Reconstruction of Mutilating Hand with Pollicization and Anteromedial Thigh Perforator Free Flap - A Case Report - (무지 형성술 및 전내측 대퇴부 천공지 피판의 연속 술기를 이용한 수부 절단 손상의 재건 - 증례 보고 -)

  • Purpose: Thumb reconstruction plays most important role in hand injuries because total loss of a thumb constitutes about 40% disability in the hand. The reconstruction can be accomplished by pollicization, free toe-to-thumb transfer, wrap around procedure and lengthening extraction. However, we sometimes need consecutive or double free flaps in the reconstruction of mutilating hand injuries. Methods: We reconstructed a mutilating hand injury in a 54-years old man. Because of severe crushing injury of right thumb and index fingers, we reconstructed a thumb with pollicization using nearly amputated middle finger. Although it survived completely, the adjacent soft tissues which had been covered by fillet flap from the space past was necrosed on 1 month. We debrided the necrotic tissues and covered it with anteromedial thigh perforator free flap consecutively because he had an anatomical variation in branches of lateral femoral circumflex artery. Results: He had an uneventful postoperative course without any complication such as infection, dehiscence and flap necrosis. Three months later, he had undergone tenolysis and defatting procedure of flap site. He recovered the some amount of grip function and was happy with the result. Conclusion: In severe hand trauma including thumb amputation, thumb reconstruction using pollicization and perforator free flap could be an alternative option. It provides minimal donor site morbidity and an acceptable functional result.

An Antitumor Constituent of the Cultured Mycelia of Hydnum repandum (턱수염 버섯 배양(培養) 균사(菌絲)의 항종양(抗腫瘍) 성분(成分))

  • To investigate antitumor constituents of Hydnum repandum, which belongs to edible basidiomycetes, the mycelia separated from the carpophore were shake-cultured and subjected to hot water extraction. The extract was concentrated under vacuum and mixed with a three-fold volume of 95% ethanol to yield precipitates. The water soluble fraction of the precipitates was dialyzed and then lyophilized to yield a water soluble protein-polysaccharide fraction (= WPPF). This exerted antitumor activity against sarcoma 180 implanted in ICR mice. When administered i.p. at the dose level of 20mg/kg once daily for ten consecutive days, it showed an inhibition ratio of 54.3%. WPPF was found to be composed of a polysaccharide moiety (42% of WPPF) and a protein moiety (28% of WPPF) when determined by colorimetric method using anthrone reagent and Folin's phenol reagent. The polysaccharide moiety of WPPF was found to contain glucose (57.4%), mannose(19.3%), galactose (10.8%), xylose (6.8%), and fucose (5.7%), when the methanolysate of WPPF was analysed by GLC method.

Fault Detection and Diagnosis for Induction Motors Using Variance, Cross-correlation and Wavelets (웨이블렛 계수의 분산과 상관도를 이용한 유도전동기의 고장 검출 및 진단)

  • In this paper, we propose an approach to signal model-based fault detection and diagnosis system for induction motors. The current fault detection techniques used in the industry are limit checking techniques, which are simple but cannot predict the types of faults and the initiation of the faults. The system consists of two consecutive processes: fault detection process and fault diagnosis process. In the fault detection process, the system extracts the significant features from sound signals using combination of variance, cross-correlation and wavelet. Consequently, the pattern classification technique is applied to the fault diagnosis process to recognize the system faults based on faulty symptoms. The sounds generated from different kinds of typical motor's faults such as motor unbalance, bearing misalignment and bearing loose are examined. We propose two approaches for fault detection and diagnosis system that are waveletand-variance-based and wavelet-and-crosscorrelation-based approaches. The results of our experiment show more than 95 and 78 percent accuracy for fault classification, respectively.

Multiple Objection and Tracking based on Morphological Region Merging from Real-time Video Sequences (실시간 비디오 시퀀스로부터 형태학적 영역 병합에 기반 한 다중 객체 검출 및 추적)

  • In this paper, we propose an efficient method for detecting and tracking multiple moving objects based on morphological region merging from real-time video sequences. The proposed approach consists of adaptive threshold extraction, morphological region merging and detecting and tracking of objects. Firstly, input frame is separated into moving regions and static regions using the difference of images between two consecutive frames. Secondly, objects are segmented with a reference background image and adaptive threshold values, then, the segmentation result is refined by morphological region merge algorithm. Lastly, each object segmented in a previous step is assigned a consistent identification over time, based on its spatio-temporal information. The experimental results show that a proposed method is efficient and useful in terms of real-time multiple objects detecting and tracking.

Cardiovascular Manifestations and Clinical Course after Acute Carbon Monoxide Poisoning (급성 일산화탄소 중독에 의한 심혈관계 독성의 임상 양상 및 경과)

  • Purpose: The aim of this study was to evaluate the cardiovascular manifestations and clinical course in patients with acute carbon monoxide poisoning. Methods: A retrospective study was conducted over a 36 month period on consecutive patients who visited an emergency medical center and were diagnosed with acute carbon monoxide poisoning. A standardized data extraction protocol was performed on the selected patients. Results: A total of 293 patients were selected during the study period. Cardiac manifestations were observed in 35.2% (n=103) of the patients: hypotension in 11 patients (3.8%), ECG abnormalities in 44 patients (15.0%) and cardiac enzyme abnormalities in 103 patients (35.2%). Echo cardiography was performed on 56 patients with cardiac toxicity: 12 patients had abnormal results (5 patients with global hypokinesia and 7 patients with regional wall akinesia). Five patients died within 3 hours after ED admission, and the remaining patients were discharged alive. At 3 months after discharge, none of these patients had died.The SOFA scores in the severe cardiac toxicity group and non-severe cardiac toxicity group at the time of arrival were $2.53{\pm}2.29$ and $2.19{\pm}2.12$, respectively (p=0.860). Conclusion: Cardiovascular manifestations occur after acute CO poisoning at arateof 35.2%. Even those with severe cardiovascular toxicity recovered well within 10 days after admission. Therefore, the importance of cardiac toxicity after acute CO poisoning is not significant in itself in the clinical course, and the short-term prognosis of cardiac toxicity is unlikely to be unfavorable in acute CO poisoning.

Unsupervised Segmentation of Objects using Genetic Algorithms (유전자 알고리즘 기반의 비지도 객체 분할 방법)

  • The current paper proposes a genetic algorithm (GA)-based segmentation method that can automatically extract and track moving objects. The proposed method mainly consists of spatial and temporal segmentation; the spatial segmentation divides each frame into regions with accurate boundaries, and the temporal segmentation divides each frame into background and foreground areas. The spatial segmentation is performed using chromosomes that evolve distributed genetic algorithms (DGAs). However, unlike standard DGAs, the chromosomes are initiated from the segmentation result of the previous frame, then only unstable chromosomes corresponding to actual moving object parts are evolved by mating operators. For the temporal segmentation, adaptive thresholding is performed based on the intensity difference between two consecutive frames. The spatial and temporal segmentation results are then combined for object extraction, and tracking is performed using the natural correspondence established by the proposed spatial segmentation method. The main advantages of the proposed method are twofold: First, proposed video segmentation method does not require any a priori information second, the proposed GA-based segmentation method enhances the search efficiency and incorporates a tracking algorithm within its own architecture. These advantages were confirmed by experiments where the proposed method was success fully applied to well-known and natural video sequences.

Online Handwritten Digit Recognition by Smith-Waterman Alignment (Smith-Waterman 정렬 알고리즘을 이용한 온라인 필기체 숫자인식)

  • In this paper, we propose an efficient on-line handwritten digit recognition base on Convex-Concave curves feature which is extracted by a chain code sequence using Smith-Waterman alignment algorithm. The time sequential signal from mouse movement on the writing pad is described as a sequence of consecutive points on the x-y plane. So, we can create data-set which are successive and time-sequential pixel position data by preprocessing. Data preprocessed is used for Convex-Concave curves feature extraction. This feature is scale-, translation-, and rotation-invariant. The extracted specific feature is fed to a Smith-Waterman alignment algorithm, which in turn classifies it as one of the nine digits. In comparison with backpropagation neural network, Smith-Waterman alignment has the more outstanding performance.

Risk assessment and distribution characteristics of N-nitrosamines in drinking water treatment plants (나이트로사민류의 국내 정수장 분포 특성 및 위해성 평가)

  • A nationwide survey of 8 N-nitrosamines in finished water samples from drinking water treatment plants (DWTPs) in Korea was conducted. The samples were pre-treated by solid-phase extraction (SPE) and analyzed using a gas chromatography coupled with tandem mass spectrometry (GC-MS/MS). According to the study results, four N-nitrosamines (NDMA, NDEA, NMOR, NDBA) were detected for three consecutive years, NMEA and NPYR were only found in samples collected in 2013. Two of these N-nitrosamines, N-nitrosodimethylamine (NDMA) and N-nitrosodiethylamine (NDEA), have received attention and were the most commonly detected. The concentration of NDMA and NDEA in this study ranged from $0.002{\mu}g/L$ to $0.013{\mu}g/L$ and in $0.001{\mu}g/L$ to $0.008{\mu}g/L$, respectively. In comparison to studies performed in EPA(UCMR2), the concentrations of NDMA (from $0.002{\mu}g/L$ to $0.630{\mu}g/L$) and NDEA (from $0.005{\mu}g/L$ to $0.100{\mu}g/L$) observed in the this study were low.

Development of animal model for Bisphosphonates-related osteonecrosis of the jaw (BRONJ)

  • Background: The aim of this study is to develop a rat model of bisphosphonates-related osteonecrosis of the jaw (BRONJ) that would be verified with clinical, radiological and histological examination, and to confirm the influence of concurrent bisphosphonates and steroids use upon the occurrence and aggravation of BRONJ. Methods: Twenty seven rats were divided into 3 groups; Saline group (I), Zoledronate group (II), Zoledronate and Dexamethasone group (III). Rats got weekly intraperitoneal injection for 4 times and extraction of left maxillary and mandibular 1st, 2nd molars were followed. Consecutive injections were performed, and blood sampling for measurements of C-terminal crosslinked telopeptide of type I collagen and tartrate-resistant acid phosphate 5b rats were performed at the time of 2, 4 and 8 weeks. And then, rats were sacrificed and evaluated clinically, radiologically and histologically. Results: 12/18 (66.6 %) of experimental group were diagnosed as BRONJ. There was no significant difference in incidence between zoledronate alone group (ll) and concurrent use of zoledronate and dexamethasone group (lll). Conclusions: Concurrent use of bisphosphonates and steroids increase incidence of BRONJ compared to saline group (l). Zoledronate alone group (ll) and concurrent use of zoledronate and dexamethasone group (lll) shows same incidence of BRONJ. Based on this study, the rat treated with bisphosphonates and steroids can be considered a novel, reliable and reproducible model to understand pathology of BRONJ.

Gesture Recognition Using Stereo Tracking Initiator and HMM for Tele-Operation (스테레오 영상 추적 자동초기화와 HMM을 이용한 원격 작업용 제스처 인식)

  • In this paper, we describe gesture recognition algorithm using computer vision sensor and HMM. The automatic hand region extraction has been proposed for initializing the tracking of the tele-operation gestures. For this, distance informations(disparity map) as results of stereo matching of initial left and right images are employed to isolate the hand region from a scene. PDOE(positive difference of edges) feature images adapted here have been found to be robust against noise and background brightness. The KNU/KAERI(K/K) gesture instruction set is defined for tele-operation in atomic electric power stations. The composite recognition model constructed by concatenating three gesture instruction models including pre-orders, basic orders, and post-orders has been proposed and identified by discrete HMM. Our experimental results showed that consecutive orders composed of more than two ones are correctly recognized at the rate of above 97%.
