Service Analysis of Community Health Nurse Practitioner using Information System (보건진료소 정보시스템을 이용한 보건진료원의 업무 분석)

  • The purpose of this study was to analyze the activities of Community Health Nurse Practitionses using the Community Health Post Information System(CHPIS). The information system that have been introduced in 1994 and used by 400 Community Health Posts(CHPs) since 1997, which is about $20\%$ of the total CHPs nationwide. Twenty-five CHPs from two provinces participated in the analysis. Seventy-two percent of the CHPs among the participating CHPs started using the system since 1996. The degree of utilization of the information system was classified into three groups (i. e., high. medium, and low). The results revealed that only $48\%$ utilized the system with high level. The areas of analysis of the information system included characteristics of community residents, environmental attributes, and job analysis of Community Health Nurse Practitioners(CHNPs). The study results indicated that primary health care and drug demand and supply system showed the highest level of satisfaction in utilizing the information system by CHNPs.

A Study on Hypertension Management of Community Health Practitioner Posts (보건진료소 고혈압 관리사업의 실태)

  • Objectives: The purpose of this study is to provide data for the improvement of hypertension management of community health practitioner posts through the study on hypertension management in community health practitioner posts. Methods: A questionnaire was mailed to 700 community health practitioners and 205 of them responded during the period from March 13, 2003 to May 13. The survey results were analyzed using SPSS program, version 11. Results: The results are as follows; 1. There are two major activities in a hypertension prevention project for community: health education and early detection. About 57% of community health care practitioners perform a health education for community people four times a year. The 64.5% of them used the materials for health education provided from a community health center and 22.1% of them performed a post-evaluation. The main method of early detection of hypertension was measurement of blood pressure of person to visit, which was 96.1%. Other methods included home visiting(89.3%), a referral from community hospitals and other resources(49.1%), health promotion events(39.5%), and a review of medical records(35.7%). 2. For the registration and management of patients with hypertension, about 36% of community health centers used a special form and more than 50% of them have registered patients who were managed by other health care institutions in the community. A computerized program was used for the management of patients with hypertension in 68.5% of them. More than 60% of them responded that it was used for report, treatment, and follow-up of patients with hypertension.

Analysis of the Results of Health Service Programs in the Primary Health Care Posts (보건진료소 보건사업 현황 분석)

  • Purpose: To analyze health programs of the PHCP (Primary Health Care Posts) Method: From August 2006 to July 2007, data on the general quality and health program of the PHCP was requested by official letter and replies were received via E-mail. From December 8 to December 30, 2007, data from 1,268 (66.8%) PHCP out of 1,897 PHCP were analyzed using SPSS 12.0 Win program. Results: The average population covered by each PHCP is 878.3 people. For the health and special programs, Community Health Practitioners report high motivation for programs on health promotion, management of chronic illness, social welfare (40-50%). Demand by the residents was reported at 10% and increases in the health of the residents were attributed to high interest and demand. Volunteer work was 83.3% for bathing, 54.5% for equipment support and 46% for exercise programs. As elders make up 30% of the population in rural areas, there is an increasing demand for volunteer work in bathing programs. Conclusions: As the number of elders in the population increases and there is an increased need for more medical treatment for older people who are sick, the role of PHCP must be strengthened to include visits to homes of community residents. Where financial support for the PHCP is difficult, it is necessary to develop sound data on demographic characteristics of the population in order to develop efficient and effective health promotion programs. The finding that 54.7% of the population need management of chronic illness has difficulty in seeing a physician indicates a need to enhance the health care delivery system by strengthening the role of the Community Health Practitioners and including them in the civil service system to ensure stability of the PHCP.

The Effect of Health Promotion Program on the Frailty of Rural Elderly Women Implemented at Primary Health Care Posts (일부 보건진료소에서 실시한 건강증진프로그램이 농촌여성노인의 노쇠에 미치는 효과)

  • Objective: This study was conducted to examine the effects of a health promotion program on the health condition of rural elderly women implemented at primary health care posts using Korean Frailty Index. Methods: The research was designed a nonequivalent control group pretest-posttest setting. The participants in this study were 50 residents (intervention group: 25, control group: 25) selected from 4 villages 2 primary health care posts in J city Gyeongsangnam-do. The health promotion program was conducted at the primary health care posts twice a week for 12 weeks. This program consisted of basic exercises(Gukseondo + Theraband muscle strength training) and additional activities(including modified Theraband activity, rubber ball exercise, ball massage, nutrition class, singing class). Collected data were analyzed by descriptive statistics, paired t-test, and repeated measures ANOVA with SPSS 21.0. Results: Results of the health promotion program showed that the health conditions(measured by perceived health status, frailty score, upper/lower flexibility, maximum grip strength, dynamic balance test Timed Up and Go) of the experimental group(25) all statistical significantly improved. Conclusion: Study findings indicate that the health promotion program implemented at primary health care posts on rural elderly women is effective and can contribute to a developed health promotion program for local residents in the future.

Prevalence of Clonorchiasis, Knowledge and Intention to Change Behavior of Village People living in the Catchment Area of Community Health Posts along the Geum River (금강유역 보건진료소 지역주민의 간흡충 감염 실태와 지식 및 행태변화 의도)

  • Purpose: The purpose of this study was to describe the prevalence of clonorchiais and analyze the knowledge level and intention of the participants to change their behavior related to clonorchiais. Method: The participants were 514 people from 7 community health posts along the Geum River. The formalin-esther sedimentation technique was used to detect the Clonorchis Sinensis (CS) eggs and a questionnaire for data on related factors. The study was carried on from December 2008 to January 2009. Results: The prevalence of CS averaged 9.3%, with a range from 0% to 24.6%. The significant factors were sex, habit of eating raw fish, and habit related to smoking and alcohol consumption. The level of knowledge was not high and not significantly different between the CS positive group and CS negative group. Intention to change their habits of eating raw fish showed various stages of change and attitudes to raw fish eating habit of others were not positive. Conclusions: These results suggest that a CS control program needs to be developed by community health services in the Geum River area to decrease the prevalence of clonorchiais.

Analysis of online parenting community posts on expanded newborn screening for metabolic disorders using topic modeling: a quantitative content analysis (토픽 모델링을 활용한 광범위 선천성 대사이상 신생아 선별검사 관련 온라인 육아 커뮤니티 게시 글 분석: 계량적 내용분석 연구)

  • Purpose: As more newborns have received expanded newborn screening (NBS) for metabolic disorders, the overall number of false-positive results has increased. The purpose of this study was to explore the psychological impacts experienced by mothers related to the NBS process. Methods: An online parenting community in Korea was selected, and questions regarding NBS were collected using web crawling for the period from October 2018 to August 2021. In total, 634 posts were analyzed. The collected unstructured text data were preprocessed, and keyword analysis, topic modeling, and visualization were performed. Results: Of 1,057 words extracted from posts, the top keyword based on 'term frequency-inverse document frequency' values was "hypothyroidism," followed by "discharge," "close examination," "thyroid-stimulating hormone levels," and "jaundice." The top keyword based on the simple frequency of appearance was "XXX hospital," followed by "close examination," "discharge," "breastfeeding," "hypothyroidism," and "professor." As a result of LDA topic modeling, posts related to inborn errors of metabolism (IEMs) were classified into four main themes: "confirmatory tests of IEMs," "mother and newborn with thyroid function problems," "retests of IEMs," and "feeding related to IEMs." Mothers experienced substantial frustration, stress, and anxiety when they received positive NBS results. Conclusion: The online parenting community played an important role in acquiring and sharing information, as well as psychological support related to NBS in newborn mothers. Nurses can use this study's findings to develop timely and evidence-based information for parents whose children receive positive NBS results to reduce the negative psychological impact.

Analysis of the Effectiveness of Community Health Programs Based on Primary Health Care Post Areas (일 지역 보건진료소 중심 지역사회 건강 프로그램 효과 분석)

  • Purpose: The study was conducted to identify the effectiveness of community health programs for local residents. Methods: This study was a one-group pretest-posttest study design. The subjects of the study were 259 residents in 26 villages of 13 primary health care posts located in G city. Data were collected using the questionnaire and analyzed using the paired t-test in SPSS 28.0. Results: As a result of performing the community health programs, prostate symptom scores improved in men, and stress and urgency urinary incontinence scores decreased in women. In addition, it was found that the degree of health-related knowledge increased among female subjects after the community health programs were provided. Conclusion: The community health programs were effective, and the effectiveness of the programs may differ according to gender. Therefore, when developing a program, it is necessary to consider a gender-sensitive approach that considers the difference between men and women.

The Effects of the Activity Program for Preventing Dementia Against Depression, Cognitive Function, and Quality of Life for the Elderly (치매예방을 위한 활동 프로그램이 지역사회 노인의 우울, 인지기능 및 삶의 질에 미치는 효과)

  • Objectives: This study is conducted to identify the effects of the activity program for preventing dementia against depression, cognitive function, and quality of life for the elderly. Methods: A non-equivalent control group with pretest-posttest design was used on 50 elderly who were assigned to the experimental and control group (25 for each group). Results: Cognitive function was significantly different between the two groups of the pretest and posttest (Z=4.217, p<.001). There were significant differences between the two groups of the pretest and posttest (Z=-2.324, p=.020) for the quality of life on the EQ-5D index. Conclusions: These findings confirmed that the activity program for preventing dementia was effective for improving cognitive functions and quality of life for the elderly. Therefore, it is further encouraged to perform the activity programs for preventing dementia within other community health posts and community centers. It is necessary to study the emotional supports from community health posts in the future.

The Effect of the Telemedicine Service System Application for the Patients with Hypertension at Community Health Practitioner Posts in Gangwon Province (강원도 보건진료소 고혈압 환자의 원격관리시스템 적용 효과)

  • Objectives: This study is to identify the effects of utilizing a telemedicine service system on patients with hypertension at home in rural areas. Methods: The study was designed to be a retrospective case-control study. The subjects of this study were 152patients with hypertension who were managed by community health practitioner posts; using telemedicine service system group (n=76), usual care group (n=76). The data was collected through EMR (electric medical record) from September to December, 2013, and analyzed by descriptive statistics of $x^2$/t-test and ANCOVA. Results: The analysis showed that telemedicine service system was an effective way to deal with body weight (F=4.723, p=.031) and BMI (F=5.225, p=.024). Conclusions: This study provides evidence for utilizing the telemedicine service system based on information technology as intervention method in the hypertension management.
