Comparison between Radiological and Invasive Diagnostic Modalities in Diagnosis of Breast Cancer

  • Background: Breast cancer is the most common cause of deaths of cancer in women. Nowadays, following completion of imaging methods, mainly fine needle aspiration biopsy (FNAB) and core biopsy methods have been used for establishing cytopathological diagnosis although discussions regarding superiority continue. Materials and Methods: Those with a complaint of "mass in breast" along with those diagnosed to have a mass as a result of routine physical examination among all patients applying to our clinic between 01.01.2009 and 31.12.2011 were retrospectively assessed. Totals of 146 and 64 patients with complete radiological observation who had undergone FNAB and core biopsies, respectively, were evaluated. Postoperative pathological results of patients of both groups receiving surgery were also taken into consideration. All results were compared in terms of false positivity/negativity, sensitivity/specifity, surgery types and distribution of postoperative results with regard to diagnoses along with those of malignant/benign masses with regard to quadrants determined. Results: Diagnostic malignancy power of mammographic BIRADS classification was 87.3%. However, the value was 75% in the core biopsy group. Sensitivity and specifity following comparison of FNAB and postoperative pathology results of those receiving surgery were 85.4% and 92.9% while they were 93.5% and 100% in the core biopsy group. Diagnostic malignancy power, calculated by determining AUC in ROC analysis, of FNAB was 89.1% while that of core biopsy was 96.7%. Conclusions: It was shown that core biopsy is superior to FNAB in terms of sensitivity, specificity and accurate histopathological classification. However; quick, cheap and basic diagnosis by means of FNAB should not be ignored. Sensitivity of FNAB is rather high in experienced hands and furthermore it would be expected to be lower than with core biopsy.

Video-Assisted Thoracic Surgery Core Needle Biopsy for Pulmonary Nodules in Patients with Impaired Lung Function: Is It Feasible and Safe?

  • Background: The number of patients with incidentally identified pulmonary nodules is increasing. This study attempted to confirm the usefulness and safety of video-assisted thoracic surgery (VATS) core needle biopsy of pulmonary nodules. Methods: Data from 18 patients diagnosed with pulmonary nodules who underwent VATS core need biopsy were retrospectively reviewed. Results: Of the 18 patients, 15 had malignancies (primary lung cancer, n=14; metastatic lung cancer, n=1), and 3 had benign nodules. Mortality and pleural metastasis did not occur during the follow-up period. Conclusion: In patients with solitary pulmonary nodules that require tissue confirmation, computed tomography-guided percutaneous cutting needle biopsy or diagnostic pulmonary resection sometimes may not be feasible choices due to the location of the solitary pulmonary nodule or the patient's impaired pulmonary function, VATS core needle biopsy may be performed in these patients as an alternative method.

골 병변에서 세침흡인 세포검사의 유용성 (The Usefulness of Fine Needle Aspiration Cytology of Bone Lesions)

  • To determine the usefulness on fine needle aspiration cytology(FNAC) of bone lesions and the complementary role of FNAC and percutaneous needle biopsy, 75 cases of FNAC taken from bone lesions were analyzed. Correlations with histopathology were possible in 47 cases, including 14 cases of simultaneous core biopsy and 33 cases of subsequent open biopsy due to inadequate aspirates. Among 75 cases, 4 cases were benign tumors and tumor-like lesion, 11 cases were malignant primary bone tumors, 17 cases were metastatic tumors, and 43 cases were nonneoplastic bone lesions. The aspirates were adequate in 35 cases(46.7%), in all of which the discrimination between benignancy and malignancy was possible. The main reason for Inadequate aspirates was due to hypocellularity. In the cases of aspiration and core biopsy simultaneously done, the diagnostic accuracy of aspiration, core biopsy, and both were 57%(8/14), 78.6%(11/14), and 92.9%(13/14), respectively. We conclude that a final diagnosis based on cytology is possible with the adequate aspirates and the clinical and radiological findings. Also we confirm the complementary role between FNAC and core biopsy in bone lesions.

유방 미세석회화 조직검사에서 X선 유도 하 조직검사와 초음파 유도 하 조직검사의 유용성 (Usefulness of X-ray Guided Biopsy and Ultrasound Guided Biopsy in Breast Microcalcification Biopsy)

  • 유방암에 관한 사회적인 관심이 증가되고 있는 가운데 진단의 가장 기본이 되는 검사는 유방 X선 촬영과 유방 초음파 검사이다. 특히 유방 미세석회화는 조직학적 진단을 필요로 하며 유방 미세석회화 조직검사를 시행하고 있다. 이에 미세석회화 진단에 기본이 되는 X선 유도 하 조직검사(needle localized open biopsy)와 초음파 유도 하 조직검사(sono guided core needle biopsy)를 분석하여 평가하고자 한다. 유방 미세석회화를 주소로 내원한 환자에서 확대 촬영을 시행한 241예를 대상으로 X선 유도 하 조직검사(needle localized open biopsy)와 초음파 유도 하 조직검사(sono guided core needle biopsy)의 연령대 별 분포와 병소의 위치를 분석하고 확대촬영 시행 후 이루어진 검사를 분류해서 X선 유도 하 조직검사와 초음파 유도 하 조직검사의 빈도를 분석하고 악성과 양성의 결과를 확인 하였다. 그 결과 X선 유도 하 조직검사(needle localized open biopsy)가 64예(26.6%)로 초음파 유도 하 조직검사(sono guided core needle biopsy) 12예(4.9%)에 비해 5.4배 높은 것으로 나타났다. 초음파 장비의 발달과 입체정위 흡입법(stereotactic vacuum-assisted biopsy)등의 발달로 미세석회화의 조직학적 진단방법이 다양해지고 있지만, 각각 검사의 특성과 한계점을 고려해 볼 때 X선 유도 하 조직검사(needle localized open biopsy)가 유방 미세석회화 병변에서는 정확한 조직학적 진단을 제공한다고 사료된다.

수술 전 중심부바늘생검을 통해 진단된 갑상선에 발생한 신경초종 1예 (A Case of Intra-thyroidal Schwannoma Diagnosed by Preoperative Core Needle Biopsy)

  • Schwannoma is a slow growing benign tumor that can occur anywhere in our body. About 25~45% cases of schwannomas occur in the head and neck, but intra-thyroidal schwannomas are very rare, and mostly are diagnosed by post-thyroidectomy pathologic study. In this article, we present a case of intra-thyroidal schwannoma diagnosed preoperatively with core needle biopsy. The patient underwent enucleation of the thyroid tumor, and the pathology of the tumor was confirmed as schwannoma. Few cases of intra-thyroidal schwannomas have been reported in the literature, but none of them have been diagnosed through core needle biopsy preoperatively. Preoperative diagnosis of intra-thyroidal schwannoma can be helpful when determining appropriate surgical extent and avoid unnecessary thyroidectomy.

Performance Indices of Needle Biopsy Procedures for the Assessment of Screen Detected Abnormalities in Services Accredited by BreastScreen Australia

  • Background: We wished to analyse patterns of use of needle biopsy procedures by BreastScreen Australia (BSA) accredited programs to identify areas for improvement. Design: BSA services provided anonymous data regarding percutaneous needle biopsy of screen detected lesions assessed between 2005-2009. Results: 12 services, from 5 of 7 Australian states and territories provided data for 18212 lesions biopsied. Preoperative diagnosis rates were 96.84% for lesion other than microcalcification (LOTM) and 93.21% for microcalcifications. At surgery 97.9% impalpable lesions were removed at the first procedure. Of 11548 Microcalcification (LOTM) biopsied, 46.9% were malignant. The final diagnosis was reached by conventional core biopsy (CCB) in 72.46%, FNAB in 21.33%, VACB in 1.69% and open biopsy in 4.52% of lesions. FNA is being limited to LOTM with benign imaging After FNAB, core biopsy was required for 38% of LOTM. In LOTM the mean false positive rate (FPR) was 0.36% for FNAB, 0.06% for NCB and 0% for VACB. Diagnostic accuracy was 72.75% for FNAB and 92.1% for core biopsies combined. Of 6441 microcalcifications biopsied 2305 (35.8%) were malignant. Microcalcifications are being assessed primarily by NCB but 6.57% underwent FNAB, 45.6% of which required NCB. False positive diagnoses were rare. FNR was 5% for NCB and 1.53% for VACB. Diagnostic accuracy was 73.52% for FNAB, 86.29% for NCB and 88.63% for VACB. Only 8 of 12 services had access to VACB facilities. Conclusions: BSA services are selecting lesions effectively for biopsy and are achieving high preoperative diagnosis rates. Gaps in the present accreditation standards require further consideration.

  • Fine needle aspiration cytology(FNAC) holds a main role in assessing thyroid nodules. But nonnegligible rate of thyroid cytology is reported as uncertain, indeterminate or inadequate for diagnosis. Recently, the microhistologic evaluation by core needle biopsy(CNB) under ultrasound sonographical guidance has been reported to show high accuracy for the diagnose of thyroid nodules. Stereotactic Vacuum-Assisted Core Biopsy Results for Non-Palpable Breast Lesions

  • Background: The increase in breast cancer awareness and widespread use of mammographic screening has led to an increased detection of (non-palpable) breast cancers that cannot be discovered through physical examination. One of the methods used in the diagnosis of these cancers is vacuum-assisted core biopsy, which prevents a considerable number of patients from undergoing surgical procedures. The aim of this study was to present the results of stereotactic vacuum-assisted core biopsy for suspicious breast lesions. Materials and Methods: Files were retrospectively scanned and data on demographic, radiological and pathological findings were recorded for patients who underwent stereotactic vacuum-assisted core biopsy due to suspicious mammographic findings at the Interventional Radiology Centre of the Florence Nightingale Hospital between January 2010, and April 2013. Statistical analysis was carried out using Pearson's Chi-square, continuity correction, and Fisher's exact tests. Results: The mean age of the patients was 47 years (range: 36-70). Biopsies were performed due to BIRADS 3 lesions in 8 patients, BIRADS 4 lesions in 77 patients, and BIRADS 5 lesions in 3 patients. Mammography elucidated clusters of microcalcifications in 73 patients (83%) and focal lesions (asymmetrical density, distortion) in 15 patients (17%). In terms of complications, 1 patient had a hematoma, and 2 patients had ecchymoses (3/88; 3.3%). The histopathologic results revealed benign lesions in 63 patients (71.6%) and malignant lesions in 25 patients (28.4%). The mean duration of the procedure was 37 minutes (range: 18-55). Although all of the BIRADS 3 lesions were benign, 22 (28.6%) of the BIRADS 4 lesions and all of the BIRADS 5 lesions were malignant. Among the malignant cases, 80% were in situ, and 20% were invasive carcinomas. These patients underwent surgery. Conclusions: In cases where non-palpable breast lesions are considered to be suspicious in mammography scans, the vacuum-assisted core biopsy method provides an accurate histopathologic diagnosis thus preventing a significant number of patients undergoing unnecessary surgical procedures.

3차원 입체정위 유방생검술의 정확도 및 정밀도 평가 (Evaluation of the Accuracy and Precision Three-Dimensional Stereotactic Breast Biopsy)

  • 본 연구는 3차원 입체정위 유방생검술의 정확도를 알아보고, 심부침생검을 이용하여 Stereotactic biopsy과 Sonoguided biopsy의 정확도와 정밀도를 평가하고자 한다. Stereotactic QC phantom을 이용하여 실제 5개의 target 위치로 3D sterotactic machine의 정확도를 측정하고, CT장비로 Scan하여 실측을 구해 X, Y, Z의 길이의 정밀도를 비교한다. 유방조직과 유사하게 제작한 Agar power phantom을 이용하여 5개의 각기 다른 needle tip Target을 통해 3D sterotactic machine과 2D ultrasound machine의 정확도를 비교하고, Z축을 장비별로 실측하여 정밀도와 신뢰도를 비교하며, 6개의 모조병소 Target을 심어놓은 Medical application phantom으로 표적하여 육안검사와 Specimen검사를 통해 정확도를 확인하였다. Stereotactic QC phantom으로 측정한 3D sterotactic machine의 정확도는 100%였으며, CT와 비교한 정밀도는 X, Y, Z축이 모두 p>0.05로 나타났다. Agar powder phantom으로 측정한 두 장비의 정확도는 100%의 정확도를 보였으며, CT와 두 장비 사이에는 p > 0.05로 차이가 없었다. 그러나 2명의 방사선사가 측정한 신뢰도분석에서 3D sterotactic machine은 ICC가 0.954였고, 2D ultrasound machine은 0.785로 2D ultrasound machine이 술자에 따라 차이가 있었다. Medical application phantom의 실험에서 3D sterotactic machine은 Sliced boneless ham을, 2D ultrasound machine은 small chalk powder group를 찾을 수 없었다. Phantom을 이용한 3차원 입체정위 유방생검술의 정확성은 우수하게 나타났고, 인체조직과 비슷한 Agar powder phantom과 유방 조직과 비슷한 Medical application phantom을 이용하여 Stereotactic biopsy과 Sonoguided biopsy의 정확도와 정밀도 모두 우수하게 나타났다. 또한 Medical application phantom의 심부침생검의 정확성 평가에서 각 검사에 따라 생검 표본이 병소의 형태에 따라 상이하게 채취되었고, 3차원 입체정위 유방생검술의 재현성이 유방 초음파검사보다 술자의 영향없이 우수하였다.

Initial Experience of Transperineal Biopsy After Multiparametric Magnetic Resonance Imaging in Korea; Comparison With Transrectal Biopsy

  • Purpose: The aim of this study is to confirm the detection rate of transperineal biopsy after multiparametric magnetic resonance imaging (mpMRI) and compared it to that of transrectal biopsy. We also examined the role of mpMRI and the rate of complications for each method. Materials and Methods: In a retrospective study, we analyzed 147 patients who underwent mpMRI before prostate biopsy because of elevated serum prostate-specific antigen and/or abnormal digital rectal examination findings at Korea University Hospital, Seoul, Korea from March 2017 to April 2018. Regions on the mpMRI that were suggestive of prostate cancer were categorized according to the Prostate Imaging-Reporting and Data System (PI-RADS v2). For transperineal biopsy, a 20-core saturation biopsy was performed by MRI-TRUS cognitive or fusion techniques and a 12-core biopsy was performed in transrectal biopsy. Results: Sixty-three and 84 patients were enrolled in transperineal group and transrectal group, respectively. The overall detection rate of prostate cancer in transperineal group was 27% higher than that in transrectal group. Classification according to PI-RADS score revealed a significant increase in detection rate in all patients, as the PI-RADS score increased. Frequency of complications using the Clavien-Dindo classifications revealed no significant differences in the total complications rate, but two patients in transrectal group received intensive care unit care due to urosepsis. Conclusions: Our results confirmed that transperineal biopsy is superior to transrectal biopsy for the detection of prostate cancer. From the complication point of view, this study confirmed that there were fewer severe complications in transperineal biopsy.
