Does a preterm labor-assessment algorithm improve preterm labor-related knowledge, clinical practice confidence, and educational satisfaction?: a quasi-experimental study (조기진통 사정 알고리즘은 실습 시 조기진통 관련 지식, 임상수행자신감, 교육만족도에 유효한가?: 유사실험 연구)

  • Purpose: Preterm birth is increasing, and obstetric nurses should have the competency to provide timely care. Therefore, training is necessary in the maternal nursing practicum. This study aimed to investigate the effects of practice education using a preterm-labor assessment algorithm on preterm labor-related knowledge and clinical practice confidence in senior nursing students. Methods: A pre-post quasi-experimental design with three groups was used for 61 students. The preterm-labor assessment algorithm was modified into three modules from the preterm-labor assessment algorithm by March of Dimes. We evaluated preterm labor-related knowledge, clinical practice confidence, and educational satisfaction. Data were analyzed with the paired t-test and repeated-measures analysis of variance. Results: The practice education using a preterm-labor assessment algorithm significantly improved both preterm labor-related knowledge and clinical practice confidence (paired t=-7.17, p<.001; paired t=-5.51, p<.001, respectively). The effects of the practice education using a preterm-labor assessment algorithm on knowledge lasted until 8 weeks but decreased significantly at 11 and 13 weeks after the program, while the clinical practice confidence significantly decreased at 8 weeks post-program. Conclusion: The practice education using a preterm-labor assessment algorithm was effective in improving preterm labor-related knowledge and clinical practice confidence. The findings suggest that follow-up education should be conducted at 8 weeks, or as soon as possible thereafter, to maintain knowledge and clinical confidence, and the effects should be evaluated.

The relationship between the satisfaction levels of education and clinical practice in the dental hygiene students (일부 치위생과 학생의 교육 만족도와 임상실습 만족도의 관련성)

  • Objectives : The purpose of the study is to investigate the relationship between the satisfaction levels of education and clinical practice in the dental hygiene students and to provide the basic data for the improvement of satisfaction with major. Methods : A self-reported questionnaire was filled out by 487 dental hygiene students in Busan, Gyeongnam, and Ulsan. Data were analyzed by SPSS 19.0 program. Results : Those who chose the dental hygiene by themselves tended to have higher satisfaction(p<0.05). Those who chose the major by teacher's advice tended to have higher satisfaction for the clinical practice(p<0.05). The satisfaction level between the education and clinical practice was equally proportional to each other. Multiple regression test showed that the influencing factors on satisfaction level included major satisfaction and lecture satisfaction(p<0.05). Conclusions : Satisfaction with the major influenced on the clinical practice performance. More systematic approach for the better curricula can bestow the students satisfaction with the major in the future.

A Study on the Clinical Practice Experiences on Nursing Activities of Nursing Students (간호학생의 임상실습 중 간호활동 경험 정도)

  • Purpose: The purpose of this study was to identify the clinical practice experiences of nursing students who completed the courses of nursing curricula in college. Method: The data was collected using a questionnaire with 191 task elements in 8 nursing standards specified by the Korean Nurses Association(2003), from December 2004 to January 2005. Three hundred thirty-three nursing students from five three-year and five four-year colleges of nursing were selected for respondents. Result: The nursing activities which over 70% of the students had performance experiences with were 23 items(12.04%) practiced frequently in clinical settings, simple, low in malpractice risk and noninvasive in characteristic. In addition, the nursing activities which under 30% of students had performance experiences with were 87 items(45.55%). The nursing activities which over 70% of students had only observation experiences with were 20 items(10.47%). Also, the nursing activities that over 30% of students didn't have any experiences with were 19 items(9.95%) not frequently seen in the clinical setting. Conclusion: It is concluded that the students are not fully qualified as the new nurses of the future. Therefore, every effort to develop a more effective clinical nursing education in school, clinical settings and in students themselves is needed.

The Need for a New Curriculum in Clinical Genetic Education (임상유전학 교육에서 새로운 교과과정 개발의 필요성)

  • It is becoming increasingly important for medical doctors to have a thorough understanding of human genetics and the ethical, legal, and social implications of genetic testing, counseling, and treatment. As genetic engineering and technology evolves, medical doctors will find themselves called in to counsel patients about a rapidly increasing number of diseases for which genetic testing and treatments are available. Medical doctors will need to master a new set of principles and clinical skills. A lack of knowledge about these issues and problems may lead to serious, lifelong or even fatal negative effects on patients. Medical genetics has moved from the study of rare conditions to the illumination of disorders that impact the entire spectrum of medical practice. This study demonstrates several areas in which medical genetics is clearly an important tool in medical practice and the necessity of establishing new curriculum for clinical genetic education in Korea. Medical students nearing graduation may lack genetic knowledge that is essential for daily practice because genetics has little or no place in clinical teaching. Medical schools should make extensive curriculum changes to increase students' awareness of clinical genetics and its ethical implications. The medical school curriculum will need creative new approaches to keeping up with the rapid pace of evolution of clinical genetics.

Domestic Research of Medical Students Trends Analysis (의과대학생에 관한 국내 연구동향 분석)

  • This study explored medical students' major research topics and research methods by analyzing 184 academic articles pertaining to the characteristics of medical students from 2007 to 2017. Results showed many papers dealing with medical students' emotional and cognitive aspects, student counseling, clinical practice education, and curriculum management. According to the medical education accreditation board, research trends were found mostly in the student and curriculum areas of learner characteristics, medical humanities, student counseling, clinical practice education, and curriculum management. Common research topics have been steadily increasing since the introduction of the evaluation accreditation standard in 2012. Medical students predominantly used quantitative research methods for the studies. In the future, it is necessary to ensure that research topics such as CQI, digital- and performance-based clinical practice, and convergent curriculum within the Fourth Industrial Revolution are being studied. In addition, it is crucial to investigate learners' unique, dynamic, and qualitative characteristics through qualitative and mixed methods.

Analysis of Research Trends in the Korean Journal of Medical Education and Korean Medical Education Review Using Keyword Network Analysis (키워드 네트워크 분석을 통한 "한국의학교육"과 "의학교육논단"의 연구동향 분석)

  • The aim of this study was to analyze the research trends in articles published in the Korean Journal of Medical Education (KJME) and Korean Medical Education Review (KMER) using keyword network analysis. The analyses included 507 papers from 2010 to 2019 published in KJME and KMER. First, keyword frequency analysis showed that the research topics that appeared in both journals were "medical student," "curriculum," "clinical clerkship," and "undergraduate medical education." Second, centrality analysis of a network map of the keywords identified "curriculum" and "medical student" as highly important research topics in both journals. Third, a cluster analysis of 20 core keywords in KMER identified research clusters related to academic motivation, achievement, educational measurement, medical competence, and clinical practice (centered on "learning," while in KJME, clusters were related to educational method and program evaluation, medical competence, and clinical practice (centered on "teaching"). In conclusion, future medical education research needs to expand to encompass other research areas, such as educational methods, student evaluations, the educational environment, student counseling, and curriculum.

A Study of Faculty Practice for Clinical Teaching (임상실습 교육을 위한 간호학 교수의 실무참여에 관한 연구)

  • Nurse educators are being encouraged to intergrated the role of faculty practice into the role expectations of the education institutes. Schools of nursing are faced with challenge of the faculty who wishes to adopt facilitating practical role. Also directors of nursing department in hospitals point out the lack of competences for nursing care of new graduated nurse. This survey study was conducted to clarify the factors that faculty who engages on practice in the clinical teaching are to facilitate or inhibit. In this study, 55 head nurses of university hospital and 30 professors of nursing school were assigned to complete the questionnaire. Results of this study are as follows : 1) Head nurse : The most actively participated nursing activities in student's clinical teaching are medication, injection, vital sign checking and bed making. The problems of clinical leaching are lack of direct care of Professors, overloaded work of head nurses, passive learning attitudes of nursing students and less priority about clinical teaching of academic administrator's perception. 2) Nursing professor Facilitators of faculty practices are negative perception about clinical practice of both nursing professor and academic administrator. Inhibitors of faculty practice are negligence of the clinical teaching, lack of the practicing capability and lack of administration system on practical education by head of the school. There, following strategies are suggested for facilitating faculty practice : 1. Faculty practice focused on clinical teaching must be emphasized for academic administration. 2. Nurse educators must keep continuing clinical practice in their specific area. 3. Collaboration between school of nursing and hospital promotes effectiveness of the clinical practice for nursing students.

A Survey on Clinical Practice Students' Perception of Clinical Performance Examination in a College of Korean Medicine Using the Actor Standardized Patient and Student Standardized Patient (연극인표준화환자와 학생표준화환자를 사용한 일개 한의과대학 진료수행시험(CPX)에 대한 임상실습 학생 인식 조사)

  • Objectives : This study compares clinical practice students' perception of Clinical Performance Examination (CPX) in a College of Korean Medicine using actors as Actor Standardized Patient (ASP) and enrolled students as Student Standardized Patient (SSP). Methods : Targeting the clinical practice students who completed the clinical practice education in the first semester of the fourth year in the Department of Korean Medicine, a survey was conducted on total six categories such as satisfaction with CPX using ASP and SSP, prior knowledge about clinical practice education, self-evaluation, difficulties in the process of performing the CPX, satisfaction with ASP/SSP, and familiarity with them. Results : In the results of the survey, the mean score of CPX program using SSP was higher than that of CPX program using ASP in every area. Among them, in the satisfaction with CPX itself and prior knowledge about various clinical practice education, the mean score of CPX program using SSP was statistically significantly higher than that of CPX program using ASP. Conclusions : Overall, the clinical practice students' satisfaction with CPX program using SSP was higher than that of CPX using ASP, and statistically significantly higher in some areas.

Presentation on Research Trends and Suggestion for further research and education on Objective Structured Clinical Examination and Clinical Performance Examination in Korean Medicine Education: Scoping Review (한의학교육에서 객관구조화진료시험과 진료수행평가에 대한 연구 동향과 후속 연구 및 교육에 대한 제언: 주제범위 문헌고찰)

  • Objectives : This study reviewed existing studies on clinical examination of Korean medicine to investigate the trends and gaps of existing research on clinical performance examination (CPX) or objective structured clinical examination (OSCE) in Korean medicine education. Methods : We conducted a scoping review according to the five steps suggested by Arksey and O'Malley. Six databases(RISS, OASIS, KMBASE, KISS, ScienceON, and DBpia) were searched and studies published from 2012 to July 15, 2022 were considered. The subjects of the study were limited to domestic studies about OSCE or CPX conducted in Korean medicine education. Results : Among the 25 selected papers, 17 studies implemented OSCE or CPX in practice. Those studies were conducted in 8 clinical departments and 9 of them used standardized patients. All 14 survey studies reported positive answers in satisfaction, efficacy, and self-assessment. OSCE, CPX scoring items were developed by modifying existing tools or developing through expert surveys or through literature review and significant cases. Conclusions : This study is expected to be used as basic data for following studies and promote clinical examination. From now on, the colleges of Korean medicine should proceed research on large-scale students about extended subjects through various research methods to obtain objective results that can be generalized and acquire validity and reliability. In order to promote such research, it is necessary to induce cooperation from institutions related to Korean Medicine and local universities to develop modules, implement, and conduct post-evaluation.

A Study on the Perception of the Importance and Performance of Patient Education of the Clinical Nurses (종합병원 간호사가 인식하는 환자교육 중요도와 수행도의 관계연구)

  • The purpose of the study are to examine the perception of the importance and performance of patient education of the clinical nurse and find out the interfering factors in practicing patient education. The data were collected from convenient sample of 256 clinical nurses working in the nursing units of adult patients except the psychiatric unit, obstetric unit, dental surgical unit and intensive care unit of one University Hospital in Seoul from September 29 to October 2, 1998. Three measurement tools of self-report- questionnaires developed by researcher used. For the content validity of the questionnaires, two sessions of panel discussion and a pilot test were done and finally factor analysis was done with Varimax method. Analysis of data was done with SAS program using frequency, percentage, means, standard deviation, Pearson's Correlation Coefficients, t-test and ANOVA. The obtained results were as follows : 1. The surveyed nurses perceived the importance of patient education at higher level with mean score of 4.08 among 5 point than their perception of practice( mean score : 3.42). 2. There was positive significant correlation(r=.29, p=0.0001)between nurses' perception of the importance of patient education and it's practice 3. Among the teaching contents for patients, 'information of diagnostic procedure and operation' and 'orientation of hospitalization' were perceived most important. And 'preparation for discharge' and 'understanding of disease and health promotion' were perceived least important 4. Among the teaching contents for patients, 'orientation of hospitalization' and 'information of diagnostic procedure and operation' were perceived highly performable. And 'understanding of disease and health promotion' and 'preparation for discharge' were perceived least performable. 5. Three types of interfering factors were identified as patient-factor, situational factor, nurse-factor. The mean degree of impediment with the interfering factors was at average level(3.09 among 5). The patient and situational factors of impediments were more interfering than nurse- factor for teaching patients. 6. In older age(p<.05), married state (p<.05), higher educational status (p<.01), higher clinical experience (p<.01) and higher position(p<.01), the score of perceived importance of patients education was more high. 7. In older age(p<.01), higher clinical experience(p<.001) and surgical unit (p<.01), the score of perceived performance of patients education was more high. In conclusion, in order to activate patient education practice in the clinical setting, the continuing education for patients education should be more emphasized and the effective teaching methods and materials should be developed to help patient teaching. And an organizational support such as budgeting for patient education and reimbursement system should be administrated.
