금관변연이 치은 및 치은열구에 미치는 영향에 관한 연구 (EFFECTS OF ARTIFICIAL CROWN MARGIN ON GINGIVA AND GINGIVAL SULCUS)

  • A total of 200 patients, ranging in age from 20 to 60 years, were selected for the study. Each had at least one tooth which was restored with complete cast gold crown and a nonrestored contralateral tooth with no clinical evidence of caries and periodontal disease. The gingival tissues adjacent to the crowned and nonrestored teeth were examined to determine the evaluation of the severity of inflammation and probed to determine individual pocket depth. The findings are listed here. 1. The average sulcus bleeding index of the gingival tissues adjacent to crowned teeth was 1.99. The average sulcus bleeding index of the gingival tissues adjacent to nonrestored teeth was 0.67. 2. The average gingival sulcus depth adjacent to crowned teeth was 2.19mm. The average gingival sulcus depth adjacent to non restored teeth was 1.68mm. 3. No difference could be found between the average gingival sulcus bleeding index and average gingival sulcus depth of male and those of female. 4. The difference between sulcus bleeding index of the gingival tissues adjacent to crowned teeth and sulcus bleeding index of the gingival tissues adjacent to nonrestored teeth increased with increased age of the cast crown.

Management of large class II lesions in molars: how to restore and when to perform surgical crown lengthening?

  • The restoration of endodontic tooth is always a challenge for the clinician, not only due to excessive loss of tooth structure but also invasion of the biological width due to large decayed lesions. In this paper, the 7 most common clinical scenarios in molars with class II lesions ever deeper were examined. This includes both the type of restoration (direct or indirect) and the management of the cavity margin, such as the need for deep margin elevation (DME) or crown lengthening. It is necessary to have the DME when the healthy tooth remnant is in the sulcus or at the epithelium level. For caries that reaches the connective tissue or the bone crest, crown lengthening is required. Endocrowns are a good treatment option in the endodontically treated tooth when the loss of structure is advanced.

  • STATEMENT OF THE PROBLEM: Recent data regarding the effects the cement type and abutment heights on the retentive force of a prosthetic crown are inconsistent and unable to suggest clinical guidelines. PURPOSE OF THE STUDY: This study evaluated the effects of different types of temporary cements and abutment heights on the retentive strength of cement-retained implant-supported prostheses. MATERIALS AND METHODS: Prefabricated implant abutments, 4 mm in diameter, $8^{\circ}$taper per side, and light chamfer margins, were used. The abutment heights of the implants were 4 mm, 5.5 mm and 7 mm. Seven specimens of a single crown similar to a first premolar were fabricated. Six commercially available temporary cements, TempBond, TempBond NE, Cavitec, Procem, Dycal, and IRM, were used in this study. Twenty-four hours after cementation, the retentive strengths were measured using a universal testing machine with a crosshead speed of 0.5 mm/min. The cementation procedures were repeated 3 times. The data was analyzed using two-way analysis of variance and a Tukey test (${\alpha}$=0.05). RESULTS: The tensile bond strength ranged from 1.76 kg to 19.98 kg. The lowest tensile strengths were similar in the TempBond and Cavitec agents. Dycal showed the highest tensile bond strength (P<0.01). More force was required to remove the crowns cemented to the long abutments (P<0.05). CONCLUSION: TempBond and Cavitec agents showed the lowest mean tensile bond strength. The Dycal agent showed more than double the tensile bond strength of the TempBond agent.

Dentoskeletal features in individuals with ectopic eruption of the permanent maxillary first molar

  • Objective: The aim of the study was to analyze the prevalence and distribution of ectopic eruption of the permanent maxillary first molar (EEM) in individuals scheduled for orthodontic treatment and to investigate the association of EEM with dental characteristics, maxillary skeletal features, crowding, and other dental anomalies. Methods: A total of 1,317 individuals were included and randomly divided into two groups. The first 265 subjects were included as controls, while the remaining 1,052 subjects included the sample from which the final experimental EEM group was derived. The mesiodistal (M-D) crown width of the deciduous maxillary second molar and permanent maxillary first molar, maxillary arch length (A-PML), maxillomandibular transverse skeletal relationships (anterior and posterior transverse interarch discrepancies, ATID and PTID), maxillary and mandibular tooth crowding, and the presence of dental anomalies were recorded for each subject, and the statistical significance of differences in these parameters between the EEM and control groups was determined using independent sample t -tests. Chi-square tests were used to compare the prevalence of other dental anomalies between the two groups. Results: The prevalence of maxillary EEM was 2.5%. The M-D crown widths, ATID and PTID, and tooth crowding were significantly greater, while A-PML was significantly smaller, in the EEM group than in the control group. Only two subjects showed an association between EEM and maxillary lateral incisor anomalies, which included agenesis in one and microdontia in the other. Conclusions: EEM may be a risk factor for maxillary arch constriction and severe tooth crowding.

한국인의 안정위시 상악중절치 노출량에 관한 연구 (A study on exposure length of Maxillary central incisor in rest position in Korean)

  • This study is intended to provide a referable information of exposed amount of maxillary central incisor of Korean by ages and gender under rest position. The result of this study will give guidelines for making prothesis. The subjects of this study are patients of Charmgoun Dental Hospital in Busan, Korea. A statistical analysis was conducted after taking digital photos of patients' teeth with a ruler and measuring the length of teeth on the computer program. The results of this study were as follows : 1. The length of maxillary central incisor that exposed under upper lip is decreased by increasing age in rest position.; the average length is 3.455mm in 20s, 2.525mm in 30s, and 1.543mm in 40s. 2. The exposal length in females is more than males, average length is 2.796mm in female and 2.342mm in male. However, there is not significant difference between the genders. 3. The exposed average length of maxiallry central incisor under upper lip is 2.618mm at rest. 4. The clinical crown average length of maxillary central incisor is 10.195mm, but incresing age, there is no significant defference. 5. There is significant difference between the genders in the clinical crown length of maxillary central incisor.; the length is 10.637mm in men, 9.90mm in women.

Cone-beam computed tomography-guided three-dimensional evaluation of treatment effectiveness of the Frog appliance

  • Objective: To evaluate the effectiveness of the Frog appliance in three dimensions by using cone-beam computed tomography (CBCT) images. Methods: Forty patients (21 boys and 19 girls), averaged 11.7 years old, with an Angle Class II division 1 malocclusion were included in our study. They had either late mixed dentition or early permanent dentition, and the maxillary second molars had not yet erupted. All patients underwent CBCT before and after the treatment for measuring changes in the maxillary first molars, second premolars, central incisors, and profile. Paired-samples t-test was used to compare the mean difference in each variable before treatment and after the first phase of treatment. Results: The maxillary first molars were effectively distalized by 4.25 mm (p < 0.001) and 3.53 mm (p < 0.05) in the dental crown and root apex, respectively. The tipping increased by $2.25^{\circ}$, but the difference was not significant. Moreover the teeth moved buccally by 0.84 mm (p < 0.05) and 2.87 mm (p < 0.01) in the mesiobuccal and distobuccal cusps, respectively, whereas no significant changes occurred in the root apex. Regarding the anchorage parts, the angle of the maxillary central incisor's long axis to the sella-nasion plane increased by $2.76^{\circ}$ (p < 0.05) and the distance from the upper lip to the esthetic plane decreased by 0.52 mm (p = 0.01). Conclusions: The Frog appliance effectively distalized the maxillary molars with an acceptable degree of tipping, distobuccal rotation, and buccal crown torque, with only slight anchorage loss. Furthermore, CBCT image demonstrated that it is a simple and reliable method for three-dimensional analysis.

Effect of abutment neck taper and cement types on the amount of remnant cement in cement-retained implant restorations: an in vitro study

  • PURPOSE. The present study aims to analyze the effect of abutment neck taper and types of cement on the amount of undetected remnant cement of cement-retained implant prostheses. MATERIALS AND METHODS. Three neck taper angles (53°, 65°, 77°) and three types of cement (RMGI: resin-modified glass ionomer, ZPC: zinc phosphate cement, ZOE: zinc oxide eugenol cement) were used. For each group, the surface percentage was measured using digital image and graphic editing software. The weight of before and after removing remnant cement from the abutment-crown assembly was measured using an electronic scale. Two-way ANOVA and Duncan & Scheffe's test were used to compare the calculated surface percentage and weight of remnant cement (α = .05). RESULTS. There were significant differences in remnant cement surface percentage and weight according to neck taper angles (P < .05). However, there were no significant differences in remnant cement surface percentage and weight on types of cement. No interaction was found between neck taper angles and types of luting cement (P > .05). The wide abutment with a small neck taper angle showed the most significant amount of remnant cement. And the types of luting cement did not influence the amount of residual cement. CONCLUSION. To remove excess cement better, the emergence profile of the crown should be straight to the neck taper of the abutment in cement-retained implant restoration.

Six-year clinical performance of lithium disilicate glass-ceramic CAD-CAM versus metal-ceramic crowns

  • PURPOSE. To assess the clinical performance of monolithic CAD-CAM lithium disilicate glass-ceramic (LDGC) crowns and metal-ceramic (MC) crowns provided by predoctoral students. This study also assessed the effects of patient and provider-related factors on their clinical performance as well as patient preference for these types of crowns. MATERIALS AND METHODS. Twenty-five patients who received 50 crowns (25 LDGC CAD-CAM and 25 MC) provided by predoctoral students were retrospectively examined. LDGC CAD-CAM crowns were milled in-house using the CEREC Bluecam system and cemented with either RelyX Unicem or Calibra Esthetic resin cements. MC crowns were cemented with RelyX Unicem cement. Clinical assessment of the crowns and the supporting periodontal structures were performed following the modified California Dental Association (CDA) criteria. Patients' preference was recorded using a visual analog scale (VAS). The results were statistically analyzed using log-rank test, Pearson Chi-squared test and Kaplan-Meier survival analysis. RESULTS. Twelve complications were observed in the MC crown group (9-esthetic, 2-technical and 1-biological). In comparison, 2 complications in the LDGC CAD-CAM crown group were observed (1-technical and 1-esthetic). The 6-year cumulative survival rates for MC crowns and LDGC CAD-CAM were 90.8% and 96%, respectively, whereas the success rates were 83.4% and 96%, respectively. Overall, patients preferred the esthetic outcomes of LDGC CAD-CAM crowns over MC crowns. CONCLUSION. The high survival and success rates, low number of complications, and the high level of patients' acceptance of monolithic LDGC CAD-CAM crowns lend them well as predictable and viable alternatives to the "gold standard" MC crowns.

  • Author examined clinical procedure, clinical application and clinical failure of castable glass ceramic (Dicor) crowns which were made at the Dept. of Prosthodontics, Seoul National University Hospital during 19 months from Jan. 1988 to July, 1989(Observation period:19 months). The author obtained the following results. 1. A total of 86 crowns were constructed ; Most of them has been set on maxillary central incisors(53 cases, 62%):maxillary lateral incisors(20 cases, 23%):maxillary canines(6 cases, 7%):others(7 cases, 8%) 2. Dicer could be used as a lamiate veneer for discolored tooth. 3. During the examination period(1-19 months), there was no fracture case. 4. Dicer could be applied on the maxillary anterior tooth with minimal occlusal force.

치아돌출이 부착치은 폭경에 미치는 영향 (The Effect of Dental Protrusion on the Width of Attached Gingiva)

  • 치아돌출정도가 부착치은 폭경에 미치는 영향을 살펴보기 위하여 치주상태가 양호한 성인 중 하악 전치부에 다소의 총생이 있는 37명을 대상으로 하악의 인상채득 후 석고모형을 제작한 다음 각 하악 절치의 상대적 및 절대적 돌출정도를 측정하고 임상치관 고경, 치주낭 깊이, 각화 및 부착치은 폭경과 비교 분석하여 다음과 같은 결론을 얻었다. 1. 비돌출측에 비하여 돌출측의 각화치은 및 부착치은 폭경이 작게 나타났다. 2. 돌출측의 임상치관 고경은 비돌출측보다 유의하게 크게 나타났으나 치주낭 깊이는 돌출측과 비돌출측간의 유의한 차이를 보이지 않았다. 3. 돌출측과 비돌출측간의 부착치은 폭경차이는 중절치보다 측절치에서 더욱 뚜렷이 나타났다. 4. 절대적 돌출도보다 상대적 돌출도가 부착치은 폭경과 더 높은 상관관계를 나타내었다. 5. 돌출도보다 임상치관고경이 부착치은 폭경과 더 높은 상관관계를 나타내었다.
