다중절편 방사선단층촬영 결과에 기반한 한국 중년 남성에서의 관상동맥 석회화와 심혈관질환 위험인자와의 연관성 (The Relationship between Coronary Risk Factors and Coronary Calcium Score Detected by Computed Tomography Coronary Angiography in Korean Middle Aged Men)

  • 본 연구는 다중절편 방사선단층촬영을 이용하여 관상동맥 석회화와 심혈관질환 위험인자와의 관련성에 관하여 알아보기 위하여 수행되었다. 이를 위하여 일개 대학병원에서 2010년 1월부터 2011년 12월까지 다중절편 방사선단층촬영을 시행한 30~59세의 성인 남성 5,899명을 대상으로 문진, 설문, 신체계측, 혈액검사 등을 시행하였다. 다중절편 방사선단층촬영 영상을 통해 대상자의 관상동맥 칼슘 점수를 확인하고, 기존의 심혈관질환 위험인자에 대해 확인하였다. 대상자 5,899명의 평균 관상동맥 칼슘 점수는 8.20이었고, 773명(13.1%)에서 관상동맥 석회화가 관찰되었다. 관상동맥 석회화 유무와 알려진 심혈관질환 위험인자(나이, 혈압, 공복혈당, 콜레스테롤, 아포지방단백, 비만)는 유의한 관련성을 보였으며, 심혈관질환 발병 위험도평가도구들과도 유의한 관련성이 있었다. 30-39세, 40-49세, 50-59세의 연령 군에서 관상동맥 석회화와 통계적으로 유의한 관련 요인은 각기 다른 양상을 나타냈다. 본 연구를 통하여 관상동맥 석회화와 전통적인 심혈관질환 위험인자와의 관련성 및 심혈관질환 발병위험도 평가도구와의 관련성을 확인할 수 있었다. 또한 연령 군에 따라 각기 다른 양상의 위험인자를 확인할 수 있었다. 따라서 향후 심혈관질환 예방을 위한 관리전략 수립에 연령에 따른 차이를 고려한 접근이 필요할 것으로 사료된다.

일 지방 도시의 종합병원 이용자들의 의료서비스 만족도와 재이용 의사에 미치는 요인 (The Effected Factors on Customer Satisfaction of Medical Service and Willingness to Revisit among Selected Hospital Users in a Local City)

  • 의료서비스 만족도에 영향을 미치는 요인을 분석하고, 만족도가 병원의 재이용 의사에 미치는 영향을 파악하고자 지방도시 일부 종합병원 환자 600명을 대상으로 2004년 2월 1일부터 2004년 4월 30일까지 설문조사를 실시한 결과, 의료서비스 만족지수는 전체 175점 만점에 $113.54{\pm}19.23$점으로 비교적 높은 점수를 나타냈고, 국공립병원 이용자가 $120.20{\pm}18.96$점으로 대학병원 이용자 $106.46{\pm}16.49$, 사립종합병원 이용자 $107.83{\pm}18.48$점 보다 유의한 수준에서 높게 나타났다(p<0.001). 만족지수를 4개영역별로 살펴보면 진료 서비스 만족지수는 총 30점 만점에 19.57점이었고, 간호사나 의사의 친절에 대한 만족지수는 총 55점 만점에 39.10점으로 비교적 높게 나타났으나 서비스 절차 및 시설이용 만족지수는 총 60점 만점에서 36.28점을 나타났으며 환경상태에 대한 영역에서는 30점 만점에 18.59점으로 나타났다. 병원이용 만족지수에 영향을 주는 요인을 알아보기 위해 다중선형회귀분석을 적용한 결과, 이용한 병원이 국공립병원인 경우 (${\beta}=0.16$), 연령이 많을수록 (${\beta}=0.15$), 의료비에 대한 인식이 싸기 때문에 이용한다 라고 생각하는 경우(${\beta}=0.15$) 타 병원 이용경험이 있는 경우 (${\beta}=0.12$)에 만족지수가 유의하게 높았다. 병원의 재이용의사 비율은 긍적적인 비율이 전체 46.8%였고, 진료비가 비싸도 다시 이용하겠다는 비율이 전체 31.3%로 국공립병원 이용자들의 재이용의사 비율(44.7%)이 대학병원 이용자(20.0%), 사립종합병원 이용자(13.3%)의 비율보다 유의하게 높게 나타났다(p<0.001). 또한 이용한 병원의 좋은 점을 주위에 알리겠다고 한 비율이 전체 41.5%이었다. 재이용 의사에 미치는 영향을 분석한 결과, 연령(${\beta}=0.09$)과, 건강검진 여부(${\beta}=0.08$)가 유의하게 영향을 미쳤고 진료 서비스 만족지수(${\beta}=0.35$), 친절 및 대인관계 만족지수(${\beta}=0.17$)가 높은 상관성을 보였다($R^2=0.37$). 이 결과로 병원내의 인적요인에 의한 만족도가 높을수록 재이용 의사가 높아짐을 알 수 있었다. 병원의 마케팅 전략 면에서 한번 방문한 의료서비스 이용자의 재이용 의사는 매우 중요하다. 재이용 의사에는 진료서비스 만족지수가 크게 영향을 미치므로 진료서비스 만족도를 높이기 위한 전략이 중요하다고 생각된다. 특히 진료 서비스 만족도와 구성원의 대인 및 친절에 대한 만족도가 가장 많은 영향을 미치는 것으로 볼 때, 병원의 환경이나 시설보다는 인적관리가 더욱 중요한 것으로 생각된다. 그러므로 마케팅 전략에서 의료인의 질적 수준을 높이기 위한 재교육 및 병원 구성원들의 친절교육을 강화하는 것이 가장 중요하다고 생각된다.

일부 고등학생들의 학교와 학원 코로나19 대응방역 신뢰도에 따른 코로나19 예방행동 실천의 차이 (Difference in the practice of COVID-19 prevention according to the reliability of COVID-19 response among high school students in Korea)

  • 본 연구는 고등학생의 학교와 학원의 코로나19 대응방역에 대한 신뢰도와 이에 따른 코로나19 예방실천의 차이를 비교함에 목적이 있다. 연구 자료는 고등학생을 대상으로 2020년 7월 2일부터 7월 17일까지 온라인 설문조사를 통해 자료를 수집하였으며, 수집된 232명의 고등학생 중 32명을 제외한 200명을 분석하였다. 카이제곱 검정과 이분형로지스틱회귀분석 등을 이용하여 분석하였다. 그 결과, 고등학생은 학교의 코로나19 대응에 대하여 학원보다 더 신뢰하고 있었으며, 학교만 다니는 학생의 코로나19 예방 실천율이 학교와 학원을 다니는 학생보다 높았다. 특히, 오염된 물건 접촉 후의 손씻기(p=.038), 엘리베이터 이용 자제(p=.038)이 유의미하게 차이가 있었다. 학교의 코로나19 대응 신뢰정도에 따라서 대중교통기피(p=.028), 일렬착석 식사(p=.011)가 유의미한 차이가 있었으며, 학원의 대응 신뢰정도에 따라 재채기 시 입가림(p=.041)이 유의미한 차이가 있었다. 학교의 대응방역 신뢰도에 영향을 주는 코로나19 예방실천은 3개로 나타났으며, 재채기 시 입가림(p=.002), 대중교통 기피(p=.043), 일렬착석 식사(p=.025)이었다. 반면에 학원의 대응방역 신뢰도에 영향을 주는 코로나19 예방실천은 1개로 물건 접촉 후 손씻기(p=.039) 였다. 학교의 코로나19 대응방역 신뢰도가 학원보다 높은 이유는 보건교사이다. 보건교사는 교육부의 지침을 받아 코로나19 방역과 학생들에게 코로나19 예방실천 교육을 실시하고 있기 때문이다. 또한, 학원은 코로나19 방역에 대한 지침은 받지만, 이를 책임지고 이행하며 관리할 기관이 명확하지 않다. 따라서, 교육부와 지자체에서는 학원을 대상으로 코로나19 방역을 관리감독하고 코로나19 예방교육을 실시할 임시 순환 보건교사 배치 및 관리 체계를 강화할 필요가 있다.

농업인의 미세먼지 노출에 대한 인지된 감수성과 마스크 착용 및 호흡기질환 예방교육 참여 태도와의 연관성 (Association between Perceived Susceptibility to Fine Dust Exposure and Wearing Masks, Attitude toward Respiratory Disease Prevention Education in Farmers)

  • 현재 농사를 짓고 있는 농업인 295명을 대상으로 일대일 면담 방식의 설문조사를 통해 농작업 중 노출되는 미세먼지에 대한 인지된 감수성, 마스크 착용, 미세먼지 노출 관련 호흡기질환 예방교육의 참여의향 등을 조사하였다. 대상자의 평균 연령은 60.3세로 농사 경력은 평균 25.6년이었고 주 작목에 따른 실내 농작업자가 178명(60.9%), 실외 농작업자가 115명(39.1%)이었다. '직업 특성상, 농작업 중 노출되는 미세먼지로 부터 자신이 취약한 계층이라고 생각하는 지'에 대해서 112명(48.1%)이 '예'라고 하였고, 응답자 중 실내 농작업자가 66.7%이었다. '농작업 중 노출되는 미세먼지에 대한 인지된 감수성'의 점수는 5점 만점에서 평균 3.8점이었으며 호흡기질환 예방교육 참여 경험이 있는 집단은 4.1점, 경험이 없는 집단은 3.6점이었다. 작목군 중에서는 화훼 농업인이 평균 4.1점으로 가장 높았다. 성별(p=.004)과 직업특성상 자신이 미세먼지로부터 취약한 계층이라고 생각하는 지 여부(p=.004), 호흡기질환 예방교육 참여 경험 유무(p=.011)에서 유의미한 차이가 있었다. 미세먼지 노출에 대한 인지된 감수성과 미세먼지 관련 호흡기질환 예방교육 참여 의향간의 상관관계는 유의미한 양의 관계(r=.219, p<.001)를 보였고 인터넷이나 문자를 활용한 교육 방법(33.6%)을 가장 선호하는 것으로 나타났다. 농업인의 농작업 관련 건강위험 인식 고조 및 미세먼지 등 유해물질의 직업적 노출 감소를 위해서 정부와 대중의 관심 증대라는 외부 기회를 활용하고, 농업인의 인식수준과 교육 욕구의 내부 강점을 사용하는 전략으로써 다양한 교육 콘텐츠 개발이 이루어져야 할 것이다.

한국 청소년의 거주지역별 및 체격별 식사의 질 평가지수의 비교 (Comparison of Indices for Diet Quality Evaluation of Korean Adolescents by Residence Area and Body Size)

  • The purpose of this study was to assess several indices of diet quality based on nutrient, food and food group intake of Korean adolescents based on several indices on diet quality according to residence area and body size. Using the data from the 1998 National Health and Nutrition Survey, twenty-four-hour-dietary recalls of a total of 1,110 Korean adolescents aged 13 - 19 years (male 543, female 567) were analyzed for nutrient adequacy ratio (NAR) , index of nutritional quality (INQ) , the number of foods (Dietary Variety Score, DVS) and food group consumed (Dietary Diversity Score, DDS) . In doing that, it was attempted to apply only the minimum amount of solid foods of Kant's without inclusion of liquid foods because of the very limited variety in Korean foods. Based on weight length index, 13.1% of the subjects were categorized as obese, 14.2%, overweight, 44.4%, normal and 28.3%, underweight. Only vitamin B2 intake was higher in the obese group than in the underweight group. There was no meaningful difference in energy, protein and fat intakes according to the grade of the body size. In terms of residence area, intake of fat, niacin, vitamin $B_6$ and folic acid were lower in the rural areas than in the metropolitan city. Only vitamin E intake was higher in the rural areas. Mean value of NARs (MAR) and INQs (mINQ) was also higher in the metropolitan city than in the rural areas, but there was no significant difference of these two values according to body size of the subjects. Mean DVS was 21.02 for total subjects, and has no difference between male and female and between metropolitan city and other medium-small city. But, the rural areas showed the lowest DVS of 19.05. Mean DDS in which five is a maximum score was 3.3 with no significant difference by sex and by residence area in male subjects. However, in female subjects, DDS in the rural areas was the lowest. According to body size of the subjects, there was no meaningful difference in both scores of DVS and DDS. In conclusion, most indices of nutrient intake and food and food group intake were not significantly different by body size of the subjects, while most indices were significantly different by residence areas: higher in the metropolitan city than in the rural area.

대학생의 특수영양 및 건강보조식품의 성별, 지역별 섭취실태 및 섭취요인에 관한 연구 -인터넷 영양교육 참여 대학생을 중심으로- (Regional Differences in Dietary Supplement Use and Related Factors among College Students Participating in Nutritional Education Programs via the Internet)

  • The purpose of this study was to investigate the regional 야fferences in dietary supplement use and related factors among college students participating in nutritional education programs via the internet. The subjects in this study were 797 college students (male: 518, female: 279). A cross-sectional survey was conducted using a self-administered questionnaire about dietary supplement use, demographic characteristics, health-related lifestyles, nutrient intake, and data were collected via the internet and by mail. Dietary supplements were taken by 82.2% of subjects (males: 76.3%, females: 85.3%). There was a significant regional difference in supplement use (p<0.01). The supplements, used most frequent by male students in the Seoul and Incheon areas were lactic acid beverages and gagogi, in that order. Male students in the Kyunggi area used gagogi and lactic acid beverages in that order. Lactic acid beverages and Chinese medicine were used most frequently by male students in the Chungcheong area. Male students in the Kyungsang area used lactic acid beverages and water-soluble vitamins, in that order. Male students in Seoul and Kyungsang areas showed significantly higher percentage of keep on taking supplements compared to other areas (p<0.05). Both male and female students in the Seoul area had more frequent medical examinations, in comparison to students in other areas (p<0.05). Male students in the Kyungsang and Seoul areas took significantly more calcium (p<0.05) and iron (p<0.05), in comparison to students in other areas. Female students id the Kyunsang area took significantly more Vitamin A, phosphorous and iron (p<0.05), while those in the Seoul area took significantly more Vitamin C (p <0.01) and calcium (p<0.05) . Male students in the Chungcheong area took significantly less seaweed, in comparison to students in other areas (p<0.05) . Male students in the Seoul area had the highest amount of animal fat and oil, while those in the Incheon area ate the lowest amount of animal oil and fat (p<0.05). Female students in the Kyunggi area ate the highest amount of fruit, while those in the Incheon area ate the lowest amount of fruit (p< 0.05) , Female students in the Incheon area ate the highest amount of seaweed, while those in the Kyunggi area ate the lowest amount of seaweed (p<0.01). Therefore, it can be concluded that it is necessary to develop dietary supplements to optimize the nutritional status of college students in different areas of Korea.

여성들의 수유양상 및 수유방식의 선택에 영향 주는 요인들 (Infant Feeding Practices and the Factors that Influence feeding Practices among Women in Seoul and the Chungbuk Area)

  • In order to obtain baseline data for the development of an educational program on breastfeeding, a survey was carried out to investigate infant feeding practices, the characteristics the subjects have in common, and the factors that influence the feeding methods among women in Seoul and the Chungbuk area. Subjects included 671 lactating mothers who visited public health centers or pediatric clinics between December 1999 and February 2000, and were interviewed using a questionnaire. The results were as follows : With respect to feeding methods, the incidence of breastfeeding, formula feeding, mixed feeding and formula feeding switched from breastfeeding was 20.6%, 29.8%, 11.6%, and 38.0%, respectively. With respect to the characteristics the subjects had in common, the incidence of women who planned their infants' feeding methods before pregnancy, during pregancy, and after delivery was 48.7%, 31.0%, and 20.3%, respectively. The incidence of women who started breastfeeding in the hospital after delivery was 38.8%. About seventy seven percet of the subjects had not previously attended an educational program on breastfeeding, and most of these wanted to participate in the future in an educational program to learn about breastfeeding. Some of the topics they were interested in were“Nutritional Management for Sufficient Breast Milk”(60.3%),“Breast Care”(25.0%), and“Correct Nursing Positions”(9.8%). Most (88.2%) of the women who breastfeed suffered from physical discomforts including discomfort of the waist, and legs and discomfort due to cracked or sore nipples. “Insufficient breast milk”was the main reason for breastfeeding cessation or for switching to formula feeding. With respect to formula feeding practices, the main reasons for selecting a specific brand of formula were“the same brand the hospital used after delivery”(34.3%) and“an advertised brand”(23.3%). The strongest factor for promoting breastfeeding was“the support of husband or parents”, next were “breastfeeding in the hospital after delivery”and“planning to breastfeed before pregnancy”in that order. The characteristics the subjects had in common relating to formula feeding were“mother's job”,“high economic level”,“Caesarian section”and “planning to breastfeed after delivery”. In conclusion, it is recommended that breastfeeding be pro-moted, and educational programs be developed and offered as soon as possible to each group which had unfavorable attitudes toward breastfeeding. In addition, the monitoring and supervision of formula advertisements is required to protect consumers from the adverse effects of exaggerated advertising.

멸치, 어묵, 다시마 및 무 조리 후 국물의 영양성분 분석 (Analysis of Nutritional Composition in Boiled Broth using Anchovy, Fish Paste, Sea Tangle and Radish)

  • This study was undertaken to evaluate the change of major nutrient components in boiled broth of anchovy, fish paste, sea tangle, and radish subjected to the boiling processing. The crude protein, fat, ash, and carbohydrates in anchovy boiled broth were 0.1%, 0.04%, 0.1%, and 0% respectively as against 0.04%, 0.1%, 0.2%, and 0.4% in fish paste boiled broth. The protein content in the anchovy boiled broth, the fat and carbohydrate in the fish paste boiled broth, and the ash in the sea tangle boiled broth were high. Major components of the minerals were sodium plus potassium and the boiled broth in anchovy and fish paste had the contents of 19.41 plus 16.19 and 76.77 plus 10.41 mg, respectively. Especially, the potassium in boiled broth of the sea tangle was shown the highest content of 85.94 mg. The Vitamin B1 content in all broth samples were below 0.01 mg per 100 g edible portion. In the case of the B2 content, the anchovy boiled broth had 0.03 mg per 100 g edible portion. On the other hand, the niacin content of the boiled broth on a wet weight basis was detected at an average 0.1 mg. Vit. A, C, tocopherol, and cholesterol was not detected in the boiled broth of any of the subjects. As a result of applying the data of boiled broth in the fish paste soup instead of anchovy and sea tangle which are non-edible, the calcium, dietary fiber, and vitamin A were shown to have a difference of less than 50%.

2005 국민건강영양조사 원자료를 이용한 우리나라 6~11세 어린이의 다소비 식품 1인 1회 섭취분량(portion size) 설정 및 성인과의 비교 (Establishment of One Portion Size of Foods Frequently Consumed by Korean Children Aged 6-11 Using 2005 Korea National Health and Nutrition Examination Survey and Its Comparison with Adults')

  • The purpose of this study was to define a one portion size of food frequently consumed by Korean children aged 6-11 for convenient use in food selection, diet planning, nutritional evaluation, and nutrition education. We analyzed using the original data on 889 persons (9.96%) aged 6-11 years among 8,930 persons to whom NHANES 2005 and selected food items consumed by the intake frequency of 10 or higher among the 500 most frequently consumed food items. A total of 172 varieties of food items of regular use were selected. Also the portion size of food items was set on the basis of the median (50 percentile) of the portion size for a single intake by a single person was analyzed. The portion size of cereals was 5 g for prosomillet to 120 g for hamburger. As for vegetables, it posted 1 g for red pepper to 50 g for nabakkimchi. The portion size of meats and products was 15 g for luncheon meat to 120 g for beef feet soup. In comparison of children's portion size with adults', the children's portion sizes of every food groups, with the exception of meats and eggs, were lower than those of adults. The portion size of the regular food items of children in this study will be conveniently and effectively used by children, children's diet planners and researchers in selecting food items for a nutritionally balanced diet and in assessing the children's diet intake.

농촌거주 외짝가족의 식생활관리 -부부가족과의 비교- (The Odd Pair Family's Dietary management in rural, Korea - Comparison with the Pair Family -)

  • Recently the rural Korea has been remarkedly changed of family and social value in accordance with the development of industry. The lower economic class made by social economic growth is widespread with increasing aged, specially odd pair family in rural. The purpose of this study was to investigate to help and keep improve health of rural lower economic class, family system by comparing and analyzing the dietary management, between pair and odd pair family, and to get the data helpful the right guidance for rural. The subjects 1870 collected in 9 provinces by sampling with probability proportional to size (PPS). Questionnaire about dietary habit, food cultivation, production and preservation survey was conducted by trained interviewers. The main results were as follows : 1) The characteristics of odd pair families, head of household was female(77%), over 65 years(84.9%), small family(1.76 persons) and lower education(male 7.5 years, female 3.1 years) status. 2) As the states of diets of odd pair family, having breakfast(87.1 %) but one or two kinds of side dishes(31.3 %) only possible to guess lower status of food intake balance. Nutritional supplements(21. 7 %) was lower than that of paired family. 3) The aspects of dietary habit of odd pair family, no instant foods(70.7%), no snack(38.4%) no dine out(69.2%) were common. 4) Dietary habit scores were 7.78 points of odd pair family compared 8.34 points of paired family. 5) Food purchase place of odd pair family was market(44.2%) but super-market(42.7%) of paired family. 6)In odd pair family, seldom traditional dish preparation(62.0%) but prepared winter kimchi(81.9%), comparing seldom traditional dish(38.6%) and winter kimchi(96.4%) in paired family. 7)The food cultivation state was surveyed, pepper( 42.2 %) and chinese cabbage( 43.9 %) were consumed after cultivation, but sesame(59.4%), bean sprout(90.2%), tofu(92.8%) and egg(93.3%) were consumed by purchase in odd pair family.8) Food cultivation score of odd pair family was 2.98/12points significantly lower than 4.50/12 points of paired family(p<0.01). 9) At the status of fermentation food production in odd pair family, Duenjang(72.1 %) and Gochujang(69.7%) Kanjang(68.3%) Kimchi(82.1 %) and Meju(68.3%) were high rate of production, but more frequently producted in pair family. 10) The score of fermentation food production of odd pair family was 8.57/12points but significantly lower than 10.24/12 points of pair family(p<0.0001). 11) Food preservation score 0.48/6 points in odd pair family was not significantly different than that of pair family(1.07/6points).
