WHIP SPRING FOR THE TREATMENT OF LOCALIZED TOOTH MALPOSITION IN MIXED DENTITION (혼합치열기의 국소적 치열부정을 위한 Whip Spring)

  • There are various types of localized tooth malpositions in the mixed dentition, such as abnormal tooth axis, anterior crossbite of some incisors, impaction, midline diastema, ectopic eruption, and so forth. We, Pediatric Dentists, have usually used removable appliances for these instances. But, removable orthodontic appliances, as is known, have marked limitations in some situations, for example, severe rotation, intrusion and extrusion, root torque, closure of large diastema, traction of impacted tooth, etc. In such cases, Whip spring, combined with fixed or removable appliance, can increase utilities of removable orthodontic appliances. The authors have applied whip springs to some cases showing localized positional and arrangement problems, and have witnessed the results as follows; 1. The refined and elaborate control of direction and magnitude of force by the operator, and accurate compliance of the patients were requisite for the treatment with it. 2. It showed special effectiveness for de rotation of incisors. Although it yields some benefit for root movement, the special consideration for incomplete roots in this age bracket was required. 3. In the localized malalignment cases in mixed dentition, uncurable with traditional removable appliances but practically unrealistic with fixed appliance therapy, the whip spring was thought to be a good alternative.

Silicone Implant-Based Paranasal Augmentation for Mild Midface Concavity

  • Background: Midface concavity is a relatively common facial feature in East Asian populations. Paranasal augmentation is becoming an increasingly popular procedure for patients with mild concavity and normal occlusion. In this study, we evaluate clinical outcomes following a series of paranasal augmentation. Methods: A retrospective review was performed for patients with Class I occlusion who had undergone bilateral paranasal augmentation using custom-made silicone implants, between October 2005 and September 2013. Patient charts were reviewed for demographic information, concomitant operations, and postoperative complications. Preoperative and postoperative (1-month) photographs were used to evaluate operative outcome. Results: The review identified a total of 93 patients meeting study criteria. Overall, aesthetic outcomes were satisfactory. Five-millimeter thick silicone implant was used in 81 cases, and the mean augmentation was 4.26 mm for this thickness. Among the 93 patients, 2 patients required immediate implant removal due to discomfort. An additional 3 patients experienced implant migration without any extrusion. Nine patients complained of transient paresthesia, which had resolved by 2 weeks. There were no cases of hematoma or infection. All patients reported improvement in their lateral profile and were pleased at follow-up. Complications that arose postoperatively included 9 cases of numbness in the upper lip and 3 cases of implant migration. All cases yielded satisfactory results without persisting complications. Sensations were fully restored postoperatively after 1 to 2 weeks. Conclusion: Paranasal augmentation with custom-made silicone implants is a simple, safe, and inexpensive method that can readily improve the lateral profile of a patient with normal occlusion. When combined with other aesthetic procedures, paranasal augmentation can synergistically improve outcome and lead to greater patient satisfaction.

Facial and occlusal esthetic improvements of an adult skeletal Class III malocclusion using surgical, orthodontic, and implant treatment

  • The aim of this clinical report is to describe the complex treatment of an adult Class III malocclusion patient who was disappointed with the outcome of a previous oral rehabilitation. Interdisciplinary treatment planning was performed with a primary indication for implant removal because of marginal bone loss and gingival recession, followed by orthodontic and surgical procedures to correct the esthetics and skeletal malocclusion. The comprehensive treatment approach included: (1) implant removal in the area of the central incisors; (2) combined orthodontic decompensation with mesial displacement and forced extrusion of the lateral incisors; (3) extraction of the lateral incisors and placement of new implants corresponding to the central incisors, which received provisional crowns; (4) orthognathic surgery for maxillary advancement to improve occlusal and facial relationships; and finally, (5) orthodontic refinement followed by definitive prosthetic rehabilitation of the maxillary central incisors and reshaping of the adjacent teeth. At the three-year follow-up, clinical and radiographic examinations showed successful replacement of the central incisors and improved skeletal and esthetic appearances. Moreover, a Class II molar relationship was obtained with an ideal overbite, overjet, and intercuspation. In conclusion, we report the successful esthetic anterior rehabilitation of a complex case in which interdisciplinary treatment planning improved facial harmony, provided gingival architecture with sufficient width and thickness, and improved smile esthetics, resulting in enhanced patient comfort and satisfaction. This clinical case report might be useful to improve facial esthetics and occlusion in patients with dentoalveolar and skeletal defects.

Clinical and Radiological Outcomes of Anterior Cervical Interbody Fusion Using Hydroxyapatite Spacer

  • Objective : This is retrospective study of clinical and radiological outcomes of anterior cervical fusion using Bongros-$HA^{TM}$ (BioAlpha, Seongnam, Korea) which is a type of synthetic hydroxyapatite (HA) spacer to evaluate the efficacy in its clinical application and usefulness as a reliable alternative to autograft bone. Methods : Twenty-nine patients were enrolled in this study and 40 segments were involved. All patients were performed anterior cervical interbody fusion using HA spacer and plating system. Indications for surgery were radiculopathy caused by soft-disc herniation or spondylosis in 18 patients, spondylotic myelopathy in 1 patient, and spinal trauma in 10 patients. Cervical spine radiographs were obtained on postoperative 1day, 1week, and then at 1, 2, 6, and 12 months in all patients to evaluate intervertebral disc height, and the degrees of lordosis. Cervical computed tomography was done at postoperative 12 month in all patients to confirm the fusion status. The mean period of clinical follow-up was 17 months. Results : Complete interbody fusion was achieved in 100% of patients. Preoperative kyphotic deformities were corrected in all cases after surgery. Intervertebral disc height was well maintained during follow up period. There were no cases of graft extrusion, graft deterioration and graft fracture. Conclusion : HA spacer is very efficient in achieving cervical fusion, maintaining intervertebral disc height, and restoring lordosis. When combined with the placement of a cervical plate, immediate stability can be achieved and graft related complication can be prevented.

The Clinical Results of Meniscus Allograft Transplantation - Comparison between medial, Lateral, Isolated and Combined Procedure Groups - (동종 반월상 연골 이식술의 임상적 결과 - 내측과 외측 및 동반 손상의 유무에 따른 비교 -)

  • Purpose: To compare the clinical outcomes after meniscus allograft transplantation between lateral and medial or isolated and combined procedure groups. Materials and Methods: Of the patients who had undergone arthroscopic meniscal allograft transplantation between Dec. 1997 and Jun. 2007, 52 patients were available for retrospective evaluation. Patients were grouped into lateral(33 cases) and medial(19 cases) transplant groups as well as those with isolated(18 cases) and combined(34 cases) procedure. The average age was 34.2 years and the mean follow-up period was 42.2 months. Postoperative range of motion (ROM), visual analog scale (VAS), International Knee Documentation Committee (IKDC) subjective score, Lysholm score, Tegner score, patient's subjective satisfaction, $2^{nd}$ look arthroscopy and MRI were evaluated retrospectively. Results: Mean postoperative ROM was $130.3^{\circ}$. The VAS showed an improvement from 5.96 to 3.05 at the last follow up. IKDC subjective score and Lysholm score also showed an improvement from 46.5 to 64.5 and from 61.9s to 79.58 respectively. Tegner score was improved from 2.9 to 3.6. Overall, 61.5% of patients reported they were completely or mostly satisfied with procedure. There were no significant differences noted between lateral and medial groups as well as isolated and combined groups. In 2nd look arthroscopy, 10 of 18 cases showed good pheripheal healing and there were 6 cases of partial and 2 of complex tear. We observed graft subluxation or extrusion in 12 of 16 cases who were evaluated with follow-up MRI. Conclusion: Meniscus allograft transplantation alone or in combination with other procedure showed an improvement in knee pain and clinical score. But there were no significant difference between lateral and medial groups or isolated and combined procedure groups.

Chemical Characteristics and Efficacy of Combined Pesticide Granules Formulated by Different Ways (제제방법별(製劑方法別) 혼합입제(混合粒劑) 농약(農藥)의 특성(特性)과 약효(藥效))

  • A comparative study of chemical characteristics, efficacy and relative merits of extruded and coated granules, containing fungicide and insecticide, was conducted under laboratory and field conditions. Probenzole (3-allyloxy-1,2-benzisothiazole 1,1-dioxide) and carbofuran (2,3-dihydro-2,2-dimethyl-7-benzofuranyl methyl carbamate) were chosen as toxicants for rice blast(Pyricularia oryzae) and brown planthopper (Nilaparvata lugens) control, respectively. Stability of active ingredients in coated granule was superior to the extruded one under accelerated temperature. Active ingredient dissolution of coated one into distilled water showed slow release pattern. Pesticide residues in rice (Oryzae sativa, Chucheongbyeo) shoot applied with extruded one at the dosage of 3kg/10a retained higher levels than those with coated one on two days after application, while the residue levels were a reversed tendency on eight days after application. Efficacy on rice blast exhibited minute differences between the granules, on the other hand efficacy on brown planthopper by coated one was of higher rank than that by extruded one. Production cost of the combined pesticide granule by coating method could be cut down by 6% as compared to extrusion method.

  • Adult patients who need implant and prosthodontic treatment often need treatment to improve the existing occlusion through orthodontic treatment for long-term stable treatment results. However, due to non-aesthetic and uncomfortable orthodontic treatment with orthodontic brackets and wires, many adult patients give up treatment even though they know the need for it. Recently, as digital dentistry has affected all areas of dentistry, clear aligner orthodontic systems have begun to be widely used, and their use is increasing in adults and old-aged people due to the esthetic advantage and convenient oral care. SERAFIN clear aligner system developed in Korea has been developed with the aim of implementing a functional occlusion harmony and is used not only for partial orthodontic treatment but also comprehensive orthodontic treatment. This patient presentation is shown the treatment using SERAFIN clear aligner system for the treatment of patient with TMD, severe extrusion of maxillary second molars, and extracted teeth.

Basic Study on the Development of Combined Granular Pesticides for Paddy Rice (수도용(水稻用) 혼합입제(混合粒劑) 농약개발(農藥開發)을 위(爲)한 기초연구(基礎硏究))

  • To obtain informations on the development of the combined granular pesticides for the simultaneous control of rice insect pests and diseases which often occur at the same time or in overlapping experiments were carried out on twelve mixture pesticides of granular type formulated by extrusion method with two fungicides and three insecticides. Each of 12 mixtures was tested for physico-chemical properties and efficacy against to rice leaf blast and brown planthoppers under the laboratory conditions. All of the mixture pesticides showed acceptable physico chemical properties as granular formulation. Most of the compounds in mixture pesticides showed a tendency to have lower stabilities than those of pesticide alone, therefore, it was necessary to add stabilizer in formulation process. Isoprothiolane combinations were found very effective against leaf blast but their control effects were similar to that of isoprothiolane alone due to its high efficacy, while probenazole-carbofuran combinations showed synergistic effects for the control of leaf blast. All of the combinations showed synergistic effects to brown planthoppers. But fenthion and 3% propoxur combinations resulted comparatively low efficacies. In the consideration of their physico-chemical properties and efficacies to rice pests, carbofuran and 6% propoxur combinations were promising combined granular pesticides for the simultaneous control of leaf blast and brown planthoppers, while fenthion and 3% propoxur combinations were not available because of their low efficacies for brown planthoppers.

  • Bio-based plastics containing the biomass content higher than 25 wt% have been considered as environment-friendly materials due to their effects on the reduction in the $CO_2$ emission and petroleum consumption as well as biodegradability after use. In this study, poly vinyl chloride, plant-derived plasticizers, by adding a biodegradable catalyst was observed a change in the biodegradability and physical properties. To produce the oxidative decomposition transparent bio film, which is broken down in the initial percent elongation and physical properties such as tensile strength, it was to test the safety of the product as a food packaging material. Poly vinyl chloride, primary plasticizer, secondary plasticizer, anti fogging agent, the combined stabilizer were mixed in a high speed mixer, then extruded using an extrusion molding machine, after cooling, winding, to produce a oxidative decomposition transparent bio film and the control film, with a thickness of $12{\mu}m$ through winder role. Mechanical properties tensile strength, elongation, and the maximum load elongation and biodegradation test. Transparent bio film produced by biodegradation catalyst is compared with the control film. Tensile strength and elongation of films were found to be no significant difference. Further, as a result of the biodegradation test for 45 days based on the ASTM D6954-04 method, biodegrability of film is 61.4%.
