1. Objectives : This study aimed to assessing the reliability of questionnaire for Japanese as one of method to diagnose Sasang Constitution and evaluating the possibility of applying domestic questionnaire to foreign site. 2. Methods : A cross-sectional survey approved by IRB (Independent Review Board) at Tohoku University was performed for 119 participants with informed consent at a particular city in Japan using self-reported questionnaires developed by KIOM (Korea Institute of Oriental Medicine). Among the entire samples, 112 were retested. The test-retest reliability was assessed by Kappa for normal variables and internal consistency was evaluated by Cronbach's alpha. 3. Results : Four of 77 questions are impossible to calculate consistency owing to unsymmetric matrix between primary reply and secondary one. Seventeen of them (22%) showed 0.4 or below in Kappa, 56 were ranged from 0.4 to 0.8 and no question marked 0.8 or over. Internal consistency was possibly checked for several questions about character, digestion, sweat, excrement, urine, cold-heat pattern. Cronbach's alpha for questions except urine and cold-heat pattern was over 0.6 4. Conclusions : On the basis of Kappa value by two self reported questionnaire, 72.7% of questions showed 0.4 or over so that reliability were highly secured. And other questions except for urine and heat-cold pattern marked 0.6 or over in order that internal consistency was also successfully maintained. Accordingly, when subsequently applying KIOM questionnaire to foreign site, domestic questionnaire should be properly adjusted in social and cultural background, lifestyle, etc.
To prevent deterioration of agricultural products during cold transportation, optimized temperature control is essential. Because the control of temperature and thermal uniformity of transported products are mainly governed by cooling air flow pattern in the transportation equipment, the accurate understanding and removal of appearance of stagnant air zone by poor ventilation is key to design of optimized cooling environment. The objectives of this study were to develop simulation model to predict the airflow and heat transfer phenomena in the cold container and to evaluate the effect of fan blowing velocity on the temperature level and uniformity of products using the CFD approach. Comparison of CFD prediction with PIV measurement showed that RSM turbulent model reveals the more reasonable results than standard $k-{\varepsilon}$ model. The increment of fan blowing velocity improved the temperature uniformity of product and reduced almost linearly the averaged temperature of product.
This study draws pattern differentiations of headache disorders on the ground of modern clinical applications and Korean medical literature. Categorization and symptoms of headache disorders are based on International Classification of Headache Disorders 3rd edition(beta version). And clinical papers are searched in China Academic Journals(CAJ) of China National Knowledge Infrastructure(CNKI). In the aspect of eight principle pattern identification, primary headache occurs due to lots of yang qi and has more inner pattern rather than exterior pattern, heat pattern rather than cold pattern, excess pattern rather than deficiency pattern. And primary headache is related with liver in the aspect of visceral pattern identification and blood stasis, wind and phlegm are relevant mechanisms. Migraine without aura is associated with ascendant hyperactivity of liver yang, phlegm turbidity, sunken spleen qi, wind-heat, blood deficiency or yin deficiency. Migraine with aura is mainly related with wind and it's major mechanisms are ascendant hyperactivity of liver yang, liver fire, yin deficiency of liver and kidney, blood deficiency or liver depression and qi stagnation. High repetition rate of tension-type headache can be identified as heat pattern or excess pattern. And trigeminal autonomic cephalalgias can also be accepted as heat pattern or excess pattern when the occurrence frequency is high and is relevant to combined pattern with excess pattern of external contraction and deficiency pattern of internal damage based on facial symptoms by external contraction and nervous and anxious status by liver deficiency. This study can be expected to be Korean medical basis of clinical practice guidelines on headache by proposing pattern identifications corresponding to the western classifications of headache disorders.
The performance experiments for a microchannel printed circuit heat exchanger (PCHE) of high-performance and high-efficiency on the two technologies of micro photo-etching and diffusion bonding were performed in this study. The microchannel PCHE were experimentally investigated for Reynolds number in ranges of 100 $\sim$ 700 under various flow conditions in the hot side and the cold side. The inlet temperatures of the hot side were conducted in range of $40^{\circ}C\;{\sim}\;50^{\circ}C$ while that of the cold-side were fixed at $20^{\circ}C$. In the flow pattern, the counter flow was provided 6.8% and 10 $\sim$ 15% higher average heat transfer rate and heat transfer performance than the parallel flow, respectively. The average heat transfer rate, heat transfer performance and pressure drop increases with increasing Reynolds number in all the experiment. The increasing of inlet temperature in the experiment range has not an effect on the heat transfer performance while the pressure drop decrease slightly with that of inlet temperature. The experimental correlations to the heat transfer coefficient and pressure drop factor as a function of the Reynolds number have been suggested for the microchannel PCHE.
Objectives: The purpose of this study was to examine the relationship between pathological patterns and hyperventilation syndrome, using pathological pattern and Nijmegen questionnaires. Methods: 33 healthy adults were asked to complete the Cold-Heat-, Phlegm-, Yin deficiency, and Lao Juan (勞倦)-pattern questionnaires, the Chalder fatigue scale, and the Nijmegen questionnaire at Kyung Hee University Hospital. We performed Pearson correlation analyses between the pathological pattern questionnaires and the Nijmegen questionnaire. The questionnaires were composed of several factors. Therefore, each factor and Nijmegen questionnaire score were also analyzed. Results: All of the pattern questionnaire scores had a positive correlation with the Nijmegen questionnaire score. The phlegm pattern, in particular, and the LaoJuan (勞倦) questionnaire scores had high correlation coefficients. The coefficient for the phlegm pattern was 0.856 and the coefficient for the LaoJuan (勞倦) pattern questionnaire was 0.855. Conclusions: The results mean that the pathological pattern questionnaires could be one of the reference materials to evaluate hyperventilation syndrome. Furthermore, improvement of pathological patterns may be helpful for treating hyperventilation syndrome, together with conventional therapies including breathing training.
Objectives We aimed to analyze traditional Chinese medicine (TCM) literatures in regards to the pattern identification and related symptoms of idiopathic short stature (ISS). Methods We searched relevant literatures published up to September 29, 2020 through three Chinese electronic databases. We performed frequency analysis of the selected studies by extracting information on pattern identification, clinical symptoms, and TCM treatments presenting pattern identification of ISS. Results Sixteen studies were included. Spleen deficiency, kidney deficiency, dual deficiency of spleen-kidney, and liver-kidney yin deficiency were frequently reported. Clinical symptoms of the spleen deficiency include sallow complexion, body constituent weakness, anorexia, lack of qi and no desire to speak, and loose stools. Herbal medicines (HMs) such as Sijunzi-tang were frequently reported. Clinical symptoms of the kidney deficiency include cold limb and fear of cold, soreness and weakness of waist and knees, and clear and long urine. HMs such as Bishendihuang-wan were frequently reported. Clinical symptoms of the dual deficiency of spleen-kidney include body constituent weakness, spirit lassitude and lack of strength, anorexia, soreness and weakness of waist and knees, and cold limb and fear of cold. HMs such as Sijunzi-tang plus Bishendihuang-wan were frequently reported. Clinical symptoms of the liver-kidney yin deficiency include tidal fever and night sweating, heat in the palms and soles, dizziness, and dry throat. HMs such as Liuweidihuang-wan were frequently reported. Conclusions This was the first study to analyze the frequency of pattern identification and related symptoms on ISS. In the future, a standardized Korean medicine pattern identification system should be established.
Objectives : The purpose of this survey is the development on the neck pain for acupuncture and moxibustion clinical guideline. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results and Conclusions : 1. The first selected pattern identification on the neck pain This study shows that the meridian pattern identification was selected 35 times(61.4%), the pattern identification based on cause of disease was selected 8 times(14.0%), the visceral pattern identification was selected 7 times(12.3%), the other pattern identification was selected 4 times(7.0%), Qi blood yin yang pattern identification was selected 2 times(3.5%), according to symptoms was selected 1 time(2.4%). 2. Meridian pattern identification Small intestine meridian of hand Taeyang was used 39 times(18.1%), Large intestine meridian of hand Yangmyeong and Bladder meridian of foot Taeyang was used 34 times(15.7%), Gall-bladder meridian of foot Soyang was 32 times(14.8%), Tripple energizer meridian of hand Soyang was used 31 times(14.4%), Governor meridian was used 30 times(13.9%), Lung meridian of hand Taeeum was used 8 times(3.7%), Heart meridian of hand Soeum and Pericarduim meridian of hand Gworeum was used 4 times(1.9%). 3. In order to investigate environmental stress inducible genes in abalone, we analyzed differentially expressed transcripts from a Pacific abalone, Haliotis discus hannai, after exposure to heat-, cold- or hyposalinity-shock by suppression subtractive hybridization (SSH) method. 1,074 unique sequences from SSH libraries were composed to 115 clusters and 986 singletons, the overall redundancy of the library was 16.3%. From the BLAST search, of the 1,316 ESTs, 998 ESTs (75.8%) were identified as known genes, but 318 clones (24.2%) did not match to any previously described genes. From the comparison results of ESTs pattern of three SSH cDNA libraries, the most abundant EST was different in each SSH library: small heat shock protein p26 (sHSP26) in heat-shock, trypsinogen 2 in cold-shock, and actin in hyposalinity SSH cDNA library. Based on sequence similarities, several response-to-stress genes such as heat shock proteins (HSPs) were identified commonly from the abalone SSH libraries. HSP70 gene was induced by environmental stress regardless of temperature-shock or salinity-stress, while the increase of sHSP26 mRNA expression was not detected in cold-shock but in heat-shock condition. These results suggest that the suppression subtractive hybridization method is an efficient way to isolate differentially expressed gene from the invertebrate environmental stress-response transcriptome.
In order to ensure the fact that eight principle pattern differentiation is used clinically as a basic guideline for Korean medicine practice, the definition, role and method of yin-yang pattern differentiation with its case report were explored at first. Yinyang Pattern Differentiation is a method of discriminating human tendencies or morbidity based on the yin and yang characteristics expressed in living bodies. And yin and yang are the two contrasting characteristics and aspects of the interaction when certain physical conditions that have a lasting effect on the human physiological metabolic function are correlated with the morbidity. Specific methods of yinyang pattern differentiation can be divided into several types of yin and yang indicators. First, time and space factors like day and night, hot and cold seasons, above and below, topographical districts. Second, colors and pulse and their/or relative clearness and muddiness, hardness and softness, moving and resting. Third, diagnose yin and yang patterns through distinguishing the true and false of a fever and cold in an emergency phase such as increase of brain pressure and shock state. Fourth, general characteristics of the propensity and constitution of a subject such as body type, speech, behavior, and physiological metabolism. And for clinical use, these were summarized again as a symptom indicators of physical signs and color, pulse, tongue and questionnaire indicators of propensity, body type, and space-time characteristics. Conclusively, it was confirmed that yinyang pattern differentiation has its own diagnostic significance which is distinct from exterior-interior, cold-heat and deficiency-excess pattern differentiation.
Purpose : There are two types of sleep disturbance: inability to sleep (不眠) and somnolence (多眠). This study is to examine treatments of the two types of sleep disturbance in the Books of Cold Damage and how those theories were established and formed. Methods : 1. Verses including the words related to inability to sleep and somnolence were extracted from "Treatise on Cold Damage Diseases" (傷寒論). 2. Among the Books on Cold Damages that are classified according to the symptoms, 17 books with contents related to inability to sleep and somnolence were selected to collect and classify data in three perspectives: mechanism of disease (病機), method of treatment (治法) and disease pattern (病證). 3. Data collected through the above methods were compared and diagramed. Results & Conclusions : 1. On Cold Damage, inability to sleep is mostly rooted when human body lacks yin energy while having excessive yang energy (陽盛陰虛) due to fire and heat (火熱). The reason could be misuse of perspiration inducing method (汗法) or purgation (下法) on the doctors' part. 2. On Cold Damage, somnolence is rooted when pathogen (邪氣) is spread to yin meridians (陰經) and the human body lacks yang while having excessive yin energy (陰盛陽虛) or when heat (熱邪) is invaded into interior parts (裏部). 3. Many scholars of Cold Damage in later periods had copied the "Treatise on Cold Damage Diseases" for treatments of sleep disturbance but many others have applied the verses from "Treatise on Cold Damage Diseases" or added new treatments. Do jeol-am (陶節庵) and Wang Geung-dang (王肯堂) particularly had deep understanding on "Treatise on Cold Damage Diseases" and utilized the content freely or suggested new remedies because they had thorough knowledge on relating formula as well.
