Comparison of ondansetron and granisetron for antiemetic prophylaxis in maxillofacial surgery patients receiving general anesthesia: a prospective, randomised, and double blind study

  • Objectives: To compare the efficacy of intravenous ondansetron (4 mg, 2 mL) and granisetron (2 mg, 2 mL) for preventing postoperative nausea and vomiting (PONV) in patients during oral and maxillofacial surgical procedures under general anesthesia. Materials and Methods: A prospective, randomized, and double blind clinical study was carried out with 60 patients undergoing oral and maxillofacial surgical procedures under general anesthesia. Patients were divided into two groups of 30 individuals each. Approximately two minutes before induction of general anesthesia, each patient received either 4 mg (2 mL) ondansetron or 2 mg (2 mL) granisetron intravenously in a double blind manner. Balanced anesthetic technique was used for all patients. Patients were assessed for episodes of nausea, retching, vomiting, and the need for rescue antiemetic at intervals of 0-2, 3, 6, 12, and 24 hours after surgery. Incidence of complete response and adverse effects were assessed at 24 hours postoperatively. Data was tabulated and subjected to statistical analysis using the chi-square test, unpaired t-test, or the Mann-Whitney U-test as appropriate. A P-value less than 0.05 was considered statistically significant. Results: There was no statistically significant difference between the two groups for incidence of PONV or the need for rescue antiemetic. Both study drugs were well tolerated with minimum adverse effects; the most common adverse effect was headache. The overall incidence of complete response in the granisetron group (86.7%) was significantly higher than the ondansetron group (60.0%). Conclusion: Granisetron at an intravenous dose of 2 mg was found to be safe, well tolerated, and more effective by increasing the incidence of complete response compared to 4 mg intravenous ondansetron when used for antiemetic prophylaxis in maxillofacial surgery patients receiving general anesthesia. Benefits of granisetron include high receptor specificity and high potency, which make it a valuable alternative to ondansetron.

  • Idiopathic pulmonary alveolar proteinosis (PAP) is a rare disorder characterized by surfactant component accumulation in the alveolar space. Idiopathic PAP has recently been recognized as a autoimmune disease of impaired alveolar macrophage function caused by autoantibodies against granulocyte-macrophage colony-stimulating factor (GM-CSF). While whole lung lavage has been the standard treatment, not every patient shows a complete response. Subcutaneous injection or inhalation of GM-CSF is another promising treatment option for PAP. A 45-year-old patient visited our hospital for dyspnea, he was diagnosed as PAP and underwent whole lung lavage. Eighteen months later, the patient had not achieved complete remission in despite of initial response. After then he was administered with GM-CSF (5 ${mu}g/kg/day$, subcutaneous injection) for fivetimes a week during 2 months. Nine months later, the abnormal shadows in high-resolution computed tomography (HRCT) decreased and the patient fully recovered in forced vital capacity. After 60 months, the HRCT scan showed complete remission of PAP.

The Neutrophil to Lymphocyte Ratio has a High Negative Predictive Value for Pathologic Complete Response in Locally Advanced Breast Cancer Patients Receiving Neoadjuvant Chemotherapy

  • Background: The neutrophil-to-lymphocyte ratio (NLR) is a strong predictor of mortality in patients with pancreatic, colorectal, lung, gastric cancer and renal cell carcinoma. The aim of this study was to determine the relationship between pathological complete response (pCR) and pretreatment NLR values in locally advanced breast cancer (BC) patients receiving neoadjuvant chemotherapy (NACT). Materials and Methods: Datawere collected retrospectively from the Akdeniz University School of Medicine Database for locally advanced BC patients treated with NACT between January 2000-December 2013. Results: A total of 78 patients were analyzed. Sixteen (20%) patients achieved pCR. Estrogen receptor (ER) positivity was lower in pCR+ than pCR-cases (p=0.011). The median NLR values were similar in both arms. The optimum NLR cut-off point for BC patients with PCR+ was 2.33 (AUC:0.544, 95%CI [0.401-0.688], p=0.586) with sensitivity, specificity, positive predictive value and negative predictive value (NPV) of 50%, 51,6%, 21,1%, and 80%, respectively. Conclusions: This study showed no relationship between the pCR and pretreatment NLR values. Because of a considerable high NPV, in the patients with higher NLR who had luminal type BC in which pCR is lower after NACT, such treatment may not be recommended.

Self-rated Health and Global Network Position: Results From the Older Adult Population of a Korean Rural Village

  • Background: Since the mid-20th century, the ways in which social networks and older adults' health are related have been widely studied. However, few studies investigate the relationship between self-rated health and position in a complete social network of one entire Korean rural village. This study highlights use of a complete network in health studies. Methods: Using the Korean Social Life and Health Project, the population-based data of adults aged 60 or older and their spouses in one myeon in Ganghwa island (Ganghwa-gun, Incheon, Korea), Incheon, Korea (with a 95% response rate), this study built a $1,012{\times}1,012$ complete social network matrix of the village. The data were collected from 2011 to 2012, and 731 older adults were analyzed. The ordered logistic models to predict self-rated health allowed us to examine social factors from socio-demographic to individual community activities, ego-centered network characteristics, and positions in a complete network. Results: From the network data, 5 network components were identified. Even after controlling for all other factors, if a respondent belonged to a segregated component, the probability that he or she reported good health dropped substantially. Additionally, high in-degree centrality was connected to greater self-rated health. Conclusion: This finding highlights the importance of social position not only from the respondents' point of view but also from the entire village's perspective. Even if a respondent maintained a large social network, when all of those social ties belonged to a segregated group in the village, the respondent's health suffered from this segregation.

  • Purpose: Accurate assessment of the lesion after treatment of patients with bone lymphoma is difficult. In this patient who demonstrated complete remission after chemotherapy, the regions of fluorine-18 fluorodeoxyglucose $(^{18}FFDG)PET$ uptake diminished more rapidly fellowing therapy, indicating a complete response at much earlier stage than did Magnetic Resonance Imaging (MRI) or CT based findings. With the conventional methods, such as MRI and CT, It was difficult to assess whether the residual tumor tissue was viable or not. Decision to complete response is very important in patients with lymphoma to plan the further treatment. We experienced a patient with primary lymphoma of bone who revealed complete response to chemotherapy on $^{18}FFDGPET$ while CT showed persistent destructive bone lesion. Thus, $^{18}FFDGPET$ study after therapy may be superior to CT in the evaluation of response to treatment in primary lymphoma of bone.

  • Lymphomatoid granulomatosis, polymorphic reticulosis, midline malignant reticulosis, or lethal midline granuloma have similar histologic features of an angiocentric and angiodestructive lymphoreticular proliferation representing the same nosologic entity. The term 'angiocentric immunoproliferative lesion' (AIL) was proposed by Jaffe, Costa, and Martin. The malignant potential of AILs is originally uncertain, but the facts that AILs have a relatively short survival, and most of them usually progress to an overt malignant lymphoma and survival is inversely proportional to the large, atypical lymphoreticular cells suggest that AILs are malignane. We experienced 17 AILs in nose during 16-year period and retrogradely analized them to recognize the problems in the diagnosis and to establish the further therapeutic strategies. The results were as follows; Twelve of total 17 patients who had diagnosed as histologic grade 1 and 2 had received radiation therapy as an initial treatment and the complete response rate was 91.7%(11/12), but 6 out of 11 had local recurrence and 5 had progress to overt maligant lymphoma within 2years. Three patients with the histologic grade 3 and 2 with unclear histologic grade had received CHOP chemotherapy and there was 1 case with complete response. Two patients with unclear histologic grade had been proved to be malignant histiocytosis by bone marrow biopsy during the clinical course. The overall duration of survival was 2 - 119 months and the 5-year survival rate was 71.9%. And the achievement of initial complete response was the most important prognostic factor of overall survival(P=0.006). Our results suggest that the treatment strategy according to the histologic grading scheme is efficient and more aggressive combination chemotherapy may be necessary to achieve complete response in patients with histologic grade III and II, because most of them progress to overt malignant lymphoma during its process.

Treatment outcome of nasal natural killer/T-cell lymphoma

  • Purpose: To evaluate the radiotherapy treatment outcome of patients in stage IE and IIE nasal natural killer/T-cell lymphoma. Materials and Methods: From August 1999 to August 2009, 46 patients with stage IE and IIE nasal natural killer/T-cell lymphoma were treated by definitive radiotherapy and chemotherapy. 33 patients were treated with chemotherapy followed by radiotherapy (CT + RT) and they received 50.4 Gy in 28 fractions. 13 patients were treated with concurrent chemoradiotherapy (CCRT) and they received 40 Gy in 20 fractions. Results: The median follow-up period was 4.6-137.6 months (median, 50.2 months) for all patients. The 4-year overall survival was 68.6% and 4-year disease free survival (DFS) was 61.9%. The 4-year locoregional recurrence free survival was 65.0%, and 4-year distant metastasis free survival (DMFS) was 66.2%. For patients treated with CT + RT, 15 patients (45.5%) achieved complete response after chemotherapy, and 13 patients (39.4%) achieved partial response. 13 patients (81.8%) achieved complete response after radiotherapy, and 6 patients (18.2%) achieved partial response. For patients treated with CCRT, 11 patients (84.6%) achieved complete response, and one patient (7.7%) achieved partial response. In univariate analysis, presence of cervical lymph node metastasis was only significant prognostic factor for DFS and DMFS. Conclusion: This study did not show satisfactory overall survival rate and disease free survival rate of definitive radiotherapy and chemotherapy for stage IE and IIE nasal natural killer/T-cell lymphoma. For patients with cervical lymph node metastasis, further investigation of new chemotherapy regimens is necessary to reduce the distant metastasis.

The Effect of Cognitive Response on Behavioral Response of Consumers to Sold Out Products On-line Shopping Malls (인터넷 쇼핑몰 품절 경험 후 인지적 반응이 행동적 반응에 미치는 영향)

  • The purpose of this study is to examine the cognitive responses and the corresponding behavior responses of consumers who have experiences in not being able to buy a product in an online shopping mall due to it being sold-out. Responses were gathered from 526 consumers between the ages of 20 to 40 years residing in a metropolitan area. Each person surveyed had experienced a situation in which a product that they wanted to purchase from an online shopping mall was sold-out. SPSS 18.0 was used to perform frequency analysis, factor analysis, reliability analysis, and regression analysis. The first set of results of this study showed positive responses of quality, discernment, scarcity, but also negative cognitive responses of careless management, manipulation of shopping mall management, and common taste. In negative cognitive responses, sold-out situations caused consumers inconvenience. The second set of results revealed that quality, discernment, and careless management had a significant effect on product replacement (Substitute, S); likewise, factors such as quality, discernment, careless management, manipulation by shopping mall designers, and common taste had a significant effect on the delay of purchasing decisions (Delay, D). Scarcity, careless management, manipulation by shopping mall designers, and common taste also demonstrated significant influence on the incomplete leaving of stores (Incomplete Leave, L1), while discernment, scarcity, careless management, manipulation by shopping mall designers, and common taste had a significant influence on the complete leaving of stores (Complete Leave, L2). Previous studies have examined the behavioral response topics of substitute, delay, and leave. These study results suggest that product sellouts at online shopping malls did not have a solely negative effect on consumers. It actually had a positive effect in terms of discernment, scarcity, and the perception of quality of sold-out products. Furthermore, both positive and negative cognitive responses had various effects on behavioral responses.

Radiation Therapy for Brain Metastases (전이성 뇌종양의 방사선치료 성적)

  • One hundred and twenty patients with brain metastases were seen and evaluated in the Dept. of Therapeutic Radiology, Seoul National University Hospital between 1979 and 1983. Of these, 90 Patients received whole brain irradiation with 2,000 ra4 in 1 week or 3,000 rad in 2 weeks for Palliative Purpose and 30 patients failed to complete the planned treatment. Carcinoma of the lung(44 cases), choriocarcinoma(11 cases), breast(8 cases) were common Primary tumors of 90 patients receiving planned treatment. Symptomatic subjective response was obtained in $92\%$ of Patients and meurologic functional improvement was obtained in $42\%$ of patients. Median survival was 6.4 months in patients with complete treatment an·d less than 2 months in Patients with incomplete treatment, overall survival rate at 1 year and 2 year were $26\%,\;16\%$ in Patients with complete treatment and $8\%,\;0\%$ in patients with incomplete treatment. Primary site, extent of metastases and interval from diagnosis of primary tumor to brain metastases were identified as prognostic factors.
