Bony Contusion of the Knees with Isolated Traumatic Meniscal Tears (외상성 반월상 연골 단독 손상에서 골타박)

  • Purpose: We studied the incidence rate and patterns of bony contusions of the knees with isolated traumatic meniscal tears. Materials and Methods: We analyzed retrospectively MRI scans and medical records of forty-two patients(42 knees) which had undergone operations for isolated traumatic meniscal tears. Mean age, 33.7 years, the number of patients with lateral, medial or both meniscal tears were 19, 18 and 5, respectively. Bony contusions were examined according to incidence, Location, and in relation to the types of meniscal tears. Results: Bony contusion was identified in 5 cases (11.9%) which had medial meniscal tear (4 cases0 or both meniscal tear (1 case). It was always located on the medial compartment of the joint. Bony contusion was found in the knee with various type of traumatic meniscal tears. Conclusion: Bony contusions in thd knees with isolated traumatic meniscal tear have very low incidence and they seem to disappear at or less than 12 months after the trauma. The bony contusions are mainly related to medial meniscal tear and located in the medial compartment of the joint.

Bilateral Oculomotor Nerve Palsy after Head Trauma: A Case Report

  • We introduce a patient who was suffered from isolated traumatic bilateral oculomotor nerve palsy after head trauma. The patient presented with bilateral ptosis and abnormal pupilary responses with slightly drowsy mentality at first. Performed images demonstrated some hematomas along subarachnoid, intraventricular, subdural spaces and multiple small supratentorial contusions. There was no bony abnormality or ligament injury. We assumed that small amount of interpeduncular hematoma might be the proper lesion associated with oculomotor nerve palsies, since the clinical symptom and signs presented bilaterally and the oculomotor neural fascicles run through the interpeduncular fossa.

  • Bilateral traumatic hemorrhage of the basal ganglia is an extremely rare neuropathologic entity. This report describes a 50year-old man with bilateral basal ganglia hemorrhage with occipital fracture of the skull after head trauma. The mechanism of development of traumatic hemorrhage of the basal ganglia has been not clear. But, it is presumed to be secondary to rupture of the lenticulostriate or anterior choroidal artery by shearing as a result of acceleration/deceleration forces. We briefly summarize our uncommon case and discuss its possible mechanisms.

  • Rhodiola rosea, also known as gold root or rose root, is a perennial plant in the family of Crassulaceae. The rhizhome of R. rosea has been widely used as a hemostatic, tonic for burns and contusions in traditional Chinese medicine. Recent studies reported its strong antioxidant and adaptogenic properties. In this paper, we attempted to isolate compounds from the methanolic extract of R. rosea rhizomes. Four compounds including one new compound (1), two kaempferol glycosides (3 and 4) were isolated from chloroform and ethyl acetate soluble fraction of R. rosea extract. The structures of 1~4 including relative stereochemistry were determined by MS and NMR analysis.

  • People with epilepsy are believed to be at a higher risk of incurring accidental injury than people who do not have seizures. The incidence of injury, either due to seizure or accident as a consequent of seizure is also high and varies from 0.03% to 3%. The most common injuries are head contusions, lacerations, burns and fractures. In this article, we present a case of quadriplegia after a generalized epileptic seizure.

Treatment of post-traumatic hematoma and fibrosis using hyaluronidase injection

  • Patients complaining of swelling and hematoma caused by contusion of the face can be easily seen in the emergency room. Most of the treatments were conservative treatments such as ice bag application, mild compression dressing, and massage. During the follow-up, fibrosis progression due to hematoma was frequently observed in the contusion site. When hematoma or fibrosis is confirmed, hyaluronidase (H-lase) 1,500 IU and 2 mL of normal saline were mixed and subcutaneously injected in crisscross manner. To evaluate the improvement of hematoma before and after hyaluronidase injection, three plastic surgeons evaluated using the Vancouver scar scale and compared preoperative and postoperative images. Hematoma and fibrosis after facial trauma improved after hyaluronidase injection for early treatment.

A Case of Eventration of the Diaphragm (횡경막성 내장탈출증: 1례 보고)

  • Eventration of the diaphragm is, by definition, abnormally high or elevated position of diaphragm as a result of paralysis, aplasia or atrophy of varing degrees of muscle fibers, and the cause of which may be congenital or acquired. The unbroken continuity of the diaphragm differentiates it from diaphragmatic hernia. The clinical manifestations of the condition, if present, are usually due to the interference of the ventilatory function of the lung and digesive dysfunction due to gastrointestinal distorsion. Treatment consists of surgical repair of the relaxed diaphragm to it`s normal position. A ease of left sided eventuration of the diaphragm, 31 year old officer, was found by chance after traffic accident with chief complaints of hemoptysis and multiple superficial contusions. Routine chest roentgenogram and barium study of the colon revealed moderately elevated left hemidiaphragm with displacement of the splenic flexure of the colon into the left chest. Past history revealed frequent attack of upper respiratory infection and some abnormal condition on his left chest announced by screen cheek of chest X-ray at the time of entrance for his army service 3 years before. Plication of the relaxed diaphragm through left thoracotomy was done and result was excellent as seen on Fig. 5. Cause of eventration of the left hemidiaphragm was due to paralysis of the left phrenic nerve which was tested during thoracotomy.

A Case of Medical Examination for the Relationship between Symptom onset of cervical spondylotic myelopathy and minor trauma (경추증성 척수증의 증상발현과 경미한 외상 사이의 관련성에 대한 의료감정 1례)

  • The Cervical spondylotic myelopathy (CSM) is degenerative compressive myelopathy which initiation of symptoms seems to be induced by minor cervical trauma or spontaneous event. There was a case of Claim medical examination which was requested to discriminate the cause of ambulatory quadriparesis. Patient asserted that the onset of his myelopathy was followed by minor trauma. The author considered the medical recordings, MRI scan, Claim reports by claim manager. The space available for cord was the smallest at C3-4 level. But on MRI findings, the spinal cord at C3-4 level seemed to be already damaged. There were no recent injury evidences such as hemorrhages, spinal cord contusions, edema, soft tissue hemorrhages. If the space available for cord was small enough to compress the spinal cord, the serious neurologic deficits, non-ambulatory quadriplegia, etc were commonly induced by cervical extension trauma. Patient's asserts did not correspond to his clinical course after cervical trauma. The author reports a case of medical examination for the relationship between symptom onset of cervical spondylotic myelopathy and minor trauma within author's experience.

A Study for Injuries due to Agricultural Machines in Kyeongsangnam Province (경상남도 농촌지역의 농기계손상에 관한 조사연구)

  • As compared before, agricultural machines are used more commonly instead of animal or manpower in rural areas and the injuries due to those are common. This study was conducted by questionnaire method in order to find out the current status of injuries due to agricultural machines for farmers who was selected from three Gun's in Kyeongsangnam Province. The study subjects were 385 persons in all(210 male persons, 175 female reasons) and the study period was from July through September 1993. The results were as follows; 1. The injury rate due to agricultural machines was high in male(p<0.05), and it was higher in younger age group and higher educated group. 2. The injury occurred high in summer and autumn seasons(77.6%), in the afternoon(60.6%), and during harvest(35.2%). 3. The major injuries were contusion, fracture, amputation in order and the injured sites were arms, legs, and chest in order. 31.7% of the injured farmers had been admitted, and they were treated at hospitals, home, drugstores and health centers in order. 4. The casualty damage was highest by cultivators, and agricultural instruments, threshing machine were followed. Among traumatic injuries concerned with cultivators contusions were most common, and fractures, amputations were followed. In case of agricultural instruments bruises were most common, and incisions, contusion were followed. In case of threshing machines fractures were most common and contusion, bruise were followed.

Inhibitory Actions of Steppogenin on Platelet Activity Through Regulation of Glycoprotein IIb/IIIa and Ca2+ Mobilization (Glycoportein IIb/IIIa와 칼슘동원의 조절을 통한 Steppogenin의 혈소판활성 억제효과)

  • The extract of Cudrania tricuspidata is used in ethnomedicine throughout Eastern Asia in China, Korea and Japan. In Korean traditional medicine, Cudrania tricuspidata has been used to treat eczema, mumps, tuberculosis, contusions, insomnia and acute arthritis. In addition, it has been reported that root extract of Cudrania tricuspidata has anti-platelet effects. Therefore, we investigated which compound in Cudrania tricuspidata has inhibitory effect on platelet aggregation. In this study, we tried to explain the inhibitory mechanism of steppogenin from Cudrania tricuspidata on human platelet aggregation. Collagen-induced human platelet aggregation and [Ca2+]i mobilization were dose-dependently inhibited by steppogenin and we determined the inhibition by steppogenin is due to the down regulation of extracellular-signal-regulated kinase(ERK) and inositol-1,4,5-triphosphate receptor type I(IP3RI) phosphorylation. In addition, steppogenin inhibited collagen-induced fibronectin adhesion to αIIb/β3 and thromboxane A2 generation. Thus, in the present study, steppogenin showed an inhibitory effect on human platelet aggregation, suggesting its potential use for preventing platelet-induced cardiovascular disease.
