소아 청소년기에서 고혈압 (Hypertension in children and adolescents)

  • Hypertension is a major risk factor for myocardial infarction, stroke, and cardiovascular mortality in adults; its treatment reduces the risk of cardiovascular events. In recent times, attention is being paid to monitoring of blood pressure from childhood to adulthood. Childhood hypertension is associated with hypertension in later life, and early intervention is important. In the Korean socioeconomic background, a rapid increase is observed in the number of obesity cases and the rate of increase in the incidence of obesity is more in childhood. The strong association of high blood pressure with obesity and the marked increase in the incidence of childhood obesity indicate that both hypertension and prehypertension are becoming significant health issues. In this comprehensive review, we acquaint the clinician with the available literature on childhood hypertension to provide recommendations for the diagnosis, evaluation, and treatment of hypertension based on the available evidence and consensus in Korean clinical conditions.

Minimally Invasive Redo Mitral Valve Replacement under Fibrillatory Arrest in a Patient with a Calcified Aorta and Patent Previous Bypass Grafts

  • A 73-year-old woman who underwent combined bioprosthetic mitral valve replacement, tricuspid ring annuloplasty, and coronary artery bypass grafting 12 years previously visited our clinic due to aggravated dyspnea caused by structural valve deterioration of the mitral prosthesis. Because aortic or femoral artery cannulation and cross-clamping would have a high risk of stroke owing to severe calcification of the ascending aorta and ilio-femoral vessels, and because there was a risk of redo sternotomy due to the patent bypass grafts, a comprehensive approach including axillary artery cannulation, a minimally invasive right thoracotomy approach, and a clampless hypothermic fibrillatory arrest technique was used during redo mitral valve replacement.

Analysis of the Relationships between Esophageal Cancer Cases and Climatic Factors Using a Geographic Information System (GIS): a Case Study of Ardabil province in Iran

  • Esophageal cancer is a mjaor health problems in many parts of the world. A geographical information system (GIS) allows investigation of the geographical distribution of diseases. The purpose of the present study was to explore the relationship between esophageal cancer and effective climatic factors using GIS. The dispersion distribution and the relationship between environmental factors effective on cancer were measured using Arc GIS. The highest degree of spread was in Germi town and the least was in Ardabil city. There was a significant relationship between effective environmental factors and esophageal cancer in Ardabil province. The results indicated that environmental factors probably are influential in determining the incidence of esophageal cancer. Also, these results can be considered as a window to future comprehensive research on esophageal cancer and related risk factors.

  • Performance-based design concepts require the next generation of codes. To implement the main concepts several design methodologies have been proposed. This paper reviews the framework of Korea Seismic Code and shows necessary modification for adoption of appropriate design methods. The selection of design earthquake levels with the introduction of risk factor is discussed for proper risk levels for all earthquake hazards. Displacement-based design, energy-based design, comprehensive design, and force-strength design methods are reviewed as one of possible next generation design methods. This paper proposes the direction of reconstruction for design earthquake levels with performance matrix, introduction of new design methods, and emphasis on non- structural components.

  • The safety of the present railway system has been established through a lot of experience from the railway accident. But nowaday scientific safety assessment methods are exactly required to improve the safety of railway system. In this study, we focused on the methodology of probabilistic risk assessment, which has been developed mainly in the field of nuclear power industry, and considered the process to adopt this method to railway system in order to establish a scientific and comprehensive way of safety assessment.

Determination of THRs - A Practical Approach for Manufacturers According to EN50129

  • The paper will outline how hazard identification and risk evaluation can effectively be performed to obtain Tolerable Hazard Rates (THR). As a target group manufacturers are addressed, who face the situation, that for a generic application THRs are needed for compliance with EN 50129 [1]. Focusing on functional hazards this paper shows a possible hazard log and the relevant analysis methods. The terms safety barrier and barrier function will be introduced and used instead of the term "safety function". As functional hazards and barrier functions depend on each other, emphasis will be put on a comprehensive and detailed definition of barrier functions and the usage of function lists. By using detailed and complete hazard and barrier function definitions THRs can be obtained while at the same time the approach becomes clear how the hazard rates (HR) will be established.

  • As the speed of technological changes increase with the investment requirements steadily expanding, private firms and government-funded research institutes experience similar pressures with respect to the necessity of risk reduction and technological alliances in R&D activities. This paper first attempts to review previous research in managing R&D projects with large, risky, and long-term investment requirements. Our primary focus is placed on the "fuzzy front-end" (FFE) projects with uncertainties at the investigation and planning stages. We analyze various elements that create FFE conditions, classify them into basic constructs, and suggest tools and methods to deal with FFE conditions. The findings suggest that both initial FFE conditions and the effectiveness of FFE management affect the performance of the project later on, and thus, especially for large projects, we must deal with FFE seriously in a comprehensive manner. We utilize in-depth panel interviews and case studies to approach the research questions.

Sacrococcygeal Teratoma : A Tumor at the Center of Embryogenesis

  • Sacrococcygeal teratoma (SCT) is an extragonadal germ cell tumor (GCT) that develops in the fetal and neonatal periods. SCT is a type I GCT in which only teratoma and yolk sac tumors arise from extragonadal sites. SCT is the most common type I GCT and is believed to originate through epigenetic reprogramming of early primordial germ cells migrating from the yolk sac to the gonadal ridges. Fetal SCT diagnosed in utero presents many obstetrical problems. For high-risk fetuses, fetal interventions (devascularization and debulking) are under development. Most patients with SCT are operated on after birth. Complete surgical resection is the key for tumor control, and the anatomical location of the tumor determines the surgical approaches. Incomplete resection and malignant histology are risk factors for recurrence. Approximately 10-15% of patients have a tumor recurrence, which is frequently of malignant histology. Long-term surveillance with monitoring of serum alpha fetoprotein and magnetic resonance imaging is required. Survivors of SCT may suffer anorectal, urological, and sexual sequelae later in their life, and comprehensive evaluation and care are required.

  • Smart city technologies such as transportation and parking systems make our daily lives more efficient and intelligent. However, it is impossible to implement a smart mobility system without analyzing the individual's behavior toward the new technology. This research study attempts to develop a framework for predicting smart mobility antecedents using SEM in primary data analysis. The Technology Acceptance Model (TAM) was the conceptual foundation for this study. To achieve the objectives of the study, one thousand five hundred and twelve effective questionnaires were collected and analyzed using Smart PLS 3.3. The results show that perceived usefulness, perceived ease of use, and perceived risk significantly affect attitudes towards adopting smart mobility systems. Our study provides a comprehensive framework to understand individual-level smart city technology adoption. This study offers implications for policymakers to update existing policies concerning road technology.

Probabilistic seismic risk assessment of a masonry tower considering local site effects

  • A comprehensive probabilistic seismic hazard analysis was carried out in Istanbul to examine the seismotectonic features of the region. The results showed that earthquakes can trigger one another, resulting in the grouping of earthquakes in both time and space. The hazard analysis utilized the Poisson model and a conventional integration technique to generate the hazard curve, which shows the likelihood of ground motion surpassing specific values over a given period. Additionally, the study evaluated the impact of seismic hazard on the structural integrity of an existing masonry tower by simulating its seismic response under different ground motion intensities. The study's results emphasize the importance of considering the seismotectonic characteristics of an area when assessing seismic hazard and the structural performance of buildings in seismic-prone regions.
