후군 코로나 바이러스(SARS-CoV-2) 감염 후의 임상 양상에 대한 보고는 매우 드물고, 이에 점액 다당류증 2형으로 효소대체요법을 받고 있던 환자에서 상기 바이러스 감염 후의 임상 양상에 대해 보고하고 관련 문헌에 대해 고찰하고자 한다. 16세 남아로 가족간 전파로 코로나바이러스감염증이 발생하였다. 콧물, 기침, 가래 등 호흡기 증상이 관찰되었다. 발열이나 산소요구도 증가는 없었으며 심박수, 호흡수, 산소 포화도는 정상 범위였고 혈액검사결과에서 백혈구 감소증이 관찰되었다. 흉부 방사선 사진에서 폐렴 소견은 보이지 않았다. 보존적 치료와 격리만으로 증상이 호전되었다. 경미한 임상 양상의 원인으로 전구 물질의 축적으로 인해 바이러스에게 불리한 숙주의 세포 환경, 바이러스와의 상호작용에 관여하는 단백질을 암호화하는 유전자 발현의 특정방향으로의 변화가 제시되고 있다. 또한 점액 다당류증 환자에서 증가된 혈청 헤파란 황산염이 SARS-CoV-2 스파이크 단백질과 숙주 세포의 상호작용에 필수적인 세포 표면의 헤파란 황산염과 경쟁하여 SARS-CoV-2의 세포 내 침투로부터 보호한다는 가설도 있다. 향후 더 많은 사례를 통해 점액 다당류증 등의 리소좀 축적질환에서 코로나바이러스감염증의 발현 양상에 대한 연구가 필요하다.
바이러스성 전염병은 전 세계 공중 보건에 가장 큰 위협 중 하나로 간주된다. 최근 중증급성호흡기증후군 코로나바이러스-2(SARS-CoV-2)로 인한 COVID-19 대유행은 신종 바이러스 감염의 위협을 극명하게 상기시켜 주었다. 효율적인 백신과 치료제 개발 및 생산은 팬데믹을 퇴치할 수 있는 유일한 대안일 것이며 COVID-19 대유행은 새로운 바이러스성 질병을 통제하고 예방하기 위한 새로운 백신 플랫폼의 필요성을 보여주었다. 기존의 백신 플랫폼인 약독화 생백신, 불활성화 백신은 백신 개발 속도, 제조 등이 광범위한 백신 적용을 위한 긴급 사용에 한계가 있다. 흥미롭게도, COVID-19 예방을 위한 SARS-CoV-2 mRNA-지질나노입자(LNP) 플랫폼은 기존 백신 플랫폼 한계에 대한 효과적인 대안임이 확인되었다. 또한 COVID-19 mRNA 핵산 백신과 나노입자 기반 플랫폼은 SARS-CoV-2 및 변종 SARS-CoV-2 모두에 효과적인 백신임이 확인되었다. 이 논문에서는 mRNA 백신, 디지털 백신 및 나노입자백신 등의 차세대 백신 플랫폼을 중점으로 백신 기술 및 플랫폼의 장단점에 대해 기술하였다.

제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
