A Case Report of HBsAg Seroclearance in Chronic Hepatitis B Patient

  • Hepatitis B surface antigen (HBsAg) seroclearance is a rare event in chronic hepatitis B virus (HBV) infection which acquires the disease early in life. A case study have examined with asymptomatic chronic hepatitis B carrier who exhibits HBsAg seroclearance in anti-HBe positive. We comprehensively studied the biochemical, virological and clinical aspects of a patient with HBsAg seroclearance. Liver biochemistry, serological markers, serum HBV DNA levels, and development of clinical complications were monitored. Mutation of hepatitis B virus is suspected serum HBsAg detected by the HBsAg assay systems of VITROS (OrthoClinical Diagnostics, USA), AxSYM (Abbott Laboratories, USA), Elecsys (Roche Diagnostics, Germany) and ADVIA Centaur (Bayer Diagnostics, USA). These four immunoassays showed negative results. Also, the patient had undetectable serum HBV DNA. Therefore, no mutation within the "a" determinant of HBsAg, which might escape detection from HBsAg immunoassay were found. Natural seroclearance was confirmed.

  • Benign paroxysmal positional vertigo (BPPV) is one of the most common clinical entities encountered in a dizzness clinic. Retrosepective review was performed for the patient's diagnosed as BPPV at Sunchon "S" hospital dizzness center. Variables for statistical analysis included age, sex, involved canal presence of recent head tramua, presence of chronic otitis media, history of middle ear surgery, underlying disease such as hypertension or diabetics, headache, central lesion. From July 2004 to May 2005, we sampled the 600 dizzness patient's who visited the dizzness center. Dizzness could be classified into BPPV, vestibulopathy. Among these patients, 256 patients had BPPV, 222 patients had vesibulopathy and 97 patients had Normal. Among these BPPV patients, 80 patients had lateral canalolithiasis BPPV (31.3%), 28 patients had lateral cupulolithiasis BPPV (10.9%), 90 patients had posterior canalolithiasis BPPV (35.2%), 7 patients had multicanalolithiasis BPPV (2.3%), 52 patients simultaneously had BPPV, other vestibular disease, and central lesion (20.3%).

  • We investigated the latest trend of u-Health and relations with clinical technologist. Since 2000, Korea has become an aging society. Korea will become an aged society by 2018, and it is expected to enter the ultra-high aged society by 2026. Increase in over 65 years population means that the desire of medical service and care demand for the elderly is greatly required. In addition, many predicted Korean national health insurance would falter financially. Medical suppression policies and regulations are also amended continuously. U-Health based on the IT industry and development of related technologies and industries contains the variety concepts of telemedicine, telehealth, e-health, u-healthcare and POCT. Especially, the use of POCT supplied quick clinical examination is extending steadily in medical center and hospital, which will generate the field friction between nurses and medical technologists. In the transition situation from provider-centric service to consumer-driven health care system, this study recommended the principal role and correspondence of clinical laboratory workers and offered information about changes in healthcare market and the basic concept of expert system, measurement and the diagnosis principle to clinical technicians throughout the investigation of the recent research and government policy trends.

Metastatic intestinal adenocarcinoma with osseous metaplasia in two Domestic Korean Shorthair cats

  • Two Domestic Korean Shorthair cats presented with dyschezia and vomiting. Computed tomography revealed a colonic mass with calcification and lymph node metastasis in case 1, and a small intestinal mass with disseminated mesenteric metastasis and calcification in case 2. Histopathology revealed intestinal adenocarcinoma with osseous metaplasia. Case 1 died two months after surgery from distant metastasis; and case 2 showed no metastasis for five months but presented with anorexia, euthanized seven months after diagnosis. Metastatic intestinal adenocarcinoma with bone formation should be considered as differential diagnosis for calcification on imaging, and lymph node metastasis at diagnosis may indicate poor prognosis.

  • Chromosomal abnormalities of abortus have also been used to investigate the most common etiology of spontaneous abortion, but the frequency and the types of spontaneous abortions have also demonstrated considerable variation among in different countries and races. A cytogenetic analysis of 75 abortuses was performed at the GenDix, Inc. from January, 2006 to December, 2007. The frequency of chromosome abnormalities in abortus was 32.0% (24/75 cases). Among the chromosomal abnormalities, trisomy was 62.5% (15/24 cases) and the most frequent trisomy was trisomy 21 with 26.6% (4/15 cases). The average maternal age of normal and abnormal karyotypes was $34.1{\pm}3.3$ and $34.3{\pm}3.3$. Cytogenetic analysis of abortus is important for diagnosis and genetic counseling for parents with spontaneous abortion.

  • Cellular phones are the most used electronic device everyday in modern life and are always in contact with our hands, Although many studies have revealed microorganisms living on our hands, there are only a few reports on the research about products or places which are in contact with our hands. Therefore, the purpose of this study is to verify microorganisms living in cellular phones. Microorganisms were scraped from cellular phones of students and professors from the clinical laboratory science department in Daegu Health College, and cultured at Brain Heart Infusion agar and MacConkey agar following API kit to identify them. The average colony number was $1.5{\times}10^2$ on BHI agar and $40{\times}10$ on MacConkey agar. There was no difference according to gender. In Gram stain result, Gram(+) Cocci showed the highest frequency. Also in BHI agar plates, Micrococcus spp and Acinetobacter baumannii identified with high frequency. Moreover, S. aureus, which is very well known as strong food poisoning bacteria, was isolated. Klebsiella pneumonia ssp pneumonia was isolated with the highest frequency from the MacConkey agar or S-S agar plate. From these results show, there are as many different microorganisms from cellular phones as from our hands. This is the first report isolating strong food poisoning bacteria in cellular phones. Since infection in hospitals have been an important issue to be aware of, it is equally necessary to investigate cell phones and products which hospital workers touch with their hands.

Effects of Green Tea Catechins (GTC) on the Treatment of Hangover and Prevention of Liver Disease

  • Over-consumption of alcohol leads to many side-effects such as malnutrition, liver disease, and neuronal disorders and many investigators have tried to identify methods for preventing the side-effects of drinking. This study was carried out to investigate the effect of the beverage contained green tea catechins (GTC) on the alcohol administered rats. We observed that blood alcohol concentration level decreased significantly in plasma. GTC (200 mg/kg) also reduced the aspartate aminotransferase (AST) and alanine aminotransferase (ALT) level of the intoxicated rats. These results suggest that GTC may be useful for the prevention and therapy of hepatotoxic pathogenesis.

A clinical case of bovine anemia due to Theileria orientalis group in a non-grazed dairy cow in the upper part of South Korea

  • This study reports the detection and genotyping of the first clinical case of bovine anemia due to Theileria orientalis group (BATOG) in non-grazed dairy cow in upper South Korea. Blood and serum tests revealed anemia and hyperbilirubinemia from animal showing clinical symptoms, and later confirmed as piroplasmosis-positive. Follow-up surveillance on the herd revealed 2 asymptomatic cows with anemia. The three animals were confirmed theileriosis-positive and genotyping revealed the clinical and one of the asymptomatic cases have Chitose, while the other has Ikeda genotype. Clinical BATOG cases were rarely reported worldwide, and asymptomatic animals left untreated could serve as parasite reservoir.

Analysis of Glucose, Cholesterol, AST and ALT in Sera of Man Workers in Jeonbuk Area (전북지역 남자 근로자의 혈청 Glucose, Cholesterol, AST, ALT 농도 분석)

  • We determined the concentration of glucose, cholesterol, AST and ALT in sera of man workers (n=3,247) had health check-up at hospitls in Jeonbuk area. The mean of concentration of glucose, cholesterol and AST of all workers were increased by 0.1%, 14.9% and 33.0% over than the mean of reference, but the mean of ALT was decreased by 0.8% from it. The rate of workers showed abnormal value of glucose, cholesterol, AST and ALT were 9.7%, 25.4%, 23.7% and 5.5%, respectively. The mean of glucose, cholesterol and AST in each age group were increased over than reference, but the mean of ALT was decreased except in fifties. The rate of the workers in each age group showed an abnormal concentration of glucose, cholesterol, AST and ALT were in 5.6~6.7%, 14.9~31.1%, 20.4~29.2% and 5.2~6.0%, respectively. These data suggest that many workers in Jeonbuk area are opened in possibility of getting diabetes, liver disease and heart disease.

Effects of Zinc and Its Chelators on ERG b-Wave Sensitivity During the Light Adaptation in Bullfrog Retina

  • Zinc plays a key role in genetic expression, cell division, and cell growth and is essential for the functions of more than 450 metalloenzyme. There are high concentrations of zinc in pigment epithelium in bullfrog eye. Zinc deficiency causes night blindness and abnormal dark adaptation. The purpose of this study was to identify ERG (electroretinogram) b-wave sensitivity during light and dark adaptation in bullfrog retina after zinc and zinc chelators treatment such as histidine and TSQ (N-(6-methoxy-8-qunolyl)-p-toluenesulfon amide). Especially, we focused whether histidine act as a zinc chelator in the Muller cell. The results of our study are summarized as follows: 1) Both zinc and histidine elevated ERG b-wave amplitude and threshold in Muller cells by accelerating rhodopsin regeneration time and increased a-peak absorbance during light adaptation. 2) TSQ reduced those by prolonging rhodopsin regeneration time and decrement of a-peak absorbance during light adaptation. 3) Zinc shortened rhodopsin regeneration time and prolonged a-peak absorbance. These results suggested that histidine may act as a zinc-mediated transporter in presynaptic Muller cell membrane rather than zinc chelator and acts as a GABA-receptor inhibitor which blocks $Cl^-$ influx to the postsynapse.
