The Effectiveness of Community-based Social Distancing for Mitigating the Spread of the COVID-19 Pandemic in Turkey

  • Objectives: The objective of this study was to demonstrate the effects of community-based social distancing interventions after the first coronavirus disease 2019 (COVID-19) case in Turkey on the course of the pandemic and to determine the number of prevented cases. Methods: In this ecological study, the interventions implemented in response to the first COVID-19 cases in Turkey were evaluated and the effect of the interventions was demonstrated by calculating the effective reproduction number (Rt) of severe acute respiratory syndrome coro navirus 2 (SARS-CoV-2) when people complied with community-based social distancing rules. Results: Google mobility scores decreased by an average of 36.33±22.41 points (range, 2.60 to 84.80) and a median of 43.80 points (interquartile range [IQR], 24.90 to 50.25). The interventions caused the calculated Rt to decrease to 1.88 (95% confidence interval, 1.87 to 1.89). The median growth rate was 19.90% (IQR, 10.90 to 53.90). A positive correlation was found between Google mobility data and Rt (r=0.783; p<0.001). The expected number of cases if the growth rate had not changed was predicted according to Google mobility categories, and it was estimated to be 1 381 922 in total. Thus, community-based interventions were estimated to have prevented 1 299 593 people from being infected. Conclusions: Community-based social distancing interventions significantly decreased the Rt of COVID-19 by reducing human mobility, and thereby prevented many people from becoming infected. Another important result of this study is that it shows health policymakers that data on human mobility in the community obtained via mobile phones can be a guide for measures to be taken.

  • Recently, with the sudden changes in the security environment, expectations towards the police and fear towards safety are increasing. Thus, the local community police operation, which creates a mutually cooperative relationship between the police and local residents, is coming to the fore as the paradigm of the nationwide police operation. The police operation in Korea has also gone through change, and as part of the local community police operation, Korea has implemented a community police system since 2003. Recently, the system has been going through change in that the operations are centralizing around not only prevention of crime including the safe city program, protection of multicultural families, response to foreign crime, protection of the socially disadvantaged but also life security of the citizens. Along these lines, this study seeks to examine conditions of the three elements of local community police operation - Strategic Oriented Policing (SOP), Neighborhood Oriented Policing (NOP), Problem Oriented Policing (POP) - and policy suggestions accordingly.

Quality Evaluation of Medication Counseling in Korean Community Pharmacies (지역약국에서 시행하는 복약지도의 질적 수준 평가)

  • Background: Medication counseling is important to improve qualify of pharmaceutical care, but there are lack of studies to investigate satisfied counseling practice in community pharmacies. Purpose: This study was to investigate current medication counseling provided by community pharmacists. Method: Questionnaires to assess medication counseling practices were mailed to nationwide 1,269 community pharmacists who were Sookmyung Women's University alumni. Result: One hundred sixty five pharmacists were responded to the questionnaires (response rate 13.63%). 16.8% of them all respondents had counseling room in their pharmacies. 75% and 29.9% of respondents provided patient counseling always for new and refilled prescriptions, respectively. Counseling was provided primarily by verbal, but especially for drug name, usage and storage, it was provided in written concurrently. 31.1% of respondents was satisfied with counseling practices by themselves, and a major barrier of unsatisfied counseling was lack of time in 64.6% of respondents. Majorities of respondents (66.3% and 76.3%) spent 1-3 minutes and <1 minute, for new prescription and refilled prescription, respectively. Approximate 99% of respondents presented necessity of standardized patient counseling information for drugs and 97.6% responded that quality of patient counseling would be improved by using software containing counseling information. Old respondent group had higher accumulative patient counseling practice scores and pharmacy owner group took much time for counseling, resulting in higher scores too. Conclusion: This study showed that qualified patient counseling practice has not been reached in community pharmacies. Much more discussion to seek ways to improve the quality would be necessary.

A Delphi Study for Development of Disaster Nursing Education Contents in Community Health Nursing (지역사회간호학 재난간호교육 콘텐츠 개발을 위한 델파이 조사)

  • Purpose: This study was conducted to develop the contents of disaster nursing education in community health nursing at universities. Methods: To validate contents, the Delphi method was used. We categorized two domains(indirect disaster management and direct disaster management) and developed 48 draft items. This study applied two round surveys and 23 experts participated in this study. The content validity was calculated using content validity ratio and coefficient of variation. Results: Indirect disaster management domain was composed of three categories including 12 items: 1) Understanding of the disaster, 2) disaster management system, and 3) response by disaster stage and recovery. Direct disaster management domain was composed of nine categories including 30 items: 1) Ethical considerations, 2) communication in disasters, 3) nursing activity by disaster stage, 4) emergency nursing in disasters, 5) patient severity classification in disasters, 6) disaster nursing for vulnerable groups, 7) disaster nursing for victims, 8) psychosocial nursing and health in disasters, and 9) cases of disaster nursing in communities. Conclusion: This Delphi study identified the contents of disaster nursing education curriculum, and confirmed the validity for disaster education program in community health nursing. Based on the results, it will be helpful for training the disaster nursing and improving the competency on disaster nursing of the nursing students.

Differences in the Characteristics of Sexual Abuse Victimization between Low- and High-Grade Elementary School Children and Correlations among the Characteristics (초등학생 저학년과 고학년의 성폭력 피해특성 차이와 상관관계)

  • Purpose: This study is attempted to figure out the characteristics of sexual abuse victimization in low- and high-grade elementary school children and furthermore to help develop appropriate preventive educational programs against sexual abuse by grade. Methods: Data were collected from 156 sexual abuse victims who were elementary school children and visited the Child Sexual Abuse Response Center in D City during the period from 2010 to 2012. Differences in general and victimization-related characteristics between low and high graders and the correlations among the characteristics were analyzed. Results: The results showed statistically significant difference between low and high graders in two variables: offender-victim relationship, and the type of sexual abuse. Offender-victim relationship was in a significant correlation with the duration of victimization (r=.576, p<.001), frequency of abuse (r=.546, p<.001), location (r=-.479, p<.001), and time (r=.435, p<.001). The type of sexual abuse was in a significant correlation with frequency (r=.175, p=.029) and time (r=.261, p=.001). Conclusion: Appropriate educational programs should be developed for preventing sexual assaults in consideration of difference in victimization-related characteristics between low and high graders. In addition, such programs should be applied fittingly to the learners'grade, gender, and cognitive level.

Utilization of Technical Resources in the Emergency Recovery Process of Flood Damage of Local Governments (지방자치단체의 홍수피해 응급복구 과정에서의 기술자원 활용방안)

  • This study analyzed appropriate equipment combination for emergency recovery after flood disasters and proposed an alternative approach for economic and rapid emergency recovery based on the utilization of human resources who have practical experience in construction business and in equipment operation. The current countermeasures and response to flood disaster including emergency procedures and methodologies are investigated and related legal framework was also analyzed. Currently, some emergency recovery works are over payed and delayed due to unclear and inefficient processes on equipment and human resources mobilization during and after flood disasters, particularly in local governments. Best practices in mobilizing equipment and human resources were compiled, theoretical foundation was investigated and analyzed, and improved emergency recovery schemes were proposed for effective flood disaster countermeasures in local government. Prioritization in equipment and human resources mobilization was also proposed considering equipment transportation distance and accessability to disaster sites. This study also proposed improved ways to utilize experts in local areas in supporting community emergency response volunteers for disaster risk management.

Association between Health Behaviors and Sexual Experience in High School Students (고등학생의 건강행태와 성경험의 관련성)

  • Objectives: This study aimed to analyze sexual experience rates according to socio-demographic factors, health behavioral factors, and internet use time, and to identify sex experience related factors in high school students using the data from the 11th Korean Youth Health Risk Behavior Web-Based Survey. Methods: A total of 33,744 students (17,346 boys and 16,398 girls) in high schools were analyzed using the SPSS WIN version 22 program. SPSS complex samples methods were used for analyses. Socio-demographic factors, health behavioral factors, and internet use time as independents variables were included. The complex samples logistic regression analyses were used to calculate the odds ratio of the sexual experience according to the socio-demographic factors, health behavioral factors and internet use time. Unweighted frequencies and percentages were represented in result tables. Results: 3.6% of girls and 9.9% of boys in high school had sexual experiences. Daily average smoking amount and daily alcohol drinking amount were a dose-response relationship with sexual experience after considering confounding factors. Students who smoke 10-19 cigarettes had 5.74 times higher risk and 20 cigarettes or more had 7.27 times higher risk of sexual experiences, comparing with non-smoking students, relatively. Likewise, students who drink soju less than 1-2 bottles and more than 2 bottles had 3.82- and 4.35 times higher chance of sexual experiences, compared with non drinking students, respectively. Conclusions: We found that there were the dose-response relationship between health behavioral characteristics and sexual experiences. Further research is needed to identify an interaction effect between smoking and drinking alcohol on sexual experiences in high school students.

Estimation of the Number of Salmonellosis Using Microbial Risk Assessment Methodology (미생물 위해성 평가 방법을 이용한 살모넬라 발생수 추정)

  • The number of foodborne salmonellosis was estimated by using microbial risk assessment(MRA) methodology and the possibility of application was studied through comparison with previous results. The contamination levels of Salmonella sp. were estimated by using published domestic studies(1997∼2000) and monitoring data (1999∼2001) from food-safety related institutes. Data on food consumption came from the 2001 National Health and Nutrition Survey, and dose-response models from studies in other countries. Simulation results showed that there were 753,368 cases of salmonellosis in Korea in 1 year, which is about 115 times that reported in previous years and lower than the WHO's estimation increase. From these results, microbial risk assessment is likely to be available for estimation of the number of foodborne illnesses and determination of the order of priority in food-safety management. Butthe verification methods are not established and most of the data on contamination levels of foodborne bacteria, food consumption, and dose-response relationships have not been established. In addition, the actual conditions of circulation, storage and cooking must be studied further.

The Study on Identifying the Components of Community Pharmacy Externship Based on Korean Community Pharmacists' Consensus (약학교과과정의 개국약국실습 방향에 관한 연구)

  • The need for and components of a contemporary community pharmacy externship for pharmacy students have not been clearly identified in Korea. Mail survey was performed among 20 college of pharmacy deans and 800 community pharmacists to analyze the current status and develop a consensus regarding major focus area and criteria of community pharmacy externship to be implemented under the separation of dispensary from medical practice in year 2000. Mail survey yielded $80\%\;and\;23.5\%$ response rate for pharmacy school deans and community pharmacists, respectively. Of the 16 pharmacy schools that responded 14 said they have externship program in hospital pharmacy, and only 8 pharmacy schools responded of having externship program for community pharmacy. However, these community pharmacy programs lacked criteria and standard guideline for the externship. The results of survey revealed that community pharmacy externship program for students should be organized and directed toward developing expert knowledge and skills in pharmacy practice activities, clinical services, communications, pharmacy management, and professionalism. Pharmacy practice components should include competencies and skills in computer application, prescription processing, dispensing, pharmaceutical compounding, Narcotics Control Law application, maintenance and provision of drug information, and laws and regulations. Clinical service components should include the ability to identify patient's drug-related problems, provide long-term patient care and appreciate drug therapy services. Communication skills should be taught to effectively express his/her professional opinion, deduce the needs of others, utilize appropriate techniques and media to communicate ideas and conduct a patient interview and to obtain patient drug history. Pharmacy management skills should be taught to be efficient in medical insurance and drug control process. It was found that professionalism, morality, pharmacy practice experience, ability to provide clinical services, collect and provide drug information and regality are important criteria of preceptors. Externship sites should possess the ability to stock various drugs, access and provide diverse pharmacy services and should have private patient counseling area. Most pharmacists agreed that top 200 drugs' generic and brand name, indications, dosage, side effects, and contraindication should be instructed during the externship. It was also found that student and preceptor should be evaluated for their performances during the externship. This information will be incorporated into teaming objectives for students and to develop Academic Extemship Program Guidelines.

Analysis of the Current Hypertension Control Program in Public Health Centers (지역사회 고혈압 관리 사업의 방향 재설정)

  • Purpose: Hypertension has been a major cause of death in Korea since the 1970s, and has resulted in being a significant economic burden to the finances of national health care. The Ministry of Health and Welfare made several efforts. but hypertension control programs in Korea are still non standardized and ineffective. We wanted to investigate the current hypertension control program in public health centers systematically and suggest the direction for future programs. Method: The design of this study is a cross sectional investigation. From September to October in 2002, we sent a set of questionnaires to all PHCs, and 179 centers responded (response rate = 74.0%). The instrument was developed based on components of National health systems. Results: 1) Resources: The department responsible for hypertension control programs is the Department of Health Promotion. Health Education Center, Community Health Center, Citizen's Health Center, etc. The chief personnel of those departments are nurses. but 27.4% of PHCs have no full time nurse for hypertension management programs. PHCs had a lot of teaching materials (nine types per a PHC) and most of the recommended contents were included. But, periodical evaluation and revision were not being made, 2) Management: Nurses' (13.03 9.46 in 23 score) actions for hypertension control were not qualitative, but regular training and evaluation were seldom carried out. Need assessment (25.9%) and evaluation (about 10-20%) for the hypertension control program were indicated as low. 3) Programs: Programs focused on individuals rather than community or public, and 2nd prevention rather than 1st, 3rd prevention. Conclusion: The Ministry of Health and Welfare has to construct the infrastructure for hypertension control programs. Related scholars and committees should develop and declare standardized manuals for hypertension control and the management system, as well.
