Neural Blockade and Trigger Point Injection for Chronic Shoulder Pain (만성 어깨통증을 위한 신경차단과 방아쇠점 주사)

  • The shoulder is a complex area which makes it vulnerable to painful pathologic processes. Chronic shoulder pain has become more common recently due to increased use of computers and a ,generally more sedentary life style among most people . Trigger point injection and neural blockade are useful for the management of chronic shoulder pain which has not improved with conservative treatment. Published articles concerning trigger point injection or neural blockade for chronic shoulder pain were reviewed to evaluate promising methods. If we are careful to remain aware of the details and complications in addition to adhering to effective treatments, these should be good armamentarium for doctors enthusiastic about the management of chronic shoulder pain.

  • We reviewed literatures of western and oriental medicine about postoperative management of spine. Traditionally, with orthoses and bed rest, many physicians restricted activity of daily life and back exercises of patients who had taken spine surgery. More aggressive early mobilization, however, such as strengthening exercise and stretching after 4 weeks of surgery gets grounds nowadays. Physical therapies including manipulation, TENS, MENS, ultrasound and cryotherapy are being used as helpful treatment modalities of postoperative pain and swelling. Failed back surgery syndrome occurs due to wrong patient, diagnosis and surgery and is managed with conservative treatment or reoperation. In oriental medicine, treatments of musculoskeletal injuries including surgical wound are based on the balance of chi(氣) and blood(血).

  • Isolated posterior cruciate ligament injuries are rare and their management is controversial. But, there is general concept that a bony avulsion of posterior cruciate ligament should be repaired. The treatments for the bony avulsion of posterior cruciate ligament were conservative treatment, open reduction and internal fixation and arthroscopic fixation. We report 2 cases of posterior cruciate ligament avulsion fractures, which were arthroscopically reduced and stabilized with cannulated screws and Kirschner wires, and introduce the arthroscopic fixation technique.

  • To adequately analyze flows in pipe or duct network system, traditional node-based junction coupling methods require the junction loss which is specified by empirical or analytic correlations. In this paper, a new finite volume junction coupling method using a ghost junction cell is developed by considering the interchange of linear momentum as well as the important wall-effect at junction without requiring any correlation on the junction loss. Also, boundary treatment is modified to preserve the stagnation enthalpy across boundaries, such as pipe-end and the interface between junction and branch. Also, the computational accuracy and efficiency of the Godunov-type finite volume schemes are investigated by tracing the total mechanical energy of rapid transients due to sudden closure of valve at downstream end.

  • Objectives The purpose of this study was to report the effects of treatments to traumatic subcutaneous hematoma on the shin using herbal medication and external application without venesection, which had no change in spite of previous 3 weeks medical conservative treatment so it had a risk of sepsis.Methods The patient diagnosed as Soyangin was prescribed with Yangkyuksanhwa-tang. The subcutaneous hematoma lesion was treated by external application of Daehwangchija-san(大黃梔子散). We evaluated the size of hematoma, pain and gait disturbance.Results and Conclusions Subcutaneous hematoma and relevant symptoms disappeared within 11 days of admission. Any kind of sequela did not show up and skin color was recovered after 4 month later follow-up.

  • The intrathecal drug administration system (ITDAS) has recently been introduced for treating chronic intractable pain patients who have failed with conservative pain treatments. The obvious advantages of its use are the direct intrathecal delivery of drugs, which yields reduced adverse effects and the increased strength of drugs, as compared to its oral or intravenous route. This article offers a review of the ITDAS with a brief review of its evidence-based effectiveness, the technical approach, and the complications.

  • Objective : The Fibromyaigia Syndrome(FMS) is frequently used to call at clinic. There are not case reports on the treatment of FMS by oriental medicine. We got a effect on three patient with FMS by oriental medicine and this case is observed the clinical symptoms and scaled by VAS with chuna and acupuncture. Methods : We used to take chuna and acupunture, though classifcated by pathologic symptom and classification criteria of American College of Rheumatology. Result : Usually, most patients with FMS on the chronic stage have chronic prognosis. But symptoms of the patient on this report were improved after above treatments. Conclusion : This case showed oriental medicine enough could be applied on diseases like this syndrome as one of conservative therapies. Of course, more accurate diagnosis and research would be needed afterwards.

  • This study discusses the effects of infrared thermography in dentistry. Infrared thermography allows the quantification of infrared rays and provides useful data for dental care and research. It has been used for the diagnosis of temporomandibular dysfunction (TMD) and proposed as an objective measure of myofascial pain. It is closely related to temperature and produces beneficial objective data. A study of the heat emissions from facial structures has the potential to benefit dental diagnosis of TMD, postoperative inferior alveolar damage, and endodontic and conservative treatments. In this paper, we assessed infrared thermography in dentistry and consider future developments of infrared thermography.

  • Although non-surgical endodontic procedures have high success rates, failures do occur, These can be managed by root canal re-treatment or surgical intervention. Intentional replantation is an accepted endodontic treatment procedure in which a tooth is extracted and treated outside the oral cavity and then inserted into its socket to correct an obvious radiographic or clinical endodontic failure. Intentional replantation is indicated when other endodontic treatments performed to maintain the tooth have failed, or when endodontic periradicular surgery is not feasible. Intentional replantation may be particularly useful in these cases because these difficult to access areas can be maximally treated while the tooth is out of the mouth without damaging the periodontal attachment in adjacent teeth. In conclusion, intentional replantation is a reliable and even predictable procedure, and should be considered more often as a treatment modality in our efforts to maintain the natural dentition.

  • Chronic recurrent TMJ dislocation results in difficulty of mastication, speaking, and swallowing due to the limitation of the mandibular movement. Etiologic factors are considered as the looseness of the capsule and ligaments, the decrease of the articular eminence, condylar morphologic change, muscular disharmony near by TMJ, and the decrease of the vertical length of the mandibular ramus. Treatment approach has been suggested that surgical methods are selected for the correction of the etiologic factors when conservative treatments are not effective. Many surgical methods have been reported such as eminectomy, eminence augmentation, condylotomy, and zygomatic arch down fracture technique. We performed the eminence augmentation through interpositional bone graft in chronic recurrent TMJ dislocation. This method leads to favorable postoperative result without recurrence and complication, so we report the case with related references.
