The Effect of Social Participation on Cognitive Impairment Degree among Korean Older Adults: Comparative Study between Urban and Rural Area (사회참여활동이 고령자 인지기능 손상 정도에 미치는 영향: 도시와 농촌 간 차이 분석)

  • The purpose of this study is to analyze the effect of social participation types on cognitive impairment degree(normal, mild, severe cognitive function) by urban and rural difference. The research was performed by multinomial logistic regression with the data of elderly 5,668 over 55 years of age from the sixth wave(2016) and the seventh wave(2018) of the Korean Longitudinal Study of Ageing. Results show that social participation reduces the risk of cognitive impairment on both urban and rural older adults regardless of types, but the types of social participation, which are effective to reduce the risk of cognitive impairment, are different between urban and rural area. According to residential area, types of social participation, which are effective to decline the risk of being mild or severe cognitive impairment and the risk of being severe cognitive impairment relative to mild, are different. These findings suggest that, in order to prevent cognitive impairment and reduce the severity, social participation is strongly recommended and appropriate supports are required. This implies policy and intervention efforts considering the urban and rural difference are needed.

A Correlation of Cognitive Function, Depression, Suicidal Idea and Serum Lipid Levels in the Elderly Over Sixty Years of Age (60세 이상 노인 인구에서 인지기능과 우울증, 자살사고, 혈청지질농도와의 연관성)

  • Objectives : We investigated a correlation of cognitive function, depression, suicidal idea and serum lipid levels in Korean elderly over 60 years old to find risk factors of deprssion and cognitive decline. Methods : 834 persons participated in this study. Clinical evaluation was done at a point by using Mini-Mental State Examination in the Korean version of CERAD assessment packet(MMSE-KC). We also evaluated 165 persons that agreed to our exact examination for Geriatric Depression Scale(GDS), Scale for Suicidal Ideation(SSI), Hachinski Ischemic Score and serum lipid profile. Correlation analysis was used to evaluate correlation among serum lipid levels, MMSE-KC and SSI. One-way ANOVA was used for comparison of serum lipid levels and cognitive function, depression and SSI. We did a post-hoc analysis. Results : Depression and suicidal ideation were related with cognitive impairment in Korean elderly over 60 years old. Those who have more severe cognitive impairment and depressive symptom, have higher Hachinski ischemic score. We can see high HDL cholesterol levels only in the normal control group. Low total, HDL, LDL cholesterol were related with depression. Conclusion : There was high prevalence of depression in the Korean elderly over 60 years old and low serum lipid level was related with severity of depression and high suicidal ideation. In the normal control group, we can see high HDL cholesterol levels. To confirm this result, we need well-designed and wide range study.

18F-THK5351 PET Positivity and Longitudinal Changes in Cognitive Function in β-Amyloid-Negative Amnestic Mild Cognitive Impairment

  • Purpose Neuroinflammation is considered an important pathway associated with several diseases that result in cognitive decline. 18F-THK5351 positron emission tomography (PET) signals might indicate the presence of neuroinflammation, as well as Alzheimer's disease-type tau aggregates. β-amyloid (Aβ)-negative (Aβ-) amnestic mild cognitive impairment (aMCI) may be associated with non-Alzheimer's disease pathophysiology. Accordingly, we investigated associations between 18F-THK5351 PET positivity and cognitive decline among Aβ- aMCI patients. Materials and Methods The present study included 25 amyloid PET negative aMCI patients who underwent a minimum of two follow-up neuropsychological evaluations, including clinical dementia rating-sum of boxes (CDR-SOB). The patients were classified into two groups: 18F-THK5351-positive and -negative groups. The present study used a linear mixed effects model to estimate the effects of 18F-THK5351 PET positivity on cognitive prognosis among Aβ- aMCI patients. Results Among the 25 Aβ- aMCI patients, 10 (40.0%) were 18F-THK5351 positive. The patients in the 18F-THK5351-positive group were older than those in the 18F-THK5351-negative group (77.4±2.2 years vs. 70.0±5.5 years; p<0.001). There was no difference between the two groups with regard to the proportion of apolipoprotein E ε4 carriers. Interestingly, however, the CDR-SOB scores of the 18F-THK5351-positive group deteriorated at a faster rate than those of the 18F-THK5351-negative group (B=0.003, p=0.033). Conclusion The results of the present study suggest that increased 18F-THK5351 uptake might be a useful predictor of poor prognosis among Aβ- aMCI patients, which might be associated with increased neuroinflammation (ClinicalTrials.gov NCT02656498).

Cognitive Dysfunction and Hippocampal Damage Induced by Hypoxic-Ischemic Brain Injury and Prolonged Febrile Convulsions in Immature Rats

  • Objective : Perinatal hypoxic-ischemic encephalopathy (HIE) and prolonged febrile seizures (pFS) are common neurologic problems that occur during childhood. However, there is insufficient evidence from experimental studies to conclude that pFS directly induces hippocampal injury. We studied cognitive function and histological changes in a rat model and investigated which among pFS, HIE, or a dual pathologic effect is most detrimental to the health of children. Methods : A rat model of HIE at postnatal day (PD) 7 and a pFS model at PD10 were used. Behavioral and cognitive functions were investigated by means of weekly open field tests from postnatal week (PW) 3 to PW7, and by daily testing with the Morris water maze test at PW8. Pathological changes in the hippocampus were observed in the control, pFS, HIE, and HIE+pFS groups at PW9. Results : The HIE priming group showed a seizure-prone state. The Morris water maze test revealed a decline in cognitive function in the HIE and HIE+pFS groups compared with the pFS and control groups. Additionally, the HIE and HIE+pFS groups showed significant hippocampal neuronal damage, astrogliosis, and volume loss, after maturation. The pFS alone induced minimal hippocampal neuronal damage without astrogliosis or volume loss. Conclusion : Our findings suggest that pFS alone causes no considerable memory or behavioral impairment, or cellular change. In contrast, HIE results in lasting memory impairment and neuronal damage, gliosis, and tissue loss. These findings may contribute to the understanding of the developing brain concerning conditions caused by HIE or pFS.

The Differences of Serum Homocysteine Levels in Mild Cognitive Impairment and Dementia of Alzheimer's Type with or without Depressive Symptoms (경도인지장애, 알쯔하이머형 치매 환자에서 우울증상 유무에 따른 혈중 호모시스테인의 차이)

  • Objectives : Mild cognitive impairment(MCI) and dementia of Alzheimer's type(AD) are characterized by progressive decline of cognitive abilities and a wide range of neuropsychiatric symptoms like depression. Among various diagnostic tools of AD, many studies showed that elevated levels of serum total homocysteine are associated with increased risk of developing Alzheimer's disease, depression and other neuropsychiatric disorders. We investigated whether elevated homocysteine concentrations are associated with depressive symptoms in MCI and AD. Methods : A total of 86 patients diagnosed with MCI or AD participated. Total serum homocysteine levels in fasting blood samples were measured. We examined cognitive symptoms by MMSE-KC, Global Deterioration Scale(GDS), Clinical dementia rating(CDR) and depressive symptoms by Korean version of Geriatric Depression Scale(K-GDS). Results : The total serum homocysteine levels were significantly higher in MCI with depression than in MCI without depression. There was no significant difference in the mean homocysteine levels between AD patients with depression and AD patients without depression. The total homocysteine levels showed a negative correlation with MMSE-KC and a positive correlation with CDR, GDS. Conclusions : These findings suggest that elevated homocysteine level is a risk factor for the decline of cognitive function and depression. We found a significant relationship between elevated serum homocysteine level and depressive symptoms in MCI. But our study had several limitations, thus more research is needed to confirm this finding.

  • The objectives of this study was to examine the effect of Interactive Video Game on cognitive information processing the elderly. Sixty elderly were attended in this study. Their ages ranged from 65 to 70, with a mean age of 67.60 years. The subjects were randomly assigned to one of three experimental conditions: (1) interactive video game group (n=20), (2) aerobic exercise group (n=20), (3) control group (n=20). The experimental design of this study was analyzed using two-way ANOVAs with repeated measures of groups and time. Cognitive function was assessed by neuroelectrical response, and ERP analysis. The results of the study showed that the interactive video game group and aerobic exercise group showed no significant statistical differences in the response time, response accuracy, amplitude and potential of the performance of the exercise in cognitive function and ERP analysis, but improved the interaction video game group and aerobic exercise (walking) group over the control group. It was concluded that long-term aerobic exercise like interactive video game is associated with attenuation of cognitive decline in the elderly.

  • Recently, studies on cognitive control revealed that the optimal level of control is determined on the basis of reward and cost. The value of reward can be subjective and therefore, the optimal control strength can vary accordingly. The inconsistent effect of aging on cognitive control can be the result of flexible adjustment of control signal strength made by the older subjects. In other words, the elderly people maintains the ability to set the optimal level of control, which is known as the function of the dorsal anterior cingulate cortex. On the other hand, the age-related decline in cognitive control is obvious in rule maintenance and inhibition, which has to do with the function of lateral prefrontal cortex. In this study, we had young and old adults perform go-no go task and compared the behavioral and neural results for different reward conditions. Both age groups showed the best performance and the largest ERN amplitude when the reward was most appealing to them. And there was no age effect in ERN amplitude even though older adults' d' and accuracy was inferior to younger participants. These findings suggest that the effect aging on different cognitive control processes can be asymmetric.

Protective effect of Phyllostachys edulis (Carrière) J. Houz against chronic ethanol-induced cognitive impairment in vivo

  • BACKGROUND/OBJECTIVES: Chronic alcohol consumption causes oxidative stress in the body, which may accumulate excessively and cause a decline in memory; problem-solving, learning, and exercise abilities; and permanent damage to brain structure and function. Consequently, chronic alcohol consumption can cause alcohol-related diseases. MATERIALS/METHODS: In this study, the protective effects of Phyllostachys edulis (Carrière) J. Houz (PE) against alcohol-induced neuroinflammation and cognitive impairment were evaluated using a mouse model. Alcohol (16%, 5 g/kg/day for 6 weeks) and PE (100, 250, and 500 mg/kg/day for 21 days) were administered intragastrically to mice. RESULTS: PE showed a protective effect against memory deficits and cognitive dysfunction caused by alcohol consumption, confirmed through behavioral tests such as the T-maze, object recognition, and Morris water maze tests. Additionally, PE attenuated oxidative stress by reducing lipid oxidation, nitric oxide, and reactive oxygen species levels in the mice's brains, livers, and kidneys. Improvement of neurotrophic factors and downregulation of apoptosis-related proteins were confirmed in the brains of mice fed low and medium concentrations of PE. Additionally, expression of antioxidant enzyme-related proteins GPx-1 and SOD-1 was enhanced in the liver of PE-treated mice, related to their inhibitory effect on oxidative stress. CONCLUSION: This suggests that PE has both neuroregenerative and antioxidant effects. Collectively, these behavioral and histological results confirmed that PE could improve alcohol-induced cognitive deficits through brain neurotrophic and apoptosis protection and modulation of oxidative stress.

Comparison of Daily Living Activities, Health Status and Nutritional-Related Characteristics of Older Adults Living Alone by Age: Based on Data from the 2020 National Survey of Living Conditions and Welfare Needs of Older Koreans (독거노인의 연령에 따른 일상생활, 건강 수준 및 영양 관련 특성 비교: 2020년 노인실태조사 자료를 중심으로)

  • Purpose: This study evaluates activities of daily living activities, health status, and nutritional-related characteristics of older adults living alone by age. Methods: This study analyzed data from aged 65 or older who participated in the National Survey of Living Conditions and Welfare. Data were collected on the following factors: demographic characteristics, comorbidities, body mass index (BMI), need for assistance with activities of daily living, exercise, subjective health status, Mini-Mental State Examination for Dementia Screening (MMSE-DS), and short form of Geriatric Depression Scale (SGDS-K). The definition of malnutrition is BMI <18.5 kg/m2 . Results: A total of 3,136 participants were included, with 1,488 classified as younger older adults and 1,648 classified as older older adults. The prevalence of hypertension, difficulties in daily living, need for daily living assistance, MMSE-DS, and SGDS-K scores were significantly higher in the older older adults than in the younger older adults. Both groups had low levels of participation in education and use of community centers or welfare centers. The distribution of cognitive function decline and depression was high in the malnourished group. Furthermore, the malnourished group without cognitive decline or depression had higher monthly income, exercise performance, and use of electronic device. Conclusion: Older adults who live alone suffer from poor health behaviors, activities of daily living, and nutritional status, and the older adults are even more vulnerable. Therefore, monitoring of older adults living alone with poor living conditions is necessary, and intensive efforts are needed to improve their health and functional status.

Comparisons of Cognitive Functions between Urban and Rural Dwelling Older Adults: A Preliminary Study (도시와 농촌거주 노인의 인지기능 비교: 예비연구)

  • Mini-Mental State Examination (MMSE) and naming tests have been used clinically as screening and diagnostic tools for mild cognitive impairment and dementia. Researches in other countries have reported that residential areas would affect one's cognitive abilities. In Korean, however, there was no systematic studies on geographical influences to cognitive ability among a sample of urban/rural residents. The aim of this study was to investigate the effect of urban/rural locality of residence on cognitive tests in normal elderly Korean. Residents aged 65 years and older (N=108) were administered the K-MMSE, confrontation/generative naming tests, and medical/socio-demographic interview. When comparing the total number of correct responses between urban residents and rural residents, the performances of rural group were significantly worse than those of the urban group in K-MMSE. Based on the results, our finding suggests that a factor of residential locality should be considered to examine the function and explain the cognitive decline in rural community-dwelling elders.
