Effects of Basic Clinical Practice Program in Academic Motivation, Critical Thinking and Clinical Nursing Competence of Nursing Students (기초임상실습프로그램이 간호대학생의 학습동기, 비판적사고 및 임상수행능력 자신감에 미치는 효과)

  • The purpose of this study was to identify the effects of basic clinical practice program(BCPP) on academic motivation, critical thinking and clinical nursing competence in nursing students. Thirty-hour BCPP was consisted of the essential of fundamental nursing skills and prior education in nursing lab before first clinical practice. The participants were 228 nursing students and data was collected with self-administered questionnaire during Feb. 18 to Feb. 28, 2013. Study design was one group pre-post test. BCPP was not related with critical thinking. But there were significant differences subcategories of learning motivation like academic stress, test anxiety, depression and self efficacy, and in clinical nursing competence. The results suggest that BCPP is effective to enhance clinical competence for novice of nursing clinical practice.

The Effect of Breathing Biofeedback Training in the Stress of Nursing Students in Clinical Practice. (호흡바이오피드백 훈련이 간호대학생의 임상실습시 스트레스에 미치는 효과)

  • The purpose of this study was to identify the effects of breathing biofeedback training on the stress of nursing students in clinical practice. The research design was a nonequivalent control group pretest-posttest design. The subjects of this study were 39 nursing students from the College of Nursing of K University. The study was conducted from July 20 to September 3, 1998. The subjects were assigned to one of two groups : the experimental group (19 students), and the control group (20 students). The breathing biofeedback training was performed 12 times with the experimental group. The level of psychological stress was measured using the State Anxiety Inventory, Profile of Mood State, and Visual Analogue Stress Scale. The level of physiological stress was measured using pulse rate and blood pressure. The data were analyzed using descriptive statistics, $x^2-test$, t-test, and repeated measures of ANOVA. The results of study are as follows : 1) State anxiety scores were not significantly different between the experimental group and the control group after the biofeedback training. 2) Profile of mood state scores were not significantly different between the experimental group and the control group after the biofeedback training. 3) Visual Analogue Stress Scale scores were significantly different between the experimental group and the control group after the biofeedback training(F=11.68, p=0.002). 4) Pulse rates were not significantly different between the experimental group and the control group after the biofeedback training. 5) Systolic blood pressures were significantly different between the experimental group and the control group after the biofeedback training(F=5.44, p=0.025). 6) Diastolic blood pressures were not significantly different between the experimental group and the control group after the biofeedback training. On the basis of the above findings, the following recommendations for further study are made ; 1) Identification of the effects of breathing biofeedback training at times of high stress during clinical practice. 2) Identification fo the effects of stress reduction according to the frequency of the breathing biofeedback training.

The Effects of Laughter Therapy on the Anxiety and Depression of Inflammatory Bowel Disease Patients (웃음치료가 염증성 장질환 환자의 불안 및 우울에 미치는 효과)

  • Purpose: The purpose of this study was to apply laughter therapy to clinical practice and investigate its effects on patients' anxiety and depression in order to increase the quality of nursing for patients with inflammatory bowel disease (IBD). Methods: The study was conducted based on the non-equivalent control group pretestposttest design. The participants include 20 patients with IBD in the control group and 17 in the experiment group. Data was collected from July 10th, 2011 to January 22nd, 2012. Laughter therapy was administered once a day for five consecutive days. Results: The anxiety score was significantly different between the two groups and indicates that laughter therapy is effective for reducing anxiety among patients with IBD. Futhermore, there were differences in the depression scores of the experiment group between the pre-test and post-test, but no significant differences were found between the two groups. Conclusion: The results show that laughter therapy was effective in reducing anxiety among patients with IBD but did not decrease depression directly. Considering that the experiment group had a bigger reduction rate in depression scores than the control group. However, it is expected that laughter therapy will serve as an emotional nursing intervention for patients with IBD.

  • Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.

The Effect of Music Therapy on Reduction of Anxiety before Gastroscopy (음악요법이 위내시경 대상자의 검사전 불안에 미치는 효과)

  • Purpose: This study examined the effect of music therapy on the pre-exam anxiety of gastroscopy clients. Method: The research period is from June 17 2002 to August 30 2002, and the subjects were 60 individuals, 30 for the experimental and 30 for the control group among adult clients. The data were analyzed by using the SPSS program. Result: The results of this study were as follows: The score of the experimental group was lower than the score of the control group for the situational anxiety (t=-12.56, p=0.000) and systolic blood pressure (t=-3.97, p=0.000) before the examination was supported. The vital sign was partially supported. There was no significant difference(t=-1.05, p=0.297) in the diastolic blood pressure (t=-1.05, p=0.297) and pulse rate(t=-1.30, p=0.199) before the examination of the experimental group and lower than the score of the control group. Conclusion: Music therapy could be useful in the nursing practice, and be utilized as a way of nursing intervention to reduce anxiety of the patients in specific clinical situations. Finally, it is recommended that further studies be conducted on this therapy by utilizing different musical selections.

Effects of an emotional regulation program on maladaptive cognitive emotion regulation, stress, anxiety, and postpartum depression among South Korean mothers of premature infants in the neonatal intensive care unit

  • Purpose: This study aimed to evaluate the effects of a rational emotive behavior therapy (REBT)-based emotional regulation program for mothers of premature infants in the neonatal intensive care unit (NICU). Methods: The study utilized a nonequivalent control group pretest-posttest design. Seventeen mothers were recruited for the experimental group and 21 mothers for the control group. Data were collected from July 9, 2018, to Oct 12, 2018 from mothers of premature infants in the NICU regarding parental stress, state anxiety, depression, and maladaptive cognitive emotion regulation. The experimental group received eight sessions of the REBT-based emotional regulation program for 3~4 weeks, and the control group only received standard nursing care. Results: Significant differences were found between the two groups in maladaptive cognitive emotion regulation (Z=-3.88, p<.001), stress (Z=-2.76, p=.006), state anxiety (Z=-3.72, p<.001), and postpartum depression(Z=-2.62, p=.009) after the intervention in the experimental group. Conclusion: The REBT-based emotional regulation program for mothers of premature infants was effective for reducing maladaptive cognitive emotion regulation, stress, anxiety, and postpartum depression. Therefore, it may be necessary to provide an REBT-based emotional regulation program for mothers of premature infants in the NICU in clinical practice.

Factors associated with clinical nurses' preconception health behavior in Korea: a cross-sectional survey

  • Purpose: Nurses have been reported to be at an increased risk for miscarriage and preterm labor. However, there is limited knowledge regarding nurses' preconception health behaviors. Therefore, this study aimed to identify factors influencing these behaviors. Methods: One hundred sixty nurses, who were planning their first pregnancy within the upcoming year, participated in an online survey from August 11 to October 31, 2021. Data on preconception health behavior, perceived health status, pregnancy anxiety, nursing practice environment, and social support were analyzed using the t-test, Pearson correlation coefficients, and multiple regression analysis. Results: Age (p=.024), educational level (p=.010), marital status (p=.003), work experience (p=.003), satisfaction with the work department (p<.001), smoking status (p=. 039), and previous health problems related to pregnancy outcomes (p=.004) were significantly associated with nurses' preconception health behaviors. Furthermore, perceived health status (p<.001), pregnancy anxiety (p=.011), nursing practice environment (p=.003), and social support (p<.001) showed significant correlations with preconception health behaviors. Social support (β=. 28, p=.001), satisfaction with the work department (β=.23, p=.032), marital status (β=.22, p=.002), and perceived health status (β=.23, p=.002) were confirmed as factors associated with preconception health behaviors. These factors explained 40.9% of the variance in preconception health behaviors (F=6.64, p<.001). Conclusion: Clinical nurses' preconception health behaviors were influenced by social support, perceived health status, satisfaction with the work department, and marital status. Interventions to improve clinical nurses' preconception health behaviors should target social support and perceived health status. A preconception health behavior education program considering clinical nurses' marital status and satisfaction with the workplace can also be implemented.

An Inquiry into Subjectivity of Fundamental Nursing Practice Attitude (간호대학생들의 기본간호실습태도에 대한 주관성 연구)

  • The study was designed to identify the attitude of nursing students to fundamental nursing practice through Q-methodology. A Q sample was developed through a review of the literature and interviews. Forty statements made up the finalized Q-sample. The P sample consisted of 25 nursing students in S College. Q statements were written on separate cards and were given to the 25 subjects to sort according to degree of agreement or disagreement. The Q-sort by each subject was coded and analyzed with QUANL PC Program. The analysis discovered three major attitudes, namely "type 1: passive demand", "type 2: active self-confidence", "type 3: practical application". The correlation was .213 between type 1 and 2, .409 between type 1 and 3, .379 between t ype 2 and 3. The results revealed three different types of fundamental nursing practice attitude; 1) Passive demand type: they were not satisfied with fundamental nursing practice time, the number of persons, practice machines. They presented anxiety and worry through fundamental nursing practice. Therefore, they will presenta passive attitude of clinical practice experience. 2) Active self-confidence type: they experienced pride as a nursing student and fascination as a nurse was an acquired recognition. 3) Practical application type: they practiced that fundamental nursing skill was applied their family and oneself. Therefore they had tension through initial fundamental nursing practice but they gained self-confidence and interest through practical study. In conclusion, the researchers suggest that the education program would be more effective if it was planned considering to each types of attitude of nursing students for fundamental nursing practicer fundamental nursing practice

The Relationships among Social Support, Hope, Anxiety, and Depression in Patients Expecting to Receive Hematopoietic Stem Cell Transplantation (조혈모세포 이식 전 환자의 사회적 지지, 희망, 불안 및 우울의 관계)

  • Purpose: This study aimed to examine the relationships among social support(family support, medical team support), hope, anxiety, and depression in patients with hematologic cancers before they received hematopoietic stem cell transplantation (HSCT) to obtain baseline data for developing a nursing intervention. Methods: The participants were 70 adult patients expecting to receive HSCT from 5 university hospitals in Seoul, Gyeonggi-do, and Jeollanam-do regions. A cross-sectional survey was done using standardized instruments for social support (Tae's Family Support Scale and Professional Medical Support Scale), hope (Kim & Lee Hope Scale), anxiety and depression (Hospital Anxiety and Depression Scale). The data were analyzed by SPSS/WIN 19.0 program using frequency, percentage, item mean and standard deviation, t-test, ANOVA, and Pearson's correlation coefficient. Results: Hope was significantly correlated with social support (r=.40, p=.001), anxiety (r=-.40, p<.001) and depression (r=-.58, p<.001). Anxiety was correlated with depression (r=.54, p<.001). Conclusion: The findings of this study show greater social support for patients who expect to receive HSCT is significantly correlated to a higher level of hope, as well as low levels of anxiety and depression. In nursing practice, clinical nurses may develop a nursing intervention to reinforce social support and hope, as well as reduce anxiety and depression for patients preparing for HSCT.

Effects of a Pre-Conception Care Program in Women with Inflammatory Bowel Disease: A Mixed-Methods Study Including a Randomized Controlled Trial

  • Purpose: The purpose of this study was to conduct a pre-conception care program for women of childbearing age with inflammatory bowel disease (IBD) in Korea and verify its effects on self-efficacy for IBD management, IBD-related pregnancy knowledge, and IBD-related pregnancy anxiety. It also aimed to explore the changes in participants through the program. Methods: A convergent mixed-methods study design was adopted. In the quantitative phase, 35 women (17 and 18 in the intervention and control group, respectively) participated. The intervention group attended a program that included small-group sessions and individual tele-coaching. To confirm the effects, data were collected before and one and four weeks after the intervention. In the qualitative stage, focus group interviews and tele-coaching were conducted with the intervention group. Results: After the program ended, significant differences were observed over time between the intervention and control groups for self-efficacy for IBD management (Wald χ2 = 4.41, p = .036), IBD-related pregnancy knowledge (Wald χ2 = 13.80, p < .001) and IBD-related pregnancy anxiety (Wald χ2 = 8.61, p = .003). Qualitative data analysis revealed the following themes: (1) improving confidence in IBD management and awareness for planned pregnancy; (2) improving IBD awareness related to pregnancy and childbirth; and (3) relieving anxiety about and actively facing pregnancy. Conclusion: This study is meaningful in that, to the best of our knowledge, it is the first to develop a pre-conception care program for women diagnosed with IBD and confirm its effectiveness. Furthermore, this program is expected to be suitable for patient counseling and education in clinical practice.
