A Review on Clinical Studies of Herbal Treatment for Infertility Caused by Polycystic Ovary Syndrome -Focusing on Chinese Clinical Trials- (다낭성 난소증후군의 난임치료에 대한 한약임상연구 경향 분석 -중국의 임상연구를 중심으로-)

  • Objectives: The purpose of this study is to review the trend of chinese clinical studies of herbal treatment for infertility caused by Polycystic Ovary Syndrome (PCOS). Methods: We searched the clinical studies published from 1979 to 2013 through CAJ (Chinese Academic Journal) with keywords of "PCOS and 不孕" or "多囊卵 巢綜合證 and 不孕". We analyzed the intervention type of clinical studies. In addition, We researched randomized controlled trials (RCTs) published from 2009 to 2013, assessed the quality of RCTs using the Jadad Scale and reanalyzed the intervention type of RCTs. Results: One hundred and seventeen clinical studies were finally selected. Clinical studies of herbal treatment for infertility caused by PCOS increased remarkably after the 2000s in China. Combined herbal medicine and western medicine was used the most frequently. RCTs published from 2009 to 2013 were fifty. Average of Jadad score of RCTs was 1.2. Combined herbal medicine and western medicine was also the most frequently used as intervention in RCTs. Conclusions: Clinical studies on effect of combined herbal medicine and western medicine for infertility caused by PCOS were mostly published in China. But Quality of studies was generally low. Further research on effect of herbal treatment for infertility caused by PCOS is needed.

  • Internal deficiency of vital Qi is origination of cancer and root cause of its development from the viewpoint of traditional Chinese medicine(TCM). For about 40 years, China has developed a combined system of TCM and Western medical system in treatment of cancers, laying stress on the therapeutic principle of strengthening the body resistance and restoring normal functioning of the body to consolidate the constitution (Fu Zheng Pei Ben). According to the results of the latest clinical and experimental studies, Chinese medicine not only increases curative effectiveness, but also reduces the side effect caused by the toxic, radio and chemical treatment of cancer cases in the western medical system as well as various complications. Long-term taking Chinese herbal medicine prevents recurrence and metastasis of tumor, and heightens long-term curative effectiveness after western medical treatments. It also helps to relieve symptoms, improve the quality of life, and prolong life of terminal cancer patients who have already lost opportunities of western medical treatments. In addition, it can prevent and cure premonitory symptoms of cancer.

A Review of Randomized Controlled Trials on the Treatment Effects of Herbal Medicine for Threatened Abortion (절박 유산의 한약 치료 효과에 대한 무작위 대조 임상시험 연구 고찰)

  • Objectives: This review is aimed to figure out the effectiveness of herbal medicine for treating threatened abortion by analyzing randomized controlled trials. Methods: We searched relevant studies using seven databases including 4 domestic databases and 3 foreign databases. Data retrieval was conducted on May 18, 2022. and the papers published from January 1, 2018 to May 18, 2022 were included. The risk of bias was assessed by using Cochrane's risk of bias tool. Results: 182 studies were searched in domestic and foreign databases, and 9 studies were finally selected. Among 9 studies, the treatment group was treated with only oral herbal medicine in 4 studies and with combined treatment of herbal medicine and western medicine in 5 studies. On the other hand, the control group was treated with only western medicine. In all 9 studies, evaluation indexes in the treatment group showed more effectiveness than those of the control group, evaluation indexes were different for each study though. Conclusions: Herbal medicine was more effective for treating threatened abortion whether using it alone or combining with western medicine compared to using western medicine alone. Additional studies which include large number are needed to confirm long-term effectiveness and safety of herbal medicine treatment.

  • Clinical studies were carried out 83 cases of patients with colorectal cancer treated by Hangamdan(抗癌丹) from January 1th 1998 to September 30th 2000. The results were summarized as follows; 1. Distribution of those attached by colorectal cancer, by sex, showed that Male is more then Female, by age, showed that the number of fifties is majority. 2. Distribution of diagnostic stage, in descending order; stage III(53%, top), stage IV(45.8%). 3. The effects of maintenance and improvement in the symptoms with traditional oriental therapy(83.3%) and combined treatment of western and oriental therapy(92.1%) were observed. The effects of the symptoms were as follows: diarrhea(37.3%), abdominal pain (25.3%), general body weakness(22.9%), nausea(20.5%) and etc. in orders. 4. Analysis of hematology attached by colorectal cancer, maintenance and increasing of WBC(89.9%), RBC(74.7%), Hgb(81.1%), Platelet(92.4%) were observed. After taken Hangamdan, the safety of the liver and kidney were as follows; maintenance and decreasing of AST(85.9%), ALT(94.8%), GTP(87.5%), Creatinine(90.9%) were observed. 5. of IL-12 and $IFN-\gammer$ attached by colorectal cancer, increasing of IL-12(53.3%), IFN-{\gammer}(80%)$) were observed. 6. Analysis of QOL attached by colorectal cancer, maintenance and improvement of combined treatment of western and oriental therapy(89.6%), traditional oriental therapy(83.3%) were observed. 7. Analysis of survival in patients with IV stage of colorectal cancer, above 7 months(18.4%), 12 months(65.8%). 8. Analysis of antitumor effects, maintenance of traditional oriental therapy(83.3%) and maintenance and improvement of combined treatment of western and oriental therapy(80.5%) were observed. Analysis of tumor marker attached by colorectal cancer, maintenance and decreasing of CEA(78.8%) were observed. 9. Analysis of curative valuation, maintenance and improvement of traditional oriental therapy(83.3%), combined treatment of western and oriental therapy(72.7%) were observed. From the above results, it is suggested that Hangamdan has significant effects of antitumor and immune activity, also could be usefully applied for colorectal cancer patients by combination with western therapy or alone.

  • Clinical studies were carried out 62 cases of patients with lung cancer treated by Hangamdan(抗癌丹) from January 1th 1998 to September 30th 2000. The results were summarized as follows; 1. The effects of the symptoms were as follows; cough(50.0%), anorexia(48.4%), chest discomfort(31.0%), sputum(24.2%), general body weakness(11.3%), hemoptysis(9.7%) and etc. in orders. 4. Analysis of hematology attached by lung cancer, maintenance and increasing of WBC(98.4%), RBC(74.2%), Hgb(71.0%), Platelet(96.7%) were observed. After taken Hangamdan, the safety of the liver and kidney were as follows; maintenance and decreasing of AST(91.5%), ALT(93.2%), ${\gamma}-GTP$(95.0%), BUN(82.7%), Creatinine(93.3%) were observed. 5. Analysis of IL-12 and $IFN-{\gamma}$ attached by lung cancer, increasing of IL-12(31.3%), $IFN-{\gamma}$(72.7%) were observed. 6. Analysis of QOL attached by lung cancer, maintenance and improvement of combined treatment of western and oriental therapy(94.0%), traditional oriental therapy(91.7%) were observed. 7. Analysis of survival in patients with IV stage of lung cancer, above 7 months(22.2%), 12 months(70.4%). 8. Analysis of antitumor effects, maintenance of traditional oriental therapy(50.0%) and maintenance and improvement of combined treatment of western and oriental therapy(80.0%) were observed. 9. Analysis of curative valuation, maintenance and improvement of traditional oriental therapy(50.0%), combined treatment of western and oriental therapy(60.0%) were observed. From the above results, it is suggested that Hangamdan has significant effects of antitumor and immune activity, also could be usefully applied for lung cancer patients by combination with western therapy or alone.

  • Objectives : This is a study of ischemic stroke patients designed for comparing Korean and Western medical treatments and Korean medical treatments alone. Methods : 44 patients were diagnosed by B-MRI scan as having suffered ischemic stroke. They had entered Dong-eui Korean Medicine hospital within seven days of attack, and remained over seven days, all between May 2005 and October 2007. Patients were divided into two groups; a group treated with Korean medical treatments, and another group treated with Korean and Western medical treatments (but in both groups examinations were done and Western medications were given for hypertension, diabetes mellitus and so on) Results : NIHss change one month later was not statistically significant for either groups but the NIHss gap between them was significant. Conclusions : NIHss change one week later was not statistically significant for either group. NIHss change one month later was not statistically significant for either groups but the Korean medical treatment group had a significant NIHss gap more than the Korean and Western medical treatment group.

Clinical study in 320 cases for cancer patients on the effect Hangamdan(抗癌丹) (항암단(抗癌丹)을 투여(投與)한 각종(各種) 암환자(癌患者) 320례(例)에 대(對)한 고찰(考察))

  • The purpose of this study was to investigate the effects of Hangamdan(抗癌丹). The clinical study was carried out 320 cases of patients with cancer treated by Hangamdan(抗癌丹) from May 1st 1998 to September 1st 1999. The results were summarized as follows; 1. The effects of improvement in the symptoms with traditional oriental cancer therapy(47.6%) were higher than combined treatment of western and oriental therapy(37.4%). 2. In the analysis of hematology, maintenance and increasing of WBC(86.2%), Hgb(87.2%), P1atelet(97.6%) RBC(81.1%) were observed. In the analyses of tumor marker, maintenance and decreasing of CEA(76%), CA19-9(88.8%), AFP(69.2%) were observed. 3. In the analysis of safety, maintenance and decreasing of AST(93.1%), ALT(95%), BUN(92.2%), Creatinine(93.6%) were observed. 4. In the analysis of QOL attached by cancer, combined treatment of western and oriental therapy(maintenance and improvement; 91.8%) was higher than traditional oriental cancer therapy(maintenance and improvement; 79.3%) 5. In the analysis of survival in patients with terminal cancer, above 6 months(46.3%), 12 months(19.2%). 6. In the analysis of antitumor effects, combined treatment of western and oriental therapy(maintenance 71.6% improvement 12.8%) was higher than traditional oriental therapy (maintenance 66.7% improvement 9.5%). 7. In the analysis of curative evaluation, combined treatment of western and oriental therapy(maintenance 40.4% improvement 41.8%) was higher than traditional oriental therapy (maintenance 23.8% improvement 46.1%). 8. In the analysis of IL-12 and IFN-$\gamma$ attached by cancer, increasing of IL-12(32.3%), IFN-$\gamma$(41.5%) were observed. From the above results, it is suggested that Hangamdan has significant effects of antitumor and immune activity, also could be usefully applied for cancer patients by combination with western therapy or alone.

The Effect of a Cooperative System of Oriental and Western Medicine in the Treatment of Allergic Rhinitis

  • Background and Purpose : Allergic rhinitis is a well-known, relatively controllable chronic disease. Although a number of methods for treating allergic rhinitis have been tried, many patients have not been satisfied with their treatment. Therefore, this study tried to evaluate the effect of a cooperative system of Oriental and Western medicine and to develop a new diagnosis protocol for treatment of allergic rhinitis. Methods : We measured improvement rate and acoustic rhinometry after the allergeninduction test and performed a filter paper test as a nonspecific hypersensitivity test with 60 patients who are allergic to house dust mite. Patients were divided into two groups, one treated with Western medicine only and one treated with both Western and Oriental herbal medicine. For the group with Western medicine only, antihistamine for one week and local steroid medicine for two weeks were prescribed. For the group with combined medicine, Oriental herbal medicine was prescribed according to the patient s constitution, along with Western medicine. After all treatments, the above tests were re-performed and the improvement rate was compared. Results and Conclusion : We observed better results in the group treated with both Western and Oriental herbal medicines, comparing improvement rate and the alteration of total nasal volume through acoustic rhinometry after the allergen induction test. In the filter paper test, there was no significant difference between the two groups. In conclusion, we showed the additive effect of Oriental herbal medicine without any severe side effects compared with treatment with Western medicine only. In this study, we set only two patient groups, but further study is required to create various experimental groups and compare among them. We suggest that it might enhance understanding of the improved effect of Oriental herbal medicine in the therapy of allergic rhinitis.

Eastern and Western Combined Medical Treatments on 11 Patients with Dry Eye Syndrome (안구건조증환자 11례의 한·양방 병용치료에 대한 임상보고)

  • Objective : The endpoint of this trial is to verify therapeutical availability of conventional western eyedrops combined with acupuncture treatment by means of BUT(tear film break-up time), Schirmer's test, which have been considered to be typical methods for diagnosing dry eye syndrome. Methods : The subjects of this trial included the outpatients who had been diagnosed with dry eye syndrome in Pusan national university hospital and underwent acupuncture treatment from March 2011 to April 2011. They received acupuncture treatment once a week, three times while constantly using Restasis or artificial tears. We collated and analyzed the outcome data of Schirmer's test and BUT conducted at the beginning of this trial and three times more after each acupuncture treatments. Acupoints, such as, Chanjuk(BL2), Eoryo, Sajukgong(TE23), Taeyang, Sabaek(ST2), limited around both eyes were selected based on their clinical effects referring to medical books. Results : 1. From BUT test results, Restasis and acupuncture combined group showed statistically significant difference, however, artificial tears and acupuncture combined group showed difference only between before and after the treatment, which is not statistically significant. 2. From Schirmer's test results, Restasis and acupuncture combined group showed statistically significant difference, however, artificial tears and acupuncture combined group showed difference only between before and after the treatment, which is not statistically significant. 3. The average duration of treatment was statistically 16.3 days for Restasis and acupuncture combined group. Conclusions : Combination of acupuncture and Restasis for dry eye syndrome can shorten the duration of treatment. Therefore, clinical benefit is expected to be worth.

Effects of Samryungbaekchul-san on Childhood Anorexia: A Systematic Review and Meta-Analysis (소아 식욕부진에 대한 삼령백출산(蔘苓白朮散)의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives This study aimed to evaluate the effect of Samryungbaekchul-san on childhood anorexia. Methods We searched 11 English, Korean, Chinese, and Japanese databases for studies published up to May 30, 2022. Randomized controlled trials (RCTs) assessing the effect of Samryungbaekchul-san on childhood anorexia were included. In the meta-analysis, relative risk (RR) and 95% confidence intervals (CI) were indicated as dichotomous variables, and mean difference (MD) or standardized mean difference (SMD) and 95% CIs were indicated as continuous variables. Results We included 12 RCTs with 1345 participants. The Samryungbaekchul-san treatment group had a significantly higher total effective rate (TER) than that of the western medicine control group (RR 1.42, 95% CI 1.23-1.64, I2 = 0%). The combined Samryungbaekchul-san and western medicine treatment group had significantly higher TER (RR 1.31, 95% CI 1.23 to 1.40, I2 = 0%) and levels of neuropeptide Y (SMD 0.93, 95% CI 0.47-1.39, I2 = 70%) and ghrelin (SMD 1.45, 95% CI 1.14-1.76, I2 = 0%) than those of the western medicine alone group. Additionally, leptin levels were significantly lower in the combined treatment group (SMD -1.19, 95% CI -1.88 to -0.51, I2 = 86%) compared with the western medicine alone group, although statistical heterogeneity was substantial. Conclusions Samryungbaekchul-san may be effective for childhood anorexia. However, owing to limitations such as high clinical heterogeneity between the studies, unclear risks of biases, and insufficient reports of adverse events and follow-ups, well-designed RCTs with a low risk of bias are needed in the future.
