Long Non-coding RNA GAS5 Functions as a Tumor Suppressor in Renal Cell Carcinoma

  • Background: Renal cell carcinoma (RCC) is a malignancy with a poor prognosis. We aimed to explore whether the expression of Long Non-Coding RNA (LncRNA) growth arrest-specific transcript 5 (GAS5) is associated with RCC genesis. Methods: We selected twelve clinical samples diagnosed for renal clear cell carcinoma and found that the LncRNA GAS5 transcript levels were significantly reduced relative to those in adjacent unaffected normal renal tissues. Results: In addition, expression of GAS5 was lower in the RCC cell line A498 than that in normal renal cell line HK-2. Furthermore, using functional expression cloning, we found that overexpression of GAS5 in A498 cells inhibited cell proliferation, induced cell apoptosis and arrested cell cycling. At the same time, the migration and invasion potential of A498 cells were inhibited compared to control groups. Conclusion: Our study provided the first evidence that a decrease in GAS5 expression is associated with RCC genesis and progression and overexpression of GAS5 can act as a tumor suppressor for RCC, providing a potential attractive therapeutic approach for this malignancy.

Review on ICF-related Research Trends in Korean Clinical Field

  • PURPOSE: This study analyzed the research trends related to the International Classification of Functioning, Disability, and Health (ICF) to suggest an ICF utilization plan in the Korean clinical field. METHODS: In the RISS, KISS, NSDL, and PubMed databases, papers published between 2016 and July 2021 were collected by applying the search terms, 'ICF', 'international classification of functioning, disability and health', 'clinic', 'patient', and 'diagno'. The 44 papers selected were classified according to the analysis criteria, and the frequency and percentage were calculated. RESULTS: In domestic clinical trials, the frequency of ICF-related studies was in the order of physical therapy (n = 19) and occupational therapy (n = 14). The frequency of each study subject was observed in the order of studies related to a specific disease (n = 34) and prior studies (n = 7). The research topics were in the order of studies using ICF as a measurement tool (n = 21) and case studies with patients with specific diseases (n = 11). There were 18 studies using ICF codes and 14 papers applying the ICF domains. CONCLUSION: Over the last five years, ICF-related research in the domestic clinical field targeted patients with more diverse diseases in more expertise fields. Research for the unification of terms should be conducted for communication among various experts in clinical practice. To promote the use of ICF in the clinical field, it will be necessary to conduct research, such as the unification of terms, standardized education, development of ICF casebook, and development of ICF coding programs and guidelines.

Mutations in the tyrosine kinase domain of the EGFR gene are rare in the Korean Oral Squamous Cell Carcinoma

  • The epidermal growth factor receptor(EGFR) protein kinase signaling is an important pathway in cancer development and recently reported that EGFR and its kinase domain molecules are mutated in various of cancers including head and neck cancer. Functional deregulation of EGFR due to mutations in coding exons and copy number amplification is the most common event in cancers, especially among receptor tyrosine kinases(TK). We have analyzed Korean oral squamous cell carcinomas (OSCC) cell lines for mutations in EGFRTK. Exons encoding the hot-spot regions in the TK domain of EGFR (exons 17 to 23) were amplified by using polymerase chain reaction(PCR) and sequenced directly. EGFR expression was also analyzed in 8 OSCC cell lines using western blotting. Data analysis of the EGFR exons 17 to 23 coding sequences did not show any mutations in the 8 OSCC cell lines that were analyzed. The absence of mutations indicate that protein overexpression might be responsible for activation rather than mutation.

  • In nursing research that the data is collected through clinical observation, analysis of clinical recording or coding of interpersonal interaction in clinical areas, testing and reporting interrater reliability is very important to assure reliable results. Procedures for interrater reliability in these studies should follow two steps. The first step is to determine unitizing reliability, which is defined as consistency in the identification of same data elements in the record by two or more raters reviewing the same record. Unitizing reliability have been rarely reported in previous studies. Unitizing reliability should be tested before progressing to the next step as precondition. Next step is to determine interpretive reliability. Cohen's kappa is a preferable method of calculating the extent of agreement between observer or judges because it provides beyond-chance agreement. Despite its usefulness, kappa can sometimes present paradoxical conclusions and can be difficult to interpret. These difficulties result from the feature of kappa which is affected in complex ways by the presence of bias between observers and by true prevalence of certain categories. Therefore, percentage agreement should be reported with kappa for adequate interpretation of kappa. The presence of bias should be assessed using the bias index and the effect of prevalence should be assessed using the prevalence index. Researchers have been reported only global reliability reflecting the extent to which coders can consistently use the whole coding system across all categories. Category-by-category reliability also need to be reported to inform the possibility that some categories are harder to use than others.

THE PRINCIPLE OF THE TOOTH-BORNE AND FREE-END REMOVABLE PARTIAL DENTURE DESIGN (치아지지및 유리단 국소의치의 설계원칙)

  • The Applegate-Kennedy classification, the drawing of removable partial denture design using color coding, the selective tissue placement impression method in case of Class I and Class II removable partial dentures, the design of the swing lock attachment of an alternative approach to conventional removable partial denture, the design of the intracoronal or extracoronal attachment, and the removable partial denture design using a rotational path were presented. The following conclusions from the above things were presented : 1. The swing lock attachment removable partial denture can be effective to an alternative approach when the design of conventional removable partial denture is improper with markedly mobile remaining teeth or missing key abutments. 2. Intracoronal or extracoronal attachments must be selected care-fully considering the conditions of the abutment teeth and alveolar ridge whether more occlusal loads to the abutment teeth or to the alveolar ridge are distributed. 3. It must be almost prerequisite that a functional impression is taken in case of Class I and class II removable partial dentures and in case of tooth-borne removable partial dentures, a removable partial denture using rotational path is strong, hygienic, esthetic, and can be accomplished successfully in the clinical aspect when it is properly designed and fabricated through the complete understanding of an indication and a principle. 4. All necessary informations must be achieved with carefully investigated surveying procedure according to each clinical case by Applegate-Kennedy classification which can be helpful and useful in the clinical application and it is important that dentists themselves must be in the habit of drawing a reasonable partial denture design using a color coding in the paper sheet.

Future Directions of Pharmacovigilance Studies Using Electronic Medical Recording and Human Genetic Databases

  • Adverse drug reactions (ADRs) constitute key factors in determining successful medication therapy in clinical situations. Integrative analysis of electronic medical record (EMR) data and use of proper analytical tools are requisite to conduct retrospective surveillance of clinical decisions on medications. Thus, we suggest that electronic medical recording and human genetic databases are considered together in future directions of pharmacovigilance. We analyzed EMR-based ADR studies indexed on PubMed during the period from 2005 to 2017 and retrospectively acquired 1161 (29.6%) articles describing drug-induced adverse reactions (e.g., liver, kidney, nervous system, immune system, and inflammatory responses). Of them, only 102 (8.79%) articles contained useful information to detect or predict ADRs in the context of clinical medication alerts. Since insufficiency of EMR datasets and their improper analyses may provide false warnings on clinical decision, efforts should be made to overcome possible problems on data-mining, analysis, statistics, and standardization. Thus, we address the characteristics and limitations on retrospective EMR database studies in hospital settings. Since gene expression and genetic variations among individuals impact ADRs, pharmacokinetics, and pharmacodynamics, appropriate paths for pharmacovigilance may be optimized using suitable databases available in public domain (e.g., genome-wide association studies (GWAS), non-coding RNAs, microRNAs, proteomics, and genetic variations), novel targets, and biomarkers. These efforts with new validated biomarker analyses would be of help to repurpose clinical and translational research infrastructure and ultimately future personalized therapy considering ADRs.

  • The human skeleton is the most common organ to be affected by metastatic cancer and bone metastases are a major cause of cancer morbidity. The five most frequent cancers in Malaysia among males includes prostate whereas breast cancer is among those in females, both being associated with skeletal lesions. Bone metastases weaken bone structure, causing a range of symptoms and complications thus developing skeletal-related events (SRE). Patients with SRE may require palliative radiotherapy or surgery to bone for pain, having hypercalcaemia, pathologic fractures, and spinal cord compression. These complications contribute to a decline in patient healthrelated quality of life. The multidimensional assessment of health-related quality of life for those patients is important other than considering a beneficial treatment impact on patient survival, since the side effects of treatment and disease symptoms can significantly impact health-related quality of life. Cancer treatment could contribute to significant financial implications for the healthcare system. Therefore, it is essential to assess the health-related quality of life and treatment cost, among prostate and breast cancer patients in countries like Malaysia to rationalized cost-effective way for budget allocation or utilization of health care resources, hence helping in providing more personalized treatment for cancer patients.

Update of Korean Standard Classification of Diseases for Rectal Carcinoid and Its Clinical Implication (직장 유암종 질병 분류 코드 변경과 임상적 의의)

  • Carcinoid tumor is called as neuroendocrine tumor and is classified into neuroendocrine tumor Grade 1, neuroendocrine tumor Grade 2, and neuroendocrine carcinoma based on the differentiation of tumors. Recently, the incidence of rectal carcinoid tumor has been increasing probably due to the increased interest on screening colonoscopy and the advancement of endoscopic imaging technology. As the rectal carcinoid shows a wide range of clinical characteristics such as metastasis and long-term prognosis depending on the size and histologic features, it is a challenge to give a consistent diagnostic code in patients with the rectal carcinoid. If the rectal carcinoid tumor is less than 1 cm in size, it can be given as the code of definite malignancy or the code of uncertain malignant potential according to International Classification of Diseases for Oncology (ICD-O) by World Health Organization (WHO). Because patients get different amount of benefit from the insurance company based on different diagnostic codes, this inconsistent coding system has caused a significant confusion in the clinical practice. In 2019, WHO updated ICD-O and Statistics Korea subsequently changed Korean Standard Classification of Diseases (KCD) including the code of rectal carcinoid tumors. This review will summarize what has been changed in recent ICD-O and KCD system regarding the rectal carcinoid tumor and surmise its clinical implication.

  • This study proposes a new paradigm hospital information system through the nursing classification system and design of the HL7 clinical document architecture (Health Level Seven CDA) for information-sharing among various healthcare institutions. Nursing information CDA are included coding systems of nursing diagnosis, nursing intervention, nursing activity and outcomes. And, we have developed CDA generator for active generation of XML document. This study aims to facilitate the optimum care by providing health information required for individuals to nursing specialists in real-time, to help improvements in health, to improve the quality of productive life. This study has the following significance. First, an expansion and redefining process conducted, founded on the HL7 clinical document architecture and reference information model, to apply international standards to Korean contexts. Second, we propose a next-generation web based hospital information system that is based on the clinical document architecture. In conclusion, the study of the clinical document architecture will include an electronic health record (EHR) and a clinical data repository (CDR), and also make possible healthcare information-sharing among various healthcare institutions.

Role of miR-511 in the Regulation of OATP1B1 Expression by Free Fatty Acid

  • MicroRNAs (miRNAs) are a family of non-coding RNA that are able to adjust the expression of many proteins, including ATP-binding cassette transporter and organic cation transporter. We sought to evaluate the effect of miR-511 on the regulation of OATP1B1 expression by free fatty acids. When using free fatty acids to stimulate Chang liver cells, we found that the expression of miR-511 increased significantly while the expression of OATP1B1 decreased. We also proved that SLCO1B1 is the target gene of miR-511 with a bioinformatics analysis and using the dual luciferase reporter assay. Furthermore, the expressions of SLCO1B1 and OATP1B1 decreased if transfecting Chang liver cells with miR-511, but did not increase when transfecting the inhibitors of miR-511 into steatosis cells. Our study indicates that miR-511 may play an important role in the regulation of OATP1B1 expression by free fatty acids.
