The First Korean case of combined oxidative phosphorylation deficiency-17 diagnosed by clinical and molecular investigation

  • Combined oxidative phosphorylation deficiency-17 (COXPD-17) is very rare and is caused by homozygous or compound heterozygous mutations in the ELAC2 gene on chromosome 17p12. The ELAC2 gene functions as a mitochondrial tRNA processing gene, and only 4 different pathogenic mutations have been reported in ELAC2-associated mitochondrial dysfunction involving oxidative phosphorylation. Affected patients show various clinical symptoms and prognosis, depending on the genotype. We report a novel mutation in the ELAC2 gene (c.95C>G [p.Pro32Arg], het), in an infant with COXPD-17 who presented with encephalopathy including central apnea and intractable epilepsy, and growth and developmental retardation. During hospitalization, consistently elevated serum lactic acid levels were noted, indicative of mitochondrial dysfunction. The patient suddenly died of shock of unknown cause at 5 months of age. This is the first case report of COXPD-17 in Korea and was diagnosed based on clinical characteristics and genetic analysis.

  • This study was conducted to examine the prognosis of canine pyometra after surgical treatment in terms of their clinical indications, hematology and serum chemistry. A total of 20 bitches diagnosed as pyometra after physical examination was hospitalized at the Veterinary Medical Teaching Hospital, Seoul National University, during the period of 1998 to 1999. After examining all profiles, ovariohysterectomy was performed to treat pyometra. All bitches had clinical symptoms of general depression, polyuria/polydipsia, anorexia and abdominal distension. In hematologic profiles, 65% of bitches had higher level of leukocyte count compared with normal level, but RBC and PCV values maintained within the normal ranges in most pyometric bitches. In serum chemistry profiler, ALT and ALP values were increased over the normal range, and BUN and creatine values were higher than normal ranges only in 10(50%) and 3 bitches (15%), respectively. Survival rate after ovariohisterectomy was 90% (18/20), but bitches that had higher ALT, ALP, BUN, creatine and potassium than normal values were died after surgical treatment. The results of this clinical investigation show that the recovery of pyometric bitches after ovariohysterectomy can be prognosed by hematologic and serum profiles including ALT, ALP, BUN, creatinine and potassium.

Biomarkers for Evaluation of Prostate Cancer Prognosis

  • Prostate cancer, with a lifetime prevalence of one in six men, is the second cause of malignancy-related death and the most prevalent cancer in men in many countries. Nowadays, prostate cancer diagnosis is often based on the use of biomarkers, especially prostate-specific antigen (PSA) which can result in enhanced detection at earlier stage and decreasing in the number of metastatic patients. However, because of the low specificity of PSA, unnecessary biopsies and mistaken diagnoses frequently occur. Prostate cancer has various features so prognosis following diagnosis is greatly variable. There is a requirement for new prognostic biomarkers, particularly to differentiate between inactive and aggressive forms of disease, to improve clinical management of prostate cancer. Research continues into finding additional markers that may allow this goal to be attained. We here selected a group of candidate biomarkers including PSA, PSA velocity, percentage free PSA, $TGF{\beta}1$, AMACR, chromogranin A, IL-6, IGFBPs, PSCA, biomarkers related to cell cycle regulation, apoptosis, PTEN, androgen receptor, cellular adhesion and angiogenesis, and also prognostic biomarkers with Genomic tests for discussion. This provides an outline of biomarkers that are presently of prognostic interest in prostate cancer investigation.

성인간호학 실습지원 웹 콘텐츠의 학습효과 (The Effects of Web-Contents Learning for Adult-Care Practice)

  • Purpose: This study investigated the effects of Web-Contents Learning by nursing students on nursing knowledge, self-directed learning and clinical competence. Method: This study was a quasi-experimental, nonequivalent control group pre-post test. For the study, 146 nursing students were selected from three colleges. Seventy-six were experimental subjects and 70 were control subjects. The subjects in the experimental group studied Web-Contents along with four weeks of clinical practice. Data were collected by structured questionnaires and analyzed using the SPSS program. Result: The experimental group showed significant increase in their nursing knowledge (t=-3.866, p=.001). However, no significant differences were observed between the two groups in self-directed learning (t=-.759, p=.449) and clinical competence (F=.891, p=.347). Conclusion: From our investigation, the present study suggests some implication for learner-related variables. Elaboration of experimental design to validate the effects of Web-Contents Learning should be carried out.

Serum Antibody Ig G and Ig M Titers for Opisthorchis felineus Correlate with Eggs in Faeces - a Comprehensive Study in Chuvash Republic, Russia

  • The Cholangiocarcinoma is a. The risk of development of cholangiocarcinoma, generally a rare type of a liver tumor, increases during infection of Opisthorchiasis. For this reason the timely detection of Opisthorchiasis is important for Cholangiocarcinoma prevention. There are many studies which concern the detection of pathogenesis of Opisthorchis viverrini infection but a little known about Opisthorchis felineus. In this study we investigate a correlation of the eggs which are found in a faeces and are comparable with a serum Ig G and Ig M antibody level that were detected with ELISA test in a large group of patients. The result is showing positive correlation between evidence of the Opisthorchis felineus eggs that were found in a faeces and antibody Ig G and Ig M level in a serum. Moreover the combination of two methods can improve the Opisthorchiasis diagnostic: the serum antibody and faeces investigation of eggs.

Meta-analysis of Inline Filtration Effects on Post-infusion Phlebitis Caused by Particulate Contamination of Intravenous Administration

  • The particulate contamination of intravenously administered fluid has been of major concern. One of the most common complications associated with long term i.v. therapy is post-infusion phlebitis (PIP). We undertook a systematic review and meta-analysis of the effect of inline filters on PIP. An electronic search of Medline, KoreaMed, and KRIST was conducted to identify randomized controlled trials evaluating the effect of inline filters. Meta-analysis was undertaken using STATA 10. A total of 62 literatures were retrieved, of which 7 were included in meta-analysis. Inline filtration for intravenous infusion significantly reduced by 39% of the incidence of phlebitis, with a relative risk of 0.61 (95% CI 0.41-0.90, p=0.012). Therefore, inline filtration is a highly effective means of decreasing the incidence of infusion phlebitis and should be considered as a part of intravenous therapy.

Huge central intravascular papillary endothelial hyperplasia of the mandible: a case report and review of the literature

  • Masson's tumor or intravascular papillary endothelial hyperplasia is an inflammatory soft tissue lesion that rarely occurs in the maxillofacial region and skeletal system. Precise clinical and para-clinical investigation is necessary for the accurate diagnosis and correct treatment of this lesion. This paper presents a massive intravascular papillary endothelial hyperplasia lesion in the bony tissue of the mandible. Histopathology features, clinical appearance, and suitable management are discussed, with a complete review of the literature. The patient underwent composite resection of the lesion as well as reconstruction. No recurrence was observed during 6 years of follow-up. To the best of our knowledge, this is the fourth case of Masson's tumor in mandibular skeletal tissue, which has unique and distinctive features due to its size and location. A rare occurrence in skeletal tissue, complex clinical presentations, and complicated histopathologic findings present diagnostic challenges for treatment of this lesion.

Investigation of serum biomarkers for neuropathic pain in neuromyelitis optica spectrum disorder: a preliminary study

  • Background: We aimed to investigate candidates for serological biomarkers of neuropathic pain in individuals with neuromyelitis optica spectrum disorder (NMOSD). Methods: We analyzed 38 sera samples from 38 participants with NMOSD in National Cancer Center. Neuropathic pain was evaluated using the painDETECT questionnaire. Pain with neuropathic components (painDETECT score ≥ 13) was observed in 22 participants, among whom 17 had definite neuropathic pain (painDETECT score ≥ 19). The remaining 16 participants had non-neuropathic pain (painDETECT score < 13). Serum glial fibrillary acidic protein (GFAP) levels were assessed using a single-molecule array assay. Several cytokines, including tumor necrosis factor-alpha (TNF-α), interleukin (IL)-6, IL-10, and IL-17A, were measured by a multiplex bead-based immunoassay. Results: In comparison of NMOSD participants with neuropathic pain components (or definite neuropathic pain) and those with non-neuropathic pain, the absolute values of serum GFAP, TNF-α, IL-6, and IL-10 levels were higher in participants with neuropathic pain components (or definite neuropathic pain), but these findings did not reach statistical significance. Conclusions: Further larger-scale investigations to find reliable serological biomarkers for neuropathic pain in NMOSD are warranted.

  • Recently, acupuncture has demonstrated extraordinary clinical results in the treatment of several categories of health conditions worldwide. The mechanisms of action of acupuncture (including immune mechanisms) have been investigated by biomedical studies over the last few decades. The immune mechanisms of representative clinical conditions and their clinical effects were thoroughly assessed, with a comprehensive investigation into the mechanisms of action of acupuncture (including immune responses) in this study. Conditions such as stroke, migraine, depression, chronic fatigue syndrome, lower back pain, hypertension, irritable bowel syndrome, sepsis, and allergic diseases were meticulously examined. This in-depth analysis aims to ensure a foundational understanding of the immune mechanisms involved in acupuncture, thereby serving as an initial step toward integrating the impact of acupuncture on the immune system.

Immune Checkpoint Inhibitors in 10 Years: Contribution of Basic Research and Clinical Application in Cancer Immunotherapy

  • Targeting immune evasion via immune checkpoint pathways has changed the treatment paradigm in cancer. Since CTLA-4 antibody was first approved in 2011 for treatment of metastatic melanoma, eight immune checkpoint inhibitors (ICIs) centered on PD-1 pathway blockade are approved and currently administered to treat 18 different types of cancers. The first part of the review focuses on the history of CTLA-4 and PD-1 discovery and the preclinical experiments that demonstrated the possibility of anti-CTLA-4 and anti-PD-1 as anti-cancer therapeutics. The approval process of clinical trials and clinical utility of ICIs are described, specifically focusing on non-small cell lung cancer (NSCLC), in which immunotherapies are most actively applied. Additionally, this review covers the combination therapy and novel ICIs currently under investigation in NSCLC. Although ICIs are now key pivotal cancer therapy option in clinical settings, they show inconsistent therapeutic efficacy and limited responsiveness. Thus, newly proposed action mechanism to overcome the limitations of ICIs in a near future are also discussed.
