석탄광업소장의 근로자 건강관리에 대한 태도 (Manager's Attitude about Health Management of Workers in Coal Mine Industry)

  • This study was planned to investigate employer's attitude about health management of workers in coal mine industry. The sample size was 38.3% (178 coal mine industries) of total 463 coal mine industries. The mailing survey was used to collecting data of coal mine industry and manager of coal mine industry. Distribution of attitude about health management of workers in coal mine industry, specifically necessity and availability of some items of health management and some apparatus of working environment, was skewed to positive attitude. While recognition of susceptability of coal workers' pneumoconiosis was low, that of seriousness of incidence of coal workers' pneumoconiosis was high.

탄광부진폐증으로 입원중 사망환자에서의 일초량을 이용한 직접사인과 여명과의 관계 (The Relationship between the Cause of Death and Life Expectancy by $FEV_1$ in Coal Workers' Pneumoconiosis)

  • Sixty-two medical records of patients with coal workers' pneumoconiosis who died in hospital as coal workers' pneumoconiosis were analysed for study of the relationship between forced expiratory volume in one second ($FEV_1$) and life expectancy in coal workers'pneumoconiosis. In the group who died of asphyxia from hemoptysis, life expectancy were well fitted with $FEV_1$(p<0.05). But others were not well fitted to simple linear regression equation. The prevalence of ECG sign of col pulmonale was more in the group of cardiorespiratory failure than asphyxia group. So, in the case of far advanced cor pulmonale, it was difficult to predict life expectancy by simple linear regression equation

발반사 요법이 탄광부 진폐증 환자의 우울과 불안에 미치는 영향 (The Effects of Foot Reflexology on Depression and Anxiety in Coal Workers' Pneumoconiosis)

  • Purpose: This study was to evaluate the effects of foot reflexology on depression and anxiety in coal workers' pneumoconiosis. Method: The research design used was a non-equivalent control group pretest-posttest design. The foot reflexology was performed for 60 minutes twice a week through five weeks in the experimental group, not in the control group. The subjects of this study consisted of both the experimental group of twenty-nine and the control group of thirty coal workers' pneumoconiosis. The data were collected from December 10, 2002 to February 15, 2003. To evaluate the effects of foot reflexology the scores of depression and anxiety were measured before and after the experiment in both groups. Data of this experiment were analyzed by Chi-square test, t-test, unpaired t-test, ANCOVA and Repeated Measures ANOVA with the SAS Program. Results: The score of depression decreased in the experimental group, not in the control group. There was a significant difference of depression between the two groups. The score of anxiety decreased in the experimental group, not in the control group. There was a significant difference of anxiety between the two groups. Conclusion: As a summary of the study results, foot reflexology was regarded that it attributed to the emotional promotion of coal workers' pneumoconiosis. Therefore the foot reflexology is considered that might be used as an effective skill of nursing intervention program for coal workers' pneumoconiosis.

푸리에 변환 적외선 분광기에 의한 사망한 탄광부 진폐증자의 폐조직내 석영농도 (The analysis of α-quartz in the worker's lung died of coal workers' pneumoconiosis by FT-IR)

  • This study was analysis of ${\alpha}$-quartz in the 37 worker's lung died of coal workers' pneumoconiosis(CWP) and in the 9 normal's lung by Fourier transform infrared spectrophotometer. The results were as follows : 1. The dried lung weight percents in wet lung weight were 24.4%, 25.4%, coal workers' pneumoconiosis and normal lungs, respectively. Ash weight percents in dried lung weight were 7.7%, 5.0%, coal workers' pneumoconiosis and normal lungs, respectively. 2. The presision (as of coefficient of variation) for the ${\alpha}$-quartz determination were 2.6% in standard quartz (ranged from 9.9 to $198.0{\mu}g$) and 3.7% in ashed lung samples. 3. The characterstic ${\alpha}$-quartz absorption spectra in lungs of CWP were only shawn by LTA ashing. 4. Geometric mean of ${\alpha}$-quartz concentration in CWP lung was 173mg/100g dried lung. 5. The concentrations of ${\alpha}$-quartz in Korean CWP lungs were similar to those of foreign coal miner's lungs.

탄광부 진폐증에서 혈장 Transforming Growth Factor-${\beta}_1$의 의의 (Clinical Significance of Plasma TGF-${\beta}_1$ in Coal Workers' Pneumoconiosis)

  • 연구배경 : 탄광부 진폐증은 석탄 분진의 흡입에 의한 폐조직의 손상에 대하여 부적절하고 과도한 염증반응이 일어나 폐섬유화를 유발하여 발병하는 것으로 생각되어지고 있다. 이 반응에는 대식세포를 비롯한 많은 염증세포들과 그 세포들에서 분비되는 매개물질들이 중요한 역할을 한다. TGF-$\beta$는 특발성 폐섬유화증, 규폐증 및 석면증의 폐섬유화 과정에 관여한다고 알려져 있다. 그러나 비슷한 기전에 의해 폐 섬유화가 진행되는 것으로 생각되는 탄광부 진폐증에서는 TGF-$\beta$의 관여여부에 대한 보고가 거의 없다. 본 연구는 탄광부 진폐증 환자에서 혈청 TGF-${\beta}_1$을 측정하여 섬유화의 정도에 따른 그 활성도의 변화를 비교하여 탄광부 진폐증의 폐섬유화 과정에 TGF-${\beta}_1$이 관여하는지 알아 보고자 하였다. 방법 : 직업력과 방사선학적 소견 상 탄광부 진폐증으로 진단 된 환자 중 단순 탄광부 진폐증 20예와 복잡성 탄광부 진폐증 20예를 대상으로 하였다. 정상소견인 자 10명을 대조군으로 설정하였으며, 각 대상을 human TGF-${\beta}_1$ immunoassay kit (R&D system, Minneapolis, MN)을 이용하여 혈장 내 TGF-${\beta}_1$을 측정하였다. 결과 : 단순 탄광부 진폐증($0.64{\pm}0.17$ ng/mL)과 정상 대조군($0.63{\pm}0.18$ ng/mL)보다 복잡성 탄광부 진폐증 ($0.79{\pm}0.18$ ng/mL)의 혈중 TGF-${\beta}_1$의 농도가 의미 있게 높았다(p<0.05). 결론 : 탄광부 진폐증의 섬유화 진행 과정에 TGF-${\beta}_1$이 관여함을 알 수 있었다. 따라서 단순 탄광부 진폐증과 복잡성 탄광부 진폐증의 감별진단과 경과 예측인자로서 혈장 TGF-${\beta}_1$이 유용하리라 생각되며, TGF-${\beta}$의 생성을 억제한다면 탄광부 진폐증의 섬유화 진행을 저지함으로써 탄광부 진폐증의 치료에 있어 중요한 역할을 할 수 있을 것으로 기대된다.

탄광부진폐증자의 혈청 면역단백 농도에 관한 연구 (Serum Immunoglobulin Levels in Coal Workers' Pneumoconiosis)

  • Serum immunoglobulin(Ig)A, IgG, IgM, levels were measured in 99 coal workers' pneumoconiosis (CWP) patients and 12 healthy coal workers and 9 non-miners to compare with each group by the radiological categories, its complications and working period in coal mine. Serum were measured by nephelometry, The findings were as follows: 1) Serum IgA levels were significantly different between three groups of CWP patient, healthy coal worker and non-miner ($mean{\pm}standard$ deviation: $226.4{\pm}87.7,\;221.3{\pm}45.1,\;170.1{\pm}65.7$ respectively). 2) There were no significant differences of Ig levels among radiological categories of CWP. 3) There were no significant differences of Ig levels among simple pneumoconiosis and its complicated disorders. 4) The three Ig levels were slightly increased in the group of mining years less than 20 years (IgA: r=0.1869, p<0.10 IgG: r=0.2902, p<0.05 IgM: r=0.2889, p<0.05).

일부 전남지역 탄광근로자의 직업병 유병율에 관한 조사 연구 (A Study on the Prevalence of Occupational Disease in Coal Mine Workers)

  • In order to investigate the prevalence of occupational disease in coal mine workers in Chonnam area, a cross sectional study was conducted in 1,097 workers from September to December in 1987. The results were as follows; 1. The prevalence of pneumoconiosis including suspected cases was 6.9% and was increased by age and working duration. 2. The prevalence of pneumoconiosis excluding suspected cases was 2.8%. 3. By the kind of job, prevalence of pneumoconiosis including suspected cases was l2.9% in tunnel drivers, 6.3% in coalface workers and 6.0% in other workers. 4. The prevalence of noise induced hearing loss was 5.1% and was increased by age and working duration. 5. The prevalence of noise induced hearing loss was the highest among tunnel drivers(7.3%), but it was not significantly different by the kind of job. 6. Occupational disease caused by vibration was not found in coal mine workers.

발반사 마사지가 탄광부 진폐증 환자의 피로와 불면증에 미치는 효과 (The Effects of Foot Reflexology on Fatigue and Insomnia in Patients suffering from Coal Workers' Pneumoconiosis)

  • Purpose: The purpose of this study was to determine the effects of foot reflexology on fatigue and insomnia in patients suffering from coal workers' pneumoconiosis. Method: This study was a quasi-experimental study of pre-test and post-test design in a non-equivalent control group. The subjects of this study consisted of both the experimental group of twenty-nine and the control group of thirty coal workers' pneumoconiosis patients. Data was collected from December 10, 2002 to February 15, 2003. Foot reflexology was performed for 60 minutes twice a week through five weeks in the experimental group, but none in the control group. To evaluate the effects of foot reflexology, the scores of fatigue and insomnia were measured before and after the experiment in both groups. Fatigue was evaluated by Fatigue Symptoms Inventory. Insomnia was measured with the visual analogue scale (VAS). Data of this experiment was analyzed by Chi-square test, t-test, unpaired t-test and Repeated Measures ANOVA with the SAS Program. Results: The scores of fatigue and insomnia decreased in the experimental group but not in the control group. There was a significant difference of fatigue and insomnia between the two groups. Conclusion: It is suggested that foot reflexology might have beneficial effects on reducing fatigue and insomnia in patients suffering from coal workers' pneumoconiosis, and can be recommended as a nursing intervention program for patients with coal workers' pneumoconiosis.

탄광부 진폐증과 용접공 폐증에 있어 흉부-X선 소견과 폐기능의 비교 (Comparision of Chest Radiographs and Pulmonary Function in Coal Workers' Pneumoconiosis and Welders' Lung)

  • 연구 배경: 탄광부 전폐증과 조선소 용접공 폐증의 흉부 X-선 소견은 유사한 규칙성 음영을 보이는 예가 대부분이지만 이를 흉부 X-선만으로 구별한다는 것은 극히 어려운 일이다. 본 연구는 탄광부 진폐증과 용접공 폐증의 흉부 X-선 소견에 따른 폐기능과 다른 요인들을 비교하여 두 질환의 진행과정과 그것에 대한 대책에 도움을 주고져 한다. 방법: 탄광부 진폐증 430명과 용접공 폐증 311명을 대상으로 흉부 X-선 소견에서 소원형 음영의 조밀도에 따라 진폐의증, 1형, 2형으로 나누어 분포, 나이, 분진의 노출기간, %폐활량, 1초율 및 환기장해의 행태, 동반된 폐질환 등을 서로 비교하였다. 1) 탄광부 진폐증에서는 2형이 54.1%로 가장 많았고 용접공 폐증에서는 1형이 55.95%, 2형이 7.4%로 탄광부 진폐증에서 2형으로 진행도가 높았다. 2) 평균연령은 용접공 폐증에 비해 탄광부 폐증에서 유의하게 높았고, 소원형 음영의 조밀도가 증가함에 띠라 탄광부 폐증과 용접공 폐증에서 유의한 증가를 보였다. 3) 분진 노출의 기간은 진폐의증에서는 유의한 차이가 없었으나, 1형과 2형의 탄광부 폐증에서 유의한 증가를 보였고, 소원형 음영의 조밀도가 증가됨에 따라 탄광부 폐증에서는 2형이 진폐의증과 1형보다 유의하게 증가하였으나, 용접공 폐증에서는 유의한 차이가 없었다. 4) %폐활량은 1형을 제외하고는 탄광부 폐증과 용접공 폐증에서 유의한 차이가 없었고, 소원형 음영의 조밀도에 따라서도 두 그룹에서 일정한 경향이 없었다. 5) 1초율은 진폐의증을 제외하고는 1형과 2형에서 탄광부 폐증에서 유의하게 감소하였고, 소원형 음영의 증가에 따라 탄광부 폐증에서는 유의한 감소를 보였으나 용접공 폐증에서는 진폐의증에 비해 2형에서만 유의하게 감소하였다. 6) 두 그룹에서 흡연과 비흡연에 따른 %VC, $%FEV_{1.0}$의 유의한 차이를 보이지 않았다. 7) 폐기능 장해의 유형에 따라 탄광부 폐증에서 구속형 상해가 21.4%, 폐색형 장해가 11.6%, 혼합형 상해가 5.6%이고, 용접공 폐증에서 구속형 장해가 21.8%, 폐색형 장해가 2.9%, 혼합형 장해가 1.9%를 보였고, 탄광부 폐증에서는 조밀도가 증가할 수 록 폐색형 장해는 증가하였다. 8) 두 그룹에서 폐결핵의 발병률이 가장 높았으며 용접공 폐증에서보다 탄광부 진폐증에서 높았다. 결론: 탄광부 진폐증과 용접공 폐증에서 흉부 X-선 소견과에 따른 폐기능 비롯한 다른 요인들을 비교하면 질병의 진행정도와 진행에 대한 대책에 도움을 줄 것으로 생각된다.

탄광부진폐증 환자에서 기관지확장제 투여 중단 후의 노력성폐활량 및 일초폐활량의 변화 (Change of FVC, $FEV_1$ after Discontinuance of Bronchodilator in Coal Workers' Pneumoconiosis Patients)

  • For the evaluation of change of FVC and $FEV_1$ after discontinuance of bronchodilator in the coal workers' pneumoconiosis patients, 17 pairs of patients were selected. They were matched by the age(${\pm}5$ y.o.) and the type of ventilatory impairment. Pulmonary function was measured 2 times bimonthly before and after the drug discontinuance discontinued after measurement of PFT for 2 times. In case group the bronchodilator was discontinued after measurement of PFT for 2 times. In control group there was no interruption of medication. FVC, $FEV_1$ decreased in both group as measurement progress. Simple linear regression coefficients against the month of measurement were calculated in both group and tested for parallelism between two groups. The results of test revealed that both regression coefficients were parallel. So in conclusively, discontinuance of medication of bronchodilator for coal workers pneumoconiosis patients has no effect on the decreasing rate of FVC, $FEV_1$.
