Purpose: This study was done to examine the effect of a multi-component cessation program on bone union by comparing the union time between the experimental group and the control group. Methods: A non-equivalent control group posttest design was used. The subjects were 33 patients in the experimental group and 33 patients in the control group, 66 patients in total. The subjects of the experimental group were provided with a face-to-face multi-component smoking cessation program. After the discharge, telephone consultation was provided once a week for 10 weeks. Results: Bone union time was 51.6±10.9 days in the experimental group and 60.9±13.83 days in the control group (p=.003). Among 33 subjects who participated in the smoking cessation program, the average union period of the participants who succeeded in quitting smoking was 44.56 days, the participants who reduced smoking were 50.67 days, and failed to quit smoking group was 60.11 days (p=.006). Conclusion: This study indicated the importance of smoking cessation in patients with fracture and the decrease in the union time by providing smoking cessation education. If the multi-component smoking cessation program is used as a nursing intervention in clinical practice, it will be effective for bone union by increasing the smoking cessation rate of patients with fracture.

The numbers of learning underachievers in college are gradually increasing. As a result, the need for extracurricular programs to increase learning in college is also growing. The purpose of this study was to analyze factors of learning difficulty and develop a model of learning consulting for college underachievers. This study also aimed to evaluate this model's validity. Using both 56 subscription forms of college underachievers and three sets of focus group interviews at B university, we found that students had difficulties in goal and career setting, management of grades and tests, learning methods, time management, failure overcome ability, lack of learning habit sustaining power and learning motivation, and so on. We developed a model of learning consulting for college underachievers based on these factors and applied the model to evaluate it's validity, testing it on 31 underachievers currently enrolled in college, five times every week. Let we say in conclusion that this model of learning consulting had positive effects on changing college underachiever's character, emotion, motivation, and behavior towards learning.

Purpose: Importance of patient satisfaction related to patient-centeredness has been emphasized, and it is known to have effect on various health outcomes including health resource utilization. However, the effect of patient satisfaction has been discussed mostly in terms of hospital marketing in Korea. This study aims to examine the effect of patient satisfaction in patient-physician communication on healthcare utilization in a nationally representative adult population of South Korea. Method: Patient satisfaction with physician communication is assessed using 4 items in the 2011 Korea Health Panel Survey. Generalized linear regression analysis is conducted using 9,325 adults' healthcare utilization in 2012. Findings: Adjusting for the socio-demographic, economic factors, individual health status, health behaviors and healthcare utilization in 2011, more satisfied individuals, more likely to utilize the outpatient service, especially in clinical setting. Practical Implications: The study findings suggests that in context of South Korea healthcare system such as insufficient medical consultation time and the absence of health delivery system, patient satisfaction as a subjective healthcare quality indicator would have effect on the individual's outpatient visit. This study contributes to stimulate patient satisfaction research and discussion in South Korea to further explore its relationship with potential and various health related outcomes. Further implications of the study are discussed.

The conventional stethoscope can not store its stethoscopic sounds. Therefor a doctor diagnoses a patient with instantaneous stethoscopic sounds at that time, and he can not remember the state of the patient's stethoscopic sounds on the next. This prevent accurate and objective diagnosis. If the electronic stethoscope, which can store the stethoscopic sound, is developed, the auscultation will be greatly improved. This study describes an amplifier for electronic stethoscope system that can extract heart sounds of fetus as well as adult and alow us hear and record the sounds. Using the developed stethoscopic amplifier, clean heart sounds of fetus and adult can be heard in noisy environment, such as a consultation room of a university hospital, a laboratory of a university. Surprisingly, the heart sound of a 22-week fetus was heard through the developed electronic stethoscope. Pitch detection experiments using the detected heart sounds showed that the signal represents distinct periodicity. It can be expected that the developed electronic stethoscope can substitute for conventional stethoscopes and if proper analysis method for the stethoscopic signal is developed, a good electronic stethoscope system can be produced.

The purpose of this study is to analyze the cost-effectiveness of four medications for treating and preventing osteoporosis -HRT therapy(conjugated equine estrogen 0.625mg for 25 days and medroxyprogesterone acetate 5mg for 01112 days), Alendronate(10mg and 5mg), Active Vitamin D(Calcitriol), and Calcium. Total costs include the direct medical cost -examination fee, consultation fee, prescription fee, fee for preparing medications, and the price of pharmaceuticals- and the indirect cost of patients such as traffic expenses and time cost. In addition, the costs of monitoring in adverse reactions are added. The effects of four medications are expressed as BMD(Bone Mineral Density) percent change measured by DEXA(Dual Energy X-ray Absorptiometry) in lumbar spine(L2-L4) and femoral neck site. A mixed model based on meta analysis provides the estimates of effectiveness, which are then appled to the hypothetical cohort consisting of postmenopausal women at the age of 50-59. HRT therapy is the most cost-effective medication at 172,433.64 won (lumbar spine site) and 546,328.28 won (femoral neck site) per BMD percent change for osteoporosis. Alendronate 10mg is more cost-effective than Alendronate 5mg as 345,971.23 won and 378,441.63 won per lumbar BMD percent change at 0.991ｇ/$cm^2$, respectively. Alendronate 10mg is more cost-effective than Alendronate 5mg as 1,329,257.89 won and 1,467,291.23 won per femoral neck BMD percent change at 0.834ｇ/$cm^2$, respectively.

Number of elderly patients requiring nerve blocks have been increasing in recent years. We had two elderly patients who suffered stroke one day and three days after lumbar facet joint block and lumbar single epidural block respectively. Both patients due to their advanced age had potential risk factor to suffer one or more of the following; stroke, hypertension, and diabetes mellitus. Due to our experience with these patients, we suggested the following: (1) Nerve blocks should be reconsidered for elderly patient who posesses a potential risk factor to suffer a stroke. (2) Prior to invasive block administration of mild sedatives or analgesics may provide beneficial effects for patients with hypertension. (3) Adequately informed consent must be fully discussed time of consultation with patient scheduled for nerve block especially for elderly and risky patient.

The concept of dental clinic have been changed along the advanced dental science to meet the need of setting up high-technology-digital appliances. As the general idea about the dental clinic has been changed from systematic connection, the expansion of zoning needs to become more accurate and mote detailed. In the spacious waiting room. the interment cafe has been established to supply the information services, and the various multi-imaged channels, which were built in, will relieve the boredom of the waiting time. The formation of water space leads to the effect of humidification and makes parents share visual and auditory sympathy All of the unit-chairs attached cameras in the consultation room enables people to diagnose more quickly and precisely through the computerized system. Partitions have been set of to put it in and to help diagnosis process to be done smoothly in the formational space. The whole atmosphere is intended to be harmony with the natural surroundings in Je-Ju. It will inspire people visiting this clinic to image the deep blue sea in Je-Ju. By using projective non-material a lot, especially material wooden windows and doors, it is believed to create the bright and stable atmosphere. It will lead patients feel comfortable.

본 논문은 누구나 참여할 수 있는 웹상에서의 시공간을 초월한 on-line 및 off-line 겸용모드의 지능형 원격 진료시스템으로서, 데이터베이스는 IIS 4.0 웹서버 상에서 ASP와 SQL을 연동한 구현하여 효율적인 자료처리를 위한 시스템 통합과 환자와 의사 및 한의사간의 on-line 상담, 그리고 off-line상에서의 진료와 환자가 지정한 약사로의 처방전 전송 및 조제, 그리고 진료데이터의 저장 및 검색으로 인한 반영구적인 진료데이터저장이 가능하며, 가족간의 진료데이터 검색 및 공유등 유전에 대한 치료효과 및 개인 병력관리를 통한 과거진료상황에 따른 빠른 치료효과 및 의사간의 협조진료등이 가능한 지능형 객체관리를 이용한 데이터베이스 구성이 되어있다.

Purpose: The purpose of this study was to explore roles of cancer care coordinator's by developing job description, job specification and job process map using dynamic job analysis. Method: The development process consisted of three stages of vertical job analysis and three stages of horizontal job analysis by modifying Song(1997)'s dynamic job analysis. Focus group interview was used to validate the content of the job analysis. Results: Cancer care coordinator's job description was categorized into six major categories, fourteen intermediate categories and one hundred forty specific jobs. Major categories are professional nursing practice, consultation and counsel, coordination and collaboration, education, research and leadership. Cancer care coordinator's job specification included master's degree with over five years of clinical experience preferably relevant clinical area, professional knowledge on pathophysiology of cancer, case management and cost control, competency for communication and counselling skills and clinical decision making. Cancer care coordinator's job process map was framed with time(horizontal) and activities(vertical). Conclusion: The Outcomes of this study will guide to develop possible areas of oncology advanced practice nurses in hospital setting and facilitate the use of oncology nurse practitioners by developing care coordinator roles in cancer care.

본 논문에서는 ASP와 SQL을 연동한 사이버한방진료와 자료처리를 위한 시스템 통합과 환자와 한의사간의 on-line 상담, 진료데이터의 저장 및 검색으로 인한 반영구적인 진료데이터의 활용(환자 및 한의사의 본 진료데이터를 이용한 보다 정확한 진료 및 처방등이 가능)이 가능하다. 본 논문에서는 사이버한방병원 자료처리 프로세스에서 전문가 상담, 유료회원 접근절차, 처방프로토콜, 전자진료차트을 구현함으로써, 웹상에서의 시공간을 초월한 on-line 및 off-line 겸용모드의 진료형태로 전환하는 한방원격진료 시스템을 설계 및 구현하였다.
