Effects of Self-Leadership, Clinical Competence and Job Satisfaction on Nurses' Job Involvement (간호사의 셀프리더십, 간호업무수행능력, 직무만족이 직무몰입에 미치는 영향)

  • Purpose: The purpose of this study was to provide basic data required to improve nursing resources management and nurse organizations. Methods: The data were collected using a questionnaire. Participants were 294 nurses who were working at 8 hospitals with more than 500 beds located in G and C cities, Korea. Data analysis was done using t-test, ANOVA, $Scheff{\acute{e}}$ test, Pearson correlation coefficient and stepwise multiple regression analysis with SPSS. Results: There were significant correlations between self-leadership, clinical competence, job satisfaction and job involvement. Factors affecting job involvement were job satisfaction, self-leadership, length of clinical career and length of career in current department. Job satisfaction was the most influential factor with an explanatory power of 41%. Conclusion: Findings show that to strengthen job involvement, identification and management of factors that affect job satisfaction and self-leadership are required and relevant training and strategies should be developed and used.

Comparison of MRI findings with clinical symptoms in temporomandibular joint internal derangement (측두하악관절 내장증 환자의 임상증상과 자기공명영상 소견의 비교 연구)

  • Purpose : To determine the clinical correlation of magnetic resonance imaging (MRI) findings of temporomandibular joint internal derangements. Materials and Methods : The MR images of 150 TMJs in 75 patients were analyzed. The clinical symptoms were pain in the preauricular area and masticatory muscles and TMJ sounds. Results : There was a statistically significant relationship between the MRI diagnoses of different types of disc displacements and clinical findings of pain, clicking, and crepitus. The risk of TMJ pain was increased when the disc displacement without reduction occurred at the same time in combination with the osteoarthrosis and effusion. Conclusion : Regardless of the results, the data indicate that each of these MR imaging variables may not be regarded as the unique and dominant factor in defining TMJ pain occurrence.

The Psychometric Properties of Distance-Digital Subjective Happiness Scale

  • This study intended to test the structure of the latent factor of a subjective happiness scale and the stability of invariance across groups of students' classifications (gender and students' status). In the large, non-clinical sample (619), students completed the subjective happiness scale. The (CFA) confirmatory factor analysis was used to investigate the factor-structure of the measure, and multiple-group confirmatory factor analysis (MGCFA) model was used to test the stability of invariance across groups of students classifications. The findings of the CFA indicated support for the original one-factor model. Additional analyses of the MGCFA method support the measurement (configural, metric and strong) invariant and practical invariant components of this model. There was an invariant across gender. There was partially invariant across groups of students' statuses. The scale exists in both groups to assess the same concepts of (single and married), excluding Items 3 and 4. Given that this study is the first investigation for the structure of the subjective happiness scale.

  • Micromanipulation procedures have been used to improve fertilization rates in patients with male factor or with unexplained infertility. Partial zona disseetion(PZD), a method using mechanical force to open the zona pellucida increase the chances of fertilization. The purpose of this study is to increase rates of fertilization and pregnacy in the ART program by using PZD. The influence of PZD on the fertilization rate was investigated in 57 couples with semen defects, antisperm antibodies(ASA), or unknown factors. PZD directly performed in 35 couples with a history of fertilization failure in previous cycle (Group 1), and PZD applied in 22 couples with the failure of initial fertilization in the same cycle (Group 2). The fertilization rates of the male facor, ASA positive factor and unknown factor in Group 1 were 37.6%, 20.0% and 59.2%, respectively. The rates of fertilization of male factor, ASA positive factor and unknown factor in Group 2 were 34.8%, 20.0% and 26.5%, respectively. The incidences of polyspermy in Group 1 and Group 2 were 5.9% and 9.0%, respectively. Among 35 patients of Group 1, one patient was pregnant and successfully delivered, whereas 1 of 22 patients of Group 2 became pregnant, but aborted at 7 weeks.

Experience of Patients with Cancer Participating in a Clinical Trial for the Development of a New Drug (암환자의 신약개발 임상시험 참여 경험)

  • Purpose: The purpose of this study was to describe the lived experience of the patients with cancer participating in a clinical trial for the development of a new drug. Method: This study was based on a phenomenological approach. The eight patients participating in a clinical trial were selected as the participations of this study. The data were obtained through in-depth interviews from the participants and were analyzed using the Giorgi method. Results: Essential experiences of the patients with cancer under a clinical trial consisted of anticipating recovery of physical health and a social role, passing the strict criteria of a clinical trial, diminishing economic burden, satisfaction with special treatment receiving, social contribution, concerns about side effects and withdrawal from the clinical trial, conflicts as a participant, pain, limited administration of other treatments, regret for giving up other treatments, strict compliance with instructions, prevention of side effects and maintaining desirable life-style. Integrated units of meaning of these components were hope, good luck, a sense of satisfaction, fear, distress, and the will of self-control. Conclusion: The most essential meaning of the cancer patients participating in a clinical trial was hope. Hope was found to be a primary factor reinforcing the will of self-management. The results of this study can be of great help to the research nurses to understand the lived experience of the patients with cancer and to plan an effective nursing intervention for the patients.

Professional Self-Concept, Critical Thinking Disposition and Clinical Competence in Nursing Students (간호대학생의 전문직 자아개념, 비판적 사고성향과 임상수행능력에 관한 연구)

  • Purpose: The purpose of this study was to identify the relationships among professional self-concept, critical thinking disposition and clinical competence in nursing students. Method: The participants in the study were 505 nursing students, 262 associate degree nursing students and 243 baccalaureate nursing students, all in schools in Gwangju or South Jeonla Province. A questionnaire survey was conducted from April 25 to May 27, 2011. Statistical processing was performed with SPSS 18.0 Version and the data were analyzed using t-test, ANOVA, Scheff$\acute{e}$ test, Pearson correlation coefficients, and multiple stepwise regression. Results: There was a positive correlation among clinical competence and professional self-concept, critical thinking disposition. The most effective factor for clinical competence was professional practice in professional self-concept, followed by prudence and objectivity in critical thinking disposition. The total explanation of clinical competence was 46.2%. Conclusion: The results of this study indicate confirm that professional self-concept and critical thinking disposition have an effect on clinical competence in nursing students. Accordingly in order to improve clinical competence, it is necessary to provide practice environment and educational curriculum that enhance the professional self-concept and critical thinking disposition of nursing students.

Characterization of the Serotyping and the Plasmid Profile of E. coli Isolated from Foods and Clinical Specimens

  • Characteristics of the food isolates and the clinical specimens isolates of E. coli harboring virulence factor and their correlations were analyzed. The predominant serogroup were 08 and 027 in the food isolates and 06 and 018 in the clinical isolates, respectively, showing the different patterns in serogrouping between them. In the test of antibiotic susceptibility, the food isolates were resistant to cephalothin, streptomycin, tetracycline and minocycline, and the clinical isolates were resistant to ampicillin, carbenicillin, streptomycin, cephalothin, trimethoprim/sulfamethoxazole, tetracyclino and minocycline, respectively. It shows that E.coli isolated from food sources and clinical specimens might be correlated. Plasmid profile in the food and clinical isolates showed wide diversity. Especially, large sized plasmid DNA such as 60 MDa, 90 MDa and 120 MDa were observed. The plasmid DNA (60 MDa) containing a gene encoding hemolysin was found in 43% of the food isolates and 35% of the clinical isolates. To study chromosomal homology, PFGE analysis was performed, showing different restriction patterns by Xbal. This result indicates that there were no genetic correlations between the foods and the clinical isolates.
