  • The construction failures can be decreased through continuous improvement of construction process based upon the information of construction failures. Herein, the information of construction failures can be utilized as the key factor for identifying and enhancing various ineffective construction processes that can prevent failures. This research proposes a process model for the continuous improvement of construction processes by using construction failure information. Extensive reviews and analyses of literatures related to construction failures are performed to investigate its definition, type, cause, and lessons learned. This research adapts process modeling methodology and case-based reasoning for the development of the proposed CIMCP(continuous improvement model of construction process), and then suggests its framework that contains modules of case retrieval, case index, and case adaptation.

  • The objective of this study is to collect and classify bridge failures that occurred in domestic from 1974 through 2001 and to draw the engineering informations by analyzing the principal causes and the trends of the bridge failures. Total 45 failures of bridge were obtained and analyzed based on the factors such as time of failures, modes of failures, classification of materials of failed elements, types of failed elements, distribution of failure cases with respect to the highway grades. The critical problems of bridge failures are associated with the construction deficiencies, external causes, maintenance deficiencies, and design deficiencies. Since the study of failures in bridges would enhance the design and construction of safe bridges in the future, it would be needed to create data bases at the national level and to alter the related codes with identifying potential deficiencies from past failures data.

  • In order to encourage construction practitioners to acknowledge failures and disseminate the information, the failure information must be documented and accumulated with a well-structured format, which contains not only the fact and result but also the circumstance and cause of the failure. In the Korean construction industry, many failures are not explained clearly and often not even reported publicly, partly because due to the lack of understanding positive aspects of failures, which can improve construction practices as a result of learning from failures. The purpose of this study is to develop a web-based construction failure information system using the case-based reasoning techniques, which can systematically accumulate, manage, and share the valuable failure information using a structured failure cases database. It can be utilized for planning proactive solutions on future failures by searching the very similar past failure cases.

  • Korea is affected by typhoon 2-3 times a year, and 50${\sim}$60 % of annual rainfall is concentrated during summer with heavy daily precipitation. Recently such natural conditions cause many of failures or damages of reservoirs and embankments. Overflow by heavy flood is the main cause that results 54.2 % of total embankment failures with damages of spillway, outlet channel and stilling basin. Since damages by overflow are triggered by scour of soils nearby the structures, use of proper backfill materials with great resistance against erosion should be considered and application of suitable construction method to protect erosion may be adopted. Most failures of levee are caused by piping along the surface of cross-structure underneath levee. Such failures may be protected by deep consideration of piping at the stage of design and good quality control during construction. Sufficient magnitude of spillway and outlet channel is the ideal way to prevent failures by the flood. For existing structures, remodeling with reinforcement to protect against flood with review of required storage of dam should be considered.

  • Construction incidents, mechanisms and causes in Geotechnical Engineering have been closely reviewed for the safety assurance during design and construction stages. The safety assurance is clearly related to the observational method in which design is reviewed during construction and contingency actions are undertaken successfully to minimise any potential failures. The observational method has been extensively applied for the construction of subways underground structures, dams and many important Civil Engineering projects in the past 20 years. But the number of serious construction incidents are growing for the past several decades. Therefore present development of observational method and improvement techniques are introduced with some case histories.

  • This study aims to present cases of rock slope failures caused by geological structures. Status of slope failures, results of cause analysis and stabilizing methods are introduced, focusing primarily on rock slope failures caused by specific geologic structures, such as intersection of faults infilled with clay, foliation and fault shear zone by dike intrusion and deep-seated clayey layer along lithologic boundary. Detailed geological survey, geophysical exploration and boring survey were conducted for cause analysis. Stabilizing method to prevent further slope failures and to ensure long-term stability of slopes were established, considering characteristics of geological structures, types of failure and geological conditions.

  • An experimental investigation of composite beams composed of wide flange steel and precast concrete is presented. The bottom flange of the steel section is encased in precast concrete. The composite beams tested in this study were designed to reduce the depth of the slab and beam. The slabs are constructed on top of the edges of the Structural Composite Hybrid System, instead of on top of the steel flange, decreasing the depth of the beams. When concrete is cast on the metal deck plate located on the edges of the precast concrete, the weight of the concrete slabs and other construction loads must be supported by the contacts between the steel and the precast concrete. This interface must not exhibit bearing failures, shear failures, and failures caused by torque due to the loading of the precast concrete. When the contact area between the concrete and the bottom flange of the steel beam is small, these failures of the concrete are likely and must be prevented. The premature failure of precast concrete must not also be present when the weight of the concrete slabs and other construction loads is loaded. This paper presents a load carrying capacity of Structural Composite Hybrid System in order to observe the failure mode. The symmetrically distributed loading that caused the failure of the composite beam was found. The paper also provides design recommendations of such type of composite structure.

  • Aluminum rolling stocks have been developed for six years in Korea and commercial trainsets are being constructed by the carbuilder. Aluminum alloys are sensitive to various imperfections. In this paper, damages and failures of the aluminum carbody taking place during the process of development are investigated and accumulated data are released. Also, remedies for the failures are suggested and design changes are introduced. It is expected that all informations can contribute to construction of reliable and safe aluminum rolling stocks.

  • As the land price in the downtown area increases, buildings are becoming bigger, deeper and higher. Consequently, the importance of underground construction has increased. Although construction engineers make every effort to complete underground construction without any problem, construction failures like landslides and the collapse of a retaining wall occur because of the uncertainty of the soil conditions as well as the unexpected risks of excavation work. In order to prevent potential excavation accidents, it is essential to understand the causes and impacts of such accidents. However, there are only a few examples of construction failures, which show the economic impact on accidents during excavation because of the sensibility of the information. This paper presents two cases of excavation accidents, which were investigated by construction insurance company. The compensation for the accidents paid by the insurance company was compared with the estimated costs calculated based on the estimation method for excavation accidents proposed by our previous study. The comparison results showed that the estimate calculated by our method was much less than the actual compensation because the estimate solely focused on the construction costs whereas the compensation included other external factors.
