The Study on theses of Chuna (推拿) (추나(推拿) 관련 논문에 대한 연구)

  • Objectives : To research the trend of the study related to Chuna(推拿) and to establish the direction of further studies into the Chuna(推拿). Methods : We reviewed and analyzed all theses published by Korean research institution. And these theses were classified by research institutions, published year, field of study, subject, research methods, thesis types, illness and symptoms. Results : The following results were obtained in this study. 1. Classified by the major field of study, oriental medicine accounted for 59 papers, followed by 18 in physical education and 2 in the science of nursing and medicine. 2. Upon classifying theses according to research method and thesis types, research related to clinical trial accounted for nearly half of all theses. It was followed by consideration of documents. 3. After 1998 deals with the effects of the Chuna on variable illnesses such as: ossified posterior longitudinal ligament, chronic coccyalgia, entrapment neuropathy, low birth weight infant, Bell's palsy, stress incontinence, paramenia, Parkinson disease, Bertolloti syndrom, TMJ (because of scoliosis) etc. 4. Two most widely cured illnesses using the Chuna therapy are scoliosis and HNP of L-spine. However, when considering the fact that several clinical tests and casuistics did not include research into Lumbago (including HNP of L-spine), it can be concluded that most of the Chuna therapies have been concentrated on Lumbago. Conclusion : As it can be seen in the above results, the possibility of curing illness through Chuna and the usefulness of Chuna have been shown factually and clinically through various consideration of documents, clinical trial & casuistics. Based upon such facts, it is regarded that further scientific research along with additional clinical approaches involving the Chuna should be performed.

The Clinical Report on 1 case of Low Back Pain and radiational pain Patient Treated by Chuna Traction and Conservative Treatment (Disc Bulging과 Stenosis 소견을 보인 요각통 환자를 대상으로 보존적 치료와 굴곡신정법(屈曲伸延法)을 병용한 치험 1례)

  • The purpose of this report is to investigate the clinical application of conservative treatment to a patient who has the low back pain and radiational pain. In this case, a female patient, 67 years old, who had low back pain and radiational pain on the right leg. We treated the patient with conservative treatment (acupuncture, herbal medicine, moxibustion) and chuna traction. In result, the in-patient's pain has decreased and walking distance has increased from 10m to 300m for 27 days. In conclusion, conservative treatment and chuna traction could be effective methods to treat the patient who has low back pain and radiational pain.

A Clinical Study on the Outcome of Chuna Treatment of Adolescent Structural Scoliosis (청소년기의 구조적 척추측만증에 대한 추나치료 결과 고찰)

  • Objectives The present study retrospectively investigated clinical outcome of patients with structual scoliosis during Chuna treatment. Methods : The study population consisted of 34 patients(14 patients of adolescent and 20 patients of adult) who were diagnosised as scoliosis with radiological finding. All patients were treated with Chuna treatment, and after treatment, evaluated with radiological measurement by calculating the scoliosis correction angle from the Initial and follow-up spine AP and lateral views. Also the evaluation of clinical outcome was done twice pre and post treatment during this study. Results : The results were summarized as follows ; 1) The 85.3 percent of patients complained physical pain. and adult group had various painful lesion compared with adolescent group. 2) Adult group had treated $18.0{\pm}7.2$ times and adolescent group treated $13.5{\pm}5.7$ times with Chuna treatment. 3) Scoliotic angle is reduced in 92.9 percent of adolescent group and had all of adult group after Chuna treatment, especially, statistically significance appeared in adult group. 4) Adult group was superior to adolescent group in correctability and scoliotic angle change. 5) Correction effect was better in the group which initial scoliotic angle was lessor, or had more time of treatment. 6) According to grade of vertebral body rotation, scoliotic angle is larger and correctability was lower. Conclusions : Chuna treatment was efficacious against scoliosis. And it is necessary for adolescent scoliosis patient to be more carefully treated and observed.

A Case Report of Spondylolytic Spondylolisthesis Treated with an Oriental Medical Treatment (한방치료를 적용한 척추분리성 척추전방전위증 환자의 치험 1례)

  • Objective: To report the effect of an oriental medicine treatment, including chuna manual therapy, for a patient suffering with spondylolytic spondylolisthesis. Methods: One patient was treated by oriental medical treatment including acupuncture, pharmacopuncture, and chuna manual therapy and the results were assessed with a Numerical Rating Scale (NRS), walking distance, and radiological examination. Results: After treatment, the NRS changed from 6 to 2 and walking distance changed from 250 m to 1000 m. Conclusions: In this study, oriental medical treatment, including chuna therapy, was an effective treatment for patients with spondylolytic spondylolisthesis. However, additional studies are needed, as are more and observations of these patients.

The Effect of Graston Technique and Chuna manual therapy combined with Korean Medical Treatment for fibromyalgia: A Case Report

  • Objectives : The purpose of this study was to report the clinical effects of the Graston Technique and Chuna manual therapy, combined with Korean Medical Treatment for fibromyalgia. Methods : We treated a patient diagnosed with fibromyalgia. We used acupuncture, the Graston Technique, Chuna manual therapy, pharmacopuncture, herbal medicine, moxibustion and physical therapy. Outcomes were evaluated using the American College of Rheumatology Preliminary Diagnostic Criteria (ACR), the Fibromyalgia Impact Questionnaire (FIQ), and the Numeric Rating Scale (NRS). Results : The widespread pain index (WPI) scale score of the ACR decreased from 12 to 9, and the symptom severity scale (SS) score of the ACR decreased from 8 to 6. The FIQ score decreased from 63.69 to 50.15. On the NRS, lower back pain & lower limb pain decreased from 6 to 2; neck pain from 6 to 3; muscle tenderness & morning stiffness from 6 to 4; fatigue from 6 to 3; urticaria from 6 to 2. Conclusion : This case study suggests that the Graston Technique and Chuna manual therapy combined with Korean Medical Treatment may be effective treatments for fibromyalgia. However, further studies are needed.

A Case Report on Snapping Hip Patient Treated by Chuna Manual Therapy for Meridian Sinew System (경근추나 치료 후 호전된 발음성 고관절 환자 치험례)

  • Objectives : This study is designed to evaluate the effect of conservative oriental medical treatment using Chuna manual therapy for meridian sinew system for snapping hip patient who have hip joint movement system impairment. Methods : 28-year old snapping hip patient who have hip joint movement system impairment was treated with conservative oriental medical treatment using Chuna manual therapy for meridian sinew system. The improvement of the patient was evaluated by Numerical rating scale(NRS), Snapping Sound Degree(SSD), Range of Motion(ROM). Results: After 13 times treatment, the patient had significant improvement in Numerical rating scale(NRS), Snapping Sound Degree(SSD), Range of Motion(ROM). Conclusions: If hip joint movement system impairment cause a snapping hip, we can treat with Chuna manual therapy for meridian sinew system.

  • Objectives This case study aimed to investigate the effect of acupotomy and fascia chuna therapy on talipes cavus-type plantar fasciitis. Methods We classified the foot arch type using a podoscope and estimated the outcome by evaluating the numeric rating scale, pain disability index, and EuroQol-5-dimension measurement. Results After treatment, heel pain decreased and the quality-of-life score improved. Conclusions This study suggests that treatment with acupotomy and muscle energy techniques based on the foot arch type may be effective for plantar fasciitis. A limitation of this study is the small number of cases. Further clinical studies are required.

  • Objectives : The objective of this study is to report the improvement of two patients with Iliolumbar ligament syndrome showed a physical examination positive. Methods : Two patients were treated by acupuncture therapy with chuna therapy at pelvic. We evaluated the effectiveness by numerical rating scale (NRS) and Oswestry Disability Index(ODI). Results : Both patients improved significantly NRS score and ODI score. And both patients' symptoms were alleviated. Iliolumbar ligament stress test was changed from positive to negative. Conclusions : We conclude that acupuncture therapy with chuna therapy at pelvic is an effective treatment to reduce the iliolumbar ligament originated pain. But there is a limit on this study due to sufficient number of cases. Further studies will be needed.

  • Objective : The purpose of the study is to compare the effects and efficacy of both treatments. Methods : 2 groups were divided among 6 patients who admitted to Jeonju Wonkwang hospital attached to Wonkwang University from 2009. jan. 1 to 2009. feb. 28 were diagnosed as herniated lumbar disc by lumbar CT or MRI. Each groups were treated by decompression therapy and chuna traetment daily and were measured under visual analogue scale and ODI score starting from the day of admission, after 1week, and second week to compare the results. Results and Conclusions : Decompression therapy and chuna treatment both showed effects on treating the patients though chuna traetment apparently revealed to be slightly more efficient. Due to the lack of abundant clinical data it was not objective in a strict sense but it was clinically meaningful enough.

  • Objectives: The aim of this study is to assess the quantity and quality of randomized controlled clinical trials (RCTs) published in the journal of Korean Society of Chuna Manual Medicine for Spine and Nerves (KSCMSN). Methods: All relevant RCTs were selected and extracted. Data extract of RCTs from all the articles published in the jounal of KSCMSN up to now, quantity assessment was made on the study design, sample size, intervention, control group and medical condition. of the consolidated standards of reporting trials (CONSORT) check list. Assessment was performed by 2 independent reviewers and disagreement was discussed based on concensus. Results: Among the 276 articles, 10 RCTs were published. 1st articles were published in 2003 and half of RCTs were published in 2011. All RCTs were parallel 2-arm designed. Average sample size was 29.2 per study and 14.6 per arm. Main intervention was consisted as acupuncture 40%, pharmacopuncture 30% and Chuna 30%. Average of adequacy of CONSORT check list was 10.3% and overall reports were insufficient. Conclusions: Though RCTs published in jounal of KSCMSN were increasing, the quality remains low. KSCMSN should make a effort to follow the CONSORT statement and improve the quantity and quality of studies.
