Current Trends of Traditional Herbal Medicine Research on Allergic Disease with Dysbiosis (알레르기 질환에서 장내미생물 조절을 통한 한약의 효과 연구동향)

  • Objectives : The purpose of this study is to analyze the current trends of various herbal medicine research on allergic disease with dysbiosis. Methods : Electronic searches were performed using Pubmed, Research Information Sharing Service(RISS), Korean studies Information Service System(KISS), Oriental medicine Advanced Searching Integrated System(OASIS). Results : We analyzed ten studies on the effect of herbal medicine on allergic disease with dysbiosis. Eight studies were animal experimental studies, and two were randomized clinical trial(RCT) study and one-group pretest-posttest research, respectively. Among the studies, three studies were on atopic dermatitis, two on allergic rhinitis, and five on asthma. All different herbal medicines were used in the studies. Changes in gut microbiota composition were observed in nine studies except for 1 RCT study. In eight animal experimental studies, there was significant reduction in allergy-related inflammatory markers. Six studies evaluated the change of metabolites related to gut microbiota and three of them showed significant increase in short-chain fatty acids(SCFA). Conclusion : This study provides current trends of studies on herbal medicine research on allergic disease with dysbiosis. Most research is conducted using animal experiments, and this is a relatively recent trend. These studies offer basic knowledge on the correlation between herbal medicine, gut microbiota, and anti-inflammatory effects in allergic disease.

A Clinical Study on the Patients with Nasal Polyps (비용(鼻茸)환자에 대한 임상적 연구)

  • Objectives : The purpose of this clinical report was to investigate the general characteristics of the patients with nasal polyps and estimate the efficacy of the oriental medical treatment on nasal polyps. Methods : We performed a clinical analysis of 50 patients who visited Kabsan oriental clinic due to nasal polyps. We inquired into distribution of age, sex, operation history related to nose, grade of nasal polyps, efficacy of treatment, etc. Results and Conclusions : 1. The sex ratio was 2.3 : 1. The average age was 40.02 years. 2. In the nasal operation history, inexperienced patients were 35 cases and experienced patients were 15 cases. 3. In the region of nasal polyps, there were 35 cases of bilateral types of nasal polyps, which is mon than in unilateral types. In the number of nasal polyps, multicentric types of 26 cases were similar in the number with solitary type of 24 cases. 4. In the predisposing or concomitant diseases, chronic sinusitis was the most frequent followed by allergic rhinitis, chronic rhinitis, septal deviation chronic otitis media, asthma, tonsilitis and allergic dermatitis. 5. In the main symptoms, nasal obstruction was the most important. In the concomitant symptoms, patients had frequent rhinorrhea, smell disturbance, postnasal drip, headache, etc. 6. The average treatment period was 3,58 months and the average number of treatment was 12.94 sessions. 1. In the amount of herbal medicine, 2 and 3 Ji(劑) were the most. The most frequently used herbal medicine to treat was Hyangbujamogeuntanggagam(香附子茅根湯加減) 8. In the change of grade after treatment, Grade I was the most. In the efficacy of treatment, 'good' was the most, followed by 'excellent' and 'uneffective'.

Thirteen-week repeated-dose oral toxicity study of KOB03, a polyherbal medicine for allergic rhinitis, in rats (알러지성 비염 한약제제 KOB의 랫드에서의 13주 반복 경구투여에 의한 독성 연구)

  • Objectives : To evaluate the safety of KOB, a polyherbal medicine for allergic rhinitis, we conducted a subchronic toxicology study. Methods : Dried extract of KOB(Lot. No. 11003, yield : 41.1%) was prepared from GLP company (Hanpoong Pharm & Food Co., Ltd). KOB was repeatedly administrated orally of male SD rats at daily dose levels of 500 (G2), 1250 (G3) and 5000 (G4) mg/kg/day for 13 weeks. We recorded the clinical signs of toxicity, body weight, food intake/consumption, optometry, urine analysis, organ weights, hematology, and conducted serum biochemical analysis, necropsy, gross and histological changes in target organs of Sprague-Dawley rats, and clinical chemistry analysis. Results : Neither death nor any toxicological signs were obserbed in KOB at all doses of 500, 1250 and 5000 mg/kg/day during the administration period for thirteen-week. Furthermore, there was no difference in body weight and food-take consumption, optometry, necropsy, organ weight, gross pathological findings, and urine analysis among the groups of rats treated with different doses of KOB, during at the observation period for thirteen-week. The hematological analysis and clinical blood chemistry data were revealed no toxic effects from repeated-dose administration of KOB in rats during the observation period. Conclusions : Based on these results, the no observable adverse effect level (NOAEL) of KOB was considered to be 5000 mg/kg/day for male rats under these study conditions.

A Clinical study on Pediatric Bronchial Asthma (소아천식에 대한 임상적 고찰)

  • Objective : This study was to investigate more effective oriental medical treatment for Pediatric Bronchial Asthma Method : Aroma therapy and Herbal medicine was given to 28 pediatric bronchial asthma patients(19 males and 9 females) for about 5months from the First, August 1999 The Fifth, January 2002. Results: 1. Demographic factor : The sample consisted of 28 persons among whom 19 were male, 9 were female. The age ranges from 1 year old to 6 year old. Less than 2 year old were 4 and 2-6 year old were 20. 2.Residence : Apartment and villar dwellers were 19(67.9%), Residential street divellers were 9(32.1%). 3.Age distribution at on set : 6(21.4%) fell ill befor 1 year old and 22(78.6%) fell ill after 1 year old 4. The period of illness : 9(32.2%) suffered during 1-3 year and 6(21.4%) suffered during 6 mouth-1year and 6(21.4%) suffered during more than 3 year. 5. Frequency of the symptoms : The symptoms appeared 2-3 times a year in the case of 16(57.2%), one time a year in the case of 1, 4 times a year in the case of 6(21.4%). 6. Concomitance symptoms : All experienced coughing sign, wheezing, 23(82.1%) experienced epistaxis, nose dripping, 13(46.4%) got fever, anorexia. 7. Past history of illness : 16(57.1%) got brochiolitis, brochitis, 12(42.9%) suffered pneumonia, 9(32.1%) had allergic rhinitis. 2 had allergic rhinitis, sinusitis, atopic dermatitis, bronchial asthma, 3 got atopic dermatitis, bronchial asthma and 7 had allergic rhinitis, sinusitis, bronchial asthma, 8. Family disease : In the case of family disease, 21(75%) had allergic dermatitis, 9(42.9%) had bronchial asthma, 8(38.1%) had allergic dermatitis. 9. The symptoms became very severe in the change of season in the case of 13(46.4%) and in the case of 11(39.3%), the change of season made no difference 10. Associated caused of induction symtoms : 28(100%) got sick by common cold, infectional disease, 8(28.6%) got sick by cold food, cold air 11.The kind of therapy : 15(53.6%) got oriental therapy after occidental therapy, 11(39.3%) took only oriental therapy. 12. Improvement degree of each symptoms : In the case of cough and wheeze that are the main symptoms of bronchial asthma, 78.6% and 64.3% of the patients replied < improvement > and in the case of dyspnea, tachypnea 41.7% of the patients replied . In other symptoms, all replied 13. Degree of satisfaction : 19(67.9%) replied , 2(7.1%) replied . Conclusion : Herbal medicine and aroma therapy proved to be a very effective oriental medical treatment for pediatric bronchial asthma.

Atopic dermatitis (아토피피부염)

  • Atopic dermatitis is estimated to affect 15-20% of the childhood population and there id considerable evidence that the prevalence is increasing. But it is frequently under diagnosed and inappropriately treated yet. Atopic dermatitis can have a large social;. emotional and financial effect on the child and their family. Atopic dermatitis also commonly predated the development of asthma and allergic rhinitis. Therefore early diagnosis and proper treatment are the key for control the atopic dermatitis itself and modify the future repiratory allergies. This review will cover the new diagnostic criteria and treatment briefly.

The role of inhaled and/or nasal corticosteroids on the bronchodilator response

  • Purpose: To compare the profiles of the bronchodilator response (BDR) among children with asthma and/or allergic rhinitis (AR) and to determine whether BDR in these children is reduced by treatment with inhaled and/or nasal corticosteroid. Methods: Sixty-eight children with asthma (mean age, 10.9 years), 45 children with comorbid asthma and AR (mean age, 10.5 years), and 44 children with AR alone (mean age, 10.2 years) were investigated. After a 2-week baseline period, all children were treated with inhaled fluticasone propionate (either 100 or $250{\mu}g$ b.i.d., tailored to asthma severity) or nasal fluticasone propionate (one spray b.i.d. in each nostril) or both, according to the condition. Before and 2 weeks after starting treatment, all children were evaluated with spirometry and bronchodilator testing. BDR was calculated as a percent change from the forced expiratory volume in 1 second ($FEV_1$) at baseline. Results: The mean BDR was 10.3% [95% confidence interval (CI) 8.3-12.4%] in children with asthma, 9.0% (95% CI 7.3-10.9%) in subjects with asthma and AR, and 5.0% (95% CI 4.1-5.9%) in children with AR alone ($P$<0.001). After treatment, the mean BDR was reduced to 5.2% (95% CI 4.2-6.3%) ($P$<0.001) in children with asthma and to 4.5% (95% CI 3.5-5.5%) ($P$<0.001) in children with asthma and AR. However, children with rhinitis showed no significant change in BDR after treatment, with the mean value being 4.7% (95% CI 3.7-5.8%) ($P$=0.597). Conclusion: The findings of this study imply that an elevated BDR in children with AR cannot be attributed to nasal inflammation alone and highlights the close relationship between the upper and lower airways.

Antianaphylactic Effect of Sinpo-tang by Anal Therapy (肛腸療法에 의한 辛蒲湯의 抗아나필락시 效果)

  • Objective : Anal Therapy is another way of taking medicine and applies to each field of clinical treatment extensively. Sinpo-tang(SPT) has been used for the treatment of a allergic rhinitis. In this study, the auther investigated the anti anaphylactic action of Sinpo-tang by anal therapy was investigated on cutaneous allergic reaction models. Methods : Results : 1. Sinpo-tang (0,001-0.1 g/kg) dose-dependently inhibited the compound 48/80-induced ear swelling response in mice. Inhibitory effect of Sinpo-tang was significant (P < 0.05) at the doses of 0.01, 0.1 g/kg. 2. Sinpo-tang (0.001-0.1 g/kg) inhibited the cutaneous allergic reaction activated by anti-dinitrophenyl IgE in rats. Of special note, Sinpo-tang (0.1 g/kg) inhibited the cutaneous allergic reaction by $68\%$. 3. This inhibitory effect of Sinpo-tang was confirmed by observation of alcian blue/nuclear fast red stained-mast cells in the cutaneous tissue. 4. Sinpo-tang (0.01-1 g/L) dose-dependently inhibited the compound 48/80-induced histamine release from the peritoneal mast cells. Conclusions :These results indicate that anal therapy of Sinpo-tang may be beneficial in the treatment of mast cell-mediated anaphylaxis by inhibition of histamine release from mast cells in vivo and in vitro.

A Study on Nutritional Status of Allergic Patients and Food allergens in Korea (알레르기 환자의 현황 및 영양상태에 관한 연구)

  • This study was designed to investigate clinical features, allergy-inducing substances as well as foods, and nutritional behavior of allergic patients. Sixty-eight adult male and 78 adult female patients were surveyed by using the self-developed questionnairs. The results were as follows: 1. The common target organs of allergic symptoms included bronchus, skin and nose. 2. 37.7% of the subjects did not recognize their allergy inducing substances. Among the known substances, house-dust mite, food, temperature and weather change, pollens, smoke, air pollution materials, drugs and even stress provoked symptoms. 3. As for the food allergy, a number of foods were found to be implicated in allergic reactions. These foods included pork, chicken, seafoods such as mackerel, crab & shrimp, milk, egg, peanut, apple and pupa. Symptoms and signs of food reactions included urticaria, asthma, and rhinitis. 4. The nutrition adequacy ratio (NAR) of energy was below 0.75 for male patients and about 0.80 for female patients. And 24.0% of the subjects were regarded as excellent, 31.5% as fair, and 44.5% as poor in food related behavior evaluation.

Clinical Study on 340 Cases of Allergic rhinitis (荊芥連翹湯加味가 알레르기性 鼻炎에 미치는 效能에 대한 臨床報告)

  • The cilinical study was done in 340 patients with allergic rhinitis who had been dosed with HYUNG GAE YUN GYO TANG GAMI. The results were summarized as follows 1. Distridution by Sex and Age $64\%\;of\;them\;were\;man,\;and\;36\%$ of them were women(It was common to men than to women) Of those who were attacted(very diverse), $25.0\%$ of them were between 10 and 19 years of age(most frequent), $23.8\%$ were between 30 and 39, $18.2\%$ between 20 and 29, $14.1\%$ between birth and 9, $12.6\%$ between 40 and 49, and $6.2\%$ of them were 50 years and over. 2. Duration $47.6\%$ of them suffered from this disease for 1 to 5 years, $13.8\%$ for 10 years or longer, $24.1\%$ for both 5 to 10 years and $7.4\%$ for 6 months to 1 years, and $6.5\%$ shorter than 6 months. 3. Symptom that appeared to them were nasal obstruction($91.5\%$,top), sneezing($90\%$), rhinorrhagir($77.4\%$), headache ($30.9\%$), itch($26.8\%$), nasal discharge through gullet($15.3\%$), and pharyngitis($15.3\%$). 4. Other diseases that occured with it were sinusitis(most often), Atopic keratoconjunctivitis, allergic dermatitis, asthma, otitis media, tonsillitis, hepertropic rhinitis, nasal polyp, and atropic rhinitis(most unusual). 5. Time and Improvement After 6 months 2 cases($20\%$) were improved excellently, 3 cases($30\%$) considerablely, after 6 months to 1 year 5 case($38.5\%$) considerably, after 1 year to 5 year 10 cases($13\%$) excellently, 20 cases ($26\%$) considerably, after 5 year to 10 years 4 cases($8.7\%$) excellently, 13 cases($28.3\%$) considerable, and after 10 years 2 cases($10\%$) improved excellently, 2 cases($10\%$) considerab. 6. Dosage and Improvemont With 10 papers 6 cases($7.8\%$) were improved excellently, 22 cases($27.8\%$) considerably, with 20 papers 4 cases($10.3\%$) were improved excellently, 6 cases($15.4\%$) considerably, with 30 papers 3 cases($15.8\%$) excellently, 2 ($10.5\%$) considerably, with 40 papers 3 cases($23.1\%$) excellently, 5($38.5\%$) considerably, with 50 papers 1 cases($10\%$) excellently, 4($40\%$) considerably, with 60and 70 papers one cases($33.3\%,\;100\%$) for each was improved considerable, with 80 papers 1 cases($100\%$) excellently, and 90 papers 2 cases($66.6\%$) considerable. 7. Improvement with additional outward application With external application 8 cases($16.6\%$) were improved excellently, 9 cases($18.7\%$) considerably without external application 10 cases($8.4\%$) were improved excellently, 34 cases($28.8\%$) considerably. 8. General Improvement 18 cases($10.8\%$) were improved excellently, 43 cases($25.9\%$) improved considerably, and 40 cases($24.0\%$) improved quite a little.thus has improvement shown in 60.8 per cent of case, and proved its efficacy.
