Fall Risk Factors and Characteristics of an Acute Hospital Setting across Clinical Departments (일 종합병원 입원환자의 진료과별 낙상특성 및 위험요인)

  • Purpose: This study was done to determine inpatient fall rates in an acute hospital setting and to explore risk factors and characteristics across clinical departments. Methods: The medical records and standardized fall reports of 416 patients admitted between January 1 and December 31, 2012 were reviewed. Descriptive statistics and statistical tests were used, including: t-test, ${\chi}^2$-test, ANOVA. Results: The total fall rate per 1,000 inpatient days was 0.49. Fall rate, fall risk factors and characteristics such as age, type of fallers and mean MFS (Morse Fall Scale) differed significantly among clinical departments. Conclusions: The analysis results show that the fall rates, fall risk factors and characteristics of acute hospital inpatient falls varied significantly across clinical departments. The findings of this study suggest that hospitals should consider differences in fall related characteristics across clinical departments when implementation fall prevention strategies and interventions.

Factors influencing the clinical competence of nursing students (간호대학생의 임상수행능력 영향요인)

  • Purpose : The purpose of this study was to identify factors influencing the clinical competence of nursing students. Methods : The participants of this study were 209 nursing students who were in 3rd and 4th grade. The collected data were analyzed using one-way ANOVA, Kruskal-Wallis test, Mann-Whitney test, Pearson's correlation coefficients, and stepwise multiple regression. Results : Clinical competence had positive relationships with self-leadership (r=.59, p <.001) and resilience (r=.64, p <.001). Clinical competence was influenced by resilience (𝛽=.45), self-leadership (𝛽=.20), and grade (𝛽=.20), with an explanatory power of 44.9%. Conclusions : Based on these results, it is suggested that future nursing education should develop a curriculum that enhances students' personal abilities such as self-leadership and resilience, rather than only teaching knowledge and skills.

Clinical Competence and Self-confidence of New Graduate Nurses with an Integrated Nursing Curriculum of Simulation with Problem-Based Learning (문제중심학습 연계 시뮬레이션 기반 통합간호교육과정을 이수한 신졸업간호사의 간호수행능력과 자신감)

  • The purpose of this study was to verify the effectiveness of an integrated nursing curriculum based on simulation with problem-based learning (PBL) by comparing the clinical competence and the self-confidence of newly graduated nurses. A non-equivalent control group post-test design was employed to compare the clinical competence and the self-confidence in the clinical performance examination using standardized patients between 39 newly graduated nurses with the traditional nursing curriculum and 35 with the integrated nursing curriculum. Data analysis involved Fisher's exact test, t-test, Pearson's correlation coefficient, and ANCOVA with the SPSS 19.0 program. The total clinical competence mean score graded by the standardized patients was not different between the two groups. However, the total clinical competence mean score graded by faculty was significantly higher in the integrated curriculum group than the traditional curriculum group. The mean self-confidence score was significantly higher in the integrated curriculum group than the traditional curriculum group. Active teaching-learning strategies including simulation or PBL in the nursing curriculum could benefit for nursing students by inducing favorable clinical competence and self-confidence. Longitudinal follow-up studies based on observation are needed to explore the patient outcomes in addition to the learner outcomes in clinical settings.

Comparison of Upper and Lower Limb Functional Ability Depends on Dominants (우세측에 따른 상지와 하지의 기능 비교)

  • Purpose. The purpose of this study was compared of left dominant and the right dominant hands about coordination. Methods. The participants were 30 occupational therapy students in M university in Changwon, from April 28th 2015 to May 1. The test tools were Jebson-Taylor Hand Function Test, Hand Strength & Pinch, Grooved Pegboard Test, Box & Block and Balance Test. Results. Right dominant hand was higher than left dominant in hand grip and dexterity, hand manipulation skills. And one leg standing and toe standing were higher left foot than right foot dominant. Conclusion. Most of all evaluation items showed a better result right dominant hand than left dominant. But shows that there is no functional difference between left-hand dominant than right-hand. So left-hand dominant coordination was evaluated by a better than right-handed.

A Comparative Study of Balance Performance in 6,7 years old by means of the Pediatric Clinical Test of Sensory Interaction for Balance (소아용 균형 감각검사(PCTSIB)를 이용한 6세 및 7세 아동의 균형 수행력 비교)

  • The purpose of this study were to compare the balance performance with 44 children, who aged 6 and 7years old by means of the Pediatric Clinical Test of Sensory Interaction for Balance(PCTSIB), second is to determine whether age and gender-relatad difference were present, third is to found correlation of weight, height with duration of balance performance. All subjects were performed with different foot position that were feet-together, heel-toes. The starting position was that subject placed their hands on the hips. The results of each test were measured by stop watch. These collected data were analyzed by using oneway ANOVA, Sheffe test t-test and correlation. The results of this study were as follows ; 1. There were all significiant difference is all instances except condition 4 with heel-toes(p<0.05). The duration of balance performance of 7-year-olds group was more increased than 6-year-olds group. 2. There was statistically significiant difference in all instances by gender(p<0.05). 3. There was no correlation between weight or height duration of balance performance (p<0.05).

Development and Enforcement of Short Case Test Items for Evaluation in Undergraduate Course of Plastic Surgery: 3 Years Experience (학부 성형외과학 과목평가를 위한 짧은 사례문항의 개발과 시행: 3년간의 경험)

  • The aim of this study is to evaluate the clinical competence of medical student using short case test items. Development of short case test items with definition of an assessment of subject, choice of clinical stations sampling, identification of components of clinical competence to be evaluated, and assessment of practicability and results exploitation. For the last three years 28, 48 and 30 topics were developed annually. Among them 20, 23 and 23 topics were selected. About 50 students were examined annually. The mean score was 72.7, 57.1, 64.3 respectively. The mean preparation time of the students for the examination was 9.8 hours. About 60% of the students prepared the test in small group activity. Our experience suggests that short case test items be feasible for evaluation of student learning in Plastic Surgery. This method should be considered as a useful tool to assess medical student's competence or even an examination for board certification.

  • In the field of clinical medicine, diagnostic accuracy studies refer to the degree of agreement between the index test and the reference standard for the discriminatory ability to identify a target disorder of interest in a patient. The receiver operating characteristic (ROC) curve offers a graphical display the trade-off between sensitivity and specificity at each cutoff for a diagnostic test and is useful in assigning the best cutoff for clinical use. In this end, the ROC curve analysis is a useful tool for estimating and comparing the accuracy of competing diagnostic tests. This paper reviews briefly the measures of diagnostic accuracy such as sensitivity, specificity, and area under the ROC curve (AUC) that is a summary measure for diagnostic accuracy across the spectrum of test results. In addition, the methods of creating an ROC curve in single diagnostic test with five-category discrete scale for disease classification from healthy individuals, meaningful interpretation of the AUC, and the applications of ROC methodology in clinical medicine to determine the optimal cutoff values have been discussed using a hypothetical example as an illustration.

Clinical Use of PFA®-100 in Pre-surgical Screening for Platelet Function Test (수술 전 혈소판 기능 검사를 위한 PFA®-100의 임상적 이용)

  • The Platelet Function Analyzer (PFA)$^{(R)}$-100 measures the ability of platelets activated in a high-shear environment to occlude an aperture in a membrane treated with collagen and epinephrine (CEPI) or collagen and ADP (CADP). The time taken for the flow across the membrane to stop (closure time, CT) is recorded. The aim of this study was to assess the potential of the PFA$^{(R)}$-100 as a primary clinical screening tool using the wide spectrum of clinical samples assessed for platelet function as well as to perform the optimal algorithm for the use of PFA$^{(R)}$-100. We established the reference interval in 460 hospital inpatients defined as having normal platelet function based on classical laboratory tests. The reference interval by using the range $5^{th}$ and $95^{th}$ percentile was 84~251 seconds for males CEPI-CT and 85~249 seconds for females CEPI-CT. A total of 1,200 inpatients were enrolled to identify impaired hemostasis before surgical interventions. The abnormal group showing prolonged CEPI-CT was 303 cases (18.9%). Only 3 cases had both abnormal CEPI-CT and CADP-CT. Several factors including sample errors, drugs, hematologic abnoralities were contributed to unexpected prolonged CEPI-CT for screening test. The von Willebrand factor (vWF:Ag) assay was performed only in one patient to verify the algorithm for the use of PFA$^{(R)}$-100. The PFA$^{(R)}$-100 was sensitive and rapid method for primary screening test of platelet dysfunction, so we can substitute it for the bleeding time in routine clinical practice.

Convergence Study on Mediating Effect of Clinical Competence on the Relationship between Grit and Retention Intention among New Graduate Nurses (신규간호사의 그릿과 재직의도의 관계에서 임상수행능력의 매개효과에 대한 융복합적 연구)

  • This study was conducted to examinate the mediating effect of clinical competence on the relationship between grit and retention intention among new graduate nurses. Participants were 232 nurses working at hospitals with over 300 beds in Korea. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficients, and multiple regression following the Baron and Kenny method and Sobel test for mediation. As a result, There were significant relationships between grit and clinical competence(r=.38, p=.011), grit and retention intention(r=.32, p=.016), and clinical competence and retention intention(r=.33, p<.001). Clinical competence showed partial mediating effect in the relationship between grit and retention intention(z=3.14, p<.001). The results of this study suggest that interventions enhancing grit would be useful to improve retention intention of new graduate nurses.

A Comparison of the Nurses Image according to Clinical Practice Experience of Nursing Students (간호대학생의 임상실습 경험에 따른 간호사 이미지 비교)

  • The purpose of this study was to compare nurses' image perceived by two group of nursing students as clinical practice experience and non-clinical practice experience. This study used 400 questionnaires from nursing students in C city to collect data from September 2 to September 9, 2014 The data were analyzed by SPSS/WIN 18.0 program using descriptive statistics, ${\chi}^2$-test, t-test, ANOVA, ANCOVA. According to the result, The nurse image was significantly different between the two groups(F=4750.2, p<.001). Also, the traditional image(F=3631.5, p<.001), social image(F=2872.5, p<.001), professional image(F=4022.9, p<.001), and personal image(F=1812.9, p<.001) of the nurse image sub-domain showed statistically significant differences. Therefore, In order to enhance the image of nurses after clinical practice of nursing college students, continuous efforts will be needed not only in clinical field and nursing college's efforts but also in policy support for nurses' independent work expansion.
