A Case of Combined Korean-Western Medicine Treatment on Chorea Hyperglycemia Basal Ganglia Syndrome Induced by Nonketotic Diabetes Mellitus (비케톤성 당뇨병으로 유발된 고혈당성 무도증의 한·양방 복합치료 증례보고)

  • Objectives: Chorea Hyperglycemia Basal Ganglia Syndrome(C-H-BG) is a syndrome with chorea-ballismus induced by hyperglycemia. This case study reports the clinical effect of Korean medicine treatment on C-H-BG. Methods: A 73-year-old male patient with left side chorea-ballismus visited Daejeon Korean Medicine Hospital. He had no diabetes mellitus(DM) history but the laboratory test result suggested nonketotic DM. We diagnosed him as C-H-BG and treated with herbal medicine, acupuncture and hyperglycemic medication. To evaluate chorea-ballismus, we checked Abnormal Involuntary Movement Scale(AIMS) score everyday. Results: After 14 days of treatment, the chorea-ballismus improved and AIMS score decreased from 38 to 4. Gait disturbance caused by the symptom disappeared. Conclusions: We suggest that Korean medicine treatment can improve the symptom of C-G-BG.

A Case Report of a Delayed Vascular Chorea Patient Treated with Sukjihwangbogan-tang-gagam (소뇌경색의 후유증으로 발생한 지연성 혈관 무도병 증례 보고 : 협진 치험 1례)

  • Objective: To evaluate the effect of Sukjihwangbogan-tang-gagam (熟地黃補肝湯加減) in a patient with delayed vascular chorea. Methods: A vascular chorea patient was treated with acupuncture, moxibustion, and herbal medicine (Sukjihwangbogan-tang-gagam). Results: Improvements in the Unified Huntington’s Disease Rating Scale (UHDRS), UFMG Sydenham’s Chorea Rating Scale (USCRS) (especially the scale of Activity of Daily Living (ADL)) (dysarthria 2→0, tongue protrusion 3→1, chorea 3→1, handwriting 3→1), and a Visual Analogue Scale (VAS) (8→2) were observed after the Sukjihwangbogan-tang-gagam treatment. Conclusions: Sukjihwangbogan-tang-gagam may be an effective treatment for patients with delayed vascular chorea.

  • Objectives: This report presents a case of late onset chorea hyperglycemia basal ganglia syndrome treated using traditional Korean medicine therapies. Methods: A patient was treated with Korean medicine therapies, including the herbal medication, Yokukan-san. We evaluated the improvements in the patient's symptoms based on the total score of the Abnormal Involuntary Movement Scale, the activities of daily living portion of the Universidade Federal de Minas Gerais (UFMG) Sydenham's Chorea Rating Scale, the numeric rating scale, and an assessment of the patient's writing. Results: After undergoing Korean medicine treatment for 15 days, there were improvements in the patient's chorea and writing as well as a decrease in the scores for all three scales. Conclusions: This clinical case study suggests that Yokukan-san might be effective for treating dyskinesia in patients with chorea hyperglycemia basal ganglia syndrome.

Objectives The primary purpose of this case is to report that a Taeurnin patients with Chorea originated after Cerebral infarction was treated with 'Yeoldahanso-tang(熱多寒少湯)' and then her symptoms improved 2. Methods We diagnosed her as Chorea originated after Cerebral infarction. So we treated her with 'Yeoldahanso-tang(熱多寒少湯)'. 3. Results After we treated her with 'Yeoldahanso-tang(熱多寒少湯)', her symptoms improved. 4. Conclusions 'Yeoldahanso-tang(熱多寒少湯)' may have an effect on Treatment Chorea originated after Cerebral infarction of Taeurnin.

  • Chorea-acanthocytosis (ChAc) is a rare hereditary disorder characterized by involuntary choreiform movements and erythrocytic acanthocytosis. Pharmacotherapy for control of involuntary movements has generally been of limited benefit. Deep brain stimulation (DBS) has recently been used for treatment of some refractory cases of ChAc. We report here on the effect of bilateral high-frequency DBS of globus pallidus interna in a patient with ChAc.

  • Even though the nonketotic hyperglycemia is a metabolic disorder, it complicates hemichorea-hemiballism rarely. Moreover, generalized chorea-ballism associated with nonketotic hyperglycemia in diabetes mellitus is very rare, so it has not been reported in Korean literature. Although the precise pathophysiologic mechanisms of these disorders are still poorly understood, deficiency of gamma aminobutyric acid (GABA) in nonketotic hyperglycemia or reduced GABAnergic inhibition by striatal lesion may increase inhibitory output to subthalamic nucleus. These result loss of pallidal inhibition and produce contralateral hemichorea-hemiballism. The striatal lesions, such as transient ischemia with reactive astrocytosis or small amount of petechial hemorrhage, are related with changes of magnetic resonance image (MRI) findings presumably. We report a diabetic old woman who developed generalized chorea-ballismus as a very rare complication of nonketotic hyperglycemia. Her brain MRI showed high signal intensity in left lentiform nucleus and right pallidum on T1 weighted images and low signal intensity in bilateral putamen on T2 weighted images with highly enhanced corresponding lesions on T1 weighted enhancement images.

  • Extrapyramidal movement disorders are divided descriptively into hypokinesias(such as parkinsonism), characterized by poverty and slowness of movement : hyperkinesias(such as chorea, athetosis, dystonia, ballism, etc.), manifested by abnormal involuntary movement. Chorea refers to widespread arrythythmic movements of a forcible, rapid, jerky, restless type Choreic movements are noted for their irregularity and variability. They are generally continuous, may be simple or quite elaborate, and affect any part of the body. Dystonla refers to abnormally increased muscular tone that causes fixed abnormal postures. Some patients with dystonia also have shifting postures, resulting from irregular, forceful twisting movement that affect trunk and produce bizarre, grotesque movements and positions of the body. The most frequent and familiar type of focal dystonia is spasmodic torticollis. It consists of an involuntary turning of the head to one side - intermittent at first, then gradually worsening to the point of being more or loss continuous. The combination of blepharospasm and oromandibular dystonia is sometimes refered to as Meige's syndrome. We report two patients with dystonia and chorea in cerebral infarction at basal ganglia. We have experienced good improvement by the oriental medicine and acupuncture treatment.'rho acupuncture points of LI 4, ST 36, TE 3, GB 34, GB 41, LR 3, GB 39 were used. The therapies of herb-medicine were treated by Zibu-Ganshen(滋補肝腎), Huoxue-Xifeng-Tongluo(活血息風通絡).

Chorea in Systemic Lupus Erythematosus: Evidence for Bilateral Putaminal Hypermetabolism on F-18 FDG PET (전신성 홍반성 루푸스에서 F-18 FDG PET상 기저핵 포도당대사 증가 소견을 보이는 무도병 1예)

  • Purpose: We describe a 54-year-old woman with systemic lupus erythematosus (SLE) who suddenly presented with chorea and had positive antiphospholipid antibodies. F-18 FDG PET showed abnormally increased glucose metabolism in bilateral putamen and primary motor cotex. Tc-99m ECD SPECT also showed abnormally increased regional cerebral blood flow in bilateral putamen. She was treated with corticosteroid and aspirin after which the symptoms improved. Four months later, follow up F-18 FDG PET showed improvement with resolution of hypermetabolism in bilateral putamen. This case suggests that striatal hypermetabolism is associated with chorea in SLE.

  • Objectives : The purpose of this report is to introduce a rare case of a patient with Chorea Hyperglycemia Basal Ganglia syndrome (C-H-BG) whose initial medical interventions were started 2 weeks after the onset, and to suggest the possibility of treatment using a combined Western-Korean medicine approach. Methods : A 75-year-old female C-H-BG patient complaining of persistent right-sided hemichorea was treated with a therapy that combined Korean and Western medicine from April 4, 2015 to April 29, 2015. Improvements of symptoms were measured by a motor assessment of Unified Huntington's Disease Rating Scale (UHDRS), Visual Analog Scale (VAS) and the number of involuntary movements. Results : Motor assessment of UHDRS, VAS and the number of involuntary movements all showed a gradually improving tendency during 26 days of admission treatment. However, the patient's hemichorea persisted. Conclusion : Rapid blood sugar control is the most important treatment for C-H-BG, because pathologic changes of basal ganglia seem to become irreversible as time goes by. A combined Western-Korean medicine approach to treating C-H-BG seems effective not only in reducing hemichorea, but also in the management of accompanying symptoms such as muscle pain and general weakness.

  • Tremor is the common symptom clinically and often be continuous with the brain cortex function. Scalp Acupuncture was designed by connecting the Meridian theory with brain cortex functional theory. We carried out four patients with tremor. We operated Scalp Acupuncture on the chorea-tremor area in connection with precentral gyrus which is in charge of the motion of the opposite part of body about 20 minutes. Herbal medication and the other oriental medical treatments were performed. After our treatments, tremor and some other symptoms were improved.
