만성골반염(慢性骨盤炎) 환자에 대한 한방치료(韓方治療) 치험 1례(例) (Clinical report on a patient with chronic PID)

  • Purpose : This study was designed to evaluate the effects of oriental medicine therapy on a chronic PID(pelvic inflammatory disease). Methods : This patient was treated with oriental medicine of Danchisoyosan(丹梔逍遙散), acupuncture at Qihai(氣海), Guanyuan(關元), Zhongji(中極), Sanyinjiao(三陰交) and Zigong(子宮) and moxibution at Zhongwan(中脘), Guanyuan(關元). Results : After the oriental medical treatments such as oriental medicine, acupuncture and moxibution, improvement was seen in the patient's symptoms such as lower abdomen pain, leukorrhea and chronic fatigue syndrome. Conclusion : This results indicates that the oriental treatment is effective in the treatment of chronic pelvic inflammatory disease.

농민의 작업환경별 노동부담 경감방안에 관한 연구(I) -겨울철 비닐하우스에서 상추재배 작업을 중심으로- (A Study for Farmers to Reduce Work Load on the Different Working Conditions (part I) - Cultivating Lettuce in the Winter Greenhouse -)

  • In this study we tried to give a decision on propriety of working conditions, to present ideas on reducing work loads. and to grope for efficiency of agricultural works. For this we examined the actual conditions of working environment, farmer's clothes, working posture, working methods, working time, resting state, fatigue recovery methods during cultivating lettuce in the winter greenhouse. And Ive improved harmful factors that affect farmer's health by considering results of previous study and farmer's subjective sensation. And we measured, compared, and analyzed the farmer's work loads before and after improvement. The results of this study are as follows ; 1. According to examine the actual conditions of cultivating lettuce in the winter greenhouse, farmers have experienced physical and mental chronic fatigue on the basis of the hot and humid crops-centered working environment, the rough ground condition, inconsistent arrangement of working stand and sorter, heavy-weared habits. and unsuitable working posture. 2. When we improved harmful factors that affect farmer's health, conformed the positive effects on important work efficiency index such as heart rate, electromyovolume, body temperature, and microclimate inside clothing and work loads were decreased by improving the hot and humid working environment, eliminating the hillock and obstacles of working path. deliver way, arranging the working stand and sorter consistantly, decreasing the clothing weight, improving the working postures and methods as using assistant appliances, alloting the working time and sequence effectively and presenting the light gymnastic exercises and rest for fatigue restoration.

And we improved harmful factors that affect farmer's health by considering results of previous study and farmer's subjective sensation. When we improved harmful factors that affect farmer's health, conformed the effects on important work efficiency index such as heart rate, electromyovolume, body temperature, and microclimate inside clothing and work loads were decreased by eliminating the hillock and obstacles of ground, decreasing the clothing weight, using proper clothing appliances such as hat and sunglasses, controlling height of working stand and sorter suitably, improving the working postures and methods as using assistant appliances, alloting the working time and sequence effectively and presenting the light gymnastic exercises and rest for fatigue restoration.

노인 퇴행성디스크 환자의 안정화운동이 척추불안정과 피로도에 미치는 영향 (The Effects of Segmental Instability and Muscle Fatigue after Stabilization Exercise Program in Degenerated Disc Disease Patients of Aged)

  • The purpose of this study was designed to find out the effectiveness of vertebral segment instability, muscle fatigue response on lumbar spine after apply lumbosacral stabilization exercise program to 4 patients with chronic low back pain and for 12 weeks. In this study, the lumbar spine motion with blind by MedX test machine and the difference of instability to lumbar vertebra segments in flexion, extension test of standing position and spinal load test(Matthiass Test) by Spinal Mouse. The stabilization exercise program was applied 2 times a week for 12 weeks in hospital and 2 times a day for 20 minutes at home. The results of the present study were as follows: 1. Instability test of lumbar vertebra segment is 2 type differential angle test between vertebrae segment and loading test of spine(matthiass) by Spinal Mouse. It appeared to improve stability of segments in sagittal plane after program. So lumbar spine curve increased lordosis toward anterior and was improved of the lumbar spine flexibility in flexion and extension. Specially, in matthiass test, ( - ) value was increased between lumbar vertebra segment when was the load on spine. And so stability improved after program. 2. Fatigue response test(FRT) results, in male, was raised muscle fatigue rate during increase weight, on the other hand female appeared lower than male. As a results, lumbosacral stabilization exercise was aided to improvement of lumbar spine vertebra segments stabilization. Spine instability patients will have a risk when in lifting a load or working with slight flexion posture during the daily of living life and it is probably to increase recurrence rate. Thus, not only lumbar extension muscle strength but also stability of vertebra segments in lumbar spine may be very important.

비진행성 갑상선암 환자에 대한 제주워터의 항피로 효능에 대한 유효성과 안전성 평가 예비 임상연구 - 무작위배정, 이중맹검, 삼군위약대조군 연구 (Effects of Vanadium Water on Cancer-related Fatigue of Non Advanced Thyroid Cancer Patients: Randomized, Three armed, Triple Blinded Controlled Trial)

  • Objective : This study was conducted to evaluate of efficacy and safety of Vanadium Water (VW) on Cancer related Fatigue of Non-advanced Cancer Patients. Methods : Experimental Group Administration. Participants in treatment group consumed three 500 ml/bottle bottles of Jeju Island's VW, which contains 40.2 ${\mu}g/L$ of vanadium, three times a day (morning, afternoon, evening) for 4 weeks (28 days) without any additional treatments for fatigue improvement. The control group participants consumed three 500 ml/bottle bottles of ordinary water (0 ${\mu}g/L$ of vanadium) three times a day for the same period of time without any additional treatments for fatigue improvement. Results : After 4 weeks of VW consumption, total score of Revised Piper Fatigue Scale (RPFS) was reduced 30% and basal score after Fisher's Exact Test resulted statistical borderline significance (p=0.0799) and result of t-test of comparison between two groups' RPFS total score and fatigue cognitive/mood changes resulted statistical significance (p=0.0112). In detail, only 36.4% of control group (N=11) showed improvement while 77.8% of treatment group (N=9) showed fatigue improvement and thyroid hormone level changes. No other lab measures indicated any significant differences between two groups. Conclusion : Daily consumption of 1.5 L of Vanadium water for 4 weeks showed improvement of non-advanced cancer patients' fatigue.

INFLUNCE OF ELECTRO-MYOSCOPE(ACUSCOPE) ON INJURED QUADRICEPS FEMORIS WITH USING ELECTRO_MYOGRAPHIC ANALYSIS BUILT INTO THE ELCETROMYOSCOPE

  • 이 실험의 목적은 Electro-Myoscope 이 과다 사용에 의해 일어나는 대퇴사두 근육 통증에 미치는 효과에 대해 근전도를 이용한 연구입니다. 이를 위해 나이 15세부터 50세 사이의 18명의 여성 자원자들이 참여했다. 대퇴직근을 Cybex 운동기기를 사용해 Terminal knee extension 운동을 통해 과다사용으로 인한 통증이 유발되게 하였다. 그리고, Electro-Myoscope을 치료 후 근전도를 통해 대퇴사두근의 motor recruit 에 대해 기록 비교하였다. t-test 와 Chi square 분석 결과 대퇴사두근의 기능이 향상된 것으로 나타났다.

피로를 주소로 내원한 환자의 생활섭생, 허손 및 피로의 상관성에 대한 고찰 (Speculation of the Correlationship between Public Health Practice, Consumption and Fatigue in Patients who Complain Fatigue)

  • Objectives : Consumption is a chronic wasting disease, in oriental medicine concept. Fatigue is a common symptom experienced by many people who have consumption. However, there has been little study about the relationship between consumption and fatigue in the Oriental medicine. For this reason, we attempted to investigate the present status of fatigue of outpatients, and its relation with consumption index, public health practice index, Chalder fatigue scale and Subjective Symptoms of Fatigue Test. Methods : The subjects were 149 outpatients who complain fatigue between Nov. 1, 2006 and Oct. 31, 2007. We measured degree of consumption by consumption index. We measured degree of fatigue by Chalder fatigue scale and Subjective Symptoms of Fatigue Test. Public health practice were investigated by "Breslow's 7 health practice". Results : Studies have shown that a group who disregard public health practices have higher numerical value, in part of $q\grave{i}-x\bar{u}$, $xu\grave{e}-x\bar{u}$, and Physical symptoms than a group more concerned with public health practice. A group who has fatigue for a period over 6 months are more likely to be $q\grave{i}-x\bar{u}$, $y\bar{i}n-x\bar{u}$, Chalder fatigue scale, neuro-sensory symptoms than a group fatigued for less than 6 months. Among the subjects, 81 (55.5%) were considered as 'fatigue patients' by the Chalder scale. Chalder fatigue scale, according to the Subjective Symptoms of Fatigue Test, consumption also, The more increase of Subjective symptoms of fatigue test, the more of consumption scale. Conclusions : This study has shown that there is a link between consumption and fatigue. Moreover, oriental medicine's consumption is more concrete than fatigue scale, so consumption index will have wide application to the study of fatigue.

여성 류마티스 관절염 환자의 피로 예측 모형 (Prediction Model of Fatigue in Women with Rheumatoid Arthritis)

  • Rheumatoid arthritis is a chronic systemic autoimmune disease. Although the joints are the major loci of the disease activity, fatigue is a common extraarticular symptom that exists in all gradations of rheumatoid arthritis. Fatigue is defined as a subjective sense of generalized tiredness or exhaustion and has multiple dimensions. Therefore fatigue is a common and frequent problem for those with rheumatoid arthritis. In fact, 88-100% of individuals with rheumatoid arthritis experience fatigue. Especially the degree of fatigue is higher in women than men with rheumatoid arthritis. Despite the importance of fatigue among the patients with rheumatoid arthritis, the mechanism that leads to fatigue in rheumatoid arthritis is not completely understood. This study was intended to test and validate a model to predict fatigue in women with rheumatoid arthritis. Especially it was intended to identify the direct and indirect effects of the variables of pain, disability, depression, sleep disturbance, morning stiffness, and symptom duration to fatigue. Data were collected by questionnaires including Multidimensional Assesment of Fatigue(Tack, 1991), numeric scale of pain, graphic scale of joints, Ritchie Articular Index, Korean Health Assessment Questionnaire(Bae, et al., 1998), Inventory of Function Status(Tulman, et al., 1991), Center for Epidemiologic Studies-Depression, and Korean Sleep Scale(Oh, et al 1998). The sample consisted of 345 women with a mean duration of rheumatoid arthritis for 10.06 years and a mean age of 49.64 years. SPSS win and Win LISREL were used for the data analysis. Structural equation modeling revealed the overall fit of the model. Pain predicted fatigue directly and indirectly through disability, depression, and sleep disturbance. Disability, sleep disturbance predicted fatigue only directly, while depression only indirectly through disability and sleep disturbance. Also morning stiffness and symptom duration predicted fatigue through disability and depression. All predictors accounted for 65% of the variance of fatigue. Depression, pain, and disability predicted sleep disturbance. Depression had reciprocal relationship with disability and they both were predicted by pain directly and indirectly. In summary, pain, depression, disability, sleep disturbance, morning stiffness, and symptom duration contributed to the fatigue of patients with rheumatoid arthritis. The best predictor of fatigue was pain. This finding indicates that the modification of pain, depression, disability, sleep disturbance, morning stiffness could be nursing intervention for relief or prevention of fatigue.

Art therapy using famous painting appreciation maintains fatigue levels during radiotherapy in cancer patients

  • Purpose: The purpose of this study was to evaluate the efficacy of art therapy to control fatigue in cancer patients during course of radiotherapy and its impact on quality of life (QoL). Materials and Methods: Fifty cancer patients receiving radiotherapy received weekly art therapy sessions using famous painting appreciation. Fatigue and QoL were assessed using the Brief Fatigue Inventory (BFI) Scale and the Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-F) at baseline before starting radiotherapy, every week for 4 weeks during radiotherapy, and at the end of radiotherapy. Mean changes of scores over time were analyzed using a generalized linear mixed model. Results: Of the 50 patients, 34 (68%) participated in 4 sessions of art therapy. Generalized linear mixed models testing for the effect of time on mean score changes showed no significant changes in scores from baseline for the BFI and FACIT-F. The mean BFI score and FACIT-F total score changed from 3.1 to 2.7 and from 110.7 to 109.2, respectively. Art therapy based on the appreciation of famous paintings led to increases in self-esteem by increasing self-realization and forming social relationships. Conclusion: Fatigue and QoL in cancer patients with art therapy do not deteriorate during a period of radiotherapy. Despite the single-arm small number of participants and pilot design, this study provides a strong initial demonstration that art therapy of appreciation for famous painting is worthy of further study for fatigue and QoL improvement. Further, it can play an important role in routine practice in cancer patients during radiotherapy.
