The Relationship between Zinc Concentration of Maternal, Umbilical Cord, and Placenta and Birth Weight (임신말 모체ㆍ제대혈 및 태반의 아연 함량과 신생아 체중)

  • The purpose of this study was to assess the maternal zinc status during pregnancy and to evaluate the relationship between the zinc concentration of maternal, umblical cord blood and placental tissue and pregnancy outcomes. Venous blood samples were drawn from 53 pregnant women just before delivery and the cord blood of their newborn babies was collected immediately after birth. In addition, placental tissues were extracted. We investigated the difference in the concentration of zinc in maternal, umbilical cord blood and placental tissue in two gestational age groups (preform delivery group [PT] and normal term delivery group [NT]) at 34.7 wk and 39.0 wk of mean gestational age, respectively). We also assessed correlations of the zinc concentration of maternal, umbilical cord blood and placental tissue. Lastly, we studied the correlations between the birth weights and the zinc concentration in the maternal, umbilical cord blood and placental tissue. The concentrations of maternal serum zinc and of umbilical cord serum zinc were significantly higher in the PT group (76.9$\pm$37.4 $\mu/dl$, 101.3$\pm$41.4 $\mu/dl$) than in those of the NT group (57.8$\pm$22.4 $\mu/dl$, 80.7$\pm$27.5 $\mu/dl$), respectively (p<0.05). The zinc concentration of the umbilical cord blood was significantly higher than that of the maternal blood in both groups (p<0.05). There was no significant correlation between the gestational age and the serum zinc concentration in the cord or the maternal serum. Our results showed that there was a negative relationship between the birth weight (r=-0.286) and the maternal serum zinc concentration. Despite there not being a significant difference, there was tendency for the highest concentrations of maternal serum zinc to be associated with the lowest birth weights. These findings support a possible relationship between the maternal zinc status and the pregnancy outcome, and suggest that zinc may play a role in the many biological processes involved in the successful outcome of a pregnancy.

Malignant Fibrous Histiocytoma of the Spermatic Cord: A Case Report (정삭에 발생한 악성섬유조직구종 -1예 보고-)

  • Malignant fibrous histiocytoma (MFH) of the spermatic cord represents an uncommon location for the most common soft tissue tumor in adults. MFH of the spermatic cord is extremely rare. No case report has been described in the Korean literature. We report a case of malignant fibrous histiocytoma, myxoid type, arising from left spermatic cord. A 77-year-old male presented with a painless left upper scrotal mass for 5 months. Simple mass excision was performed for a diagnosis. Grossly, the mass closely contacted with the left spermatic cord. Since a myxoid type of malignant fibrous histiocytoma was diagnosed from histopathological findings, left radical orchiectomy with high ligation of the spermatic cord was performed additionally. There were no evidence of local recurrence or metastasis at 6 months after surgery.

The Effect of Complex Korean Medical Treatment on Spinal Cord Infarction : A Case Report (척수경색 환자에 관한 한방 복합치료 효과 : 증례 보고)

  • Objectives : The purpose of this study is to report the clinical effect of Korean medical treatment on spinal cord infarction. Methods : We treated a patient who was diagnosed with spinal cord infarction. We used acupuncture, bee venom pharmacopuncture, herbal medicine, moxibustion and physical therapy. We evaluated the patient through Manual Muscle Testing (MMT) and International Standards for Neurological Classification of Spinal Cord Injury (ISNCSCI) score. Results : MMT grade improved grade from 1 to 5 and ISNCSCI score increased from 2 to 14. Conclusion : According to these results, this report suggests that Korean medical treatment could be effective for spinal cord infarction.

A Clinical Study on 29 Cases of Vocal Cord Paralysis caused by Neoplasm (종양에 의한 성대마비 29예에 대한 임상적 고찰)

  • The authors observed the clinical status of 29 patients with vocal cord paralysis caused by tumor from April, 1983 to September, 1993 at Department of Otorhinolaryngology, Yongdong Severance hospital, Yonsei University College of Medicine. The results were as follows: 1) In the kinds of neoplasms, the most frequent were lung Ca. with 13 cases(44.8%), followed by 8 cases by thyroid Ca., 3 cases by neurogenic tumor, 2 cases by mediastinum tumor, cervical esophagus Ca., tracheal Ca., glomus jugulare were 1 case each. 2) In sex distribution, there were 18 cases of males and 11 cases of females with the male to female ratio being 1.8:1. In age distribution, most of the cases(10 cases ; 34.5%) were in the 7th decade. 3) In chief complaints, most of the cases(17 cases : 58.6%) had hoarseness only and aspiration, stridor, dyspnea, cough, dysphagia were present in some cases. 4) In site of the paralysed vocal cord, 21 cases were in the left cord. 5 cases in the right cord and 3 cases in the both cords. 5) In the position of paralysed vocal cord, most of the cases(23 cases : 79.3%) were in the parmedian position.

Micronucleus Frequencies in Human Umbilical Cord Blood by the Supravital Staining Method (인체에서의 초생체 염색법을 이용한 제대혈내 소핵 출현 빈도)

  • This study was conducted to quantify of micronucleus frequencies in human umbilical cord blood by supravital staining method with acridine orange, and to find some factors that affected on micronucleus frequncies in humans. In this study, we used umbilical cord blood of new born infants that have sufficient reticulocytes compared with adult peripheral blood. The cord bloods were taken after childbirth from 60 normal infants in industrial and coastal region in Korea. The total of 3 ${mu}ell$ cord blood was applied to slide coated with acridine orange, and micronuclei were observed under fluorescent microscopy. Demographic factors and independent variables were collected from mothers by questionnaire. The frequencies of micronuclei in umbilical cord blood of new born infants were 0-5 per 2,000 reticulocytes by supravital staining method, and mean value and standard deviation were 1.75$\pm$0.97. There were no significant difference by the regions, smoking habits of father or mother. However, age of mother showed significant positive correlation with frequencies of micronuclei (p<0.05). Smoking at home by fathers also was found as a significant variable by muliple regression analysis. Therefore, further studies would be needed for genotoxicological evaluation of new born infants by microneuli test using supravital staining method.

Multiple Hemangioblastomas on Cerebellum and Spinal Cord in a Patient of Von Hippel-Lindau Disease - A Case Report - (본 히펠 린다우병 환자에서 소뇌와 척수에 동시에 발생한 혈관아세포종 - 증례보고 -)

  • Hemangioblastomas are rare benign tumor of the central nervous system that commonly occur in the posterior fossa around the 4th ventricle. In case of von Hippel-Lindau disease, hemangioblastomas involve multiple regions such as cerebellum, spinal cord and brainstem but, rarely show simultaneous involvement of cerebellum and spinal cord. We have experienced a case of multiple hemangioblastomas that were located at the cerebellum, cervical cord and conus medullaris and also had multiple lesions that a part of von Hippel-Lindau disease ; retinal angioma, syringomyelia, multiple cyst on kidney and pancreas, renal cell carcinoma on left kidney. Hemangioblastomas on cerebellum and spinal cord were removed totally, retinal angioma was treated with laser photocoagulation and renal cell carcinoma was also totally excised. The authors report a case of von Hippel-Lindau disease had multiple located hemangioblastomas on cerebellum, cervical cord and conus medullaris with review of literature.

Muscular Adaptations and Novel Magnetic Resonance Characterizations of Spinal Cord Injury

  • The spinal cord is highly complex, consisting of a specialized neural network that comprised both neuronal and non-neuronal cells. Any kind of injury and/or insult to the spinal cord leads to a series of damaging events resulting in motor and/or sensory deficits below the level of injury. As a result, muscle paralysis (or paresis) leading to muscle atrophy or shrinking of the muscle along with changes in muscle fiber type, and contractile properties have been observed. Traditionally, histology had been used as a gold standard to characterize spinal cord injury (SCI)-induced adaptation in spinal cord and skeletal muscle. However, histology measurements is invasive and cannot be used for longitudinal analysis. Therefore, the use of conventional magnetic resonance imaging (MRI) is promoted to be used as an alternative non-invasive method, which allows the repeated measurements over time and secures the safety against radiation by using radiofrequency pulse. Currently, many of pathological changes and adaptations occurring after SCI can be measured by MRI methods, specifically 3-dimensional MRI with the advanced diffusion tensor imaging technique. Both techniques have shown to be sensitive in measuring morphological and structural changes in skeletal muscle and the spinal cord.

  • An air spring which is a part of the railroad vehicle suspension system is used to reduce and absorb the vibration and the noise. Main components of the air spring are a cord reinforced rubber bellows, a upper plate, a lower plate and a stopper rubber spring. The characteristics of the air spring which are the load capacity, the vertical and the horizontal stiffness are determined by the configuration of the rubber bellows, the angle of cord, and the mechanical properties of cord. Computer simulations using a commercial finite element analysis codes are executed to predict and evaluate the load capacity and the stiffness. The appropriate shape and cord angle of the air spring are selected to meet the required specifications of the air spring. Several samples of the air spring are manufactured and experimentally evaluated. It is shown that the results by computer simulation arc in close agreement with the test results.

  • When a person was suffered from vocal cord paralysis or glottic insufficiency, injection materials (e.g Teflon, Bovine collagen, Autologous fat & tendon, Gelfoam) into the vocal cord have been widely used. But each injection material has some disadvantage. We introduce the Restylane which is composed of a hyaluronic acid, artificially producted. It has advantage of rate foreign body reaction, proper endurance, easy to injection. The patient was 55-year-old woman who showed left vacal cord paralysis after pneumonectomy due to aspergillosis, taken the type I thyroplasty and arytenoid adduction. The middle portion of left vocal cord has some atropic mucosal change, slight chink was noted. The restylane injection into vocal cord was done with suspension laryngoscopy under general anesthesia. In the 3 month follow-up after Restylane injection, the quality of voice has been better progressively. We report a case of Restylane injection as a new method for the improvement of quality of voice.

Phrenic Nerve Stimulation for Diaphragm Pacing in a Quadriplegic Patient

  • Chronic hypoventilation due to injury to the brain stem respiratory center or high cervical cord (above the C3 level) can result in dependence to prolonged mechanical ventilation with tracheostomy, frequent nosocomial pneumonia, and prolonged hospitalization. Diaphragm pacing through electrical stimulation of the phrenic nerve is an established treatment for central hypoventilation syndrome. We performed chronic phrenic nerve stimulation for diaphragm pacing with the spinal cord stimulator for pain control in a quadriplegic patient with central apnea due to complete spinal cord injury at the level of C2 from cervical epidural hematoma. After diaphragmatic pacing, the patient who was completely dependent on the mechanical ventilator could ambulate up to three hours every day without aid of mechanical ventilation during the 12 months of follow-up. Diaphragm pacing through unilateral phrenic nerve stimulation with spinal cord stimulator was feasible in an apneic patient with complete quadriplegia who was completely dependent on mechanical ventilation. Diaphragm pacing with the spinal cord stimulator is feasible and effective for the treatment of the central hypoventilation syndrome.
