한의학 교육에서 진료수행평가에 대한 학생 만족도 및 환자 - 의사관계 점수의 채점자간 비교 연구 (Student Satisfaction Study and Interrater Comparative Study on Patient-Physician Interaction Score of Clinical Performance Examination in Korean Medical Education)

  • As for medical education, not only acquirement of knowledge but also practical clinical competence is important because it is needed on primary medicine. Under this trend, Clinical Performance Examination(CPX) is already being practiced in medical college. But in spite of its importance, CPX is not yet practiced in Korean medical college. So, by contemplating necessity and outcome of CPX, We try to offer basic data for future imposition and improvement of CPX in Korean medical education. 49 students of grade 3 in a Daejeon Korean Medical College were targets of investigation. After educated about CPX in advance, They treated simulated patient and answered questionnaire about CPX. Then, their treatment was scored by Professor and simulated patients. Total 49 members responded to the survey and the results of the analysis were as follows. The answer that CPX is useful(Mean=4.12) and essential for acquiring professionalism of treatment(Mean=4.02) was got high scores. But many respondents replied about necessity of better CPX environment and felt difficulty about PPI(45%). Meanwhile, Professor group(Mean=9.24) tended to give more high score than Simulated patient group(Mean=7.94). This study can be very useful for composing basic data of CPX in Korean medical College and improving practical clinical competence of students. But this study has also some limits like area, respondent selection, group module etc. So, more detailed and comprehensive survey is needed.   • Obesity is one of the most serious problems in human life which may cause various diseases, i.e., cancer, hypertension and diabetes. The biochemical and clinical researches on this issue have been widely studied worldwide, and the mechanism and improvement of obesity have been gradually clarified. In this workshop, three distinguished speakers from clinical, biochemical and pharmaceutical development fields have been invited. Professor Yasushi Saito is a high prestigious worldwide in terms of the study of obesity.

간호학과 교수의 직무 스트레스와 교수 효능감이 조직 몰입도에 미치는 영향 (Effects of Job Stress and Teaching Efficacy on Organizational Commitment of Nursing Professors)

  • Purpose: This study aimed to identify how job stress and teaching efficacy impacted organizational commitment. Methods: Data were collected from 158 nursing professors via an online survey, from Jan to June 2019. Results: The study found that four factors affected the organizational commitment of nursing professors: i) type of nursing institution in which they are currently employed (β=-.16, p=.030), ii) position as an assistant professor (β=-.37, p=.012) and an associate professor (β=-.44, p=.002), iii) salary in the 50-59 million won range (β=.20, p=.024), and above 60 million won (β=.41, p<.001), and iv) professor's teaching efficacy (β=-.18, p<.016). Conclusion: To increase the organizational commitment of four-year university professors, job characteristics should be considered. In the case of lower positions and salaries, additional compensation and programs that increase school affiliation should be introduced. Teaching methods training, lecture evaluation monitoring programs, and clinical training may also improve teaching efficacy.

Effects of early clinical and basic laboratory exposure program on premedical students: a questionnaire survey

  • Background: Because premed students do not take courses related to medicine during their first 2 years, they cannot establish their identity as students at medical schools, making it difficult for them to set goals as future doctors. We conducted an early clinical and basic laboratory exposure program for premed students and studied the effects of the program and student satisfaction levels. Methods: We performed an early clinical and basic laboratory exposure program for premed students for 2 days and evaluated the effects of the program and student satisfaction with it. The program consisted of two types: type 1, where two to four students formed a group, which was assigned to a particular department to participate and make observations during ward rounds, outpatient clinics, examinations, procedures, and surgeries (in the case of basic laboratory work, the students partook in experimental observations); and type 2, where one student followed a medical school professor to observe the professor's day. After the program ended, an online survey was conducted to investigate the effects on students, their thoughts, and satisfaction levels. Results: In total, 114 students (91.2%) responded to the survey. Approximately 94% of them were satisfied with the program. They found that the program would be useful for deciding on future career paths, gaining knowledge about a department of interest, studying for a medical program after premedical studies, and befriending residents and professors in certain departments. Conclusion: Early clinical and basic laboratory exposure programs are recommended for premedical students.

간호학생의 학업성취도와 임상수행능력 및 임상수행자신감의 관련성 (The relationship among academic achievement, clinical competence, and confidence in clinical performance of nursing students)

  • Purpose: The purpose of this study was to determine relationships between academic achievement, clinical competence, and confidence in the clinical performance of nursing students, and to identify factors that influence them. Methods: This was a descriptive correlation study. Data were collected from 118 nursing students at a nursing college in Seoul. One-way variance analysis, Pearson's correlation coefficient, and multiple linear regression analysis were used to examine relationships between academic achievement, clinical competence, and confidence in clinical performance. Results: Clinical competence was related to academic achievement in students' previous semester and to students' academic scores in a fundamental of nursing course. Clinical competence showed a weak positive correlation with academic achievement in students' previous semester and academic scores in a fundamental of nursing course in both evaluations by the professor and students' self-evaluations. However, confidence in clinical performance had no significant correlation with academic achievement. The factor affecting the clinical competence was academic scores in a fundamental of nursing course, and factors affecting confidence in clinical performance were health status, personality, major suitability, and class satisfaction in a fundamental of nursing skills course. Conclusion: These findings indicate that students with high academic achievement have better clinical performance, but confidence in clinical performance is not related to academic achievement.

Effect on Preference of Clinical Practice Subjects

  • This study was a cross-sectional descriptive survey study that confirms the effect on subjects that prefer clinical practice in order to prepare basic data for efficient clinical practice guidance for nursing college students. The study participants were 201 students attending C University, and the data collection period was from October 1 to October 15, 2022. The collected data were analyzed using SPSS 18.0 as descriptive statistics, Pearson correlation, Chi square test, ANOVA test, and Multiple regression test. As a result of the analysis, it was found that clinical decision-making and critical thinking were correlated under the statistical significance level (r=.730, p<0.01). The most favorite clinical practice department was community nursing, and male students preferred community nursing the most (Male=45.6%, χ2=.000), female students were found to prefer similar levels of practical subjects with child nursing , adult nursing, and maternal nursing(χ2=000).Clinical decision-making was found to be higher in students who preferred community nursing at a statistical significance level than those who preferred child nursing (F=4.91, p<0.01). Critical thinking was higher among students who preferred adult nursing than those who preferred other subjects (F=4.65, p<0.01). Through the analysis results, it was found that general characteristics vary, but clinical decision-making ability and critical thinking affect the preference of clinical practice subjects. Therefore, based on the results of this study, the professor of clinical practice suggests the development of a program to foster clinical decision-making and critical thinking to make students interested in clinical practice subjects.

  • The authors congratulate the cerebrations for the 30 years of the Korean Society of Medical Physics (http://www.ksmp.or.kr/). The paper is published to recognize the anniversary. Geon-Ho Jahng invited Professor Z. H. Cho to join to submit this manuscript because he has been one of the leaders in the field of magnetic resonance imaging (MRI) during the last 40 years. In this review, we describe the development and clinical histories of MRI internationally and domestically. We also discuss diffusion and perfusion MRI, molecular imaging using MRI and MR spectroscopy (MRS), and the hybrid systems, such as positron emission tomography-MRI (PET-MRI), MR-guided focused ultrasound surgery (MRgFUS), and MRI-guided linear accelerators (MRI-LINACs). In each part, we discuss the historical evolution of the developments, technical developments, and clinical applications.

간호학교수 임상연수프로그램 효과 분석 (The Effect of Clinical Training Program for Nursing Faculty)

  • It is important for nursing faculties to try to improve their professional knowledge and skill in nursing education. Thus it is essential for them to participate in clinical training program. The purpose of this study was to identify the effect and satisfaction of faculty clinical training programs which were managed by Korean Academic Society of Nursing Education and 4 Hospitals(Samsung Medical Center, Seoul National University Hospital, Seoul, Asan Medical Center, Yeonsei Sebrance Medical Center) in Seoul. The data collection was done from July, 1999 to December, 1999 and the 125 subjects were used for the analysis. The purpose of participation of professor to clinical training course was found that to observe of up-to-date treatment method($3.57{\pm}.60$) and to connect between knowledge and practice ($3.63{\pm}.50$). The professors were highly satisfied with 'can see the high-tech institution and facilities($3.45{\pm}.62$)' and 'can confirm in advanced nursing field and practice ($3.38{\pm}.58$)' in clinical training program. And it was found that nursing professors applied the experience of participation in clinical training program to lecture and clinial practice for students. But they were dissatisfied with 'they had no or not enough time to practice'. In conclusion, the clinical training program was useful for nursing professor to educate and instruct the students. It is suggested that the more advanced course and practical program have to be developed for nursing faculties.

Estimation of Maximal Tolerated Dose in Sequential Phase I Clinical Trials

  • The principal aim of a sequential phase I clinical trial in which the toxicity reponses of a group of patient(s) determine the dose level of the next patient(s) group is to estimate the maximal tolerated dose(MTD) of a new drug, In this paper we compared with a simulation study the performance of the MTD estimates that are determined by a stopping rule in a design and also those that are determined by analyzing the data after a clinical trial is terminated. To the latter belong the mean median mode and maximum likelihood estimates. For the Standard Methods the stopping rule MTD is quite inefficient but the median MTD has a best efficiency and is robust with respect to the three different toxicity curves. The problem of non-convergence of MLE MTD is severe. A more improved MTD estimate is produced by combining the advantages of the various MTD estimates and its efficiency is better than the single median MTD estimate especially for the toxicity curve of an unlucky choice of dose levels. The simulation results suggest that simple types of phase I designs can be combined with relatively standard analytic techniques to provide a more efficient MTD estimate.

전문대학 간호과의 교과과정 및 교육여건에 관한 조사 연구 (A Study for the Nursing Curriculum and educational environment in 3-year college)

  • The role of nurses in Korea is undergoing significant change because of the economic development, medical insurance and the change of family structure. It will be predicted that the nurse's role is extended more and more in the 21C. So the nursing curriculum in nursing education is the most important thing to prepare a good quality of nursing. Furthermore qualified curriculum calls for the development of desirable attitudes in nursing students. The purpose of this descriptive study was to collect data which can be used to facilitate the process of encouraging every college to assume greater responsibility for needed curriculum and educational conditions related to preperation of nursing students for various developed society toward 21C. A total sixty one nursing college were surveyed by mail during August and October 1997. Thirty six college(59%) responded. The data was analyzed by using SAS program. The results are as follows : 1) 32 college(88.9%) answered that they have Nursing educational philosophy and goal. 2) Total credits are 136credits, and the students who don't have teacher's course need 128 credits. 3) Nursing essential subjects comprise 84 credits, and nursing clinical practice subjects are 19 credits. 4) 15 colleges(41.7%) have their own hospital for student's clinical practice, 36 college(100%) take the community nursing practice in public health center, 34college(94.4%) take the school health nursing practice. 5 college(13.9%) have industrial nursing practice. 5) 3-year nursing college have 6.4 professor, 3.5 assistent professor, 1.9 and 3.4 full faculty and 7.4 part time faculty. We sugest that 3-year college have to have nursing educational philosophy and goal in terms of nursing educational standard. To improve the quality of nursing education every college should try to increase the number of professor and to contact qualified hospital for students clinical practice.
