Reinterpretation of Taeyang disease(太陽病) in "Shanghanlun(傷寒論)" Based on the Pathologic Perspective of Sasang Constitutional Medicine (사상체질의학적 병리관에 의한 "상한론(傷寒論)" 태양병(太陽病)의 재해석)

  • 1. Objectives: Purpose of this paper is to study the reinterpretation of Taeyang disease(太陽病) in "Shanghanlun(傷寒論)" based on the pathologic perspective of Sa-sang Constitutional Medicine by comparing factors as pathologic mechanisms, clinical symptoms, and remedies. 2. Methods: The texts referred to pathologic mechanisms, clinical symptoms, and remedies of Taeyang disease(太陽病) described in "Donguisusebowon Gabobon(東醫壽世保元 甲午本)", "Donguisusebowon Sinchookbon(東醫壽世保元 辛丑本)", and Shanghanlun(傷寒論)" in "Donguibogam(東醫寶鑑)" were analysed. 3. Results and Conclusions 1) Early phase of Ulgwang symptomatic pattern(鬱狂證 初證) and of Mangyang symptomatic pattern(亡陽證初證) of Kidney Heat-based Exterior Heat disease(腎受熱表熱病), a category of Soeumin(少陰人) diseases, were described by adopting pathologic models of Taeyang-sangpung symptomatic pattern(太陽傷風證) and Sanghan-hyeol symptomatic pattern(傷寒血證) from "Sanghanlun(傷寒論)". 2) Soyang-sangpung symptomatic pattern(少陽傷風證) of Spleen Cold-based Exterior Cold diseae (脾受寒表寒病) and Hyunggyeok-yeol symptomatic pattern(胸膈熱證) of Stomach Heat-based Interior Heat disease(胃受熱裏熱病), categories of Soyangin(少陽人) diseases, were described by adopting pathologic models of Taeyang-yangsangpunghan symptomatic pattern(太陽兩傷風寒證), Soyang-sangpung symptomatic pattern(少陽傷風證) and Tayangbyong-sahak symptomatic pattern(太陽病似瘧證) from "Sanghanlun(傷寒論)". 3) Baechu-pyo symptomatic pattern(背顀表病輕證) and Hangual symptomatic pattern(寒厥證) of Esophagus Cold-based Exterior Cold disease(胃脘受寒表寒病), a category of Taeeumin(太陰人) diseases, was described by adopting pathologic models of Taeyang-sanghan symptomatic pattern(太陽傷寒證) and Hangual symptomatic pattern(寒厥證) from "Sanghanlun(傷寒論)". 4) Je-Ma Lee reinterpreted various diseases classified as Taeyang disease(太陽病) with the pathologic perspective of Sa-sang Constitutional Medicine. Different from existing medicine, diseases were analysed and treated by the standard, constitution of the patient.

A Case Study of a Taeeumin Patient with Advanced Parkinson's Disease Diagnosed as Dry-heat Symptomatic Pattern (태음인(太陰人) 조열증(燥熱證)으로 진단한 진행된 파킨슨병 환자 치험례)

  • Objectives This case study was about a Taeeumin patient with advanced Parkinson's disease diagnosed as Dry-heat symptomatic pattern. In this study, we reported significant improvement of motor and non-motor symptoms of this patient after Sasang constitutional medicine treatment. Methods The patient was diagnosed with Taeeumin Dry-heat symptomatic pattern and treated with Taeeumin's constitutional medications and acupuncture. The primary outcome measure for this study is Unified Parkinson Disease Rating Scale(UPDRS) to assess the overall function of patient. Secondary outcome assessments include Global Assessment Scale(GAS), change of patient's sleep, stool and urine. Results and Conclusions The patient treated with Taeeumin's constitutional medications and acupuncture had reduction in UPDRS (specifically Part II, Part III and Part IV) and GAS of motor symptoms. Furthermore, patient's sleep, stool and urine were reported to be normal after treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with Advanced Parkinson's disease diagnosed as Dry-heat symptomatic pattern.

A Literature Study on the Concept and Classification of Soyangin Gochang ("동의수세보원(東醫壽世保元)"에 나타나는 소양인(少陽人) 고창(鼓脹)의 범주(範疇)에 대한 고찰(考察))

  • 1. Objective This study aims to establish the concept and classification of the Gochangl(鼓脹) pathology introduced in the Sasang Constitutional Medicine literature. 2. Methods Different editions of Donguisusebowon, including the Gabo version and the Sinchuk version, were compared for differences and similarities in the concept, symptomology, pathology, and therapeutics concerning the Soyangin Gochang disease. 3. Results and Conclusion (1) In the Gabo edition, Gochang(鼓脹) is introduced to Eumheohwadongwiyeolyukran(陰虛火動胃熱肉爛) disease category of the Interior-Based Symptomatic Pattern in the "Soyangin Interior Large Intestine Disease" section and is introduced to the conditions progressed from edema included in the Exterior-Based Symptomatic Pattern in the "Articles added to the Soyangin Bladder and Large Intestine Disease" section. (2) In the Sinchuk edition, Gochang(鼓脹) is introduced ro the conditions progressed from Jungso(中消) of the Interior-Based Symptomatic Pattern in the "Soyangin Interior-heat-disease by the fever in the stomach" section and "Articles added to the Soyangin" section. (3) It is reasonable to think that Soyangin's Gochang is considered to Interior-Based Symptomatic Pattern because the Sinchuk edition is developed from the Gabo edition and Gochang is introduced to Jungso(中消) category after the distinction between Gochang and edema in the Sinchuk edition.

The Study on Clinical Characteristics of Taeeumin Dry-Heat Symptomatic Patten (태음인(太陰人) 조열병증(燥熱病證)의 임상적 특징에 대한 연구(硏究))

  • 1. Objective The object of this study was to understand the clinical characteristics of the Taeeumin Symptomatic Pattem. 2. Methods The patients visiting the department of Sasang Constitution of Traditional Korean Medicine clinic from June to November of 2008 were used in this study. Those who showed a favorable turn after over ten days of medication and who showed up for at least two follow-up sessions were chosen among first-time visitors. Of these, 75 patients identified and verified to be of the TE type were chosen for the study sample. The Taeeumin were verified into two groups according to clinical symptoms into Exterior Cold Disease and Interior Heat Disease groups, of which the Interior Heat Disease group was further subdivided into the Galgeun-medicament group and the Cheongshimyeonja-tang group, based on questionnaire results. The Exterior Cold Disease group was set as the control group in order to contrast and compare it with the study group, the Interior Heat Disease group. 3. Results and Conclusions The questionnaire items shown to differentiate the Exterior Cold Symptomatic Patterm and the Interior Heat Symptomaic Pattern (indicating Heat-affceted Liver Symptomatic Pattern) were related to "dryness of stool," "dark urine colorm," "dryness of nose," and "dryness of lips." The items shown to differentiate the Galgeun-medicament group and the Cheongshimyeonja-tang group (of the Dry - Heat Symptomatic Pattern) were related to "nausea," "stifling sensations and palptation," "disturbed sleep," and "dizziness." Therefore, the sleep pattern, palpitation, dizziness, and nausea can be said to be indicators of the Interior Heat Symptomatic Pattern, and Dry-Heat symptoms are better distinctions between the Chongshimyeonja-tang group and the Galgeun-medicament group than Heat-affected Liver symptoms.

A Case Study of a Taeeumin Patient with Meniere's Disease Treated with Cheongsimyeonja-tang (청심연자탕으로 호전된 메니에르병 환자 치험 1례)

  • Objectives The aim of this study was to report significant improvement of vertigo, tinnitus and aural fullness after treatment with Cheongsimyeonja-tang in a Taeeumin patient with Meniere's disease. Methods The patient was diagnosed with Taeeumin Dry-heat symptomatic pattern and treated with Cheongsimyeonja-tang and acupuncture. The patient's subjective symptoms of vertigo, tinnitus and aural fullness were observed using Global Assessment Scale (GAS) during the treatment period. Results & Conclusions The symptoms of vertigo, tinnitus and aural fullness decreased from GAS 100 to GAS 0 for seven weeks. Furthermore, the patient's symptoms of insomnia, urinary frequency, chest discomfort and fatigue were reported to be improved after treatment. In conclusion, this study shows that Sasang Constitutional Medicine can be effective treatment for Taeeumin patient with Meniere's disease diagnosed as Dry-heat symptomatic pattern.

  • 1. Objective This study aims to establish the concept and classification of the Janggyeol(藏結) pathology introduced in the Sasang Constitutional Medicine literature. 2. Methods Different editions of Dongui suse bowon, including the Gabo version and the Sinchuk version, were compared for differences and similarities in the concept, symptomology, pathology, and therapeutics concerning the Soeumin Janggyeol disease. 3. Results and Conclusion (1) In the Gabo edition, Janggyeol(藏結) is introduced in the "Soeumin Interior Stomach Disease" section and the "Soeumin Summaries" section. However, instead of being included in the interior-based pathology, it is proposed as an independent disease category that is similar to and yet separate from the Taeeumbyeong Simhabiman (epigastric congestion) variations of Sugyeolhyung and Hansilgyeolhyung. Further, passages from the "Soyangin Summaries" section outline the underlying pathology in detail to show that this symptomatic pattern can be included in the exterior-based pathology. (2) In the Sinchuk edition, Janggyeol(藏結) is introduced in the "Soeumin Cold-based Stomach Interior-Cold Disease" section. Similarly to the Gabo classification, it is proposed as an independent disease category not necessarily included in the interior-based pathology that can be compared to the Taeeumbyeong Simhabiman(太陰人 心下 滿) category. The detailed pathological explanations in the "Soyangin Summaries" section suggest that this disease pattern is included in the exterior-based pathology. (3) Janggyeol(藏結) can be understood as a Taeyangbyeong Wigasil(太陽病胃家實) pathology in the Gabo edition and as a Taeyangbyeong Wigasil(太陽病胃家實) pathology in the Sinchuk edition.

  • Objectives The aim of this study was to report significant improvement of hyperhidrosis after treatment with Hyungbangsabaek-san (荊防瀉白散), in a Soyangin Mangeum Symptomatic pattern Patient. Methods The patients were diagnosed with Soyang Sin-Yeol Du-Tong Mang-Eum Symptomatology(身熱頭痛亡陰證) and treated with Hyungbangsabaek-san(荊防瀉白散). The primary outcome measure for this study were sweat using a questionnaire with visual analogue scale(VAS). Secondary outcome assessment included change of odinary symptoms such as patient's sleep, feces and digestion. Results The symptoms of hyperhidrosis disappeared by the end of the accure period without side effect. Original symptoms were also changed. Conclusions This result show Hyungbangsabaek-san(荊防瀉白散) can be used to treat hyperhidrosis in a Soyangin Heat-related diarrhea accompanied by headache Mangeum(身熱頭痛亡陰) symptomatic pattern patient. Meaning and process of hyperhidrosis are different according to Sasang Constitutions.

  • Objective The aim of this study was to report significant improvement of primary insomnia in a Soyangin Cold-related diarrhea accompanied by abdominal pain Symptomatic pattern Patient. Methods The patients were diagnosed with Soyangin Cold-related diarrhea accompanied by abdominal pain Symptomatology(身寒腹痛亡陰證) and treated with Hyungbangjihwang-tang(荊防地黃湯). The primary outcome measures for this study were condition of sleep using a questionnaire with Pittsburgh Sleep Quality Index (PSQI) Secondary outcome assessment included change of original symptoms such as patient's digestion, sweating and feces. Result The symptoms of primary insomnia improved by the end of the a treatment period without side effect. original symptoms were also changed. Conclusions This result show Hyungbangjihwang-tang(荊防地黃湯) can be used to treat primary insomnia in a Soyangin Cold related Mangeum Symptomatic Pattern accompanied by abdominal pain Symptomatology(身寒腹痛亡陰證). Meaning and process of primary insomnia are different according to Sasang Constitutions.

Objective : This study aims to report symptomatic improvement of a Soyangin patient reporting general weakness, dizziness, gait disturbance, cold sensitivity, nocturnal sweating, and anorexia after treatment with Hyeongbangjihwang-tang. 2. Method : The patient's subjective and objective symptoms were observed daily, and the VAS scores for the main symptoms were recorded daily throughout the hospitalization period. 3. Result : The general condition of the patient improved by the end of the hospitalization period, where the VAS score dropped from VAS 10 to VAS 5 and the symptoms of dizziness, general weakness, cold sensitivity, and nocturnal sweating disappeared by day 4 of admission without recurrence. 4. Conclusion : Although the patient did not report diarrhea, one of the main Mangeum symptoms, the general symptoms improved with Hyeongbangjihwang-tang medication. This indicates that Hyeongbangjihwang-tang can be used to treat chief and subsidiary symptoms related to Mangeum symptomatic pattern when applied to the appropriate base and manifesting symptomology.

  • Objectives The purpose of this study was to report significant improvement of peripheral coldness in patient, who was diagnosed with 'Soyangin Chest-heat symptomatic pattern' and applied medication based on Sasang Constitutonal medicine. Methods The patient was treated with herbal medications, according to his own symptomatology. The objective outcome was measured by thermography and the subjective sensation was assessed by using Visual Analogue Scale (VAS). Results The symptoms of peripheral coldness improved remarkedly without any side effects. Accessory symptoms and original symptoms also changed for the better. Conclusions This results show that it is available that peripheral coldness is treated with herbal medications according to Sasang Constitutional medicine, especially ingenious in terms of the fact that some patient can be diagnosed with heat-based condition on the basis of original symptoms, although his/her chief complaint is classified as cold pattern.
