Diagnostic and Clinical Differences in Obstructive Sleep Apnea Syndrome and Upper Airway Resistance Syndrome (폐쇄성 수면 무호흡 증후군과 상기도 저항 증후군의 진단적 및 임상적 차이)

  • It has been controversial whether upper airway resistance syndrome (UARS) is a distinct syndrome or not since it was reported in 1993. The International Classification of Sleep Disorders classified UARS under obstructive sleep apnea syndrome (OSAS) in 2005. UARS can be diagnosed when the apnea-hypopnea index (AHI) is fewer than 5 events per hour, the simultaneously calculated respiratory disturbance index (RDI) is more than 5 events per hour due to abnormal non-apneic non-hypopneic respiratory events accompanying respiratory effort related arousals (RERAs), and oxygen saturation is greater than 92% at termination of an abnormal breathing event. Although esophageal pressure measurement remains the gold standard for detecting subtle breathing abnormality other than hypopnea and apnea, nasal pressure transducer has been most commonly used. RERAs include phase A2 of cyclical alternating patterns (CAPs) associated with EEG changes. Symptoms of OSAS can overlap with UARS, but chronic insomnia tends to be more common in UARS than in OSAS and clinical symptoms similar with functional somatic syndrome are also more common in UARS. In this journal, diagnostic and clinical differences between UARS and OSAS are reviewed.

  • Neuropathy is a common and often debilitating complication of diabetes, Diabetic peripheral neuropathy(DN) includes a variety of different disorders that affect the peripheral nervous system. The most common type of DN is the predominantly sensory distal polyneuropathy. Typically, symptoms begin in the foot and proximally during the course of the discease, reflecting the fact that longer fibres are involved earlier that shorter ones. Reviewed the pathogenesis, the diagnosis of DN, the gait pattern and the excercise, the treatment of pain in DN patient.

Differential diagnosis of vertigo (어지럼증의 감별진단)

  • Vertigo and dizziness are common symptoms with various etiologies and pathogeneses. Vertigo is an illusion of motion due to disease of the vestibular system, usually a sense of rotation. Dizziness, a term that represents a wide range of non-vertigo symptoms, is commonly associated with non-vestibular disorders including old age, cardiac syncope, orthostatic hypotension, metabolic disease, anxiety, and drugs. Vertigo should be determined whether the cause is central or peripheral. Peripheral vertigo is usually benign but central vertigo is serious and often require urgent treatment. The careful history and detailed physical examinations(pattern of nystagmus, ocular tilt reaction, head impulse test and positional tests such as Dix-Hallpike maneuver) provide important clues to the diagnosis of vertigo. Most of patients have benign peripheral vestibular disorders - vestibular neuritis, benign paroxysmal positional vertigo (BPPV), and Meniere's disease. BPPV is a leading cause of peripheral vertigo and can easily be cured with a canalith repositioning maneuver. In this review, a focus is on the differential diagnosis of common vestibular disorders with peripheral and central causes.

Wind Attribute Time Series Modeling & Forecasting in IRAN

  • A wind speed forecast is a crucial and sophisticated task in a wind farm for planning turbines and corresponds to an estimate of the expected production of one or more wind turbines in the near future. By production is often meant available power for wind farm considered (with units KW or MW depending on both the wind speed and direction. Such forecasts can also be expressed in terms of energy, by integrating power production over each time interval. In this study, we technically focused on mathematical modeling of wind speed and direction forecast based on locally data set gathered from Aghdasiyeh station in Tehran. The methodology is set on using most common techniques derived from literature review. Hence we applied the most sophisticated forecasting methods to embed seasonality, trend, and irregular pattern for wind speed as an angular variables. Through this research, we carried out the most common techniques such as the Box and Jenkins family, VARMA, the component method, the Weibull function and the Fourier series. Finally, the best fit for each forecasting method validated statistically based on white noise properties and the final comparisons using residual standard errors and mean absolute deviation from real data.

  • Public library user studies are required for better services and circulation records can be used as specific library data to identify common user patterns of the libraries. This study examined 754,198 of circulation data set - number of users, frequencies of circulation, volume of time of circulation and etc. The results of this study indicate the possibility of using circulation data to analyze the usage patterns in public libraries and to provide major types of users, collections, time of circulation, and common library user behaviors for better public library services.

  • Melasma is a common disorder that causes dark colored patches. It generally causes brown spots on the face, especially on the forehead, cheeks, and upper lips. The pattern of patches has bilateral symmetry in yellowish-brown to gray-brown colors. It is much more common in women than in men. Melasma is considered to be caused by environmental and physical constitutional factors and often occurs when a woman's hormone changes by pregnancy or the use of oral contraceptive pills. It is important to combine oriental medicine and Sasang constitutional medicine for treatment of Melasma. Normal treatment of Melasma includes warming oneself, removal of the emotional stress factors, and good nutrition.

A Novel Synthetic Route to Highly Cross-Linked Poly(vinyl ethers): Ⅲ. Synthesis and Free Radical Polymerization of Aryloxyethyl Vinyl Ethers Having an Electron Acceptor in ortho- or meta-Position

  • ο-(2-Vinyloxyethoxy)benzylidenemalononitrile (3a) and methyl ο-(2-vinyloxyethoxy)-benzylidenecyanoacetate (3b), m-(2-vinyloxyethoxy)benzylidenemalononitrile (4a), and methyl m-(2-vinyloxyethoxy)benzylidenecyanoacetate (4b) were prepared by the condensation of ο-(2-vinyloxyethoxy)benzaldehyde (1) and m-(2-vinyloxyethoxy)benzaldehyde (2) with malononitrile or methyl cyanoacetate, respectively. Bifunctional vinyl ether monomers 3a and 3b polymerized quantitatively with radical initiators in γ-butyrolactone solution at 65 ℃, while meta-isomers 4a and 4b gave lower yields of polymers under the same conditions. The polymers 5-6 obtained from the monomers 3-4 were insoluble in common solvents due to cross-linking. Under the same polymerization conditions ethyl vinyl ether polymerized well with model compounds ο-methoxybenzylidenemalononitrile 7a, methyl ο-methoxybenzylidenecyanoacetate 7b, m-methoxybenzylidenemalononitrile 8a, and methyl m-methoxybenzylidenecyanoacetate 8b, respectively, to give 1:1 alternating copolymers 9-10 in high yields. Cross-linked polymers 5-6 showed a thermal stability up to 300 ℃, and showed a double phase degradation pattern in their TGA thermograms. Polymers 5-6 showed broad endothermic bands around 75-110 ℃ without any characteristic Tg peaks in DSC thermograms. Alternating copolymers 9-10, except copolymer 9b were soluble in common organic solvents. The inherent viscosities of polymer 9-10 were in the range of 0.35-0.62 dL/g. Polymer films cast from acetone solution were cloudy and tough and Tg values obtained from DSC thermograms were in the range of 118-165 ℃.

Bilateral unusual branching pattern of the external carotid artery in a human cadaver

  • Comprehensive understanding of the variations in the branching of the external carotid artery (ECA) is essential to minimizing vascular complications during cranio-facial and neck surgical procedures. We demonstrate a rare case of unusual branching of ECAs in both carotid triangles and anomalous origin of the left ascending pharyngeal artery (APA) during dissection of embalmed cadaver. The right and left common carotid arteries (CCA) bifurcated at the level of the upper border of the thyroid cartilage. The right superior thyroid artery (STA) originated anterior to the carotid bifurcation (CB), while the left STA originated from the anterior aspect of the left CCA. The right ECA trifurcated into linguofacial trunk, APA, and distal ECA, 15.7 mm from CB. On the left side, lingual artery and APA arose as a short common linguopharyngeal trunk, 1.9 mm from CB. The left facial and occipital arteries originated anteromedially and posteriorly at the same level.

A Study on Pull-out Capacity and Shear resistance strength change by grasses (초본류에 의한 인발저항력 및 전단저항력 변화에 관한 연구)

  • It was tested in field that a Pull-out Capacity and Shear resistance strength change of reed, common reed and sedge which were planted by mat-type turf and used for revegetation of bank. The testes were done for 9 weeks from end of May and the grasses were planted on sandy soil. Roots grew fastly after planted and increasement of a common reed and sedge root were reduced after 4 weeks but increasement of reed roots were not reduced. The difference of increasement of roots is due to a difference of propagation method. Sedge propagate by seed. Reed and commom reed propagate by seed and subterranean stem and reed has bigger subterranean stem than common reed. So increasement of common reed and sedge roots were slow than reed. By root growth pattern, increasement of pull-out capacity and shear resistance strength showed very similar way of root growth, those of common reed and sedge were fast in early stage of cultivation but were reduced. But increasement of pull-out capacity and shear resistance strength of reed was not reduced. A Maximum shear resistance strength called critical shear resistance strength of common reed and sedge can be Analyzed at 11 weeks after planted.

Characteristics of Small Group Discussions About Friction in Terms of the Formation of Common Context (공통맥락 형성의 관점에서 살펴본 마찰력에 대한 소집단 토론의 특징)

  • In this study, we observed the characteristics of students' small group discussions concerning the four friction problems. Participants in this study were 22 students of upper-level mechanics course and their small group discussions have been transcribed. As a result, we found that the phenomenon in this study is well defined by 'common context.' The process of formation of the common context was explicitly observed when students discussed about the identification of the problem situation (especially the movement of A in the second problem), the nature of friction and various forces, inertial frame, and noninertial reference frame. Meanwhile, the formation of common context was tacit when students thought they already had a common context. For example, students did not discuss about the friction rule itself because they had confidence about the knowledge. We also found that the presence of the questioner, receiver, and the other opinion were important for positive group discussions. The result of this study would be meaningful because it analyzed how the theme affects the group discussion beyond the limit of previous studies of just analyzing the form or pattern of discourse.
