Status of Kidney Function in Hospitalised COVID-19 Patients in the Southern Gyeonggi Province, South Korea (경기 남부 일개 병원에 입원한 코로나 19 환자들의 신기능 현황)

  • Coronavirus disease 2019 (COVID-19) is a contagious disease caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). This study aimed to investigate the status of renal function in patients with COVID-19. The study surveyed a total of 649 patients hospitalized with COVID-19 at a hospital located in southern Gyeonggi Province, South Korea over a one month period in January 2021. The parameters analyzed were blood urea nitrogen (BUN), creatinine, sodium, potassium, chloride, and estimated glomerular filtration rate (eGFR). The BUN and creatinine of the COVID-19 patients were found to be higher than the normal reference range, specially in males, and in the elderly (60s and 80s or older). The serum electrolyte levels of the patients were observed to be within the reference intervals. Of the subjects, males over 80 years of age had a Chronic Kidney Disease Epidemiology Collaboration (CKD-EPI) of 60 mL/min/1.73 m2 or less. Recent research suggests that some severe cases of COVID-19 are showing signs of kidney damage, even in those with no prior underlying kidney disease. Thus, assessment of kidney function using multiple indicators could help diagnose abnormal renal function in patients with COVID-19.

  • Prior studies of physiologically active substances in grapes have focused only on the fruit, pericarp, and branches, as well as the pulp and seeds. The present study assessed the changes in quercetin-3-O-glucuronide (Q3OG) and flavonol glycoside content using grape leaves from Vitis labruscana B. and V. coignetiae at different growth stages and provided basic data for quality control. Content analysis showed that both varieties differed in Q3OG and flavonol glycoside content by growth stage, and the components were found to be high in the order of fruit, maturation, and coloration. Also, Vitis labruscana B. has a better vascular relaxation effect than Vitis coignetiae. These results suggest that in the use of grape leaves as a functional raw material, Q3OG and flavonol glycosides can be used as indicator components. In addition, if raw materials for each growth stage are mixed in a particular ratio, it will be a way to manage the specific efficacy and content of indicator components.

Changes in sedimentary structure and elemental composition in the Nakdong Estuary, Korea (낙동강 하구역 퇴적구조 및 원소조성 변화에 관한 연구)

  • To understand the sedimentary environment of Scirpus planiculmis habitat (Myeongji and Eulsuk tidal flats) in the Nakdong Estuary, this study analyzed the statistical parameters (sorting, skewness, and kurtosis) of grain size data and the major (Al, Fe, Mn, Mg, Ca, Na, K, Ti, and P), minor (Li, Sc, V, Cr, Co, Ni, Cu, Zn, Sr, Zr, Cs, Pb, Th, and U), and rare earth elements (REEs) in sediment cores. For Myeongji, the sediment structure of the upper part of the cores was poorly sorted, more finely skewed, and more leptokurtic due to construction of the West gate. By contrast, the Eulsuk cores all differed due to the contrasting floodgate operation patterns of the West and East gates. The linear discriminate function (LDF) results corresponded to the statistical parameters for grain size. At the Eulsuk tidal flat (sites ES05 and ES11), elemental distributions were representative of Al-, Fe- and Ca-associated profiles, in which the elements are largely controlled by the accumulation of their host minerals (such as Na- and K-aluminosilicate and ferromagnesium silicate) and heavy detrital minerals at the sites. Detrital minerals including the aluminosilicates are major factors in the elemental compositions at ES05, diluting the REE contents. However, clay minerals and Fe-oxyhydroxides, as well as REE-enriched heavy minerals, appeared to be controlling factors of the elemental composition at ES11. Therefore, the mineral fractionation process is important in determining the elemental composition during sedimentation, which reflects the depositional condition of riverine-saline water mixing at both sites.

25-Hydroxyvitamin D level is associated with mortality in patients with critical COVID-19: a prospective observational study in Mexico City

  • BACKGROUND/OBJECTIVES: Considering the high number of deaths from coronavirus disease 2019 (COVID-19) in Latin American countries, together with multiple factors that increase the prevalence of vitamin D deficiency, we aimed to determine 25-hydroxyvitamin D (25[OH]D) levels and its association with mortality in patients with critical COVID-19. SUBJECTS/METHODS: This was a prospective observational study including adult patients with critical COVID-19. Data, including clinical characteristics and 25(OH)D levels measured at the time of intensive care unit admission, were collected. All patients were followed until hospital discharge or in-hospital death. The patients were divided into those surviving and deceased patient groups, and univariate and multivariate logistic regression analyses were performed to determine independent predictors of in hospital mortality. RESULTS: The entire cohort comprised 94 patients with critical COVID-19 (males, 59.6%; median age, 61.5 years). The median 25(OH)D level was 12.7 ng/mL, and 15 (16%) and 79 (84%) patients had vitamin D insufficiency and vitamin D deficiency, respectively. The median serum 25(OH)D level was significantly lower in deceased patients compared with surviving (12.1 vs. 18.7 ng/mL, P < 0.001). Vitamin D deficiency was present in 100% of the deceased patients. Multivariate logistic regression analysis revealed that age, body mass index, other risk factors, and 25(OH)D level were independent predictors of mortality. CONCLUSIONS: Vitamin D deficiency was present in 84% of critical COVID-19 patients. Serum 25(OH)D was independently associated with mortality in critical patients with COVID-19.

Development of Optimal Antiviral Coating Method for the Air Filtration System of Subway Station (지하역사 승강장 공조 시스템 필터용 항바이러스 코팅 성능 및 재생 성능 평가)

  • In this study, a novel antiviral coating method for the air filtration system of subway station was investigated. Using dry aerosol coating process, we developed a high-performance antiviral air filter with spark discharger and carbon brush type ionizer. Silver nanoparticles were produced by a spark discharge generation system with ion injection system and were used as antiviral agents coated onto a medium grade air filter. The pressure drop, filtration efficiency, and antiviral ability of the filter against aerosolized MS2 virus particles as a surrogate of SARS-CoV-2 virus were tested with dust contamination. Dust contamination caused the increase of the filtration efficiency and pressure drop, while the antiviral agents (in this study, silver nanoparticles) coating did not have any significant effect on the filtration efficiency and pressure drop. Using these properties, we suggested a novel method to maximize the antiviral performance of the antiviral air filter that was contaminated by dust particles. Moreover theoretical analysis of antiviral ability with dust contamination and re-coated antiviral agents was carried out using a mathematical model to calculate the time-dependent antiviral effect of the filter under actual conditions of subway station. Our model can be used to apply on antiviral air filtration system of subway station for prevention of pandemic diffusion, and predict the life cycle of an antiviral filter.

Long-Term Impact of Stigma on Coping Strategies and Depressive and Posttraumatic Stress Symptoms in MERS Survivors (메르스 대유행 생존자들이 인식한 낙인이 대처전략 및 우울 및 외상후 스트레스 증상에 끼치는 장기적 영향)

  • Objectives : This study intends to assess the associations among perceived stigma at the time of infection, coping strategies adopted 12 months later and depressive and posttraumatic symptoms 24 months later in Middle Eastern Respiratory Syndrome (MERS) survivors. Methods : A nationwide cohort study was conducted on 63 survivors of 2015 MERS outbreak. Demographic data, illness severity of MERS, depression and posttraumatic stress symptoms, coping strategies and MERS-related stigma were collected 12 and 24 months after the MERS outbreak, respectively. Results : Higher levels of perceived stigma at the time of outbreak were associated with higher levels of dysfunctional coping strategies after 12 months (p=0.003) and more severe depressive (p=0.058) and posttraumatic stress symptoms (p=0.011) after 24 months. Moreover, higher levels of dysfunctional coping strategies after 12 months were significantly associated with more severe depressive (p=0.002) and posttraumatic stress symptoms (p<0.001) after 24 months. Conclusions : Social stigma against people who have contracted an emerging infectious disease can leave a negative impact on the mental health of the survivors in the long term. In case of novel pandemics in the future, prompt rectification of stigma and promotion of adaptive coping strategies in survivors are needed.

Dynamic of heat production partitioning in rooster by indirect calorimetry

  • Objective: The objective of this study was to describe a methodological procedure to quantify the heat production (HP) partitioning in basal metabolism or fasting heat production (FHP), heat production due to physical activity (HPA), and the thermic effect of feeding (TEF) in roosters. Methods: Eighteen 54-wk-old Hy Line Brown roosters (2.916±0.15 kg) were allocated in an open-circuit chamber of respirometry for O2 consumption (VO2), CO2 production (VCO2), and physical activity (PA) measurements, under environmental comfort conditions, following the protocol: adaptation (3 d), ad libitum feeding (1 d), and fasting conditions (1 d). The Brouwer equation was used to calculate the HP from VO2 and VCO2. The plateau-FHP (parameter L) was estimated through the broken line model: HP = U×(R-t)×I+L; I = 1 if t<R or I = 0 if t>R; Where the broken-point (R) was assigned as the time (t) that defined the difference between a short and long fasting period, I is conditional, and U is the decreasing rate after the feed was withdrawn. The HP components description was characterized by three events: ad libitum feeding and short and long fasting periods. Linear regression was adjusted between physical activity (PA) and HP to determine the HPA and to estimate the standardized FHP (st-FHP) as the intercept of PA = 0. Results: The time when plateau-FHP was reached at 11.7 h after withdrawal feed, with a mean value of 386 kJ/kg0.75/d, differing in 32 kJ from st-FHP (354 kJ/kg0.75/d). The slope of HP per unit of PA was 4.52 kJ/mV. The total HP in roosters partitioned into the st-FHP, termal effect of feeding (TEF), and HPA was 56.6%, 25.7%, and 17.7%, respectively. Conclusion: The FHP represents the largest fraction of energy expenditure in roosters, followed by the TEF. Furthermore, the PA increased the variation of HP measurements.

A Case of Multisystem Inflammatory Syndrome in Children (MIS-C) with Acute Myocarditis

  • After initial reports of multisystem inflammatory syndrome in children (MIS-C) in April 2020 in Europe, this disease has been known to occur in children with recent history of coronavirus disease 2019 (COVID-19) and most cases have been reported in Europe and the Unites States of America. We report a case of a 14-year-old girl who was diagnosed with MIS-C with acute myocarditis and successfully treated with intravenous immunoglobulin (IVIG), methylprednisolone, and anakinra. At initial presentation, she had persistent high fever, generalized rash, generalized swelling, abdominal pain, and low blood pressure. She showed a remarkably elevated level of inflammation and cardiac enzyme markers and had a previous history of COVID-19 5 weeks before the initial presentation. After extensive work up, other infectious and non-infectious causes were excluded. She was diagnosed with MIS-C and initially treated with IVIG and high-dose methylprednisolone; however, despite treatment, her heart function deteriorated and coronary artery dilatation progressed. Therefore, anakinra, an interleukin-1 receptor antagonist, was administered on hospital day 6, after which her cardiac function exhibited improvement. She was discharged on hospital day 19 without any symptoms, and follow-up echocardiography after 1 month revealed fully recovered heart function with normal coronary arteries.

Impact of the Coronavirus Disease Pandemic on Patients with Head Injuries in South Korea

  • Objective : The coronavirus disease 2019 (COVID-19) pandemic is affecting the characteristics of patients with head injuries. This study aimed to evaluate the effect of the COVID-19 pandemic on patients with head injuries at a regional emergency medical center in South Korea. Methods : From April 2019 to November 2020, 350 patients with head injuries were admitted to our hospital. The study period was divided into the pre-COVID-19 (n=169) and COVID-19 (n=181) eras (10 months each). Patients with severe head injuries requiring surgery (n=74) were categorized into those who underwent surgery (n=41) and those who refused surgery (n=33). Results : Head injuries in pediatric patients (<3 years) were more frequent in the COVID-19 era than in the pre-COVID-19 era (8.8% vs. 3.6%, p=0.048). More patients refused surgery in the COVID-19 era than in the pre-COVID-19 era (57.9% vs. 30.6%, p=0.021). Refusal of surgery was associated with old age (67.7±14.5 vs. 52.4±19.1, p<0.001), marital status (married, 84.8% vs. 61.0%, p=0.037), unemployment (42.4% vs. 68.3%, p=0.034), COVID-19 era (66.7% vs. 39.0%, p=0.021), and lower Glasgow coma scale scores (6.12±3.08 vs. 10.6±3.80, p<0.001). Multivariable logistic regression analysis revealed that refusal of surgery was independently associated with old age (adjusted odds ratio [OR], 1.084; 95% confidence interval [CI], 1.030-1.140; p=0.002), COVID-19 era (adjusted OR, 6.869; 95% CI, 1.624-29.054; p=0.009), and lower Glasgow coma scale scores (adjusted OR, 0.694; 95% CI, 0.568-0.848; p<0.001). Conclusion : We observed an increased prevalence of head injuries in pediatric patients (<3 years) during the COVID-19 pandemic. Additionally, among patients with severe head injuries requiring surgery, more patients refused to undergo surgery during the COVID-19 pandemic.

Sensitive and Selective Electrochemical Glucose Biosensor Based on a Carbon Nanotube Electronic Film (탄소나노튜브 전자 필름을 이용한 고감도-고선택성 전기화학 글루코스 센서)

  • This work presents a non-destructive and straightforward approach to assemble a large-scale conductive electronic film made of a pre-treated single-walled carbon nanotube (SWCNT) solution. For effective electron transfer between the immobilized enzyme and SWCNT electronic film, we optimized the pre-treatment step of SWCNT with p-terphenyl-4,4"-dithiol and dithiothreitol. Glucose oxidase (GOx, a model enzyme in this study) was immobilized on the SWCNT electronic film following the positively charged polyelectrolyte layer deposition. The glucose detection was realized through effective electron transfer between the immobilized GOx and SWCNT electronic film at the negative potential value (-0.45 V vs. Ag/AgCl). The SWCNT electronic film-based glucose biosensor exhibited a sensitivity of 98 ㎂/mM·cm2. In addition, the SWCNT electronic film biosensor showed the excellent selectivity (less than 4 % change) against a variety of redox-active interfering substances, such as ascorbic acid, uric acid, dopamine, and acetaminophen, by avoiding co-oxidation of the interfering substances at the negative potential value.
