요통 치료를 위한 흉요추 이행부 (Thoracolumbar Junction) 와 배유혈(背兪穴), 화타협척혈(華他夾脊穴)의 상관성 에 관한 연구 (A Study on the Correlation between Thoracolumbar Junction and Back-su points(背兪穴), Hwatahyeopcheok points(華他夾脊穴) for Treatment of Low Back Pain)

  • Objectives : This study was designed to investigate the correlation between thoracolumbar junction and back-su points, Hwatahyeopcheok points for treatment of low back pain in the thoracolumbar junction syndrome that was suggested by Maigne R. Method : We Investigate the acupuncture points that was correlated with the location of thoracolumbar junction area. And We tried to find out a common point between thoracolumbar junction and back-su points, Hwatahyeopcheok points for treatment of low back pain. Results and Conclusion : 1. It is considered that these points such as $BL_{20}$, $BL_{21}$, $BL_{22}$, and Hwatahyeopcheok points that are located from 11th thoraic spinous process to 2nd lumbar spinous process are correspond to the thoracolumbar junction area. 2. It is suggested that acupuncture treatment on $BL_{20}$, $BL_{21}$, $BL_{22}$, and Hwatahyeopcheok points can release the tenderness of the muscles, recover autonomic nervous function and release smooth muscles and vascular contraction, so it can treat low back pain caused by thoracolumbar junction.

고관절 전치환술을 시행 후 내원한 만성 강직성 척추염 환자 치험 1례 (A Case of Chronic Ankylosing Spondylitis with Total Hip Replacement - A Case Report -)

  • 본 증례는 약 30년전 임신 후부터 강직성 척추염의 증상이 나타나기 시작하였고, 15년 전부터 경추 부까지 강직이 진행되었으며, 10년전부터 고관절의 침습이 나타났고 이후 계속 강직이 진행되어 좌측 고관절에 전치환술을 시행하고 2주후 본원에 내원한 만성 강직성 척추염 환자로서, 그동안 적절한 치료를 꾸준히 받아오지 못하여 척추부 및 고관절의 강직과 흉협통 뿐 아니라 견관절, 슬관절 및 거의 전신관절에 강직과 통증이 나타났고 식욕부진, 소화 장애와 전신피로감까지 호소하여 증상의 별다른 호진을 기대하기 힘들 것으로 예상하였으나 64일간 본원에 입원하여 한의학적인 치료와 운동요법 및 물리요법으로 이학적 검사상의 호전과 전신증상의 호전을 보였다. 식욕부진, 소화불량, 피로감, 체중감소, 빈혈, 발열(發熱), 도한(盜汗) 등 만성 강직성 척추염 환자에게서 나타날 수 있는 전신증상은 한의학에서 허증(虛證)의 범주에 가깝고, 변증시치를 통한 한의학적인 치료로 전신증상의 개선을 도모하면서 꾸준한 운동치료, 호흡치료 등의 재활치료를 병행한다면 보다 나은 임상적 치료효능을 거둘 수 있을 것으로 생각된다.

  • Oryeong-San, Pangpungtongseong-San, Rangkyeoksanwha-Tang, Sipeemikwanjoong -Tang and Taeumjoui-Tang are not only examined using the code which are related to overweight in the QRIS but are also investigated the level of Free Radical using the Free Radical Measurement after steeping those 5 prescriptions in water. The results are as follows: 1. We indicated in the study of QRIS that 5 kinds of medicines which used most frequently In the treatment of the obesity appeared to affect the Immune system, spleen, kidneys, pancreas, the fatigue toxicity, TSH, and the metabolic disability but did not influence high on the contents of overweight and those of fatty cell, as well. in addition, there were no significant differences between the prescriptions as regards testosterone and progesterone. 2. In the Free Radical Measurement, Rangkyeoksanwha-Tang evaluated significantly high level of Free Radical, whereas others appear to have the similar level of Free Radical. These findings suggest that the treatment of the obesity affects particular body parts with respect to the control of overweight, although those medicines are not related directly to the areas(such as fatty cell Code), it is possible that they influence on the cure for the obesity. Furthermore, they indicate that with soaking prescription, Free Radical is not produced as much as we expected.

정상 성인에서 체간 굴근, 신근의 근력과 요추 전만각의 상관관계에 관한 연구 (A Study on Correlation between Power of Trunk Flexors, Extensors and Lumbar Lordotic Angle in Normal Adults)

  • Objectives : The purpose of this study was to investigate the correlation between lumbar lordotic angle and the power of trunk flexors, extensors in normal adults Methods : 34 normal participants participated in this study. Their lumbar lordotic angle(L1-S1 Cobb's angle and L1-L5 cobb's angle) was measured by x-ray taken on lateral direction, erect cross-arm position. And muscle power of trunk flexors and extensors of each participant measured using Cybex HUMAC NORM. Results : 1. The average of L1-S1 Cobb's angle was $47.21{\pm}8.88^{\circ}$ and the average of L1-L5 Cobb's angle was $36.32{\pm}9.62^{\circ}$(Table IV). 2. The average ratio of trunk flexors/extensors was $6.44{\pm}19.31%$(Table V). The average power of the trunk flexors was $165.18{\pm}55.05$(Newton-Meter/kg), and the power of trunk extensors was $257.18{\pm}85.53$ (Newton-Meter/kg)(Table VI). 3. Lumbar lordotic angle has no relation to the ratio of trunk flexors/extensors(Table VII, Fig. 4). 4. Lumbar lordotic angle has no relation to both the power of the trunk flexors and extensors(Table VIII, Fig. 5, Fig. 6). Conclusions : These results suggest that the lumbar lordotic angle measured by radiograph could not evaluate the power and ratio of trunk flexors, extensors.

모아레 체형측정과 양하지 근육 발달비율의 상관관계에 관한 조사 (Correlation between Moire Topography and Muscle Development Ratio of Both Lower Limbs)

  • Objectives : This study was performed to find the correlation between moire topography and muscle development ratio of both lower limbs. Methods : 88-general persons who examed general health checkup were enrolled this study. Who coincided the exclusion criteria were left out. The authors practiced Moire Topography by using IBS-2000 and measured difference of contour line and interval between vertical base line of neck and pelvis. We made use of X-SCAN to analyse body component and calculate muscle development ratio of both lower limbs. Results : In this study, we found out that the more interval between vertical base line of neck and pelvis grown large, the more muscle development ratio of both lower limbs got small. So, there was correlation between imbalance of the dorsum muscle and muscle development ratio of both lower limbs.(p>0.05). But others did not have. Conclusion : According to this results, we found out there was correlation between imbalance of lumbar and gluteal region and muscle development ratio of both lower limbs. Apply to clinic, we are able to improve the efficacy of diagnosis and medical treatment.

요추추간판탈출증환자 49례의 탈출양상에 따른 한방치료에 대한 임상적 고찰 (The clinical study on difference of sex, age, medical history, part, or type in patients with lumbar spine herniation of nucleus pulpous)

  • Objective : In treatment of lumbar spine herniation of nucleus pulpous, the different result in accordance with difference of sex, age, medical history, part, or type. Methods : After 49 patients who were diagnosed as HNP of L-spine were grouped by sex, age, medical history, part, or type, they were compared and analyzed on the basis of difference between measuring VAS and ODI as they were hospitalized and as they were discharged. Results : 1. In terms of differences by gender, males' average degree of VAS improvement was 63.92 and females' average degree of VAS improvement was 59.8. Also, Males' average ODI improvement index was 12.36 and females' average ODI improvement index was 12.5. 2. In terms of differences by age, Teens recorded highest degree of VAS improvement and ODI improvement index. 3. In terms of differences by medical history, Most acute or acute patients showed higher degree of VAS improvement and ODI improvement index than subacute or chronic patients. 4. There were no significant distinguishing markscomparing between one diseased part and other. 5. As the research that focused on different type, Extrusion type had better improvement by medical treatment than Protrusion type. Conclusion : This research drew the result as above, but recognizable statistical relation could not be found in the result. Therefore there needs much sustainable research to deduce meaningful result.

척추측만증 검진을 위한 모아레 체형측정법과 설문조사의 상관성 연구 (Clinical Study on the Correlation between 3-Dimensional MOIRE Topography and Questionnaire for School Screening in Scoliosis)

  • Objectives : The purpose of this study is researching for correlation between Moire topography results and questionnaire which include obesity rate, height, weight, posture, pain aspect (low back pain, headache, fatigue, neck pain etc), studying posture, school attendance method, menarche. Methods : The authors practiced Moire topography and questionnaire for primary & middle school 24,419 students [male 12,843, female 11,576] in Jeonbuk, korea in 2002. After we distinguished students who had abnormal finding in Moire topography and statistically analysed the data. Results : According to this research, the abnormal finding in Moire topography results was 37.3% (9,105 students), and students needed X-ray re-examination were 3.9% (953 students). According to statistical analysis, male/female and primary/middle school students have no difference in deformity degree of the body surface examined by Moire. Height and weight have low correlation with deformity degree of the body surface examined by Moire and menarche, pain aspect have no correlation. But, school attendance method by bicycle and $90^{\circ}$upright sitting in study posture is the best way to decrease the deformity degree of the body surface examined by Moire. Conclusions : Following this research, throughout Moire topography could detect trunk asymmetry that couldn't be found existing X-ray examination, and school attendance by bicycle and $90^{\circ}$upright sitting in study posture is good for school students to prevent back deformity.

교통사고 후 발생한 요통에 대한 봉약침 치료의 효과 (Effect of Bee-venom Acupuncture on Low Back Pain by Traffic Accidents)

  • Objectives : The purpose of this study was to investigate the effect of Bee-venom acupuncture on low back pain by traffic accidents. Methods : We included 34 patients with low back pain caused by TA for this study. The experimental group was treated by usual therapy plus Bee-venom acupuncture therapy, the control group was treated by usual therapy such as acupunture therapy and herbal medicine therapy. Oswestry Disability Index(ODI) and Visual Analog Scale(VAS) were evaluated at 4 week from baseline for the treatment effect in the both group. Results : After the subjects being treated by Bee-venom or general acupuncture therapy. It was detected that the scores of ODI and VAS in both groups decreased at the 4 week from baseline. There was significant difference between both groups in the VAS. However, there was no significant difference between both groups in the ODI. Conclusions : The results suggested that bee-venom acupuncture was effective on low back pain by traffic accidents.

초.중학생의 척추비틀림과 통증과의 관계고찰 - 경근무늬(모아레) 측정법과 설문지 조사를 통해 - (A Study of the Relationship between Spine Distortion and Pain of Elementary and Middle School Students - through moire measurement and questionnaire investigation -)

  • Objectives : The aim of this study is to investigate the relationship between spinal distortion and pain in elementary and middle school students. Methods : Using the phase-shifting moire developed by KAIST(Korea Advanced Institute of Science and Technology) we measured the back surface distortion, and using questionnaire investigation, we investigated the partial pain in 1008 elementary and middle school students(532 boy students(52.8%), 476 girl students(47.2%)) in Seoul in 1999. And, we statistically analyzed the relationship between spinal distortion and partial body pain Results : This study show that the more age increase, the more the pain in head, neck, shoulder, back and low back Increase considerably among the students aged 9 to 15. The ratio of each partial body pain was 28.4%, 19.7%, 18.2%, 18.7%. 16.0% at the age of 9 and 59.7%, 67.4%, 51.8%, 51.8%, 64.4% at the age of 15 respectively. And, it was shown there was a significant correlation between spine distortion and partial body pain area. When the spinal distortion was found at the upper thoracic and cervical area, the students complained the more pain on head, neck and shoulder(p<0.05) and when the spinal distortion was found at the sacral base, the more headache(p<0.05) Conclusions : It is suggested that the significant correlation exist between the spinal distortion and pain and the pain related to spine disorder get more increase as students grow. We have to study further more about Juvenile postural distortion and it's prevention.

백굴채(白屈菜)가 손상된 배양척수감각신경세포에 미치는 영향 (Effects of Herbar Chelidonii on the Cultured Spinal Sensory Neurons Damaged by XO/HX)

  • Objectives and Methods : To evaluate the mechanism of oxidative damage by xanthine oxydase(XO) and hypoxanthine(HX)-induced oxygen radicals, MTT assay and NR assay were carried out after the cultured mouse spinal sensory neurons were preincubated for 4 hours with various concentrations of XO/HX. And the amount of total protein. neurofilament EIA. lipid peroxidation and LDH activity were measured, to evaluate the protective effect of Herbar Chelidonii(HC) water extract on cultured spinal sensory neurons damaged by XO/HX. after the cultured mouse spinal sensory neurons were preincubated with various concentrations of HC water extract for 3 hours prior to exposure of XO/HX. Results : XO/HX decreased significantly the survival rate of the cultured mouse sensory neurons by NR assay and MTT assay In proportion to concentration and exposed time. In proportion to concentration and exposed time on cultured spinal sensory neurons, XO/HX showed the quantitative decrease of neurofilament by EIA. the decrease of total protein amount by SRB assay and the Increase of lipid peroxidation as well as LDH. HC showed the quantitative increase of neurofilament and total protein, but showed the decrease of lipid peroxidation and LDH activity against the neurotoxicity of XO/HX. Conclusions : From the above results, it is concluded that XO/HX have a neurotoxic effect on cultured spinal sensory neurons and that the herbs extract, such as HC, prevent the toxicity of XO/HX effectively in that they decrease lipid peroxidation and LDH activity.
