A Clinical Study on Soyangin Prescription (소양인처방(少陽人處方) 활용(活用)에 대한 임상적(臨床的) 연구(硏究))

Objectives Due to a lack of detail guidelines for prescriptions to ordinary and pathological symptoms in ${\ulcorner}$DongyiSuseBowon${\lrcorner}$, doctors depend on their own decision when give a prescription. This study aimed to compare ordinary symptoms and pathological symptoms in regards to six prescriptions being applied to Soyangin to utilize them as guidelines in practices. 2. Methods Date were collected from September, 2003 to August, 2004 at the Sasang Constitutional clinic of Pohang Oriental Medicine Hospital. Patients were first asked to fill Out a questionnaire of symptoms that they had at the time of visiting. Data from 273 Soyangin patients whose chief complains got alleviated after taking the prescribed drug were analyzed to examine if there were differences in prescriptions according to ordinary or pathological symptoms complained. 3. Results and Conclusions 1. Hyungbangpaedog-san(Paedog) was given more to the patients having good digestive function than Hyungbangjihwang-tang(Hyungji) and Hyungbangdojeok-san (Dojeok) ; Dojeok was given for bloating symptom compared to Hyungbangsabaek-san(Sabaek). Yangkyuksanhwa-tang(Yangkyuk) and Dojeok were more like for patients with stomachache compared to Paedog. Doghwaljihwang-tang(Dogji) was prescribed to patients complaining frequent voiding compared to Sabaek while Sabaek was given for patients who prefer cold water and feel their body warm compared to other prescriptions. Yangkyuk and Dojeok were prescribed more likely to patients with feeling of suffocation than Sabaek. 2. In symptom comparisons between one prescription and the group of the rest of 5 prescriptions, Paedog was found to be prescribed when patients had good digestive function while Dojeok was given in symptoms cases of indigestion, bloating, constipation and dizziness. Sabaek was applied more for patients having tendency of preferring cold water and feeling of heat body as well as good digestive function. Hyungji was used for patients complaining a symptom of fatigue, in addition Yangkyuk was given to patients who were nervous, sensitive, or impatient when compared to Other prescriptions. Dogji was applied to frequent voiding, insomnia, tingling of extremities, backache or leg pain, or in case of tendency of catching a common cold.

The Complaining Rate of Physical Self -Consciousness Symptoms for College Students in Taegu, Korea (대구지역 대학생의 신체자각증상 호소율)

  • This research was conducted to understand and analyze the physical awareness symptoms of college students and get fundamental research data essential to provide efficient student guidance and public health service at schools. The survey was carried out for 1 month from May 7th, 2001 to June 7th, 2001. The interviewees were 950 enrolled students at junior colleges in Daegu. 26.5% of them were men and 73.5% women. Among the surveyed, public health majors were 76.9%. Daytime students were 64.1 %, and the majority of the students, 42.8%, were atheistic. 56.0% of the students grew up in big cities, 22.2% in medium and small-sized cities, and 21.8% in farming and fishing communities. 66.1 % used public transportation. The majority of the students, 48.7%, answered that the satisfaction level for their majors was average. The result shows that the ocular fatigue was indicated as a greatest complaint, and the female students showed the higher complaint rate than the males. In the study of the complaining rate of physical symptoms by major, the public health majors showed the highest complaining rate in gasping when going up/down stairs, shoulder pain, a burning feeling on empty stomach, and leg/knee pain. On the other hand, the highest complaint factors for the technical majors were ocular fatigue and the feelings of heavy head. In the study of the complaining rate of physical symptoms by transportation, gasping during going up/down stairs and shoulder pain were most common, which were 52.2% and 36.0% respectively. In the study of the insecurity response distribution by transportation, college students who drive a car showed 3.8% and students using public transportation 3.7%. In the study of the insecurity response by subject satisfaction, students who answered dissatisfactory were 8.3%, average 3.9%, satisfied 3.0%. Therefore, it was concluded that students who were dissatisfied with their subjects showed the higher rate of insecurity response. it can be recommended that a school health education program regarding ocular fatigue and shoulder pain should be developed.

Collaborative Study of Oriental-Western Medicine on HIVD (요추추간판탈출증의 동·서의학적 협진연구)

  • Herniated lumbar intervertebral disc(HIVD) is the most common reason causing low back pain and leg radiating pain. The purpose of this study is to see the good of Oriental-Western medicine on HIVD. In order to this syudy, HIVD patients divided into two groups: Group with Oriental medicine(GOM) and Group with Oriental-Westem medicine(GOWM). The results of treatments were summarized as follows. 1. If We investigate a state of patients, GrIII type is 17 cases in GOM and is 16 cases in GOWM. In the GOM, Gr II shows better than the others and in the GOWM, GrI, III shows better than the others. 2. The results of treatments in GOM and GOWM is the good in a general way. It shows excellent type at 10 cases in the GOM, and shows excellent type at 14 cases in GOWM. 3. In Acute stage(in one month), subacute stage(one month~six months) and chronic stage(over six months), the therapic point of GOM and GOWM is 2.9~3.5 on the average. 4. If we notice the effect point of HIVD, in GOM, the point of bulging type is 3, protruded type is 3.5 extruded type is 3. But in GOWM, the point of bulging type is 3.7, protruded type is 3.5 extruded type is 2.7.

  • The person who gets a ALS(Amyotrophic Lateral Sclerosis) has language disability and physical disability together. A common first symptom is a painless weakness in a hand, foot, arm or leg, which occurs in more than half of all cases. Other early symptoms include muscle weakness of speech. In the early stage of this disease they can communicate with other persons, but it will become increasingly difficult. In our research we have designed and implemented communication tools for them. We have implemented Chunjiin(the Korean computer keyboard) at PDA(personal digital assistant). And we have also implemented software which is consisted of frequently used words.

  • Advances in FPGA technology have enabled fast real-time simulation of power converters, filters and loads. FPGA based HIL (Hardware-In-the-Loop) simulators have revolutionized control hardware and software development for power electronics. Common time step sizes in the order of 100ns are sufficient for simulating switching frequency current and voltage ripples. In order to keep the time step as small as possible, ideal switching function models are often used to simulate the phase legs. This often produces inferior results when simulating the discontinuous conduction mode (DCM) and disabled operational states. Therefore, the corresponding measurement and protection units cannot be tested properly. This paper describes a new solution for this problem utilizing a discrete-time PI controller. The PI controller simulates the proper DC and low frequency AC components of the phase leg voltage during disabled operation. It also retains the advantage of fast real-time execution of switch-based models when an accurate simulation of high frequency junction capacitor oscillations is not necessary.

A Case Report of Pancreatic Cancer with Liver Metastasis Patient Treated with Integrative Cancer Treatment (췌장암 간전이 환자의 통합 암 치료에 대한 증례보고)

  • Objective: The purpose of this study is to report effects and potential of Integrative Cancer Treatment (ICT) on metastatic pancreatic cancer patient. Methods: A 79-year-old pancreatic cancer patient diagnosed with metastasis on liver visited the Daejeon Korean medicine hospital of Daejeon university East West Cancer Center (EWCC) on May 2021. The patient has been received chemotherapy (gemcitabine plus abraxane) and concurrently treated with ICT since May 2021. The clinical outcomes were measured by computed tomography, laboratory findings including tumor markers (CEA, CA19-9) and numeric rating scales (NRS). Laboratory analysis and National Cancer Institute Common Terminology Criteria for Adverse Events (NCI-CTCAE), version 5.0 were used to evaluate the safety of ICT. Results: After treatment, constipation was relieved from NRS 5-6 to 2, both leg numbness was improved from NRS 9 to 2. Tumor size was generally decreased accompanying by reducing the levels of tumor markers. There were no severe adverse events induced by ICT based on NCI CTCAE version 5.0. Conclusion: This case study suggests that ICT in combination with chemotherapy may help in the treatment of patients with metastatic pancreatic cancer.

Epidemiology and Incidence of Orthopedic Fractures in the Military of the Republic of Korea

  • Purpose: Fractures are common in the military population, but limited studies have investigated the incidence of fractures among Korean military personnel. Hence, this study aimed to clarify this issue. Methods: Eligible subjects were patients who had sustained a fracture and were registered in the N-DEMIS (the medical records system of participating hospitals) from June 2017 to May 2019. Fractures were categorized according to the fracture site, patients' age, sex, and type of duty. Results: In total, 23,687 patients with 23,981 fractures were included. There were 216 patients with multiple fractures, of whom 156 had fractures at two sites, 42 had fractures at three sites, and 18 had fractures at four sites. Of the 23,687 patients, 23,340 were men and 347 were women. The incidence of fractures in men and women was 12.96 per 1,000 person-years and 0.19 per 1,000 person-years, respectively. In terms of the broad location of fractures, the percentage of fractures was the highest in the hand, followed by the foot and lower leg. When the location of fractures was analyzed more specifically, the percentage of fractures was the highest in the phalanx (thumb and fingers), followed by the ankle and metacarpal bones. Conclusions: Hand, foot, ankle, and wrist fractures were the most commonly encountered fractures in the Korean military population. To prevent the loss of combat power due to non-battle-related injuries, thorough preparation is necessary, including protective equipment and preliminary training for areas with a high frequency of fracture occurrence.

Korean Medical Treatment for Partial Rupture of Gastrocnemius Muscle Observed by Ultrasonography: A Case report (초음파를 통해 경과관찰한 비복근 부분파열 환자의 한의학적 치료: 증례보고)

  • Gastrocnemius muscle partial rupture is a common muscle injury. This case is report on a patient with a gastrocnemius muscle partial rupture who was continuously observed with ultrasonography while receiving Korean medicine treatment. Acupuncture, pharmacoacupuncture, herbal medicine, physical therapy and rehabilitation treatment were performed on a patient diagnosed with gastrocnemius partial rupture. The improvement of symptoms was evaluated using Numeric Rating Scale (NRS), Range of Motion (ROM) of ankle, circumference of calf and size of hematoma by using ultrasonography. NRS decreased more than 90%, ROM of ankle joint gradually improved, calf circumference improved, and hematoma reduced by more than 90% when measured by ultrasonography. In this study, gastrocnemius muscle partial rupture was observed with ultrasonography, and it is considered that the Korean medical treatment is useful for gastrocnemius muscle partial rupture.

Case Report of Chemotherapy-Induced Peripheral Neuropathy Treated with Korean Medicine (항암화학요법 유발 말초신경병증에 대한 한방치험 1례)

  • Introduction: The aim of this study is to report on the effectiveness of Korean medicine to improve symptoms of chemotherapy-induced peripheral neuropathy (CIPN). Some patients are treated with medication, but medications may have no effect in some patients. Korean medicine treatments have been used in such patients, but few reports exist. Case Presentation: A 58-year-old female patient with CIPN reported symptoms of bilateral limb pain and tingling sensations as well as cold sensations in bilateral lower legs and hands, fatigue, and bilateral limb weakness. To reduce the patient's symptoms, we provided Korean medical treatment, including herbal medicine (Uchashinki-hwan), acupuncture, and moxibation. To evaluate the results of this treatment, we used a numeric rating scale (NRS), the manual muscle test (MMT), the National Cancer Institute Common Toxicity Criteria for Adverse Events (CTCAE), and the European Organization for Research and Treatment of Cancer quality-of-life questionnaire about CIPN (EORTC-QLQ-CIPN20). After 49 days of treatment, the NRS scores decreased for bilateral limb pain and tingling, for bilateral lower leg and hand cold sensation, and for fatigue. The MMT results for the upper and lower extremities were improved. CTCAE Grade and EORTC QLQ-CIPN20 score were also decreased. Conclusion: According to these results, Korean medicine treatment may be considered an effective treatment for CIPN. Prospective studies are needed in the future to confirm and expand these findings.

Bilateral foot drop caused by T12 infectious spondylitis after vertebroplasty: a case report

  • The most common cause of foot drop is lumbar degenerative disc herniation, particularly at L4/5. We present a rare case of spinal cord injury accompanied by a thoracolumbar lesion that presented with bilateral foot drop. A 69-year-old male patient presented with sudden-onset severe bilateral leg pain and bilateral foot drop. Radiologic findings revealed T12 spondylitis compressing the conus medullaris. He had undergone vertebroplasty for a T12 compression fracture after a fall 6 months before. A physical examination showed bilateral foot drop, paresthesia of both L5 dermatomes, increased deep tendon reflex, and a positive Babinski sign. An acute bilateral L5 root lesion and a conus medullaris lesion were suspected based on electromyography. A surgical procedure was done for decompression and reconstruction. After the operation, bilateral lower extremity muscle strength recovered to a good grade from the trace grade, and the patient could walk without a cane. The current case is a very rare report of bilateral foot drop associated with T12 infectious spondylitis after vertebroplasty. It is essential to keep in mind that lesions of the thoracolumbar junction can cause atypical neurological symptoms. Furthermore, understanding the conus medullaris and nerve root anatomy at the T12-L1 level will be helpful for treating patients with atypical neurological symptoms.
