보건계열 학생의 환자안전 지식 및 수행 자신감 : 융복합적 접근 (The Knowledge and Confidence in Performance on Patient Safety among Health-Related Majors : Convergent Approach)

  • 본 연구는 보건계열 학생을 대상으로 환자안전 지식과 환자안전 수행자신감을 파악하기 위해 수행되었다. 보건계열학생 349명이 참여하였고, 자료분석은 SPSS 21.0을 이용하여 분석하였다. 연구결과 환자안전지식 정답률은 평균 65.7%였다. 환자안전 수행자신감은 평균 $7.11{\pm}1.74$점이었으며 문항별로 살펴보면 환자확인이 높았으며 효과적 의사소통이 낮게 나타났다. 환자안전지식과 환자안전 수행자신감의 하부영역인 환자확인, 의사소통, 수술시술, 낙상, 환자안전사고보고는 양의 상관관계였고, 환자안전지식과 감염관리, 시설환경은 음의 상관관계였다. 보건계열학생들에게 임상실습 전 환자안전에 대한 중요성을 높이고 안전한 임상실습이 수행될 수 있도록 환자안전교육프로그램을 개발하는 것이 필요하다.

  • 임상병리 검사분야에 있어서 환자로부터 유래된 조직이나 세포의 형태학적 변화, 세포 생리, 세포 내 분자의 추적 및 신호전달 체계 등의 임상검사 및 관련 연구를 위한 빼놓을 수 없는 주요한 진단과 연구장비로서 현미경이 가지는 의미는 크다고 할 수 있다. 현미경에 대한 포괄적인 지식과 이해를 바탕으로 현미경의 올바른 사용, 관리와 유지보수는 신뢰도 높은 이미지 획득과 그에 따른 정확한 데이터 분석을 통한 질병의 진단을 위해서 반드시 요구되는 부분이라고 할 수 있다. 광학현미경의 표준 운영 절차(standard operating procedure, SOP)는 현미경의 작동 절차와 함께 검사실 규모에 따른 현장 사용자의 체계적인 현미경 장해 해결 방안과 기계적 원리에 대한 핵심 정보가 함께 수록되어야 한다. 현미경 유지관리 업무에는 대물, 접안렌즈와 현미경 내부 광학필터의 청소, 광원의 교체와 교정, XY재물대 유지보수, 공초점 레이저 주사 현미경(confocal laser scanning microscope)에서의 점확산함수(point spread function, PSF) 측정, 형광현미경에서의 검사 품질관리(quality control, QC)와 체계적인 현미경 장해 해결방안 등이 포함되어야 한다. 본 종설에서는 국제적 기준에 따른 레이저의 위험도에 따라 일부 현미경에 장착된 레이저 광원에 대한 안전지침과 보호장구에 대한 내용을 함께 소개하였다. 현미경을 통해 획득된 이미지는 촬영된 시점의 검체에 대한 모든 정보를 제공한다고 할 수 있으며, 적절한 유지보수 프로그램과 그에 따라 적합하게 관리된 현미경만이 이미지 데이터를 통한 정보의 획득, 올바른 해석과 정확한 진단에 반드시 필요한 선제 조건들이라고 하겠다.

간호대학생의 환자안전에 대한 지식, 간호전문직관과 환자안전관리활동의 관련성 (The Relationship Between Knowledge of Patient Safety, Nursing Professionalism and Patient Safety Management Activities in Nursing Students)

  • Purpose: The objective of this study was to identify the relationship between knowledge of patient safety, nursing professionalism and patient safety management activities of nursing students with clinical practical experience. Methods: Self-administered questionnaires survey on knowledge of patient safety, nursing professionalism, and patient safety management activities were conducted for the $3^{rd}-year$ and $3^{th}-year$ nursing students. 139 questionnaires were distributed, of which, 131 were used for data analysis. Results: The scores of nursing students' knowledge of patient safety, nursing professionalism and patient safety management activities were $6.76{\pm}1.26$, $65.11{\pm}7.97$ and $67.99{\pm}7.26$, respectively. Knowledge of patient safety differed significantly according to the grade. Nursing professionalism had a difference with major satisfaction, clinical practical satisfaction, and experience of patient safety accident. Patient safety management activities were positively correlated (p<.01) with knowledge of patient safety and nursing professionalism. Patient safety management activities increased significantly with increase in the scores of knowledge of patient safety and nursing professionals. The factors that were related to patient safety management activities of nursing students were knowledge of patient safety and nursing professionalism. Knowledge of patient safety and nursing professionalism were selected as significant variables for explaining the patient safety management activities of nursing students, of which the coefficient of determination was 9.8%. Conclusion: To promote patient safety management activities of nursing students, training programs for patient safety management activities are required. Also, there is the need to increase the knowledge of patient safety and nursing professionalism of nursing students using various educational method.

공감만족-공감피로(CS-CF) 모델에 근거한 임상간호사의 소진 구조모형 (Structural Equation Modeling on Burnout in Clinical Nurses based on CS-CF Model)

  • Purpose: The purpose of this study was to construct and test a structural equation modeling on burnout of clinical nurses based on CS-CF model. Methods: A survey using a structured questionnaire was conducted with 557 clinical nurses. Data were analyzed using structural equation modeling. Results: The modified hypothetical model yielded the following ${\chi}^2=289.70$, p<.001, RMSEA=.09, GFI=.93, TLI=.91, CFI=.94, PCFI=.65, AIC=363.21, SRMR=.05 or less and showed good fit indices. Nursing work environment, patient safety culture and resilience showed indirect effects on burnout while compassion fatigue and compassion satisfaction had direct effects. Conclusion: Results of this study suggest that compassion fatigue must be decreased and compassion satisfaction has to be increased, while burnout is lowered by enhancing the clinical nursing work environment, patient safety culture and resilience. In addition, more variables and longitudinal studies are necessary to validate the clear cause-and-effect relationship between the relevant variables.

WHW$^{(R)}$의 랫드에서의 반복경구투여 독성에 관한 연구 (Thirteen-week repeated-dose oral toxicity study of the Modified Wenpitang-Hab-Wulingsan (WHW$^{(R)}$) in Sprague-Dawley rats)

  • Objectives : We investigated the repeated-dose toxicity of Wenpitang-Hab-Wulingsan(WHW), a Korean traditional medicine prescribed with twelve herbs, which has been used for the treatment of renal disease. Methods : WHW extract prepared by GLP company. WHW was supplemented by gavage at 0, 100, 500 and 1000 mg/kg/day for 13-week consecutive days. We recorded the clinical signs of toxicity, body weight, organ weights, hematology, gross and histological changes in target organs rats and clinical chemistry analysis for all rats. Results : WHW extract at all doses was shown no mortality or abnormal clinical signs in rats during at the observation period. Furthermore, there was no difference in body weight and food-take consumption, organ weight, gross pathological findings, and urine analysis among the groups of rats treated with different doses of WHW extract. The hematological analysis and clinical blood chemistry data were revealed no toxic effects from WHW-treated rats. Conclusions : The results suggest that WHW extract in rats is a wide margin of safety on a acute toxicity.

Single-Dose Intramuscular Toxicity Test Using No-Pain Pharmacopuncture in Sprague-Dawley Rats

  • Objectives: This study aimed to evaluate the potential toxicity of a recently developed and clinically used No-Pain pharmacopuncture (NPP) solution. We also assessed the lethal dose of the NPP agent following a single intramuscular injection in Sprague-Dawley (SD) rats. Methods: Animals were divided into two groups: the NPP test material group and the normal saline control group. A single intramuscular injection of the NPP agent (1.0 mL/animal) was administered to rats of the NPP test material group. The control group rats received the same volume of normal saline. Both female and male rats were included in each group. All rats were monitored for clinical signs and body weight changes for 14 days after administration of the test substance or saline. At the end of the observation period, a gross necropsy was conducted and localized tolerance at the injection site was analyzed. Results: No mortality was observed in the NPP test material and control groups. Moreover, no test substance-related effects were observed on clinical signs, body weight, necropsy findings, and localized tolerance at the injection site. Conclusion: The approximate lethal dose of the NPP agent is greater than 1.0 mL/animal under the conditions used in this study. Additional toxicity evaluations and clinical studies are needed to confirm the safety of NPP use in clinical practice.

프라이머를 이용한 3.0% 과산화수소 미백 패치의 임상효과 (Clinical effect of 3.0% Hydrogen peroxide bleaching patch with primer)

  • Purpose The The purpose of this study was to clinically evaluate the efficacy and safety of a self-whitening patch containing a primer containing taurine and 3.0% hydrogen peroxide. Methods A double-blind randomized clinical trial was conducted on 55 subjects. The whitening patches containing 3.0% hydrogen peroxide were applied to the labial surfaces of maxillary six anterior teeth once daily for 30 minutes using a primer, and whitening efficacy was measured by △E* values before application and at 3, 5, 7, and 10 days after application. Stability was determined using the Gingival index (GI) and visual analogue scale (VAS). Results Changes in △E* values were clinically recognizable as early as day 5 after patch application, and whitening effects were visible by day 7. There was no statistically significant difference in gingival index (p=0.069). Conclusions Self-whitening patches using primer and 3.0% hydrogen peroxide applied once daily for 30 minutes showed effective whitening effect from the 5th day after application and could be used safely without significant side effects.

임상간호사의 안전간호활동 영향요인 (Factors Affecting on the Clinical Nurse's Safety Nursing Activity)

  • 본 연구는 대학병원 임상간호사를 대상으로 환자안전문화에 대한 인식과 안전간호활동에 미치는 영향을 파악하기 위한 서술적 조사연구이다. 자료는 2015년 9월 1일부터 10월 1까지 하였다. 수집된 자료는 SPSS 23.0을 이용하여 t-test, ANOVA, Pearson correlation coefficients, stepwise multiple regression으로 분석하였고, 사후분석은 Scheffe' test로 하였다. 그 결과 대상자의 환자안전문화에 대한 인식은 5점 만점에 $3.30{\pm}.22$점이었고, 안전간호활동은 5점 만점에 평균 $3.89{\pm}.44$점이었으며, 환자안전문화에 대한 인식은 환자안전간호활동과의 양의 상관관계(r=.364, p=.000)가 나타났다. 또한 안전간호활동에 간호사의 총 경력, 직위, 직속관리자/책임자, 의사소통 절차, 사건보고 빈도가 영향을 받으며, 38.6%의 설명력이 있는 것으로 나타났다. 따라서 안전간호활동을 증진시키기 위해서는 적절한 경력자 관리, 조직간 원활한 의사소통, 자발적 사건보고를 할 수 있는 실무지침과 효과적인 교육프로그램 개발이 필요하다.

가습기살균제 피해신청자들의 노출등급 분류 및 폐질환 발생 영향요인 분석 (Affecting Factors of Lung Disease and Classification of Exposure Rating of Applicants for Injuries from Humidifier Disinfectants)

  • Background: Lung injuries due to exposure to humidifier disinfectants (HDs) were reported in 2011 in South Korea. As a result of the government's epidemiological investigation and toxicity test study, it was found that HDs caused health damage such as lung disease. Objectives: The purpose of this study was to classify HD exposure ratings and analyze the affecting factors that could identify the relationship with lung disease. Methods: Exposure assessment for HDs was conducted using a questionnaire during face-to-face interviews with the applicants. Ratings of high exposure (Class 1) and low exposure (Class 2) were cross-tabulated with clinical ratings (acceptable and unacceptable). Logistic regression analysis was carried out by setting the clinical rating of lung disease as a dependent variable and the socio-demographic and exposure characteristics obtained through the questionnaire as independent variables. Results: The concentration in air of polyhexamethylene guanidine (PHMG) was 71.96±107.47 ㎍/m3, and the exposure concentration was 15.21±23.28 ㎍/m3 . The exposure rating was overestimated with 97.1% of affected subjects having high exposure using margin of exposure (MOE), but only 9.9% matching the clinical class. In the overestimated group, it could be explained by the fact that the exposure time was long and the subjects had already recovered from damage symptoms. As a result of logistic regression analysis, ten variables were found to be significant influencing factors. Conclusions: A new exposure rating could be calculated based on the MOE, and factors affecting lung disease could be estimated through comparative evaluation with the clinical rating.
