요추 척추관 협착증에 대한 침구치료의 국내 임상근거: 체계적 문헌고찰 (Acupuncture for Lumbar Spinal Stenosis in Korean Literature: a Systematic Review)

  • Objectives : This study aims to evaluate currently available clinical evidence for the use of acupuncture in patients with lumbar spinal stenosis in the context of Republic of Korea. Methods : Five Korean databases were searched for all clinical studies employing acupuncture for patients with lumbar spinal stenosis. The exceptions were case reports and series involving less than 10 patients from their inception to September 2011. Only studies reported in Korean databases were searched. Results : Five retrospective audits among 22 initially identified studies were eligible for analysis in this review. There was no randomized controlled trial for this topic. The number of patients varied between 15 and 119 in each audit, and all the patients analyzed were inpatients. In all the included studies, acupuncture was conducted in combination with other concomitant treatments, including moxibustion, cupping, herbal remedies, acupoint injection, Chuna, and various physical therapies. All the included audits reported symptomatic and/or functional improvements compared to baseline. None of the included audits indicated whether any adverse events were reported. Conclusions : Current evidence for the use of acupuncture in patients with lumbar spinal stenosis in the context of South Korea is based solely on five retrospective audits that are very likely prone to a high risk of bias. Thus, further prospective and methodologically sound clinical trials to overcome this huge gap between clinical practice and the available evidence for the use of acupuncture in patients with lumbar spinal stenosis are needed.

Protocol for Systematic Review of Controlled Trials of Korean and Chinese Herbal Treatments for Stroke

  • Background: Many stroke patients receive traditional medical care in Korea and China. Stroke patients comprise the largest proportion of inpatients in traditional Korean medicine and traditional Chinese medicine hospitals. The purpose of this study is to identify the types and effects of Chinese medicine widely used in China and Korea, and to apply them to future studies of stroke. Methods: Nine Korean and Chinese databases will be surveyed for clinical studies of herbal medicines for stroke, published between the inception of the database and August 2017. Clinical studies of decoctions or modified decoctions will be included without restriction on study type. The frequencies and patterns of formulas or single herb usage and the any type of herbal medicine utilization will be analyzed. Dissemination: The results of the systematic review will be published in a peer-reviewed journal and disseminated electronically and in print. Updates of the review will be conducted to inform and guide healthcare practices.

임상연구에서 나타난 고혈압 뜸치료법에 대한 고찰 (Review of Moxibustion Treatment for Hypertension in Clinical Trials)

  • Objectives : The aim of this study is to review clinical trials using moxibustion on hypertension and to assess their methodology and results. Methods : Electronic literature searches for clinical trials (randomized trial, non-randomized trial, before-after study) of moxibustion were performed in nine electronic databases (four international databases and five Korean databases) and handsearch. English, Korean or Chinese articles were included. Laboratory or animal studies were excluded. Results : A total of twelve studies met the inclusion criteria. Seven randomized controlled trial, three non-randomized trials and two before-after studies were included. Seven studies used direct moxibustion(two are scarring one) and five used indirect one. Five of twelve studies used moxibustion treatment just one time, except for that most frequency was once daily or 2-3 times per week for 1-2 months. Of ten randomized or non-randomized studies, three used antihypertension drug control and another three used waiting list control. Compare to baseline, change of blood pressure after moxibustion treatment was significant in all studies. However, the results of effect in blood pressure between moxibustion and controlled were not consistent. Methodological quality of clinical trials included in this review was low and has risk of bias, especially in blinding of parcitipant. Conclusions : There are little high-quality clinical trials of moxibustion for hypertension. To evaluate the effect of moxibustion, more rigorous trials are warranted. Also, methodology of clinical trials have to be descripted in detail.

소아 야제에 대한 추나요법 효과; 체계적 문헌고찰 (Chuna Manual Therapy for Nocturnal Crying; A Systematic Review)

  • Objectives To evaluate the evidence supporting the effectiveness of Chuna manual therapy for nocturnal crying. Methods We conducted search across the 3 electronic databases (Pubmed, CAJ and Oasis) to find all of randomized controlled clinical trials(RCTs) that used Chuna manual therapy as a treatment for nocturnal crying. The methodological quality of each RCTs was assessed using the Cochrane Risk of Bias tool and nRCTs was assessed using the Cochrane Risk of Bias Assessment tool for Non-randomized Study tool. Results Two RCTs and three nRCTs met our inclusion criteria. The meta-analysis showed positive results for the use of Chuna manual therapy in terms of the efficacy rate when compared to medication treatments alone. Positive results were also obtained, in terms of the efficacy rate, when comparing Chuna manual therapy combined with acupoint application therapy to Chuna manual therapy alone, but was not statistically significant. Conclusions Our systematic review found encouraging but limited evidence of Chuna manual therapy for nocturnal crying. We recommend clinical trials which compare the effectiveness of Chuna manual therapy with usual care to obtain stronger evidence without the demerits of trial design.

국내 작업치료 임상실습 연구에 관한 체계적 고찰 (A Systematic Review of Domestic Research on Clinical Fieldwork in Occupational Therapy)

  • 본 연구는 국내 임상실습 관련 연구에 관한 체계적 고찰을 실시함으로써 연구동향을 분석하고, 향후 임상실습교육 발전방안을 모색하기 위해 필요한 기초자료를 제시하고자 한다. 2000년부터 2019년까지 RISS, KISS, Google Scholar, 국립중앙도서관 데이터베이스에 등록된 논문을 검색하였다. 주요 검색용어는 "Occupational Therapy" AND "Clinical Fieldwork"로 하였으며, 최종적으로 총 8편의 연구가 선정되었다. 선정된 연구의 질적 수준을 분석한 결과, 분석대상 연구 8편(100.0%) 모두가 Level IV(조사연구)에 해당하여 질적 수준이 낮은 편이었다. 또한 연구동향을 분석한 결과 자료수집 방법은 설문조사가 6편(75.0%)으로 가장 많았으며 작업치료 전공 학생을 연구대상으로 한 연구가 6편(75.0%)으로 가장 많았다. 주요 주제는 임상실습만족도가 4편(50.0%)으로 가장 많았으며, 임상실습교육체계와 임상실습에 따른 변화가 각각 2편(25.0%)으로 나타났다. 앞으로 국내 작업치료 임상실습 관련 연구의 양적, 질적 성장이 필요함을 확인할 수 있었고, 이를 통해 향후 실제적이고 체계적인 임상실습교육 발전방안을 확립하는데 기여 할 수 있을 것으로 기대한다.

치위생(학)과 학생들의 임상실습 스트레스에 관한 체계적 문헌고찰 (A Systematic Review on the stress of Clinical Dental Practice for Dental Hygiene Students)

  • 본 연구는 치위생(학)과 대학생의 임상실습 스트레스에 대한 선행연구의 결과를 체계적으로 수집, 분석하여 임상실습 스트레스의 측정도구와 관련요인에 대한 분석을 실시하였다. PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analysis)의 체계적 문헌고찰 가이드라인에 의거하여 수행되었으며 최종분석에 사용된 문헌은 24편이었다. 임상실습 스트레스를 측정하기 위하여 사용된 측정도구는 4개였다. 문헌 분석결과 만족도가 낮을수록 임상실습 스트레스가 높은 것으로 나타났고, 대인관계, 역할 및 활동, 이상과 가치의 요인이 스트레스에 유의한 영향을 미치는 것으로 나타났다. 본 연구에서는 임상실습 스트레스와 다양한 변수들 간의 상관관계 및 영향요인에 국한되어 분석하였기에 향후 본 연구 결과를 기초로 하여 치위생(학)과 대학생의 임상실습 스트레스에 관하여 다면적인 연구들이 활발하게 진행되길 기대한다.

도라지(Platycodon grandiflorum)의 임상적 효과에 대한 체계적 문헌 고찰 (The Clinical Effects of Platycodon grandiflorum: A Systematic Review)

  • Platycodon (Platycodon grandiflorum) is used as a food and medicinal ingredient worldwide, particularly in Northeast Asia. Although a large number of studies have been conducted to determine its medicinal efficacy, there is still a paucity of literature summarizing the clinical activity of Platycodon. In this systematic review, we aimed to summarize the clinical efficacy of Platycodon. All literature describing the clinical efficacy of Platycodon was collected from international databases, and relevant papers were carefully evaluated for eligibility. Data were extracted from the selected papers and quality evaluation was conducted in accordance with the standards provided by the National Institutes of Health (NIH), with all procedures being conducted in accordance with PRISMA 2009 checklist guidelines. After the removal of 342 papers, 644 studies were fully screened for their titles and abstracts, among which, 19 studies were finally selected for full summarization. Extraction of data indicated evidence of the effectiveness of Platycodon in the treatment of respiratory system-related diseases and cancer, and in the provision of immunity. However, we found that the quality and objectivity of evaluation, management of variable factors, sample size, and reproducibility were generally poor. Although Platycodon has shown potential antiviral, antibacterial, anticancer, and antidepressant effects, and promotion of immunity in clinical settings, it was not possible to integrate objective indicators to conduct a meta-analysis due to the lack of literature and insufficient studies. More investigations in the clinical setting should be conducted to confirm the functional effects of Platycodon.

COVID-19 소아 환자의 임상 양상 및 실험실적 특징: 체계적문헌고찰 및 메타분석 (Clinical and Laboratory Features of Pediatric Patients with COVID-19: Systematic Review and Meta-analysis)

  • Background: Although the identification of clinical and laboratory features in pediatric COVID-19 patients is essential in establishing an appropriate treatment plan, a systematic review and meta-analysis on the topic has yet to be reported. Methods: We searched MEDLINE, Embase, and Web of Science to access clinical and laboratory characteristics as well as clinical outcomes of children with COVID-19 infection. A meta-analysis using random-effect model was performed to estimate pooled prevalence and 95% confidence intervals. Results: Among the 532 studies initially collected, 12 articles were finally included in the meta-analysis. Among the investigated 320 pediatric patients with COVID-19, fever (48.2%) and cough (39.3%) were the most common symptoms. Almost one third of patients (30.4%) were asymptomatic. In laboratory findings, only 11.4% of pediatric patients experienced lymphocytopenia. Increased inflammatory markers including c-reactive protein (18.6%) and procalcitonin (32.4%) were observed. Only a few patients needed mechanical ventilation and intensive care support, and only one death was reported. Conclusion: Pediatric patients with COVID-19 infection exhibited milder symptoms and more favorable outcomes compared to adults. However, considering the high rate of asymptomatic pediatric patients, close monitoring is required to prevent community infection in asymptomatic conditions and hidden disease progression.

Diagnostic Accuracy of Clinical Test for Anterior Cruciate Ligament Injury: Systematic Review

  • Purpose: The aim of this study was to conduct a systematic review of randomized controlled studies from 2012 to present that explore the diagnostic accuracy of clinical tests used for diagnosing anterior cruciate ligament (ACL) injury. Methods: Study design: Systematic review. Literature search of the PubMed and Scholar databases was conducted using keywords related to diagnostic accuracy of clinical tests for ACL injury. The PRISMA Guidelines were followed to conduct this study. The Cochrane Risk of Bias Tool was utilized to assess the quality of each included study. Results: As a result, 8 studies were included, and 6 clinical tests used in ACL tears were evaluated for diagnostic accuracy. The pivot shift test was reported as having the highest +LR (29.5) value with a sensitivity of 59% and a specificity of 98%. However, the test with the lowest -LR value was the lever test, and the values were as follows: -LR (0.08), +LR (4.7), specificity (80%), sensitivity (94%). Conclusion: In this study, it was concluded that a single clinical test is not sufficient to determine the presence of ACL injury. Test combinations have a higher diagnostic accuracy than a single test. In this study, the accuracy of the clinical tests was examined without considering the amount of ACL rupture and acute-chronic condition. Further research is required to examine the impact of these two factors on diagnostic accuracy of clinical test.

암 환자의 임상시험 참여 의향과 연관된 요인: 탐색형 서술적 문헌고찰 (Factors associated with willingness to participate in clinical trials for cancer patients: an exploratory narrative review)

  • Objectives : This study aimed to investigate both cancer patients' willingness to participate (WTP) in clinical trials and its associated factors. Methods : We searched MEDLINE (PubMed) for studies that had investigated cancer patients' WTP in clinical trials. Search terms included 'willingness to participate', 'cancer' and 'trial'. Studies published within the last five years up to July 26, 2016 were potentially eligible. Results : Of 44 initial hits, 14 studies (12 quantitative and 2 qualitative) were included. Two out of the 14 studies investigated WTP in an acupuncture trial. Study designs, such as WTP measurement methods, investigation settings, and type of cancer, varied. The proportion of participants with a WTP was widely distributed between the studies (median 48.6 %, range 11.1 % to 88.3 %). Factors associated with WTP included various socio-demographic factors, clinical factors, participants' knowledge/attitudes toward clinical trials, travel distance, information from physicians, and trust toward physicians. Conclusion : WTP and its associated factors were variable across the reviewed studies, which might be due to different research methods and contexts. Whether or not common factors associated with cancer patients' WTP in clinical trials exist remains unclear and thus warrants further research.
