Purpose: This study was done to better understand how sick children and their families define chronic illness; what behaviors they used for managing chronic illness; and how they perceived the socio-cultural context of Korea. Methods: This study was a secondary analysis of qualitative studies. Articles on children with a chronic illness and their families located in electronic databases were selected for review. Twenty one qualitative studies were reviewed. Qualitative studies that had used an analysis tool, the Family Management Style Framework were reviewed. Results: Children with chronic illness and their families tended to accept illness as a negative outcome and thought that they were deprived of the context of normality. In the traditional Korean family style, parents-in-law demand absolute obedience from their daughter-in-law, leading to a conflict between the two parties, which, in turn, may have negatively affected their perceptions of chronic illness. Western and oriental medical treatments were used, and participants sought an array of folk remedies. Conclusion: Culturally specific findings can help to better understand the difficulties faced by children with a chronic illness and their families and can provide invaluable input into the development of culturally appropriate and sensitive nursing interventions.
Objectives: This study aimed to examine factors that affect chronic illness anticipated stigma among patient living with Parkinson's disease. The attributes of stigma measured in this study were labeling, expectation of prejudice, negative stereotyping, and discrimination. Chronic illness anticipated stigma was correlated with measures of mental and physical health as well as certain health behaviors. Methods: The sample included 250 adults with idiopathic Parkinson's disease that attended D university hospital in B metropolitan city from June to August 2014. Results: The chronic illness anticipated stigma were significantly correlated to depression, anxiety, Schwab & England ADL, Hoehn & Yahr stage, and age. The significant predictors of chronic illness anticipated stigma were anxiety, depression, and age. These factors accounted for 47% of variance in chronic illness anticipated stigma. Conclusions: In order to reduce the chronic illness anticipated stigma, nurse involved in the treatment and care of patients with Parkinson's disease should encourage patients to attend programs of depression and anxiety management in Parkinson's disease and systemic education programs on stigma.
Purpose: This study was performed to identify the illness intrusiveness, quality of life, and their relationships in patients with chronic liver disease. Method: Data was collected using scales of illness intrusiveness developed by Devins et al and Korean health related quality of life (KoQoLS) developed by Shim et al to measure the illness intrusiveness and quality of life from 141 chronic liver disease patients. Result: The total mean score of illness intrusiveness was $38.85{\pm}18.45$. The domain of illness intrusiveness which showed the highest mean score was health ($4.60{\pm}2.02$), and the lowest mean score domain was relationship with spouse ($20.4{\pm}1.69$). The subcategory of KoQoLS which showed the highest mean score was bodily pain ($6.96{\pm}2.98$), and the lowest mean score subcategory was vitality ($2.75{\pm}1.20$). There were negative relationships between illness intrusiveness and KoQoLS in all subcategories. Abovel all, Illness intrusiveness had the strongest negative relationship with role limitation (r=-0.641) among the KoQoLS subcategories. Conclusion: Because the illness intrusiveness had negative influence on the quality of life in patients with chronic liver disease, further research will need to specify detailed illness intrusiveness and to explore influencing factors on quality of life in them.
Background: Patients with the chronic physical illness are more likely to experience depression, and the accompany of chronic physical illness and depression is particularly high in middle-aged and elderly women. Considering that depression is associated with somatization and the decline of therapeutic compliance when accompanied by chronic physical illness, middle-aged and elderly women who experience depression among chronic physical illness may increase their use of medical services. This study is to identify the effect of depression on the use of medical services, especially among middle-aged and elderly women with chronic physical illness. Methods: This study used the 2016 Korean Health Panel. For analysis, it used T-test, negative binomial regression, and multivariate regression combining propensity score matching. Results: First, depressive groups had a higher number of medical service utilization and total medical expenditure than the non-depressive group. Second, depression significantly increased medical service utilization (β=0.17, p=0.04) at the 5% significance level. Also, depression significantly increased total medical expenditure (β=0.37, p=0.08) at the 10% significance level. Conclusion: For those who have chronic physical illness among middle-aged and elderly women, the experience of depression was confirmed to be a factor affecting the use of medical services. In the end, it is important to come up with policy countermeasures for middle-aged and elderly women accompanied by depression and chronic physical illness.
Purpose: This ethnography was done to explore the meaning of illness in Korean Americans with chronic hepatitis B. Methods: The participants were 6 patients with chronic hepatitis B and 6 general informants who could provide relevant data. Data were collected from iterative fieldwork with ethnographic interviews within Korean communities in two cities in the United States. Data were analyzed using causal chain analysis developed by Wolcott. Results: The analyses revealed three meanings for the illness: hidden disease, intentionally hidden disease, and inevitably hidden disease. The contexts of meaning of illness included characteristics of the illness, social stigma, structure of health care system and communication patterns and discourse between health care providers and clients. Conclusion: The meaning of illness was based on folk illness concepts and constructed in the sociocultural context. Folk etiology, pathology and interpretation of one's symptoms were factors influencing illness behavior. These findings could be a cornerstone for culture specific care for Korean Americans with chronic hepatitis B.
The purpose of this descriptive study was to identify the nursing needs of patients with chronic illness. Method: The subjects of this study were 636 patients with chronic illness, 323 general nurses, and 106 public health nurses. The instruments used for this study were questionnaires including perceived functions of nursing from the center for chronic illness, preference to placement, intention to use, needs to receive services from the center for chronic illness, strategies management for nursing, and nursing needs of chronically ill patients. Results: The mean of perceived functions for nursing from the center were 3.1(0.5) in public health nurses, 2.9 (0.59) in general nurses, and 2.4(1.33) in chronically ill patients. Regards of needs to receive on services of the nursing centers were, the regularly physical examination, for health educational services which was perceived highest request amongst chronically ill patients. We found the means of each specific need as 2.2(0.6), for physical health, 2.1(0.7), for psychosocial health, and 1.8(0.6) for spiritual health. Conclusion: From the results of this study, it is suggested that establishing a nursing center for chronically ill patients consider physical, psychosocial, for spiritual health needs of chronically ill patients. It is also a consideration that direct care for symptom management and health education in the nursing center be implemented.
Purpose: This study was to identify the relationship between family support, self-efficacy and self-care performance in patients with chronic illness. Methods: The subjects were 100 in-and out-patients who were diagnosed with chronic illness at university hospital in Cheonan. The data analyzed using descriptive statistics, Pearson correlation coefficient and stepwise multiple regression with SPSS WIN 12.0. Results: The level of family support, self-efficacy and self-care performance were high. The family support in patients with chronic illness indicated a significant positive correlation to self-care performance but no significant correlation to self-efficacy. The self-efficacy in patients with chronic illness indicated a significant positive correlation to self-care performance. Family support and self-efficacy account for 24.4% of variance of the self-care performance. The most significant predictor which influenced self-care performance in patients with chronic illness was family support. Conclusion: The results of this study showed that family support and self-efficacy are very important variables in explaining the self-care performance. Therefore, these variables should be considered in nursing intervention development and education.
Purpose: The purpose of this study was to identify the factors influencing Symptoms of Stress in patients with chronic illness. Method: Data were collected by questionnaires from 1,748 patients with chronic disease in General Hospital in Seoul. Chronic diseases of were cardiac disease including hypertension, peptic ulcer, pulmonary disease included COPD and asthma, DM, and chronic kidney disease. The data were analyzed using descriptive statistics, pearson correlation coefficients, and stepwise multiple regression. Result: 1. The level of symptoms of stress was moderate(M=2.17). 2. The score of symptoms of stress showed significantly positive correlation with the score of mood state(r=.58, p=.00), perceived stress(r=.57, p=.00), and ways of coping(r=.33, p=.00). The symptoms of stress showed significantly negative correlation with the score of social support(r=-.37, p=.00) and self-esteem(r=-.19, p=.00). 3. The most powerful predictor of symptoms of stress was mood state and the variance explained was 34%. A combination of mood state, ways of coping, perceived stress, social support, and duration of illness account for 45% of the variance in symptoms of stress of the patients with chronic illness. Conclusion: This study suggest that mood state, ways of coping, perceived stress, and social support are significantly influencing factors on symptoms of stress of the patients with chronic illness.
The purposes of this study were to identify the relationship between parent and child with chronic illness and developmental disability, and to review the strategy for improvement of their relationship. The effects of chronic illness and developmental disability is that the child has delayed growth and development, and his(her) parent has sadness, guilty feeling, anxiety, grief, disappointment, low self-esteem, anger and resentment. Chronic illness and developmental disability also have a negative effect to the parental marital relationship. The reaction of parent and child varies in age of onset, a developmental transition period, crisis and parent-child relationship. Through this study, I propose that parent-child relationship could be improved by touch, communication between them and education for parents.
A Health Interview Survey at Chuncheon city was conducted to assess level of illness by stratified random sampling method. Sample size was 6,007 and the prevalence of illness in period of 15 days was studied. 1) Prevalence rate of illness among Chuncheon citizen was 20.8 percent, and that of acute condition was 10.8 percent and chronic condition, 10.0 percent. 2) There was significant difference in prevalence rate by sex, especially the prevalence rate of chronic illness. 3) High positive correlation was noted between aging and prevalence rate of chronic illness. But negative correlation was noted in case of acute illness. 4) Multiple regression analysis on morbid state by socioeconomic variables was not seen to be appropriate for this survey, especially the analysis of acute illness. 5) Estimation of anual prevalence rate could not be possible because of seasonal variation of disease prevalence.
