Enhanced Recovery after Surgery for Gastric Cancer Patients Improves Clinical Outcomes at a US Cancer Center

  • Purpose: Enhanced recovery after surgery (ERAS) protocols for gastric cancer patients have shown improved outcomes in Asia. However, data on gastric cancer ERAS (GCERAS) programs in the United States are sparse. The purpose of this study was to compare perioperative outcomes before and after implementation of an GC-ERAS protocol at a National Comprehensive Cancer Center in the United States. Materials and Methods: We reviewed medical records of patients surgically treated for gastric cancer with curative intent from January 2012 to October 2016 and compared the GC-ERAS group (November 1, 2015-October 1, 2016) with the historical control (HC) group (January 1, 2012-October 31, 2015). Propensity score matching was used to adjust for age, sex, number of comorbidities, body mass index, stage of disease, and distal versus total gastrectomy. Results: Of a total of 95 identified patients, matching analysis resulted in 20 and 40 patients in the GC-ERAS and HC groups, respectively. Lower rates of nasogastric tube (35% vs. 100%, P<0.001) and intraabdominal drain placement (25% vs. 85%, P<0.001), faster advancement of diet (P<0.001), and shorter length of hospital stay (5.5 vs. 7.8 days, P=0.01) were observed in the GC-ERAS group than in the HC group. The GC-ERAS group showed a trend toward increased use of minimally invasive surgery (P=0.06). There were similar complication and 30-day readmission rates between the two groups (P=0.57 and P=0.66, respectively). Conclusions: The implementation of a GC-ERAS protocol significantly improved perioperative outcomes in a western cancer center. This finding warrants further prospective investigation.

A Method of Commerce Activation of the Old City-Center of Jeju City Using Augmented Reality Technology (증강현실을 활용한 원도심 상권 활성화방안)

  • Jeju City is one of the cities where decline of old city-center is appearing due to the reduction of residential and floating population. The decline of old city-center of Jeju City has been on the rise since the 1990s, and has been intensifying in the 2000s. Jeju City is currently activating specific policies to ensure activation and diversification of the local commercial area. In this paper, we propose a new method to increase the number of visitors to Jeju-Mokkwana by using Augmented Reality technology for activating commerce of old city-center. Various Augmented Reality contents based on smart-phone are implemented and operated in Jeju-Mokkwana. Also the survey is conducted to 574 visitors who have experienced online/offline promotion for analyzing effects. Survey results show that the proposed method can be effectively used to activate the old city-center by increasing the floating population.

  • U-City includes various information devices and network techniques, which connect among several information devices. Integrated Management Center, which is the core element of u-City, is designed to manage all services of u-City and carry out the control function for a city. Accordingly, u-City needs the methods of user authentication and security, so these methods must be implemented to integrated management center. This paper is devoted to describe some conventional authentication techniques, and authentication methods and procedures that may be available to u-City network context. Proposed u-City integrated authentication model assigns IP to only right user after authenticating information terminals and users in u-City and authorizes users according to the policy, so this model plays an important role for the security of integrated management center.

City center Regeneration in Busan Metropolitan Using GIS (GIS를 활용한 부산시 도심재생에 관한 연구)

  • Donut phenomenon of city centers has resulted from the decreasing residential population and the fluid population with curved commercial function, but the analysis can bring other problems owing to the simplified method. And As the survey and the analysis the research can offer the characteristic methods for regeneration in the city of Busan. To figure out the functional regions of city center the research has analyzed 15 Dongs in city center of the three years of 1996, 2000 and 2004 to find out the donut phenomenon and the regeneration of city center. The survey has chosen 20 variation factors using factor analysis and cluster analysis. Major factors in each year are shown 3 factors, presenting 84.2%, 87.1% and 85.5% of the accumulated explanation rate. These factors explain residential centered, commercial centered, and management centered factor. The results are as following. First, it is necessary to plan the regeneration of the total city center owing to the total donut phenomenon regarding the functions. Second, the methods to regenerate city centers should be established according to the various regional characterizations.

Building Web Database for WHO Healthy City Wonju (원주시 건강도시 웹 데이터베이스 구축)

  • The purpose of this study is to develop Web database for healthy city that contains healthy city indicators for making city health plans, setting project priorities, monitoring projects, and evaluating healthy city projects, effectively. Using Delphi survey method for identifying indicator domains and indicators, we extracted nine domains with thirty-four healthy city indicators. Based on the appraisals of DB users about the contents of DB, a web database for healthy city Wonju was constructed. We developed a web database system for the purposes of sharing high quality health related data for managing and evaluating healthy city projects. The web database currently provides variety data in the web address, http://healthycity.wonju.go.kr/index.html. The web DB comprised with major healthy city indicators that are the most important indicators, healthy city indicator data that have a variety data set for encompassing all domain areas such as city infrastructure, health medicine, economies, and all other related areas and qualitative data that contains policy reports, research results, healthy city information and all other tips. A database of healthy city is very essential and important because it makes healthy city projects alive by managing and sharing healthy city related data effectively. But we need to fill out some blank cells in DB because there are currently unavailable data for some indicators. In conclusion, we expect the web DB contributes information sharing of healthy city project teams and improving healthy city project quality at Wonju city in Korea.

  • This study was a basic study for the establishment of basic policies for the operation of Sejong City Library. It was intended to establish and operate the functions and roles of Sejong City Library by conducting a general survey on the current status, user demand, expert opinion, and librarian opinion survey. As a result, the Sejong City Library is the representative library of Sejong Special Self-Governing City, which oversees all public and small libraries in Sejong Special Self-Governing City and carries out support and cooperation projects, and should improve the development of the local community and the quality of life of local residents by providing more convenient and equal access to provide information services and cultural programs. Second, apart from providing library services to local residents in Sejong Special Self-Governing City, the functions of the Sejong Special Self-Governing City's representative library and hub library, knowledge and information center, lifelong education center, social and cultural center, leisure culture center, leisure and convenience center, and furthermore, it should function as a policy information support center. Finally, in order to enhance the brand value of Sejong City Library, the Sejong City Library as a landmark in Sejong area, the Sejong City Library as an art and cultural hub in Sejong area, the Sejong City Library as a complex cultural space in Sejong area, and the Sejong City Library as a center for Sejong Studies were proposed.

Information Security Policy in Ubiquitous-Ecological City (u-Eco City에서의 정보보호 정책)

  • In this paper, the requirements for information security are presented in the ubiquitous ecological(u-Eco) city. The various definition of ubiquitous city is analyzed first, the concept of the u-Eco City, services and major projects are then presented. The framework of the integrated operating center for u-Eco city is proposed, the privacy, data security and network facility protection in the center are analyzed. Unlike to previously proposed security algorithms, the light-weight encoding algorithms(such as block/stream encoding, pseudo-random generator, hash function, and public key encoding) in the u-Eco city center are required to communicate the information in the ubiquitous sensor network. Furthermore, the principal policies guaranteeing the secrecy and authentication for the private information are also presented.

  • For the development and management of u-City(ubiquitous city), Ubiquitous City Construction Law was enacted(March 3, 2008) to form the legal basis of u-City plan, construction and managements afterwards. Among u-City elements, the u-City Management Center has its own meaning and importance in roles which are crucial to the u-City developments, and its legal establishment is one of the core u-City realization factors. In spite of its crucial position for u-City, the legal arrangement of its basis concept, physical/systematic structures, and managerial finance sources and their standardizations are very immature. Therefore, the purpose of this study is to analyse its overall problem issues and then to suggest the basic directions and enforcement strategies to form its legal basis.

  • As adopting ubiquitous technology into civil engineering, new city model is suggested called U-City. This paper proposes the framework of U-City management center to support effective services operation. The aims of the framework are to provide the development and operation environment for U-City services. Basically, these objectives are achieved by adopting the semantic web service technology to the framework. In this paper, OWL-S is mainly conducted to represent the description of U-City services. In addition, this paper insists that fine grained unit services are required to guarantee reusability, compatibility, and scalability of the services on U-City management center. The documentations conducted by OWL-S are provided as an example of service descriptions. At the last section, this paper also presents the architecture of U-City management center which enables automatic service discovery, selection, composition and interoperation.

The effect of visual feedback training on balance ability in patients with hemiparetic (시각적 되먹임 훈련이 편마비 환자의 균형능력에 미치는 영향)

  • Purpose : The purpose of present study was to determine effects of a visual feedback training on balance ability in poststroke hemiparetic subjects. Methods : Fourteen chronic stroke patients participated. Participants were randomly divided into either visual feedback training group(7 experimental group, 7 control group). All of participated were inpatients at local rehabilitation center and had been receiving a traditional rehabilitation program, five days a week. Exprimental group have additionally undergone for four weeks, three days a week, the visual feedback training but control group was not received any additional program except the traditional rehabilitation program. The Berg Balance Scale(BBS), the FICSIT-4, the MTD-balance system to measure a balance ability were carried out twice before and after training. Results : After participation in the program, subjects of visual feedback training demonstrated a significant improvement in the scores of the BBS, the FICSIT-4, the MTD-Balance system. The control group had no change on the any tests. After the training, the result to improve significantly in visual feedback training group compared to post-test of control group were the score of BBS and the FICSIT-4 and the MTD-Balance system. Conclusions : The present study suggests that the visual feedback training program may become a useful strategy for enhancing balance ability in the rehabilitation of stroke patiens.
