뇌졸중 환자의 균형, 인지, 일상생활 평가의 상관성 연구 (The Study of Correlation Between the Balance, Cognition and Activity of Daily Living in Stroke Patients)

  • 목적: 본 연구는 뇌졸중 환자를 대상으로 BBS, MoCA-K, MBI와의 상관관계를 제시하고 각 변수들과의 영향력을 분석하여 뇌졸중 환자의 기능적 수행능력 평가에 기초연구가 되고자 한다. 방법: 연구 대상자는 고양시에 소재하는 Y재활병원에 입원하여 치료받고 있는 뇌졸중 환자 34명을 대상으로 2017년 12월부터 2018년 3월까지 실시하였으며 선정기준은 첫째, 뇌졸중으로 진단받고 6개월 이상 경과한 자, 둘째, 의사소통이 가능하며 MMSE-K 20점 이상인 자, 셋째, 편측무시가 없는 자, 넷째, 하위운동신경 병변이 없고 양하지 및 척추에 정형외과적 질환이 없는 자, 다섯째, 시청각계 이상이 없고 운동기능에 영향을 주는 약물 또는 수술을 받지 않은 자, 여섯째, 환자본인이 연구 참여에 동의한 자로 하였다. 평가는 재원 중 물리치료사와 작업치료사에 의해 BBS, MoCA-K, MBI를 측정하였으며 균형능력 평가 시 안전을 위하여 한명의 보조자가 동반하여 측정하였다. 결과: BBS와 MoCA-K r=.459, BBS와 MBI r=.550, MoCA-K와 MBI r=.565로 비교적 높은 상관관계가 있었다(p<.01). 결론: 본 연구는 임상적인 평가도구로서 BBS, MoCA-K, MBI의 적극적인 활용과 그 유용성에 대한 근거를 제시하였다는데 의미가 있다.

  • Background: Most developmental screening tools in Korea are adopted from foreign tests. To ensure efficient screening of infants and children in Korea, a nationwide screening tool with high reliability and validity is needed. Purpose: This study aimed to independently develop, standardize, and validate the Korean Developmental Screening Test for Infants and Children (K-DST) for screening infants and children for neurodevelopmental disorders in Korea. Methods: The standardization and validation conducted in 2012-2014 of 3,284 subjects (4-71 months of age) resulted in the first edition of the K-DST. The restandardization and revalidation performed in 2015-2016 of 3.06 million attendees of the National Health Screening Program for Infants and Children resulted in the revised K-DST. We analyzed inter-item consistency and test-retest reliability for the reliability analysis. Regarding the validation of K-DST, we examined the construct validity, sensitivity and specificity, receiver operating characteristic curve analysis, and a criterion-related validity analysis. Results: We ultimately selected 8 questions in 6 developmental domains. For most age groups and each domain, internal consistency was 0.73-0.93 and test-retest reliability was 0.77-0.88. The revised K-DST had high discriminatory ability with a sensitivity of 0.833 and specificity of 0.979. The test supported construct validity by distinguishing between normal and neurodevelopmentally delayed groups. The language and cognition domain of the revised K-DST was highly correlated with the K-Bayley Scales of Infant Development-II's Mental Age Quotient (r=0.766, 0.739), while the gross and fine motor domains were highly correlated with Motor Age Quotient (r=0.695, 0.668), respectively. The Verbal Intelligence Quotient of Korean Wechsler Preschool and Primary Scales of Intelligence was highly correlated with the K-DST cognition and language domains (r=0.701, 0.770), as was the performance intelligence quotient with the fine motor domain (r=0.700). Conclusion: The K-DST is reliable and valid, suggesting its good potential as an effective screening tool for infants and children with neurodevelopmental disorders in Korea.

건강사정실습 수업에서의 플립러닝이 간호대학생의 학업적 자기효능감, 성취목표, 인지적 관여에 미치는 영향 (The Effect of Flipped Learning in Health and Physical Assessment Practice Classes on Academic Self-efficacy, Achievement Goal, Cognitive Engagement of Nursing Students)

  • 본 연구는 플립러닝 교수-학습법이 간호대학생의 학업적 자기효능감, 성취목표, 인지적 관여에 미치는 효과를 확인해보기 위해 시도된 단일군 전·후 실험설계의 유사실험 연구이다. 2학년 학생을 대상으로 건강사정실습 수업 5차시, 10시간 동안 플립러닝을 적용하였으며 연구에 동의하고 전·후 설문에 응답한 91명의 자료를 분석하였다. 자료수집은 2022년 9월 26일부터 11월 25일까지 이루어졌으며 자료분석은 SPSS WIN 24.0을 이용하여 기술통계와 paired t-test를 실시하였다. 연구결과 학업적 자기효능감의 하위영역인 학습 자기효능감은 사전 21.22±5.09에서 사후 25.27±5.02로(t=-5.445, p<.001), 인지적 관여는 사전 32.44±9.04에서 사후 35.12±8.29로(t=-2.120, p=.035) 상승하여 통계적으로 유의하였다. 따라서 플립러닝 교수-학습법은 학생들의 학습 자기효능감과 인지적 관여에 효과가 있었다. 그러나 일반화 및 연구설계의 제한점이 있어 반복 연구가 필요하며 간호대학생의 학습 능력과 임상 수행 능력을 향상시키기 위한 다양한 교수-학습 전략을 활용할 수 있는 방안들에 대한 탐색이 필요하다.

고혈압 청소년에서 tissue Doppler imaging과 strain rate imaging을 이용한 좌심실 기능 이상에 대한 연구 (Left ventricular dysfunction measured by tissue Doppler imaging and strain rate imaging in hypertensive adolescents)

  • 목 적 : 고혈압에서 이완기 기능 이상은 수축기 기능 장애가 나타나기 전에 질환 경과에 일찍 발견되고 흔하게 관찰된다. 이완기 심기능을 평가하기에는 TDI가 고식적 심초음파보다 더 우수한 것으로 알려져 있다. 그러나 소아 및 청소년에서의 연구는 많지 않은 실정이다. 청소년 고혈압에서 심실 기능 이상을 발견하는데 있어서 SRI의 유용성을 알아보고자 본 연구를 실시하였다. 방 법: 수축기 혈압이 140 mmHg 이상이거나 이완기 혈압이 90 mmHg이상인 고혈압을 가진 16-17세 사이의 38명의 청소년을 대상군으로 하였고, 같은 연령의 정상 혈압을 가진 청소년 19명을 대조군으로 하였다. M-mode 심초음파로 심실 중격 두께, 좌심실 후벽 두께를 측정하였고, 이면성 심초음파로 ejection fraction (EF), myocardial performance index (MPI)를 측정하였다. TDI를 이용하여 심근 수축기 심근속도, E 심근속도, A 심근속도를 측정하였고, SRI를 이용하여 strain과 strain rate을 측정하였다. 결 과: M-mode 심초음파 소견상 심실벽 두께, 좌심실 후벽 두께가 고혈압군에서 유의하게 증가하였다. EF, MPI와 modified MPI는 두군간에 유의한 차이가 없었다. 고식적 심초음파 검사결과 고혈압군에서 A 혈류속도가 유의하게 증가되었고, TDI 검사상 A 심근 속도는 고혈압군에서 유의하게 증가하였고 E/A 심근속도비는 유의하게 감소되었다. SRI에 의한 E strain rate은 고혈압군에서 기저, 중간 부위, 심첨부에서 유의하게 감소되었고, strain은 중격에서 고혈압군은 $19.15{\pm}8.65%$, 정상군은 $22.63{\pm}5.55%$으로 고혈압군에서 유의하게 감소하였다(P<0.05). 결 론 : 고식적 초음파로는 좌심실 이완기 기능 이상만 관찰되었으나 SRI로 좌심실 이완 기능 외에 수축 기능이 유의하게 감소됨을 알 수 있었다. SRI를 이용하여 심실 기능의 이상을 조기에 발견할 수 있었다. 앞으로 더 많은 고혈압 청소년을 대상으로 한 연구가 필요할 것으로 생각한다.

관절염 환자의 운동행위 예측모형 (Pender의 재개정된 건강증진 모형에 의한) (Prediction Model of Exercise Behaviors in Patients with Arthritis (by Pender's revised Health Promotion Model))

  • The aims of this study were to understand and to predict the determinent factors affecting the exercise behaviors and physical fitness by testing the Pender's revised health promotion model, and to help the patients with rheumatoid arthritis and osteoarthritis perform the continous exercise program, and to help them maximize the physical effect such as muscle strength, endurance, and functional status and mental effects including self efficacy and quality of life, and improve the physical and mental well being, and to provide a basis for the nursing intervention strategies. Of the selected variables in this study, the endogenous variables included the physical fitness, exercise score, exercise participation, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue) and the exogenous variables included personal sociocultural factor(education level), personal biologic factor(body mass index), personal psychologic factor(perceived health status) and prior related behavior factors(previous participation in exercise, life-style). We analyzed the clinical records of 208 patients with rheumatoid arthritis and degenerative arthritis who visited the outpatient clinics at H university hospital in Seoul. Data were composed of self reported qustionnaire and good of fitness score which were obtained by padalling the ergometer of bicycle for 9 minutes. SPSS Win 8.0 and Window LISREL 8.12a were used for statistical analysis. Of 75 hypothetical paths that influence on physical fitness, exercise participation, exercise score, perceived benefits of action, perceived barriers of action to exercise, activity-related affect(depression) and perceived self-efficacy, interpersonal influences(family support), situational factors(duration of arthritis, fatigue), 40 were supported. The physical fitness was directly influenced by life-style, perceived health status, education level, family support, fatigue, which explained 12% of physical fitness. The exercise participation were directly influenced by life-style, education level, past exercise behavior, perceived benefits of action, perceived barriers of action, depression and duration of arthritis, which explained 47% of exercise participation. Exercise score were directly affected by perceived self efficacy. BMI, life-style, past exercise behavior, perceived benefits of action, family support, perceived health status. perceived barriers of action, and fatigue, which explained 70%. Perceived benefits of action was directly influenced by BMI, life-style, which explained 39%. Perceived barriers of action were directly influeced by past exercise behavior, perceived health status, which explained 7%. Perceived self efficacy were directly influeced by level of education, perceived health status, life-style, which explained 57%. Depression were directly influeced by past exercise behavior, BMI, life-style, which explained 27%. Family support were directly influeced by life-style, perceived health status, which explained 29%. Fatigue were directly influeced by BMI, life-style, perceived health status. which explained 41%. Duration of arthritis were directly influeced by life-style, past exercise behavior, BMI, which explained 6%. In conclusion, important variables for physical fitness were life-style, and variable affecting exercise participation were life-style. Perceived self-efficacy of exercise was a significant predictor of exercise score. BMI, Life-style, perceived benefits of action, family support, past exercise behavior showed direct effects on perceived self-efficacy. Therefore, disease related factor should be minimized for physical performance and well being in nursing intervention for patients with rheumatoid arthritis, and plans to promote and continue exercise should be seeked to reduce disability. In addition, Exercise program should be planned and performed by the exact evaluation of exercise according to the ability of the patients and the contents to improve the importance of exercise and self efficacy in self control program, dedicated educational program should be involved. This study suggest that the methods to reduce the disease related factors, the importance of daily life-style, recognition of benefit of exercise, and educational program to promote self efficacy should be considered in the exercise behavior promotion and nursing intervention for continous performance. The significance of this study is also thought to provide patients with chronic arthritis the specific data for maximal physical and mental well being through exercise, chronic therapeutic procedure, daily adaptation and confrontation in nursing intervention.

알코올의존 환자에서의 수면장애에 대한 아침 광치료의 임상시도 (Open Clinical Trial of Morning Light Therapy in Sleep Disturbance of Alcohol Dependent Patients)

  • 연구목적: 알코올의존 환자의 수면 장애에 대한 치료에 있어서 기존의 진정수면제 등의 약물은 교차 내성을 일으키고, 수면의 질을 악화시키기도 한다. 광치료는 일주기리듬장애, jet-lag, 순환제교대근무에서 나타나는 수면장애와 노화와 관련된 수면 장애의 치료에 효과적인 비약물적 치료중재이다. 따라서 저자들은 이미 여러 질환의 수면장애에 시도되고 있던 광치료의 회복기 알코올의존환자의 수면장애에 대한 치료적 효과를 알아보고자 하였다. 연구방법: 알코올 금단 증상을 보이지 않는 13명의 알코올의존 환자들을 대상으로 하여 2500Lux의 광상자로 하루에 오전 1시간 씩 3일에 걸쳐 광치료를 시행하였다. 총수면시간, 실제수면시간, 수면잠복기, 수면효율, 각성빈도 등을 수면기록지를 통해 평가하였으며, visual-analogue scale을 통해 주관적인 수면의 만족도를 평가하였다. 오전 8시, 오후 2시, 오후 8시 3차례에 걸쳐 연구대상자의 졸림 정도를 스탠포드졸림척도를 통해 평가하였으며, visual-analogue scale을 통해 불안, 우울 기분 및 피로감을 평가하였다. 수면상태를 반영하는 수행 능력의 평가를 위하여 기초단계와 연구 4일째에 기호잇기검사(trail making test)A, B 및 기호바꿔쓰기검사(digit symbol substitution test)를 시행하였다. 각 측정값은 반복측정분산분석법을 이용하여 비교하였으며 수행능력의 평가는 사전사후차이검증을 이용하였다. 결 과: 수면기록지로 평가된 수면잠복기, 수면효율은 광치료의 효과를 확인할 수 있었으며 기초 단계에 비해 연구 1일째부터 유의한 차이를 보였다. 수면에 대한 만족도도 광치료에 따라 유의하게 증가하였으며 연구 1일째부터 유의한 차이를 나타내었다. 오후 2시에 평가된 졸림은 광치료의 효과를 확인할 수 없었지만 오전 8시에는 광치료에 따라 졸림이 유의하게 감소하였고 오후 8시에는 유의하게 증가하였다. 기호잇기검사와 기호바꿔쓰기검사에서 환자들은 광치료 전후 수행 시간이 유의하게 감소하였다. Visual analogue scale에 의한 피로감의 평가에서 오후 2시와 오후 8시에 광치료에 따라 유의하게 피로감의 감소가 나타났으며 우울기분과 불안은 광치료의 효과를 확인할 수 없었다. 결 론: 본 연구에서는 금단 증상에서 회복된 후에도 지속적으로 수면장애를 호소하는 알코올의존 환자들에 있어 비록 제한적이지만 광치료의 효과를 확인할 수 있었고, 임상 실제에서 알코올중독 환자들의 수면장애에 단기간의 광치료가 효과적인 치료도구가 될 수 있음을 제시하고 있다. 추후 보다 객관적인 수면 평가 도구를 이용한 장기간의 대조군 연구가 필요할 것으로 생각된다.

단일 병원에서 소아 중환자의 예후인자 예측을 위한 PIM2 (pediatric index of mortality 2)와 PRIMS III (pediatric risk of mortality)의 유효성 평가 - 후향적 조사 - (Performance effectiveness of pediatric index of mortality 2 (PIM2) and pediatricrisk of mortality III (PRISM III) in pediatric patients with intensive care in single institution: Retrospective study)

  • 목 적: 저자들은 중환자실에 입원하는 환아들에 대한 소아사망률지표 2 (pediatric index of mortality 2, PIM2)와 소아사망위험도 III (pediatric risk of mortality III, PRISM III)의 유효성을 평가하고자 하였다. 방 법: 2003년 1월부터 2007년 12월까지 단일 기관 중환자실에 입실하여 치료받았던 환아의 의무기록을 후향적으로 조사하였다. 중환자실에 입실하여 2시간 이내에 사망하거나 절망적인 상태의 퇴원인 경우는 제외하였다. 환아들의 일반적인 특성에 대해서 Student's t-test와 ANOVA를, PIM2와 PRISM III 항목에 대해서 생존한 환아와 사망한 환아 사이에 상관분석을 시행하였다. 또한, 사망률 예측의 정도에 대한 정확성을 위해서 Hosmer-Lemeshow 적합도에 대한 다중회귀분석과 수용자 작업특성곡선을 사용하였으며 예측사망율의 과대 또는 과소 평가는 표준화된 사망비를 이용하여 검증하였다. 결 과: 193 증례의 의무기록을 검토하였으나 3예가 중환자실에 입실한 2시간이내에 사망하여 190예에 대하여 분석을 시행하였다. PIM2의 항목들은 수술이나 술기 후에 입원한 경우와 저위험군의 항목을 제외하고 생존과 연관성이 있었다. PRISM III에서는 심혈관/신경계 징후, 동맥혈가스분석의 항목이 관련성이 있었으나 생화학과 혈액학적 검사소견은 연관성이 유의하지 않았다. 수용자 작업특성곡선으로 확인한 예측도는 모두 의의가 있었으며 PIM2의 곡선하면적이 0.858 (95% 신뢰도: 0.779-0.938), PRISM III가 0.798 (95%신뢰도: 0.686-0.891)이었다. 또한, 표준화된 사망비는 두 가지 지표 모두 1에 가까웠으며 다중회귀분석을 이용한 Hosmer-Lemeshow 적합도에서 PRISM III가 ${\chi}^2(13)=12.899$, P=0.456이었으며, PIM2는 ${\chi}^2(13)=14.986$, P=0.308이었다. 그러나 PIM2가 가능도비검정에서 PRISM III보다 유의한 특성을 가지고 있었다(${\chi}^2(4)=55.3$, P<0.01). 결 론: 저자들은 중환자실에 입실하는 소아 환자에서 사망률을 예측하는 두 가지 지표(PIM2, PRISM III)가 의미가 있다는 사실을 확인하였다. 저자들은 PIM2가 PRISM III보다 보다 정확하고 적절하다고 생각된다.

간호학생의 효과적인 자율학습을 위한 조사연구 (A Study on the Effective Independent Study of Nursing Student)

  • This survey was made for a month starting from November 15 to December 15, 1977 covering 711 students taking the junior. (3rd-yea.) and the senior. (4th-year) at nine college of nursing in Seoul concerning their perception and Attitude toward the profession of nursing, motive for the necessity of learning, environment of study, attitude of study and particulars relevant with study performance, particulars of library, references and reading, assignments and particulars of the degree of confidence for the learning achievement. Through the survey of the above Particulars, the following results were obtained by classifying all subject matters and by analysing motive of the selection of their course, awarding or not awarding of scholarships. 1. General characteristics: it was revealed that 406 students (57.1%) were attending at the junior. while 305 students (42.9%) were taking the senior. Thus, the total number was 711 and their average age was 21.4 years. Their dwelling category was; 73.9 percent of them resided at their parent's home, 214 students (30.1%) were awarded with scholarships. The reason to be attracted by nursing science was the possibility of continuing social life after graduation (43.5%). 2. Their perception and attitude toward the profession of nursing: According to the perception of profession by the students of each grade, students of the 4th grade showed comparatively strong conception. Also, students of the 4th grade showed more positive attitude in the purchase and reading of magazines relative with the science of nursing, in the reading of Code for Nurses and in their interest in the activity of nursing field. For the necessity of mission of nurse, 97.7 percent of the entire number of students covered responded to necessity. For the necessity of the particular humanity and particularity in the character of nurses, 95.8 percent of those students responded to necessity. By the each grade, students of the 4th grade showed more response. 3. As to professional field desired after completing the professional course: 57 percent of those students desired for clinician nurse while 55 percents desired for community health-nurse. 4. As to the environment of study: they were mostly satisfied with their present residential environment. However, they complained of inconvenience at their lecture-halls. Students of the 3rd grade showed more complain. As to their attitude toward the adjustment of environment of study, they showed a affirmative response. As to the opinion of factors which interfere with their study, comparatively strong response was showed in their scepticism in the science of nursing, insufficient comprehension in general learning, relation with professors n4 discrepancy in the method of study. According to opinions of students at each grade, students of the 4th grade showed more scepticism. 5. Particulars relative with their attitude and performance of study : As to their knowledge of the objectives of their study of subject, the majority was to study with a partial knowledge of the objectives of their study. As to the plan of study, a low percentage indicated management of routine life under regular scheduling. Students of the 4th grade responded to rather planned life. As to time spent in independent study, response to concentrated study when necessary was stronger than that to regular daily study. Students of the 4th grade showed stronger response to regular study than that of the 3rd grade. As to the contents of their note-taking, 67.4 percents of those students responded to such regulatory procedure performing in the lecture-hall as they listen to lectures. 17.3 percents of those students showed response to adding supplementary informations from references to what was entered in choir note-taking at their lecture-halls. 6. Particulars of library, references and reading books: As to receiving of instruction for the utilization of library and time of receiving such instruction 64.7 percents of those students had received such instruction. 66.7 percents of the those responded received such instruction at orientation conducted for freshmen. As to the convenience of the utilization of library, 49.9 percents of those students responded to convenience. However, students of the 3rd grade showed a much stronger response to inconvenience. As to the time of the utilization of library,92.5 percents of those students showed a response to occasional utilization for particular purpose than regular utilization. 53.2 percents of those students responded to ordinary in quantity that library have references. 34.2 percents of those students responded to insufficient. As to the particular relative with the method and field of reading: 53.5 percents of those students responded to intensive reading and was the majority. As to the reading field, fiction u as the majority. When read any books for their major, they usually rend Korean text-b, oks. 7. Particular relative with giving assignment: All respondents were well aware of the objectives of giving home tasks. As to the attitude toward assignments and performing home tasks, 54.8 percents of those students to making ostentatious study because of an excessive quantity of assignments imposed. For performing assignment, they showed comparatively positive response. Also, 52.2 percents of those students responded that they usually submitted complected assignment with references. 8. As to motive to realize the necessity of study : 55.6 percents of those students responded that they realized such necessity in communication with patients when they were engaged in clinical practice. Also, 8.6, the lowest percents of those students responded that they realized such necessity in the course of conversation with nurses when they were engaged in clinical practice. 9. As to the determination of their confidence in the performance of study relative with clinical experience: They showed a general inclination of having in nursing. The major response was that they came to well comprehend the patients families. the lowest response was that they could apply what was learned at lecture-hall to practice. This response incidentally showed the distance the lecture-hall and practical study. In general items, students of the 4th grade showed more favorable response than students of the 3rd grade and there was a significant difference. 10. As to the perception and attitude toward profession according to the motive of selecting the nursing science : Those who selected the nursing voluntarily showed stronger conception than those who selected the nursing through indirect influence. However, there was no significant difference on this point. Only there was a remarkable difference in the reading of Code for Nurses. 11. Those who showed a stronger conception in the profession of nursing according to the motives of attractive nursing science indicated a strong will and ability to manage stable life and comparatively strong response was shown in the management of good home life because of the good adaptability of the science to their character. This group showed a strong conception of the profession than those who responded that they prefer this profession out of a longing for the work of a hospital and for the easy obtaining of opportunity to immigrate to over seas and for economic cause and for high school grade. There was significant difference between these two groups, 12. As to the conception and attitude toward the profession of nursing according to benefits by scholarships, those who were benefitted by scholarship showed stronger conception of profession than those who did not receive scholarship and there was a remarkable difference between these two categories. However, there was no remarkable difference between these two categories in the extent of interest of the activities of nursing fields and in the reading of Code for Nurses. 13. As activation for study according to the benefits of scholarships, those who were benefitted by scholarships showed stronger response to the motive for study comparing with those who receive. 14. As to tile field of reading according to the benefits by scholarships, those who received scholarships tended to read autobiographies and essayers to a considerable extent. Those who did not receive scholarships tended to read novels. Those who received scholarships more read nursing boots than those who did not receive scholarships. 15. As to the attitude of study and doing of assignment according to benefits of scholarships, those who received scholarships managed a favorable life with schedules for study, More students of receiving scholarship showed a regular study for more than one hour per day. Also, in the method of doing home tasks, more students of receiving scholarship showed reference to relative books frequently for the submission of completed assignments.

학동 전기 소아에서 폐활량 측정의 질관리와 성공률 (The quality control and acceptability of spirometry in preschool children)

  • 목 적:국내 학동 전기 소아를 대상으로 폐활량 측정법을 시행하여 기존에 제시된 여러 지표를 기준으로 검사 결과의 적정성을 평가함으로써 이 연령대에서의 폐기능 검사의 성공률을 알아보고자 하였다. 방 법:만성 기침이나 천식이 의심되어 내원한 2세 이상 6세 이하의 176명을 대상으로 폐활량 측정법을 시도하여 총 155 명에서 측정 결과를 얻을 수 있었다. ATS/ERS 에서 제시한 기준에 따라 후외삽용적(Vbe)이 80 mL 미만이고 FVC의 12.5% 미만일 때를 시작 기준에 적합하다고 정의하였다. 검사의 반복성을 확인하기 위해 선별한 두개의 유량-용적 곡선에서 FVC, FEV1의 차이를 구하여 FVC와 $FEV_1$의 절대적 차이값이 100 mL 또는 10% 미만인 경우를 적합하다고 정하였다. 검사의 종료에 대한 기준은 학동 전기 연령에서 수치적으로 정의되어 있지 않지만 유량-용적 곡선이 초기에 급격하게 증가해서 후기에 완만하게 감소하는 것을 합당하다고 보았다. 또한 유량-용적 곡선이 적절하게 나온 대상자(107명)를 선별하여 다른 문헌에 발표된 여러 정도 관리 기준에 맞추어 각 항목별로 성공률을 비교하였다. 결 과:전체 폐기능 검사의 성공률은 59% 이었고 연령이 증가할수록 폐활량 측정법의 성공률이 증가하였으며, 특히 3세부터 급격하게 성공률이 증가하였다(2세 14.3%, 3세 53.7%, 4세 65.1%, 5세 69.7%, 6세 70.8%). 폐활량 측정법의 실패 원인으로는 시작 기준에 맞지 않는 경우가 전체의 6.5%, 반복성 기준에 맞지 않는 경우가 12.3% 또 최대 호기량의 부족, 조기 종료 등으로 인해 검사 종료 기준에 맞지 않는 경우가 전체의 31% 로 가장 높았다. 폐활량 측정법 성공군와 실패군을 비교하였을 때 나이에서만 유의한 차이가 있었다(P<0.01). 정도 관리에 대한 몇몇 연구결과에 따라 각 항목별로 성공률을 보았을 때 연령이 증가할수록 성공률도 증가하였다. 결 론:학동 전기 소아에서도 약 60% 정도 폐활량 측정법을 성공적으로 수행하였다. 성공률은 연령에 따라 급격히 변화하므로 각 연령에 적합한 정도 관리 기준을 개발하여 이용한다면 검사의 성공률을 높일 수 있을 것으로 사료된다.
