Accuracy evaluation of dental models manufactured by CAD/CAM milling method and 3D printing method

  • PURPOSE. To evaluate the accuracy of a model made using the computer-aided design/computer-aided manufacture (CAD/CAM) milling method and 3D printing method and to confirm its applicability as a work model for dental prosthesis production. MATERIALS AND METHODS. First, a natural tooth model (ANA-4, Frasaco, Germany) was scanned using an oral scanner. The obtained scan data were then used as a CAD reference model (CRM), to produce a total of 10 models each, either using the milling method or the 3D printing method. The 20 models were then scanned using a desktop scanner and the CAD test model was formed. The accuracy of the two groups was compared using dedicated software to calculate the root mean square (RMS) value after superimposing CRM and CAD test model (CTM). RESULTS. The RMS value ($152{\pm}52{\mu}m$) of the model manufactured by the milling method was significantly higher than the RMS value ($52{\pm}9{\mu}m$) of the model produced by the 3D printing method. CONCLUSION. The accuracy of the 3D printing method is superior to that of the milling method, but at present, both methods are limited in their application as a work model for prosthesis manufacture.

Biomechanical behavior of CAD/CAM cobalt-chromium and zirconia full-arch fixed prostheses

  • PURPOSE. To verify the influence of computer-aided design/computer-aided manufacturing (CAD/CAM) implant-supported prostheses manufactured with cobalt-chromium (Co-Cr) and zirconia (Zr), and whether ceramic application, spark erosion, and simulation of masticatory cycles modify biomechanical parameters (marginal fit, screw-loosening torque, and strain) on the implant-supported system. MATERIALS AND METHODS. Ten full-arch fixed frameworks were manufactured by a CAD/CAM milling system with Co-Cr and Zr (n=5/group). The marginal fit between the abutment and frameworks was measured as stated by single-screw test. Screw-loosening torque evaluated screw stability, and strain analysis was explored on the implant-supported system. All analyses were performed at 3 distinct times: after framework manufacturing; after ceramic application in both materials' frameworks; and after the spark erosion in Co-Cr frameworks. Afterward, stability analysis was re-evaluated after 106 mechanical cycles (2 Hz/150-N) for both materials. Statistical analyses were performed by Kruskal-Wallis and Dunn tests (α=.05). RESULTS. No difference between the two materials was found for marginal fit, screwloosening torque, and strain after framework manufacturing (P>.05). Ceramic application did not affect the variables (P>.05). Spark erosion optimized marginal fit and strain medians for Co-Cr frameworks (P<.05). Screw-loosening torque was significantly reduced by masticatory simulation (P<.05) regardless of the framework materials. CONCLUSION. Co-Cr and Zr frameworks presented similar biomechanical behavior. Ceramic application had no effect on the biomechanical behavior of either material. Spark erosion was an effective technique to improve Co-Cr biomechanical behavior on the implant-supported system. Screw-loosening torque was reduced for both materials after masticatory simulation.

  • 하악 완전 무치악 환자의 보철수복에 있어 임플란트 식립 후 피개의치 제작은 첫 번째 치료 옵션으로 고려된다. 코너스 타입 부착장치를 이용한 피개의치는 견고한 지지를 제공하고 cross arch stabilization을 이루도록 하여 교합압을 균일하게 분산시키며, 의치에 의한 이차 고정으로 장기간의 안정성을 제공한다. 최근 소개된 팔라듐-은(Pd-Ag) 합금(Innovium)은 팔라듐, 은, 금 등의 귀금속 함량이 72% 이상인 CAD-CAM 시스템 전용 귀금속 합금으로 코너스 타입 부착장치를 제작하는데 적합한 물성을 가지고 있다. 본 증례에서는 완전 무치악 환자에서 상악에는 통상적인 총의치, 하악에는 임플란트를 4개 식립한 후에 코너스 타입 부착장치 피개의치 수복을 하였다. CAD-CAM 시스템을 이용하여 팔라듐-은 합금(Innovium)으로 제작된 코너스 타입 부착장치를 장착하였으며 이를 통해 의치의 유지와 안정을 향상시켰으며 만족할 만한 결과를 얻었기에 보고하고자 한다.

전치부 개방교합을 보이는 법랑질형성부전증 환자의 CAD/CAM system을 이용한 전악 수복 증례 (Full-mouth rehabilitation in an amelogenesis imperfecta patient with anterior open bite using CAD/CAM system)

  • 법랑질 형성 부전증은 유전적인 결함으로 인해 구조적으로 약한 법랑질이 형성되는 질환이다. 이들 환자들은 이른 나이부터 진행되는 법랑질 마모에 의한 시린 증상과 비심미적인 치아를 주소로 치과에 내원하게 되며, 성장기 이후에는 전악 보철 수복을 통해 치아의 기능성과 심미성을 회복해 주게 된다. 법랑질 형성 부전증 환자에서 보여지는 전치부 개방 교합은, 구치의 교합면 마모 및 보상성 맹출에 의한 수직적 수복 공간 문제와 결부되어 보철 치료를 어렵게 하는 요인이 된다. 따라서 전치 길이의 결정 및 교합 고경의 거상 여부, 전방유도의 설정은 신중히 결정되어야 한다. 근래에는 Computer aided design-computer aided manufacturing (CAD/CAM) 기술을 이용하여 진단 및 최종 수복으로의 이행이 용이해 졌다. 본 증례에서는 전치부 개방교합을 가지고 있는 법랑질 형성 부전증 환자에서, CAD/CAM을 이용한 전악 수복을 시행한 후, 양호한 경과를 보이고 있기에 이를 보고하고자 한다.

How adjustment could affect internal and marginal adaptation of CAD/CAM crowns made with different materials

  • PURPOSE. Recently introduced hybrid and reinforced glass ceramic computer-aided design/computer-aided manufacturing (CAD/CAM) materials have been used for full-coverage restorations. However; the effect of adjustment and type of materials on internal and marginal adaptation are unknown. This study aimed to evaluate and compare the marginal and internal adaptations of crowns made of three different CAD/CAM materials before and after adjustment. MATERIALS AND METHODS. One acrylic resin maxillary first molar was prepared and served as the master die. Thirty-six restorations were fabricated using CAD/CAM system (CEREC Omnicam, MCXL) with three materials including lithium disilicate (IPS e.max CAD), zirconia-reinforced lithium silicate (Suprinity), and hybrid ceramic (Enamic). Internal and marginal adaptations were evaluated with the reference point matching technique before and after adjustment. The data were analyzed using mixed ANOVA considering α=.05 as the significance level. RESULTS. The effect of adjustment and its interaction with the restoration material were significant for marginal, absolute marginal, and occlusal discrepancies (P<.05). Before adjustment, Suprinity had lower marginal discrepancies than IPS e.max CAD (P=.18) and Enamic (P=.021); though no significant differences existed after adjustment. CONCLUSION. Within the limitations of this study, crowns fabricated from IPS e.max CAD and Suprinity resulted in slightly better adaptation compared with Enamic crowns before adjustment. However, marginal, axial, and occlusal discrepancies were similar among all materials after the adjustment.

  • Computer-aided engineering (CAE) is the broad usage of computer software to aid in engineering tasks. It includes computer-aided design (CAD), computer-aided analysis (CAA), computer-integrated manufacturing (CIM), computer-aided manufacturing (CAM), material requirements planning (MRP), and computer-aided planning (CAP). In this study, the stress of mold analyzed using CAE technique. Punch loads were same difference between 0.5 % and 1.0 % of clearance, but punch load was decreased according to increasing of clearance. Punch load of pre-piercing process worked a little smaller than piercing process. Therefore, the hole of fine blanking process is also more efficient to manufacture the true size after pre-piercing.

구치부 지지 소실 및 무너진 교합관계를 보이는 환자에서 Dental CAD-CAM system을 활용한 완전 구강 회복 증례 (Full-mouth rehabilitation of a patient with loss of posterior support and collapsed occlusion utilizing dental CAD-CAM system)

  • 다수 치아의 상실 상태로 장시간 지속시 구치부 지지의 부족 및 치아들의 과정출이 발생되어 교합 부조화, 수직고경 상실 및 기능 장애 등의 심각한 문제가 초래된다. 본 증례는 장기간 다수치아 결손으로 인하여 대합치가 정출하면서 교합평면의 붕괴가 일어난 환자의 완전 구강 회복에 관한 증례이다. 상기 환자는 서울대학교 치과병원 치과보철과로 내원한 68세 남환으로 오래된 상악 의치를 빼다가 치아가 함께 발거되었으며, 하악 전치부가 상악 전치부와 강하게 닿아서 불편함을 호소하며 내원하였다. 여러가지 평가를 통해 수직고경을 4 mm 거상하였으며, 무치악부는 진단 및 치료계획시 설계한 최종 보철 수복물 제작을 고려하여 정확한 임플란트 식립을 위해 CAD-CAM (Computer-aided design-computer-aided manufacturing) 기술을 이용한 Computer guided implant surgery를 시행하고, 임플란트 고정성 보철 수복으로 진행하였고, 환자는 저작, 기능 및 심미 모두 큰 개선에 만족하였다.

Influence of different universal adhesives on the repair performance of hybrid CAD-CAM materials

  • Objectives: The aim of this study was to investigate the microshear bond strength (${\mu}SBS$) of different universal adhesive systems applied to hybrid computer-aided design/computer-aided manufacturing (CAD-CAM) restorative materials repaired with a composite resin. Materials and Methods: Four types of CAD-CAM hybrid block materials-Lava Ultimate (LA), Vita Enamic (VE), CeraSmart (CS), and Shofu Block HC (SH)-were used in this study, in combination with the following four adhesive protocols: 1) control: porcelain primer + total etch adhesive (CO), 2) Single Bond Universal (SB), 3) All Bond Universal (AB), and 4) Clearfil Universal Bond (CU). The ${\mu}SBS$ of the composite resin (Clearfil Majesty Esthetic) was measured and the data were analyzed using two-way analysis of variance and the Tukey test, with the level of significance set at p < 0.05. Results: The CAD-CAM block type and block-adhesive combination had significant effects on the bond strength values (p < 0.05). Significant differences were found between the following pairs of groups: VE/CO and VE/AB, CS/CO and CS/AB, VE/CU and CS/CU, and VE/AB and CS/AB (p < 0.05). Conclusions: The ${\mu}SBS$ values were affected by hybrid block type. All tested universal adhesive treatments can be used as an alternative to the control treatment for repair, except the AB system on VE blocks (the VE/AB group). The ${\mu}SBS$ values showed variation across different adhesive treatments on different hybrid CAD-CAM block types.

  • The treatment of craniofacial anomalies has been challenging as a result of technological shortcomings that could not provide a consistent protocol to perfectly restore patient-specific anatomy. In the past, wax-up and impression-based maneuvers were implemented to achieve this clinical end. However, with the advent of computer-aided design and computer-aided manufacturing (CAD/CAM) technology, a rapid and cost-effective workflow in prosthetic rehabilitation has taken the place of the outdated procedures. Because the use of implants is so profound in different facets of restorative dentistry, their placement for craniofacial prosthesis retention has also been widely popular and advantageous in a variety of clinical settings. This review aims to effectively describe the well-rounded and interdisciplinary practice of craniofacial prosthesis fabrication and retention by outlining fabrication, osseointegrated implant placement for prosthesis retention, a myriad of clinical examples in the craniofacial complex, and a glimpse of the future of bioengineering principles to restore bioactivity and physiology to the previously defected tissue.

CAD/CAM으로 제작된 지르코니아 코어의 지대치 형태에 따른 변연 및 내면 적합도에 관한 연구 (Marginal and internal fit according to the shape of the abutment of a zirconia core manufactured by computer-aided design/computer-aided manufacturing)

  • Background: In this study, zirconia copings were fabricated by setting clinically acceptable inner values for prostheses using computer-aided design/computer-aided manufacturing (CAD/CAM). The processed copings were evaluated for the marginal and internal fit of each abutment shape with a CAD program using the silicone replica technique. Methods A total of 20 copings was produced by selecting models commonly used in clinical practice. After injecting the sample, the minimum thickness, internal adhesion interval, and distance to the margin line were set to 0.5, 0.05, and 1.00 mm using a dental CAD program, respectively. It was measured using a 2D section function in a three-way program of the silicon replication technology. Although the positions and number of measurements of the anterior and posterior regions differed, nine parts of each pre-tube were designated and measured by referring to a previous study to compare the two samples. Results As a result, the average margin of the mesial, distal, and buccal (labial) surfaces was 59.90 ㎛ in the anterior region and 60.40 ㎛ in the posterior region. The mean axial wall margin was 67.25 ㎛ in the anterior region and 69.25 ㎛ in the posterior region. In occlusion, the anterior teeth (77.70 ㎛), posterior teeth (77.60 ㎛), and both anterior and posterior regions were within the clinically acceptable range. Conclusion The edge and inner fit of zirconia coping manufactured using the CAD/CAM system showed clinically applicable results. To reduce errors and increase accuracy, materials and machine errors that affect the manufacture of prosthetics should be investigated. Based on our results, the completeness of prosthetics could increase if the inner value and characteristics of the material are adjusted when applied in clinical practice.
