Stress and Satisfaction from Simulation-based Practice and Clinical Practice on High-risk Newborn Nursing (고위험 신생아간호에 관한 시뮬레이션 실습과 신생아집중치료실 실습의 스트레스와 만족도)

  • Purpose: The purpose of this study was to compare practice stress and practice satisfaction between clinical practice and simulation-based practice groups. Methods: A total of 159 nursing students (85 in the simulation group and 74 in the NICU group) participated in the study. Research tools measuring practice stress and practice satisfaction were used. Data was analyzed utilizing a t-test, Mann Whitney U-test, and descriptive statistics. Results: The simulation group showed a lower level of practice stress(practice instruction, practice evaluation, interpersonal relationship) a higher level of practice satisfaction compared with the NICU group. Conclusion: Simulation-based practice is an effective learning method for high-risk newborn nursing and can be used as an alternative to clinical practice.

Nursing Students' Stress and Anger Experience in the Clinical Setting (간호학생의 임상실습과 관련된 스트레스와 분노경험)

  • The purpose of this study was identify the level of stress, anger and anger expression in the clinical setting and identify the relation ships between anger and stress of nursing students in the clinical setting. A convenient sample of 213 junior college of nursing students was used for the study. The data were analyzed by descriptive statistics and Pearson Correlation Coefficients. The results of this study were summarized as follows : 1. The average stress score in the clinical setting of subjects was 3.87. The higher level of stress in the clinical setting was 4.10 in domain of nurses, 4.10 in domain of human relationships, 4.07 in domain of report and 3.89 in domain of relationship between nursing theory and practice. 2. The average state-anger score of subjects was 1.54. 3. The average anger-out score of anger expression mode was 1.66 and 2.36 in anger-in. 4. The state-anger was found significantly related to stress of nursing students in the clinical setting (r=0.256 p<0.001). In the relationship between state- anger and stress in the clinical setting, environment, patient relationship, between nursing theory and practice, nurses, report, orientation were significantly positive correlation . 5. The anger-out of anger expression mode was not found significantly related to stress of nursing students in the clinical setting. 6. The anger-in of anger expression mode was found significantly related to stress of nursing students in the clinical setting (r=0.205 p<0.01). In the relationship between anger-in of anger expression mode and stress in the clinical setting, environment, patient, nurses, human relationships, conference were significantly positive correlation.

Development of Construction Model of Disease Classification on Clinical Diagnosis in Ophthalmology (임상진단명에 따른 질병분류체계 구축모형 개발 - 안과를 대상으로 -)

  • Background : ICD-10 Classification, which is used domestically as well as internationally, has limited use in the clinical practice since it is developed for at disease statistics and epidemiology. Therefore, the purposes of this study were to improve the quality of diagnosis by constructing a new disease classification based on the diagnoses doctors currently make in the clinical setting and connecting this classification with OCS and EMR, and to meet the demands of doctors for high quality medical study data in medical research. Methods : The specialists in each ophthalmic subfield collected clinical diagnoses and abbreviations based on the ophthalmology textbooks and confirmed the classifications. Total number of clinical diagnoses collected was totaled 672, for which ideal diagnoses had been selected and a new model of disease classification model in connection with ICD-10 was constructed. The constructed classification of clinical diagnoses consisted of six steps: the first step was the classification by ophthalmic subspecialty field; the second to fifth steps were the detailed classification by each specialty field; the sixth step was the classification by site. Results : After introducing the new disease classification, research on the use and a pre-post comparison was conducted. The result from the research on the use of the clinical diagnoses in inpatient and outpatient care has shown a gradually increasing tendency. From the pre-post comparison of EMR discharge summary diagnoses, the result demonstrated that the diagnosis was stated correctly and in detail. Since the diagnosis was stated correctly, code classification became correct as well, which makes it possible to construct high quality medical DB. Conclusion : This construction of clinical diagnoses provides the medical team with high quality medical information. It is also expected to increase the accuracy and efficiency of service in the department of medical record and department of insurance investigation. In the future, if hospitals wish to construct a classification of clinical diagnosis and a standard proposal of clinical diagnosis is presented by a medical society, the standardization of diagnosis seems to be possible.

Essential Basic Nursing Skills and Clinical Practice Satisfaction of Nursing Students

  • In this paper, we propose basic materials for effective education of essential basic nursing skills and empirical base supporting the strengthening of essential basic nursing skills. An investigation was conducted with nursing college students ready for an employment on the relationship between the students' knowledge level of essential basic nursing skills and their satisfaction level against the clinical practice. The collected data were analyzed using descriptive statistics, t-test, one-way ANOVA, scheffē test and Pearson's correlation coefficients. in SPSS WIN 21.0 program. The findings of this study were as follows. 1) The knowledge level of essential basic nursing skills received a score of 88.95. 'Admission management' item received the highest score of 4.96 point on a 5 point scale while 'Retention urinary catheterization' item received the lowest score of 3.06. The higher the difficulty level of essential basic nursing skills was, the lower the knowledge level of it was. 2) The satisfaction against the clinical practice received a score of 3.28. In the clinical practice area, the satisfaction against the curriculum received a score of 3.84, the satisfaction against clinical practice contents a score of 3.39, the satisfaction against clinical practice environment a score of 3.17, the satisfaction against clinical practice time a score of 3.15. There was a significant correlation between the knowledge level of essential basic nursing skills and clinical practice satisfaction. These findings indicate that a systematic education of essential basic nursing skills could improve nursing students' satisfaction against the clinical practice.

The Analysis of Satisfaction with Clinical Training and the Related Factors in the Korean Medicine Obstetrics and Gynecology (한방부인과학에서 임상실습교육의 만족도와 관련요인 분석)

  • Objectives: The purpose of this study is to analyze csatisfaction with clinica1 training in the department of Korean Medicine Obstetrics and Gynecology and the related factors. Methods: All the students on the graduating class of school of Korean Medicine, Dong-Shin University must be trained for 1 year in the department of Korean Medicine Obstetrics and Gynecology according to clinical training guideline including Objective Structured Clinical Examination (OSCE). After completing clinical training, we distributed questionaries to them about a reflex of the object of study, usefulness, difficulty, satisfaction and preference. And then, we analyzed the related factors including descriptive statistics, frequency analysis and correlation analysis by SPSS 12.0. Results: In general, a reflex of the aim of study (7.31±1.59), satisfaction with contents (7.44±1.58), and difficulty (6.13±1.34) were evaluated restectively. There is no statistically significant difference with sex. There is a statistically significant positive correlation between a reflex of the aim of study and satisfaction with contents. The primary preferences of clinical practice were Clinic Training, Ward Training, PBL (Problem Based Learning), OSCE. In OSCE, a reflex of the aim of study (7.04±1.83) and usefulness (7.02±1.97) were highly evaluated. There is no statistically significant difference with sex. The primary preferences for clinical OSCE practice were breast examination, genital examination, ultrasound scan, and pap smear. Facilities and equipment (6.84±2.01), contents of clinical training (7.29±1.83), the handout (7.20±1.98) and time scheduling (7.29±1.90) were evaluated respectively in general composition of clinical training. Conclusions: Clinical training program in the department of Korean Medicine Obstetrics and Gynecology can be highly estimated from the viewpoint of satisfaction with clinical training and the related factors.

Revised Clinical Ladder System Model Based on Nurses' Clinical Competence and Professional Activities for Nurses in General Hospitals (종합병원 간호사의 간호역량, 전문성 활동에 따른 임상경력관리체계 수정모형)

  • Purpose: The purpose of this study was to propose a revised Clinical Ladder System(CLS) Model for nurses based on the evaluation of clinical competence and professional activities of nurses working in general hospitals. Methods: Data were collected between September 10 and October 30, 2017. Participants were 50 head nurses from 10 general hospitals with over 400 beds located in Seoul City and Gyeonggi Province. Each head nurse evaluated clinical competence, qualifications, and professional activities of 5 staff nurses at each of the 5 levels of CLS in her unit. The total number of the nurses evaluated was 245. Data were analyzed with descriptive statistics and t-test, one-way ANOVA, and $Scheff{\acute{e}}$. Results: Over 80% of the nurses were university graduates. As the CLS levels increased, clinical competence, qualifications, and professional activities also increased significantly. Education material development and quality improvement activities were carried out by nurses from level 2, research and evidence based practice activities were carried out from level 3, and nurses at level 4 or 5 participated in most of the professional activities as leaders. Conclusion: In order to retain excellent nurses in general hospitals, recognizing and rewarding nurses according to the revised model of the CLS are recommended.

A Study on the Satisfaction of Physical Therapy Students with the Clinical Training Programs in Busan, Ulsan, and Gyongnam (부산, 울산 및 경남 소재 물리치료(학)과 임상실습 프로그램에 대한 만족도 조사 연구)

  • Purpose: The purpose of this study was to provide basic data to establish effective clinical training programs by identifying various factors that influence the satisfaction of physical therapy students with those programs. Methods: The study subjects were 205 students from six colleges (two 4-year and four 3-year colleges) who participated in clinical training programs. The colleges have physical therapy departments and are located in Busan, Ulsan, and Gyeognam. A survey consisting of 75 questions was conducted between March and December, 2003. Survey responses were analyzed through a frequency analysis, descriptive statistics, and the multiple responses approach, and correlations among the questions were analyzed using a chi-square test. Results: The level of satisfaction with clinical training programs did not differ according to the school system or the size of the clinical training institution. However, several factors led to a higher level of satisfaction, including more active participation of students in the clinical training, more intensive instruction from school professors, a larger number of subjects to complete before the clinical training, and a higher level of student perception that their institution's clinical training program was systematic Conclusion: This study surveyed physical therapy students located in Busan, Ulsan, and Gyeongnam who attended selected schools and training institutions that have their own individual characteristics; therefore, this study may have limitations for comparative analysis. However, if more extensive studies are per formed regionally in the future using the approach taken here, clinical training programs could be developed that can satisfy both schools and the industry.

The Effects of Responsibility and Critical Thinking Disposition on Clinical Competence of Nursing students who have Completed Clinical Practice (임상실습을 수료한 간호대학생의 책임성과 비판적 사고성향이 임상수행능력에 미치는 영향)

  • The purpose of this study was to investigate the responsibility & critical thinking disposition and identify influencing factors of clinical competence of nursing students who have completed clinical practice. Data collection was performed from Dec. 11th, 2019 to Dec. 13th. A total of 119 4th grade nursing students were selected. Data were analyzed with descriptive statistics, t-test, ANOVA, Scheffé test, Pearson's correlation and stepwise multiple regression analysis using SPSS 25.0. As a result of this study, the correlated factors of clinical competence included responsibility(r=.53, p<.001), critical thinking disposition(r=.71, p<.001). The factors influencing clinical competence were critical thinking disposition(𝛽=.56, p<.001), responsibility(𝛽=.32, p<.001), and clinical practice satisfaction(𝛽=.13, p=.012). These factors accounted for 52% of the clinical competence(p<.001). Therefore, it is necessary to develop and apply educational programs so that concrete and practical education for reinforcing critical thinking disposition and responsibility can be performed together with clinical practice.

Clinical data analysis in retrospective study through equality adjustment between groups (후향적연구의 집단 간 동등성확보를 통한 임상자료분석)

  • There are two types of clinical research to figure out risk factor for disease using collected data. One is prospective study to approach the subjects from the present time and the other is retrospective study to find the risk factor using the subject's information in the past. Both approached and study design are different but the purpose of the two studies is to identify a significant difference between two groups and to find out what the variables to influence groups. Especially when comparing the two groups in clinical research, we have to look at the difference between the impact clinical variables by group while controlling the influence of the baseline characteristics variables such as age and sex. However, in the retrospective study, the difference of baseline characteristic variables can occur more frequently because the past records did not randomly assign subjects into two groups. In clinical data analysis use covariates to solve this problem. Typically, the analysis method using the analysis of covariance of variance, adjusted model, and propensity score matching method. This study is introduce the way of equality adjustment between groups data analysis using covariates in retrospective clinical studies and apply it to the recurrence of gastric cancer data.

Factors Influencing Nursing Professionalism in Nursing Students: Clinical Learning Environment and Attitude toward Nurse-Physician Collaboration (간호대학생의 전문직관에 영향을 미치는 요인: 임상실습교육환경과 의사-간호사 협력에 대한 태도를 중심으로)

  • Purpose: This study was done to identify the influence of clinical learning environment and attitude toward physician-nurse collaboration on professionalism in nursing students. Method: The sample consisted of 317 nursing students. Data were collected from October 10 to December 20, 2014 and were analyzed using t-test, ANOVA, Pearson correlation and multiple linear regression with IBM SPSS statistics version 19. Results: The mean score for clinical learning environment was 3.15 out of 5. The mean score on the Jefferson scale attitude toward physician-nurse collaboration (JSAPNC) was 3.31 out of 4. The mean score for professionalism was 3.70 out of 5. The predictors of professionalism were major satisfaction, 'care vs cure' for the JSAPNC and 'patient relationship', 'student satisfaction' for clinical learning environment. Conclusions: Based on these results, educational programs to improve attitude toward physician-nurse collaboration and clinical learning environment should be developed.
