본 연구는 LUT이 고지방식이로 비만이 유도된 C57BL/6 마우스의 대장암 발생에 미치는 영향을 분석하기 위해 실험동물을 각 10마리씩 정상식이 (ND)군, 고지방식이 (HFD)군, HFD + 0.0025% LUT 보충 (HFD LL)군, 그리고 HFD + 0.005% LUT 보충 (HFD HL)군의 4군으로 분류하였다. 각 실험군은 AOM을 1회 복강 주사하고 AOM 투여 1주일 후 총 3 cycle의 1 ~ 2% 농도의 DSS를 음용수로 공급하여 대장암을 유발하였다. 실험식이는 AOM 발암시점부터 총 11주간 급여하였다. 연구결과, 군간 식이섭취량의 차이는 없었으나 HFD 급여군에서 체중과 식이효율의 유의적인 증가가 나타났으며 HFD군과 비교했을 때 LUT 보충에 따른 체중의 변화는 없었다. 그러나 LUT 보충은 ND군에 비해 HFD군에서 나타난 대장 무게/길이 비, 대장종양 수, 혈장 $TNF-{\alpha}$ 농도, 대장 iNOS와 COX-2 발현을 유의적으로 감소시켰으며 그 효과는 HFD HL군이 HFD LL군보다 높았다. 이러한 결과는 체중조절과는 별개로 LUT이 고지방식이에 의한 대장의 염증반응 억제를 통하여 비만과 연관된 대장암 발생을 억제할 수 있음을 제시하며 향후 비만에 의한 인슐린 저항성 및 adipokine 분비, 그리고 장내 균총 변화에 따른 대장 점막세포 증식과 대장암 발생에 LUT이 어떤 영향을 미치는지에 대한 연구를 더 깊이 있게 수행한다면 비만으로 인한 대장암 발생에 LUT이 효과적인 화학적 예방 (chemoprevention)제로 활용될 수 있을 것으로 기대한다.
Ziyuglycoside-II ($3{\beta}-3-{\alpha}$-1-arabinopyranosyloxy-19-hydroxyurs-12-en-28-oicacid)는 오이풀(Sanguisorba officinalis L.)의 주요 활성 화합물 중 하나로 지혈작용, 항산화활성, 항염증활성 등의 활성이 보고되어 있다. 그러나 염증성 대장암에서의 ziyuglycoside-II의 활성에 관해서는 아직 보고되지 않아 본 연구에서 azoxymethane (AOM)/dextran sulfate sodium (DSS)으로 유발된 대장암 모델에서 ziyuglycoside-II 항종양활성을 조사하였다. 염증성 대장암을 유발하기 위해 BALB/c 마우스에게 AOM을 1회 복강 주사하고 AOM 투여 1주일 후 총 3 cycle의 2% 농도의 DSS를 음용수로 공급 하였다. Ziyuglycoside-II (1 또는 5 mg/kg)는 1주일에 3회 경구 투여하고, 64일에 대장을 적출하였다. 대장 조직에서의 종양 개수를 관찰한 결과 ziyuglycoside-II의 투여가 종양의 형성을 유의적으로 감소시키는 것을 확인하였다. 또한 Western blot 방법과 면역 조직학 분석의 결과, ziyuglycoside-II의 투여가 대장 조직에서 nuclear factor kappa-B 양성 세포와 염증 관련 단백질의 양을 현저히 감소시킴을 관찰하였다. 또한 ziyuglycoside-II 투여에 의해 대장조직내의 세포사멸이 촉진되었고 cleaved-caspase 3, cleaved-caspase 7과 같은 세포사멸 관련 단백질의 발현이 증가함을 확인하였다. 이러한 결과는 ziyuglycoside-II의 투여가 염증반응을 억제하고 세포 자멸을 유도하여 염증성대장암의 발생을 억제함을 시사하고 있다.
목적 대장암 환자에서 선행화학요법(neoadjuvant chemotherapy; 이하 NAC)의 반응을 CT를 이용한 종양퇴행등급(CT-based tumor regression grade; 이하 ctTRG)으로 예측할 수 있는지를 알아보고자 한 연구이다. 대상과 방법 총 53명을 대상으로 NAC 전후 종양의 길이, 두께, 장벽의 패턴과 모양으로 ctTRG를 정하고 부피도 측정했다. 병리 종양퇴행등급(pathologic TRG; 이하 pTRG)을 반응 평가의 기준으로 ctTRG와 연관성을 평가했다. Rödel's TRG 2, 3 그리고 4를 반응군으로 분류하였다. ctTRG와 부피변화로 반응군 및 병리완전퇴행(pathologic complete remission; 이하 pCR)을 예측하는 성능을 비교하였다. 결과 ctTRG와 pTRG는 moderate의 연관성을 보였다(ρ = -0.540). 관찰자간 신뢰도는 substantial으로 보였다(weighted κ = 0.672). 반응군을 예측하는데 ctTRG와 volume 변화의 성능은 유의미한 차이를 보이지 않았다(ctTRG의 Az = 0.749, 반응기준: ctTRG ≤ 3, volume 변화의 Az = 0.794, 반응기준: ≤ -27.1%, p = 0.53). pCR을 예측하는 두 방법 간의 성능도 차이가 없었다(p = 0.447). 결론 ctTRG는 대장암에 NAC 후 반응을 예측할 수 있었고, 그 성능은 종양부피변화 방법과 차이가 없었다.
Aurora kinases represent a novel family of serine/threonine kinases crucial for cell cycle control. Aurora-2 kinase is mainly involved in centrosome function, mitotic entry, and spindle assembly. Aurora-2 kinase overexpression causes centrosome amplification and the formation of multipolar mitotic spindles, which leads to tumor aneuploidy and so it has been found to play an important role in tumorigenicity in many cancers such as colorectal cancer, breast cancer and cervical cancer. Hence, the goal of this study is to identify the correlation of clinicopathlogical factors and overexpression of Aurora-2 kinase in oral squamous cell carcinoma. We studied the immunohistochemical staining of Aurora-2 kinase in 20 specimens of 20 patients with oral squamous cell carcinoma and the relationships between Aurora-2 kinase over expression and each of the clinico-pathological parameters were analyzed by Pearson correlation analysis. Statistical significance was set at P < 0.05. The results were as follows. 1. In the immunohistochemical study of poorly differentiated and invasive oral squamous cell carcinoma, the high level staining of Aurora-2 kinase was observed. 2. The correlation between immunohistochemical Aurora-2 kinase expression and histopathological differentiation of specimens was significant. These findings suggest that overexpression of Aurora-2 kinase may play a important role in carcinogenesis of oral squamous cell carcinoma.
Background: Around 200,000 breast disorders are annually diagnosed all over the world. Fibrocystic changes are the most common breast disorder and fibroadenoma is the most prevalent benign breast tumor. The present study aimed to determine the spectrum, type and prevalence of breast masses in women referred to Shiraz University of Medical Sciences between 2004 and 2012. Materials and Methods: A cross-sectional study was conducted on the diagnostic reports data. Results: A total of 640 samples were studied. Most 57.3% of masses were detected in the left breast, 65%, 28.2% and 6.1% of cases presenting with benign, malignant, and inflammatory lesions, respectively. Among all the samples the most prevalent diagnosis (37.7%) was fibroadenoma and fibrocystic lesions (17%). 174 samples (96% of the malignant cases) were invasive. 6.5% of the benign, and 37% of the malignant cases occurred in post menopause women and the differences were statistically significant. Among those with malignant tumors lymph nodes were involved in 25.6% of menopausal women and 44.2% of non-menopausal ones, and the difference was statistically significant. Conclusions: Regular clinical breast examination beside mammographic follow-ups, especially during menopause, should be carried out as a priority and a national organized program should be designed for screening breast disorders.

Background: Gastro-entero-pancreatic neuroendocrine neoplasms (GEP-NEN) are relatively rare tumors, not equally distributed in gastro-intestinal system. In 2010, a revised version of the WHO classification of GEP-NENs was published. This study reports for the first time the distribution and characteristics of GEP-NEN in a Lebanese population. Materials and Methods: This descriptive retrospective study concerns all the digestive neuroendocrine tumors with their characteristics diagnosed in $H\hat{o}tel$ Dieu de France in Beirut, Lebanon from 2001 to 2012, all the pathology reports being reanalyzed according to the latest WHO 2010 classification. The characteristics and features of GEP-NEN analyzed in this study were age, gender, grade and site. Results: A total of 89 GEP-NENs were diagnosed, representing 28.2% of all neuroendocrine tumors. The mean age of GEP-NEN patients was 58.7 years and the M/F sex ratio was 1.2. The primary localization was as follows: 21.3%(19) pancreatic, 18% (16) gastric, 15.7% (14) duodenal, 11.2% (10) appendix, 10.1% (9) intestinal, 10.1% (9) colorectal (7.9% colonic and 2.2% rectal), 5.6% (4) hepatic, 2.2% (2) ampulla, 1.1% (1) esophageal and 7.9%(5) NOS digestive (metastatic with unknown primary). Of the 89 patients with GEP-NEN, 56.2% (50) were diagnosed as grade I, 11.2% (10) as grade II, 20.2% (18) as grade III and 12.4% (11) were considered as mixed adeno-neuroendocrine carcinomas (MANEC). Conclusions: This study, one of the rare examples based on the 2010 WHO classification of neuroendocrine tumors in the literature, indicates that in the Lebanese population, all duodenal and appendicular tumors are G1 and the majority of MANEC tumors are gastric and pancreatic tumors. Moreover, more duodenal tumors and fewer rectal tumors were encountered in our study compared to European reports.
Splenic metastasis from colon carcinoma are rare and usually occur in the presence of disseminated visceral metastasis. The liver is the most common site of metastatic spread from colon cancer. Several hypotheses have attempted to explain the low incidence of splenic metastasis. It should be difficult for colorectal cancer cells to reach the spleen through the portal venous system, in which the blood flow is usually from the spleen to the liver. Reticuloendothelial system or rhythmic contraction of the spleen may squeeze out the tumor in the spleen. The absence of afferent lymphatic to the spleen, phagocytic activity and humoral anticancer substances are considered to be other reason for low incidence of splenic metastasis. We report the case of $^{18}F-FDG$ PET/CT finding in a 70-year-old woman who develop isolated splenic metastasis of sigmoid colon cancer.
Ginsenoside Rg3 (G-Rg3) is one of protopanaxadiol ginsenosides characteristic of red ginseng, steamed and dried ginseng (Panax ginseng), which has recently attracted much attention for its antitumor properties in vitro and in vivo animal models. Experimental studies have demonstrated that it could promote cancer cell apoptosis, inhibit cancer cell growth, the apoptosis of cancer cells, adhesion, invasion and metastasis, and also prevent an angiogenetic formation in prostate, breast, ovarian, colorectal, gastric, liver and lung cancer etc. It has shown the antitumor activities by modulation of diverse signaling pathways, including regulation of cell proliferation mediators (CDKs and cyclins), growth factors (vascular endothelial growth factor), tumor suppressors (p53 and p21), cell death mediators (caspases, Bcl-2, Bax), inflammatory response molecules ($NF-{\kappa}B$ and COX-2), protein kinases (JNK, Akt, and AMP-activated protein kinase) and Wnt/${\beta}$-catenin signaling. In addition, the combination of Rg3 and chemotherapeutic agents have synergistically enhanced therapeutic efficacy and reduced antagonistically side effects. Furthermore, it can reverse the multidrug resistance of cancer cells, prolong the survival duration and improve life quality of cancer patients. Taken together, accumulating evidences could provide the potential of G-Rg3 in the treatment of cancers and the feasibility of further randomized placebo controlled clinical trials.
In the last 5 years the Epidermal Growth Factor Receptor (EGFR) has emerged as one of the most important targets for drug development in oncology. Monoclonal antibodies targeting the external domain of EGFR have been shown to have clinical benefits in colorectal and head and neck cancer when combined with chemotherapy and/or radiation. Also the targeting of the epithelial growth factor receptor (EGFR) kinase domain using the closely related inhibitors gefitinib and erlotinib has generally been ineffective against solid tumors, many of which over express the receptor. We found that there were some differential expressions according to primary antibodies of the EGFR protein which being used as one of the histological tumor markers for non-small cell lung cancer (NSCLC). We also found that there are some differential expressions according to antibodies, the pH of the antigen retrieval (AR) buffer solutions and kinds of enzymes. There were some differential expressions according to the secondary antibodies and the detection systems. We analyzed the correlations between the immunohistochemical expressions of the EGFR protein and the gene mutations of the EGFR. The differences between automatic stainers and manual staining methods were also evaluated.
Human CDX2 is known as a caudal-related homeodomain transcription factor that is expressed in the intestinal epithelium and is important in differentiation and maintenance of the intestinal epithelial cells. The caudal-related homeobox proteins bind DNA according to a helix-turn-helix structure, thereby increasing the structural stability of DNA. A cancer-tumor suppressor role for Cdx2 has been shown by a decrease in the level of the expression of Cdx2 in colorectal cancer, but the mechanism of transcriptional regulation has not been examined at the molecular level. We developed a large-scale system for expression of the recombinant, novel CDX2, in Escherichia coli. A highly purified and soluble CDX2 protein was obtained in E. coli strain BL21(DE3)RIL and a hexahistidine fusion system using Ni-NTA affinity column, anion exchange, and gel filtration chromatographies. The identity and secondary structure of the purified CDX2 protein were confirmed by MALDI-TOF MS, Western blot, and a circular dichroism analyses. In addition, we studied the DNA-binding activity of recombinant CDX2 by ELISA experiment and isolated human CDX2-binding proteins derived from rat cells by an immobilized GST-fusion method. Three CDX2-binding proteins were found in the gastric tissue, and those proteins were identified to the homeobox protein Hox-D8, LIM homeobox protein 6, and SMC1L1 protein.
