CAD/CAM fabricated complete denture using 3D face scan: A case report (3D face scan을 이용한 CAD/CAM 제작 의치 증례)

  • In the past, computer-aided design / computer-aided manufacturing (CAD/CAM) technology was the closed system that users had to use the components of only one manufacturer. At present, it has changed to the open system with the flexibility to select and use the components of various manufacturers' components according to their needs. Despite the development of dental materials and prostheses manufacturing methods, denture manufacturing has followed conventional manufacturing methods for nearly 100 years. However, studies on CAD/CAM fabricated denture have been recently carried out to overcome the disadvantages of conventional denture manufacturing. Some commercialized products using milling or 3D printing have already been applied clinically. This case report confirms the possibility of CAD/CAM dentures using 3D face scan and compared them to conventionally fabricated dentures.

  • Recently computer-aided technology has been widely used in dentistry. DENTCA$^{TM}$ CAD/CAM denture system (DENTCA Inc.), one of CAD/CAM systems for fabricating complete denture, tries to collect and store all of a patient's information at the first visit. This system aims to deliver denture at the second visit through utilizing the CAD/CAM software to access the stored data for designing the 3D denture model. The 3 dimensional (3D) denture will then be fabricated with 3D printer. Many case reports have evaluated clinical application of CAD/CAM system for fabricating complete dentures. This case report is about fabricating of complete dentures using DENTCA system and conventional method in same patient. With two cases, usefulness and limitation of DENTCA system could be evaluated.

  • In recent years, precision machining of the dental prosthesis by computer assisted system is becoming pervasive in clinical dentistry. Prosthesis fabricating system that is designed by computer software and made by computer devices is called as a CAD/CAM (Computer-Aided Design/Computer-Aided Manufacturing) system. By the use of dental CAD/CAM system, the improvement of marginal compatibility and mechanical properties in prosthesis can be obtained more effectively, an aesthetic quality by using new materials such as zirconia can be increased. Also, the restoration process can be simple and efficient, the production time can be shortened, the process of manufacture can be standardized, and the mass production is possible. What is clear is that these benefits are theoretically possible, but the dentist or dental technician must understand the CAD/CAM basic principles and limitations for obtaining the maximum advantages of CAD/CAM system. For this reason, this article will be presented about the basic principles of CAD/CAM system and the factors of error that might occur in the CAD/CAM process based on my empirical study.

  • Dental computer-aided design (CAD) and computer-aided manufacturing (CAM) technology have rapidly progressed over the past 30 years. The technology, which can be used in the dental laboratory, the dental office and the form of production centers, has become more common in recent years. This technology is now applied to inlays, onlays, crowns, fixed partial dentures, removable partial denture frameworks, complete dentures, templates for implant installation, implant abutments, and even maxillofacial prostheses. Dentists and dental technicians, who want to use these techniques, should have certain basic knowledge about that. This article gives an overview of CAD/CAM technologies, histories and how it applies in prosthetic dentistry.

  • 오늘날 제조업에 있어서는 시장의 다양화, 원가절감 및 리드타임의 단축이라는 대내외적 요구에 부응할 수 있는 기술 생산체재의 확립이 요구되고 있다. 이에 따라 설계 및 기술변경(Engineering Change)에따른 원가상승을 막기 위해 CAE(Computer aided engineering), CAD(Computer aided design)의 도입을 서두르게 되었고 CAD데이타의 효율적 이용에 의한 가공 및 생산적 측면에서 CAM(Computer aided manufacturing)의 도입도 검토하게 되었다. 본 논문은 이러한 개념의 발상동기와 이를 뒷받침하기 위한 한국 아이 비 엠이서 제공하는 하드웨어 및 소프트웨어를 소개하고 공장자동화를 위한 기술 생산 통합시스템의 구축이라는 측면에서 아이 비 엠이 추구하는 방향을 검토하고 있다.

  • Nowadays, digital dentistry is generally applied to prosthodontics with fabrication of inlays or any other fixed prostheses by utilizing CAD/CAM (computer-aided design/computer-aided manufacturing) technology and intraoral scanner. However, in fabricating removable prosthesis, there are some limitations for digital technology to substitute conventional casting method. Therefore, approaching removable prostheses fabrication with CAD/CAM technology would be a meaningful trial. In this case report, Kennedy class III mandibular edentulous patient who was in need of increasing the vertical dimension of occlusion was treated with removable partial denture using CAD and rapid prototyping technique. Surveying and designing the metal framework of the partial denture was performed with CAD, and sacrificial plastic pattern was fabricated with rapid prototyping technique. During the follow up period of nine months, the removable partial denture has provided satisfactory results in esthetics and function.

  • In patients with fully edentulous jaw, treatment of complete dentures should be carried out in many stages when following the conventional methods. Therefore there were disadvantages such as multiple visits to dental clinic is inevitable. In addition, errors caused by polymerization shrinkage, which happens during the fabrication of denture, and difficulties in reproduction of damaged or lost denture were considered as disadvantages. But nowadays, computer-aided design and computer-aided manufacturing (CAD/CAM) system is widely used in dentistry and it has begun to expand its spectrum in manufacturing complete dentures. Using CAD/CAM system to fabricate complete dentures can reduce the number of patient's visit and clinical chair time, since taking impression, recording jaw relation, and selection of artificial teeth are performed at the same time during the first visit, and delivering of dentures during the second visit is possible. In addition, because 3D-Printing technology is used, errors by polymerization shrinkage can be reduced. Among the companies that fabricate complete dentures using CAD/CAM system, DENTCA CAD/CAM denture (DENTCA Inc., Los Angeles, CA, USA) is the most commercialized company. In this case, we treated patients of complete dentures using conventional complete denture method and DENTCA CAD/CAM denture system in the same patient. We would like to report this case because we have achieved good results not only in functional aspects of pronunciation, chewing, and swallowing but also in aesthetic aspects.

  • The implant-supported fixed dental prosthesis in irradiated maxilla needs meticulous treatment planning due to low bone healing capacity. All-on-4 concept implantation can reduce the number of implants to be placed avoiding bone grafting procedure. Conventionally, prefabricated angled abutments for tilted implants have been used. However, in this case, it was replaced with computer-aided design and computer-aided manufacturing (CAD/CAM) abutment. This case report described all-on-4 concept implantation and fabrication of CAD/CAM zirconia fixed dental prostheses using CAD/CAM titanium abutments.

  • Computer aided design and manufacturing (CAD/CAM) technology today is the standard in manufacturing industry. The application of the CAD/CAM technology, together with the emerging 3D medical images based virtual surgical planning (VSP) technology, to craniomaxillofacial reconstruction has been gaining increasing attention to reconstructive surgeons. This article illustrates the components, system and clinical management of the VSP and CAD/CAM technology including: data acquisition, virtual surgical and treatment planning, individual implant design and fabrication, and outcome assessment. It focuses primarily on the technical aspects of the VSP and CAD/CAM system to improve the predictability of the planning and outcome.

Full mouth rehabilitation utilizing computer guided implant surgery and CAD/CAM (Computer guided implant surgery와 CAD/CAM을 활용한 전악 수복 증례)

  • Computer aided design and manufacturing and implant surgery using a guide template improve restoration-driven implant treatment procedures. This case utilized those digital technologies to make definitive prostheses for a patient. According to the work flow of digital dentistry, cone beam computed tomography established the treatment plan, which was followed to make the guide template for implant placement. The template guided the implants to be installed as planned. The customized abutments and surveyed fixed restorations were digitally designed and made. The metal framework of the removable partial denture was cast from resin pattern using an additive manufacturing technique, and the artificial resin teeth were replaced with the zirconia onlays for occlusal stability. These full mouth rehabilitation procedures provided functionally and aesthetically satisfactory results for the patient.
